A Clinical Case Report of Chronic Urticaria Treated with Gagamtongcheong-san (가감통청산(加減通淸散)으로 치료한 만성 두드러기 치험 1례)

  • Objectives : The purpose of this study is to report 1 clinical experience of korean medicine treatment on chronic urticaria identified as "Wind Heat" pattern. Methods : We treated a 63-year old woman patient with Gagamtongcheong-san, acupuncture and pharmacopuncture. The improvement of patients evaluated using CU-Q2oL, UAS, and photography. Results : CU-Q2oL score decreased from 95 to 18, UAS decreased from 6 to 0. Conclusions : The complex treatments of korean medicine and was effective to improve of chronic urticaria. It is necessary to have more observations and case reports.

2 Cases of Olfactory Disorders due to Chronic Sinusitis treated with Korean Medical Treatment (한의약 치료로 호전된 만성부비동염으로 인한 후각장애 2례)

  • Objectives : The purpose of this study is to report the effect of Korean medicine on olfactory disorders due to chronic sinusitis. Methods : Two patients with chronic sinusitis complaining of olfactory disturbances, nasal obstruction, and purulent discharge were treated with herbal medicines and acupuncture treatment. The nasal endoscopic examination and paranasal sinuses x-ray series (PNS x-ray) were performed. In addition, numeral rating scale (NRS) of olfactory disturbances and degrees of nasal obstruction and purulent discharge were evaluated. Results : In cases 1 and 2, NRS of olfactory disturbances decreased from 9 to 1. Nasal obstruction and purulent discharge decreased to no symptoms or intermittent level at moderate or severe level. The endoscopic findings and PNS x-ray findings were improved after treatments. Among various treatments of Korean medicine, Gamitonggyu-tang may play a crucial role in the reduction of sinusitis and olfactory disorders. Conclusions : This case report can be used as a basis for the use of Korean medicine, Gamitonggyu-tang, for olfactory disorder due to chronic sinusitis.

A Case Report of Miller-Fisher Syndrome with Bilateral Abducens Nerve Palsy (밀러-피셔 증후군 환자의 양안 외전신경마비 치험 1례)

  • Objectives : The purpose of this study is to report the effect of Korean medicine treatment on limitation of eye movement in Miller-Fisher Syndrome. Methods : A patient was treated with herbal medicines, acupuncture(including electropuncture, phamacopuncture), Moxibustion and cupping therapy. To evaluate the result of this treatment, we used photographs of eye movement, Scott and Kraft score of both eye, length of eyeball movement and visual analogue scale(VAS) for subjective symptoms. Results : After treatment, the limitation of eye movement and diplopia were remarkably improved. Also, other symptoms such as dizziness, gait disturbance, facial nerve disorder were disappeared. Conclusions : This study suggests that Korean medicine treatments are effective for patient with limitation of eye movement in Miller-Fisher Syndrome.

Peripapillary Retinal Nerve Fiber Layer Thicknesses Did Not Change in Long-term Hydroxychloroquine Users

  • Purpose: To evaluate changes in the peripapillary retinal nerve fiber layer (RNFL) thicknesses using spectral-domain optical coherence tomography (SD-OCT) in hydroxychloroquine (HCQ) users. Methods: The medical records of HCQ users were retrospectively reviewed. In these HCQ users, an automated perimetry, fundus autofluorescence photography, and SD-OCT with peripapillary RNFL thickness measurements were performed. The peripapillary RNFL thicknesses were compared between the HCQ users and the control groups. The relationships between the RNFL thicknesses and the duration or cumulative dosage of HCQ use were analyzed. Results: This study included 77 HCQ users and 20 normal controls. The mean duration of HCQ usage was $63.6{\pm}38.4$ months, and the cumulative dose of HCQ was $528.1{\pm}3.44g$. Six patients developed HCQ retinopathy. Global and six sectoral RNFL thicknesses of the HCQ users did not significantly decrease compared to those of the normal controls. No significant correlation was found between the RNFL thickness and the duration of use or cumulative dose. The eyes of those with HCQ retinopathy had temporal peripapillary RNFL thicknesses significantly greater than that of normal controls. Conclusions: The peripapillary RNFL thicknesses did not change in the HCQ users and did not correlate with the duration of HCQ use or cumulative doses of HCQ. RNFL thickness is not a useful biomarker for the early detection of HCQ retinal toxicity.

  • Objectives : The aim of this study is to report the effects of Yangdokbaekho-tang and Hwangryunhaedok-Tang wet dressing on the treatment of erythrotelangiectatic rosacea patient. Methods : A patient with Erythrotelangiectatic Rosacea treated with Yangdokbaekho-tang, Hwangryunhaedok-Tang wet dressing and acupuncture. We evaluated the score(frequency ${\times}$ intensity) of the hot flush, the main symptom of rosacea and NRS(numeral rating scale) for the subjective symptom. Results : After treatment, the score of the hot flush decrease from 20 to 6. the NRS changed and NRS score improved from 7 to 1. There are no adverse effects and no relapse of erythrotelangiectatic rosacea after 18 days of discharge. Conclusions : According to the result, Korean Medicine such as Yangdokbaekho-tang and Hwangryunhaedok-tang wet dressing can be effective for the treatment of erythrotelangiectatic rosacea.

  • Objectives : The purpose of this study is to report three cases of Korean medical treatment for sudden sensorineural hearing loss(SSHL). Methods : This study conducted with three sensorineural hearing loss patients who hospitalized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean Medical Hospital. Three patients were treated with herbal medicine(Sunkihwalhyeol-tang, Ikgibohyeol-tang, Gongjindan) and acupuncture. After treatment, we evaluated Siegel's Criteria with pure tone audiometry and subjective symptoms alteration. Results : All three patients were completely recovery about Siegel's Criteria and improved subjective symptoms after treatment. Conclusions : This study suggests that Korean medical treatment may be effective about SSHL.

  • Objectives : The purpose of this study is to report three cases of Korean medical treatment for acute benign paroxysmal positional vertigo(BPPV). Methods : This study was conducted with three acute benign paroxysmal positional vertigo patients who hospit alized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean medical Hospital. Three patients were treated with Korean medical treatment including herbal medicine(Banhabaeckchulchunma-tang), acupuncture, p harmacopuncture(Hwangryunhaedok-tang). After treatment, we evaluated subjective symptoms. Results : The results of three patient showed remarkable improvements on subjective symptoms after the treatment. Conclusions : It is considered that Korean medical treatment can be applied to the treatment of BPPV.

A Study of Korean Medicine Hospital Inpatient Consulted by Korean Medicine Ophthalmology, Otolaryngology & Dermatology Department for Dermatology and Surgical Disease (피부과 및 외과 질환으로 한방안이비인후피부과에 의뢰된 한방병원 입원환자에 대한 고찰)

  • Objectives : The purpose of this study is to retrospectively analyze 3 years of inpatient dermatologic and surgical consultation to Korean medicine ophthalmology, otolaryngology & dermatology department in Korean medicine hospital. Methods : We classified dermatological and surgical inpatient consultations between September 1st, 2019 and August 31st, 2022, based on electronic medical record by gender, age, hospitalized department, categories of diseases. Results : Total number of consultations were 336. The proportion of female and male were 2.5:1, and the average of age was 65.6 and 60, respectively. The most frequently referred skin disease category was infectious skin disease(39.9%), followed by eczema(20.2%), dermatoses due to physical forces(10.1%), pigment anomalies(7.1%), nevi and tumors(4.8%), diseases of the skin appendages(4.2%), urticaria(3.9%), and others(4.9%). Consultations from department of Korean rehabilitation medicine, and Korean internal medicine were most common, because they have a comparatively large number of inpatients and long hospitalization period. In case of infectious disease and skin ulcer, patients from Korean internal medicine were frequently consulted due to characteristics of stroke and internal diseases. Conclusions : This study identified the frequency of consultation and the disease group with many requests for each department. Identifying the characteristics of diseases with a high frequency of consultation will prevent various dermatological problems that may occur by department. In addition, it was found that a prompt consultation was needed for accurate diagnosis and treatment in dermatological problem.

A Case of Acute Radiation Dermatitis Improved after Korean Medicine Treatment (한방 복합 치료 이후 호전된 급성 방사선 피부염 치험 1례)

  • Objectives : The purpose of this study is to report a case of Korean medicine on acute radiation dermatitis. Methods : This study was conducted with an acute radiation dermatitis patient who hospitalized in ophthalmology, otolaryngology & dermatology clinic of Korean medical hospital. The patient was treated with Korean medicine including acupuncture, wet dressing, herbal ointment and phototherapy. We evaluated the patient's subject symptoms before and after treatment using the VAS scale and CTCAE. Results : The patient's subjective symptoms were improved after treatment. Conclusions : It is considered that Korean medicine could be applied to the treatment of acute radiation dermatitis.

  • Painful ophthalmoplegia due to idiopathic granulomatous inflammation of the cavernous sinus/superior orbital fissure has been termed Tolosa-Hunt syndrome(THS). The syndrome is characterized by pain behind, above or around the eye, involvement of the cranial nerves which pass through the cavernous sinus, spontaneous remissions and exacerbations, and a favourable response to steroid therapy. There was the 56 years old man who suffered from painful ophthalmoplegia and facial palsy. The oriental medical treatment without steroids and analgesics reduced the symptoms of the patient remarkably during 5 weeks. There is few report that treat THS with oriental medical method. If more clinical trials like this are proved to be effective, we can expect that oriental medical treatment will be a good method in THS.
