Surgical Managements for Refractory Pupillary Block Glaucoma after Phacoemusification in a Dog (초음파 유화흡인술 후 발생된 난치성 동공 차단 녹내장의 수술적 치료)

  • A 13-year-old, castrated male Yorkshire terrier was presented with acute blepharospasm 2 months after phacoemulsification with intraocular lens implantation in the left eye. Menace response was weakly positive in the left eye. Intraocular pressure (IOP) was 98 mmHg in the left eye. On ophthalmic examination using slit lamp biomicroscope, episcleral injection and corneal edema were observed in the left eye. Aqueous flare (1+) and iris bombe with peripheral anterior synechia were also detected in the left eye. In spite of medical treatments, IOP was remained more than 50 mmHg. Synechiolysis and radial iridectomy with irrigation/aspiration of anterior chamber were performed in the left eye. One week after the surgery, tissue plasminogen activator was injected to resolve the blood and fibrin clot in the left anterior chamber. Menace response was recovered completely 1 month after the surgery and IOP was maintained less than 20 mmHg more than 6 months.

Protective effect of Kohl-Chikni Dawa against galactose-induced cataract in rats

  • The efficacy of Kohl-Chikni Dawa (KCD), a compound ophthalmic formulation of Unani medicine, to control cataract development was explored in rats fed 25% galactose with the diet for 6 months. When one drop of 3% KCD solution was applied (once and twice daily) in both eyes for 6 months in galactose-fed rats, a significant reduction of lens opacification was noticed. The morphological changes of the lenses were observed every month by slit-lamp biomicroscope. These results suggest that the local application of 3% KCD solution possesses anticataract effect in galactose-fed rats.

  • Over the last three decades, the development of Ti:sapphire femtosecond lasers has led to advancements in scientific and industrial fields. In particular, these advanced lasers show great potential for applications with bio-imaging and medical surgery, such as two-photon microscopy, nonlinear Raman microscopy, optical coherence tomography, and ophthalmic surgery. Herein, we present a detailed description of the theoretical and experimental physics of Kerr-lens mode-locked femtosecond Ti:sapphire lasers and its two-photon microscopy.

Inherited Metabolic Disorders Involving the Eye (유전성대사질환에서 눈이상)

  • Inherited metabolic disorders (IMD) are a large group of rare disorders affecting normal biochemical pathways. The ophthalmic involvement can be very varied affecting any part of the eye, including abnormalities of cornea, lens dislocation and cataracts, retina and the optic nerve, and extraocular muscles. Eye disorders can be initial symptoms of some IMD and can be clue for diagnosis of IMD. However, eye disorders can evolve later in the natural history of an already diagnosed metabolic disorder. Awareness of IMDs is important to facilitate early diagnosis and in some cases instigate early treatment if a patient presents with eye involvement suggestive of a metabolic disorder. Ophthalmological interventions are also an important component of the multisystem holistic approach to treating patients with metabolic disorders.

A Study on Digital Healthcare Optometry System Using Optometry DB

  • Recently, digital health care technology is spreading and developing in various fields. Therefore, in this paper, we realized that the field to which digital healthcare technology is not applied is the field of optometry, and implemented a digital healthcare optometry system for precise lens manufacturing. A device called Phoroptor is used to manufacture the lens, and this device sets the lens by measuring the visual acuity of the person who requested the glasses. And when the person to be measured wears glasses, a device called a PD meter is used to align the pupil center and lens focus. However, there is a limit to the convenience of precise lens production and optometry due to the absence of a database and program that can accumulate and analyze the PD measurement error, inconvenience and error due to manual control of the Phoroptor, and optometric information. Therefore, in this paper, PD meter design for more accurate PD measurement, Phoroptor design and Phoroptor control application design for automatic Phoroptor control, and a database and analysis program that automatically set lenses using optometry information for each subject had been designed. Based on this, ultimately, a digital healthcare optometry system using an optometry database has been implemented.

Comparison of Tear Distributions by the Corneal Eccentricity when Fitted with Spherical and Aspherical RGP Lenses (구면 및 비구면 RGP렌즈 피팅 시 각막 이심률별 눈물분포 비교)

  • Purpose: The present study was aimed to compare the tear volume and distribution by corneal eccentricity when fitted with spherical and aspherical RGP lenses. Methods: Spherical and aspherical RGP lenses were fitted in best alignment on a total of 77 subjects (136 eyes) in their twenties and thirties without any ocular disease or ocular surgery experience. The tear volume was analyzed by estimating the concentration of tear stained with fluorescein in the center of RGP lens as well as at the mid-peripheral and peripheral areas, and the difference of tear distributions was analyzed according to corneal eccentricity. Results: Tear distribution from the center to the peripheral area was not significantly different when spherical RGP lenses were fitted on the corneal eccentricities of e < 0.38 and $0.68{\leq}e$, indicating the relatively even tear distribution compared with other corneal eccentricity. In the case of aspherical RGP lenses, the difference of tear distribution between the central and peripheral areas was smaller than spherical RGP lenses. The significant difference of tear distribution according to RGP lens design was observed in the corneal eccentricity of 0.48 < e < 0.68. In other words, more even tear distribution was shown when aspherical RGP lenses were fitted on the cornea with eccentricity of $0.48{\leq}e<0.68$ and spherical RGP lenses were fitted on the cornea with eccentricity $0.68{\leq}e$. Furthermore, tear volume in the mid-peripheral area increased with higher corneal eccentricity. Conclusions: The results suggest that the appropriate selection of RGP lens design according to corneal eccentricity is necessary since tear volume and distribution by the regions of spherical and aspherical lenses are affected by corneal eccentricity.

The Error and the Graphical Presentation form of the Binocular Vision Findings (양안시기능 검사 값의 오차와 그래프 양식)

  • The stimulus of accommodation A, the stimulus of convergence C and the prism diopter ${\Delta}$ are reviewed and redefined more obviously. How the A and C are managed in the practice are reviewed and summarized. As a result, the common practical process of the binocular vision findings is most suitable in the case of the $l_c=26.67mm$, where the near distance is measured from the test lens to the near target and its value is 40 cm and the average of the P.D equal to 64 mm. The $l_c$ is the distance between the test lens and the center of rotation. Those values were used at calculating the various values in this paper. The error of the stimulus of accommodation values which are evaluated by the practically used formula (5) are calculated. Where the distance between lens and the principle point of eye is 15.07 mm ($=l_H$). The incremental stimulus of convergence values P' caused by the addition prism $P_m$ are evaluated by the recursion computation method. The P' are varied with the $P_m$, the distance $p_c$ between the prism and the center of rotation, the initial convergence value (or inverse target distance) $C_o$ and the refractive index n of the prism material. The recursion computation method and the other formulas are described in detail. In this paper n=1.7 is used. The two factors by which the P' is increased are exist. The one which is major is the property by which the values of convergence whose unit is ${\Delta}$ are not added in the generally way. The other is the that the actual power of the prism is varied with the angle of incidence light. And the P' is decreased remarkably by an increase in the $p_c$ and $C_o$. The $P^{\prime}/P_m$ are calculated and graphed which are varied with the $p_c$ and $C_o$, where the $P_m=20{\Delta}$, P.D=64 mm and n=1.7. The index n dependence of the $P^{\prime}/P_m$ is negligible (refer to fig. 6). The $p_c$ are evaluated at which the P' equal to the $P_m$ for various $P_m$ (refer to table 1). The actual values of the stimulus of convergence and accommodation which are manipulated simply in the practice are calculated. Two graphical forms are suggested. The one is like as the commonly used one. But the stimulus of convergence and of accommodation values in the practice are positioned at the exact positions when the graphic is made (refer to fig. 9). The other is the form that the incremental stimulus of convergence values caused by the addition prisms are represented at actual positions (refer to fig. 11).

One Year Follow-up for Successfully Treated Children with Accommodative Dysfunction (조절이상이 성공적으로 치료된 어린이에 대한 1년 후의 추적검사)

  • Purpose: The purpose of this study is to evaluate the long-term stability of the improved symptoms and accommodative functions after completion of accommodative therapy. Methods: Seven children (mean age${\pm}$SD: $12{\pm}1.41$ years) who were successfully treated with a vision therapy program for either accommodative insufficiency or infacility were followed for 1 year. The visual symptoms of the subjects were measured by the College of Optometrists in Vision Development Quality of Life (COVD-QOL) checklist, and this was followed by measurement of the monocular and binocular accommodative facility with ${\pm}2.00$ D flipper lens. Results: The mean visual symptoms at the 1 year follow-up examination ($15.14{\pm}8.59$) showed a small increase, but there was no significant difference (p=0.446) from post-therapy ($11.86{\pm}7.22$). There was small regression in the monocular (left eye, $13.86{\pm}3.93cpm$) and binocular ($11.14{\pm}3.13cpm$) accommodative facility at the 1 year follow-up examination, but there were no significant different from the monocular ($15.86{\pm}4.14cpm$, p=0.147) and binocular ($13.21{\pm}3.76cpm$, p=0.066) accommodative facility measurements at post-therapy. Also, every subject met the normative values of ${\geq}7$ cpm for monocular accommodative facility and ${\geq}5$ cpm for binocular accommodative facility in the long-term. Conclusions: There was long-term maintenance of the improved visual symptoms and accommodative functions, and so it is clear that the positive therapeutic effects persist with accommodative therapy.

Changes in Drug Elution Concentration and Physical Characteristics of Soft Contact Lenses Depending on the Initiator and Crosslinker (개시제와 교차결합제 농도의 변화에 따른 소프트콘택트렌즈의 물리적 성질과 약물용출 농도의 변화)

  • Purpose: The material properties of contact lenses were measured by varying the amounts of an initiator and a cross-linking agent that are the basis for the manufacture of contact lenses, and the drug delivery effects depending on the material properties of contact lenses were compared. Methods: Contact lens samples were manufactured using HEMA by varying the concentration of the cross-linking agent and the initiator. To investigate the changes in physical characteristics depending on the material properties, the results of the experiments on the amount of drug elution, water content, refractive index, and the amount of protein adsorption were compared. Results: For the contact lenses manufactured by varying the amount of the initiator, the water content hardly changed, and the refractive index also hardly changed. The amount of drug elution was not much affected by the changes in the initiator, but the amount of elution increased as the water content increased. The amount of protein adsorption was hardly affected by the changes in the initiator, but the amount of adsorption increased as the water content decreased. Conclusions: The changes in the properties were hardly affected by the changes in the amount of the initiator, but were significantly affected by the changes in the amount of the cross-linking agent. As the amount of the cross-linking agent increased, the water content decreased, while the refractive index increased. Also, when the water content increased, the amount of drug elution increased, while the amount of protein adsorption decreased.

The adsorption pattern of protein to the soft contact lens and its effect on the visible light transmission and the contact angle (소프트콘택트렌즈의 단백질 부착 양상 및 가시광선투과도와 접촉각에 미치는 영향)

  • Protein deposit buildup on soft contact lenses(SCLs) is responsible for discomfort, optical defects and is even damaging to the eye. To better understand protein deposit formation on SCL, we investigated the pattern of protein attachment on patient-worn SCLs and the effect of the protein on the contact angle of SCLs and the transmission of visible light. This study used data from a clinical trial where subjects wore either daily-wear SCL(etafilcon A, N=40) for various period up to 14 days, or extended-wear SCL(balafilcon A, N=40) for 8 h day or night. The adsorption of protein in daily-wear SCLs was significantly increased in patient-worn time-dependent manner although the lenses were cleaned everyday by using the multi-purpose SCL solution. After 2 weeks wearing, the protein on the SCL influenced the contact angle of the lenses. Furthermore, the transmission of visible light of the lenses decreased up to almost 7%. Extended-wear SCLs worn during the closed eye condition were found to deposit significantly more protein than SCLs worn during the open eye condition. The amounts of protein adsorbed both after daily SCL wear and after overnight SCL wear were not affected the contact angle and the transmission of visible light of the lenses. The protein deposit buildup on SCL related to the corneal temperature and pH of tear fluid.
