The Contamination Level of Lens Cases by Various Wearing and Storage Periods of Soft Contact Lens and the Actual Condition of Lens Cases Care (소프트 콘택트렌즈 착용 기간과 보관 기간에 따른 보관 용기의 오염도 및 보관 용기 관리 실태)

  • Purpose: This study was conducted to investigate the actual condition of caring soft contact lens cases and the change in contamination level of lens cases by wearing and storage period of soft contact lens. Methods: The actual condition of caring soft lens cases was surveyed with sixty-three of contact lens wearers. Soft contact lenses were worn for 1, 3, 7 and 14 days, and the lenses were separately stored in 12 lens cases for 1, 3 and 7 days. The contamination level of lens cases was determined by counting the numbers of microorganisms, gram negative bacteria and fungi. Results: Only 14% of survey answerers cleaned their lens cases with a brush, rinsed and dried them upside down before next usage of lens case when those lenses were kept for a long time. Furthermore, 54% of total answerers just kept soft contact lens in the case without changing the multipurpose solution. Microorganisms, gram negative bacteria and fungi were detected in lens cases even when soft contact lens were kept for 1 day in the lens case after wearing for 1 day and cleaning it with multipurpose solution. The numbers of microorganisms, gram negative bacteria and fungi in lens cases increased with storage periods of soft contact lens, and also contamination level of lens cases was increased in accordance with wearing period of lens. In the lens cases without changing multipurpose solution, the numbers of microorganisms, gram negative bacteria and fungi increased with storage period of lens with statistical significance. Conclusions: This study showed that lens wearers' concerns on caring contact lens case were not satisfactory. As the result, the contamination of lens cases was related to increase of lens wearing and storage periods suggesting that the proper instruction and education for lens wearers are required.

A Comparison of the Movement of Aspheric RGP Lens on Cornea by the Amounts of Keratometric Astigmatisms using Keratometer and Corneal Topography (각막곡률계와 각막지형도를 이용한 각막난시 측정값에 따른 비구면 RGP 렌즈의 각막에서 동적움직임 비교)

  • Purpose: The present study was conducted to analyze any difference in the movement of aspheric RGP lens by the amounts of keratometiric astigmatisms using keratometer and corneal topography. Methods: Corneal curvatures in thirty six eyes of males and females of with-the-rule keratometric astigmatisms in their twenties were measured by a keratometer and worn aspheric RGP lenses. Then, lens rotations, vertical and horizontal movements of lens by blinking were measured to compare with lens movements when aspheric RGP lenses were fitted by total keratometric astigmatisms using corneal topography. Results: The case having higher amount of central keratometric astigmatism was 61.1% of subjects, however, 36.1% of subjects showed higher total keratometric astigmatism indicating that central keratometric astigmatism was not always bigger than total keratometric astigmatism. Since over 0.25 diopter difference between total and central keratometric astigmatisms was shown in 19 eyes (52.8% of subjects), the prescription for lens fitting could be changed. Significant difference in horizontal movement was detected with increase of astigmatism when it compared based on the amount of keratometric astigmatism measured by a keratometer. However, there was no significant difference in lens rotation, horizontal and vertical movements by comparison with the amount of total keratometric astigmatism using a corneal topography. When central keratometric astigmatism measured by keratometer was bigger than total keratometric astigmatism estimated by corneal topography, bigger lens rotation was shown compared with opposite case. Also, the tendency of bigger lens rotation was measured with the increase of keratomatric astigmatism in the case of same prescription having same base curves with same amount of keratometric astigmatism but different curvatures. Conclusions: From the present study, we concluded that lens movements on cornea were not totally different when aspheric RGP lens fitted on with-the-rule astigmatism by keratometer and corneal topography. However, there was some difference in certain lens movements. Therefore, we concluded that further study on the relationship between the prescriptions for lens fitting should be conducted for improving the rate of successful lens fitting by keratometer or for the proper application of corneal topography for lens fitting.

A Comparison of Lens Centrations on Cornea with RGP Lens Fitting by the Measured Values using Keratometer and Corneal Topography (각막곡률계와 각막 지형도 검사에서의 측정값을 이용한 RGP 렌즈 피팅시 각막에서의 중심안정위치 비교)

  • Purpose: The present study was conducted to compare the centration of RGP lens on cornea when lens was fitted based on keratometric astigmatisms measured by keratometer and the lens centration when fitted by corneal topography. Methods: Thirty eight eyes of 19 male and female in their twenties were applied RGP lens with 9.9 mm of diameter by the keratometric astigmatisms classified by the measurement with a keratometer. Then, lens centrations were estimated using high speed camera and compared with the lens centration when fitted by total keratometric astigmatism using corneal topography. The relationship of the steepest location of cornea and lens centration was further compared. Results: With the rule astigmatism, lens centration was not changed even with the difference in central and total keratometric astigmatisms. When the relationship of the steepest part of cornea measured by corneal topography and lens centration was analyzed, the lens centration in vertical direction was exactly correlated with the steepest part of cornea in 52.3% of subjects. In the case of non-correlation, the steepest part of cornea was mostly upper part of cornea, however, lens centration was located on lower part of cornea. The lens centration in horizontal direction was exactly correlated with the steepest region of cornea in 65.6% of subjects. In non-correlated case, the difference in cornea curvatures between the steepest and the flattest parts was smaller than 0.05 mm in 76.9% of subjects. Conclusions: From these results, we conclude that corneal topographic patterns may more contribute the centration of RGP lens on cornea than the difference in central and total keratometric astigmatisms.

The Comparison of Lens Movement by the Fitting States of Soft Contact Lenses in Normal and Dry Eyes (정상안과 건성안에서 피팅상태에 따른 각막에서의 소프트렌즈 움직임 비교)

  • Purpose: The study was performed to compare the differences in lens rotation, lens movement by blinking and lens centration with alignment or steep fitting of soft contact lens in normal and dry eyes. Methods: Total 40 eyes (aged 20~30 years) were classified into the normal (n=20) or dry eye group (n=20) by the diagnosis methods for dry eyes and worn soft contact lens (polymacon material) with alignment fitting or steep fitting. Lens rotation, lens movement by blinking and lens centration were separately measured immediately after lens wearing and after stabilization of tear film and compared by fitting states of soft contact lenses. Results: With steep fitting of soft contact lens in dry eyes, averaged lens rotation immediately after lens wearing was not significantly different from that of the normal eye group with alignment fitting however, lens rotation after stabilization in dry eyes was significantly larger than that in normal eyes. Any significant difference in lens movement by blinking was not shown in normal eyes. However, lens movement by blinking in dry eyes was increased with steep fitting. The range of lens centration on cornea in normal eyes with alignment fitting was more vertically distributed. On the other hand, the range of lens centration on cornea in dry eyes with alignment fitting was more horizontally distributed. Lens centration was shown to be changed by stabilization of tear film. That is, lens centrations were somewhat vertically widespread immediately after lens wearing and restrictively distributed in horizontal direction, respectively, with steep fitting in dry eyes. Conclusions: These results suggested that lens movements and centration in dry eyes were different from those of normal eyes. Especially, those differences between normal and dry eyes were much bigger with steep fitting of soft contact lenses. Thus, those differences should be considered for the comfortable and safe fitting of soft contact lens in dry eyes.

Validating a New Approach to Quantify Posterior Corneal Curvature in Vivo (각막 후면 지형 측정을 위한 새로운 방법의 신뢰도 분석 및 평가)

  • Purpose: Validating a new research method to determine posterior corneal curvature and asphericity(Q) in vivo, based on measurements of anterior corneal topography and corneal thickness. Methods: Anterior corneal topographic data, derived from the Medmont E300 corneal topographer, and total corneal thickness data measured along the horizontal corneal meridian using the Holden-Payor optical pachometer, were used to calculate the anterior and posterior corneal apical radii of curvature and Q. To calculate accurate total corneal thickness the local radius of anterior corneal curvature, and an exact solution for the relationship between real and apparent thickness were taken into consideration. This method differs from previous approach. An elliptical curve for anterior and posterior cornea were calculated by using best fit algorism of the anterior corneal topographic data and derived coordinates of the posterior cornea respectively. For validation of the calculations of the posterior corneal topography, ten polymethyl methacrylate (PMMA) lenses and right eyes of five adult subjects were examined. Results: The mean absolute accuracy (${\pm}$standard deviation(SD)) of calculated posterior apical radius and Q of ten PMMA lenses was $0.053{\pm}0.044mm$ (95% confidence interval (CI) -0.033 to 0.139), and $0.10{\pm}0.10$ (95% CI -0.10 to 0.31) respectively. The mean absolute repeatability coefficient (${\pm}SD$) of the calculated posterior apical radius and Q of five human eyes was $0.07{\pm}0.06mm$ (95% CI -0.05 to 0.19) and $0.09{\pm}0.07$ (95% CI -0.05 to 0.23), respectively. Conclusions: The result shows that acceptable accuracy in calculations of posterior apical radius and Q was achieved. This new method shows promise for application to the living human cornea.

Impact of Indoor Air Quality on the Eye Conditions of Occupants in Newly-built University Buildings (새 학교 건물 내 실내공기질이 재실자의 안구에 미치는 영향)

  • Purpose: This study was to find out the influence on eyes of indoor air quality in college students taking course, such as Sick-building syndrome symptoms and effects on the eye in new-built university buildings. Methods: We selected a new building in a university located in Metro Seoul and college students in a department for the study. The number of total participants was 33, to whom questionnaire surveys were conducted in advance to check individual traits (gender, age, whether to smoke, whether to wear contact lenses, or whether to drink). The first questionnaire surveys and checking of ocular symptoms to first indoor hazardous materials were conducted in October and two months later the second surveys and checking were carried out in December. The indoor air quality was measured when conducting the first questionnaire surveys and the second questionnaire surveys; especially measurements of gaseous materials such as aldehydes and VOCs in the indoor air were conducted. Results: Indoor air quality of the new building was as follows: formaldehyde level was 22.90 ${\mu}g/m^3$ in the first measurement and 16.79 ${\mu}g/m^3$ in the second measurement. In addition, most materials showed higher value in the first measurement. The level of TVOC was statistically significant (p<0.05) decreased on 448.54 ${\mu}g/m^3$ in the first and 62.55 ${\mu}g/m^3$ in the second. In clinical assessments to check ocular symptoms caused by eye irritations, dry eye syndrome was found in the first and second exposures. When comparing the first and second assessments, dry eyes deteriorated in the morning of the second attempt compared to the first one. Conclusions: In the survey of ocular symptoms and the measurement of indoor air quality, the level of formaldehyde was measured higher in the second attempt than the first; thus, it was confirmed the influence of indoor air quality in a new building upon ocular symptoms of occupants.

Polymerization of HEMA by Electron beam Irradiation and Fabrication of Soft contact lens (전자빔조사에 의한 HEMA의 중합과 소프트콘택트렌즈 제조)

  • Purpose: Polymerization of HEMA(2-hydroxyethyl methacrylate) which can be used in the soft contact lens has been performed by using electron beam(EB) irradiation, and examined the best condition for the polymerization. Comparing the physical properties of the contact lenses to the one fabricated by thermal polymerization method, we check the use possibility of the EB irradiation to the fabrication of the soft contact lens. Methods: We investigated the degree of polymerization of the HEMA according to the composition of the monomer, the additive ratio and the dose of electron beam (0~120 kGy). The degree of polymerization was measured depending on the EB dose to research the best synthetic condition under the EB irradiation. The physical properties of the contact lens such as water content(%), oxygen transmissibility(Dk/t) and optical transmittance were analysed by using the FT-IR results with comparing the two different polymerization method (thermal and electron beam polymerization) with same additive ratio. Results: When the dose of electron beam was above 100 kGy, the degree of polymerization of HEMA was above 99% with regardless using cross-linker and initiator. The water content of the lens fabricated by EB method showed 10% higher than the one by the thermal method which was 40%. The lens fabricated by EB method also showed higher oxygen transmissibility(Dk/t) as same with the water content, and showed twice higher value in the lens fabricated by pure HEMA. According to the FT-IR results, hydrophilic property of the lens fabricated by EB method was increased due to increasing the intermolecular hydrogen bonding. It showed above 90% optical transmittance in the visible range of wavelength on the contact lenses fabricated by the both of two different polymerization method. Conclusions: The polymerization of HEMA without cross-linker and initiator was successful above 100 kGy of EB irradiation. Moreover the lens fabricated from the polymer synthesized by pure HEMA with 100 kGy of EB showed the highest water content and oxygen transmissibility. Therefore EB irradiation is another possible method to synthesize the polymer which can be used for the soft contact lens.

Associations of Metabolic Syndrome with Glaucoma in Korean - Based on the Korean National Health and Nutrition Examination Survey 2005, 2007-9, 2010 (한국인의 대사증후군과 녹내장 간의 상관관계 -2005, 2007-9, 2010국민건강영양조사 이용)

  • Purpose: This study was conducted to assess the association of metabolic syndrome and its components with glaucoma. Methods: We investigated the associations of metabolic syndrome and its components (abdominal obesity, impaired fasting glucose, high blood pressure, and dyslipidemia) with glaucoma using data from 19,162 adults aged 40 or above among the Korean National Health and Nutrition Examination Survey III (2005), IV (2007-9), V (2010). The logistic regression analysis was used for assessing those associations after adjusting demographic, lifestyle, and social economic status and for assessing the association of metabolic medication intakes with glaucoma risks. Results: After adjusting for age and sex, the risk of glaucoma was significantly higher in the subjects with impaired fasting glucose(Odds Ratio (95% confidence interval): 1.78 (1.25, 2.53)) and metabolic syndrome (1.45 (1.01, 2.08)) than subjects without those. These associations increased when additional adjusting for smoking, alcohol use, regular physical activity, income, education status(impaired fasting glucose: 1.89 (1.29, 2.77), metabolic syndrome: 1.52 (1.03, 2.25)). Glaucoma prevalence was borderline significantly increased according to the number of metabolic abnormalities(age and sex adjusted p for trend = 0.055). Use of antihypertensive medication was significantly associated with the risk of glaucoma. Conclusions: Metabolic syndrome and impaired fasting glucose were significantly associated with the increased glaucoma risk. Use of antihypertensive medication was also significantly associated with the increased glaucoma risk.

Effect of Vision Training using Fresnel Prism Lens on Fusional Vergence and Accommodation (프레넬 프리즘렌즈를 이용한 시기능 훈련이 조절과 융합버전스에 미치는 영향)

  • Purpose: Of the various methods of vision training, the essay aims to explore the effective ways of using the Fresnel prism lens in order to expand the positive fusional vergence for the patient having specific condition of convergence insufficiency or basic exophoria. Methods: 15 students of city of Daejeon university without an eye disease (average age $22.73{\pm}1.68$) were selected and underwent the subjective refraction test and binocular vision test, and recording their test results before vision training and replacing an identical frame with a lens of same quality after the full calibration, the lens was then adhered with the Fresnel prism lens and continued to train for thirty minutes daily during two weeks. Afterwards, the binocular vision test was reattempted. The observation of the change in the results of the binocular vision test in use of the fresnel prism lens in the vision training test was researched. Results: After training, the positive fusional vergence had increased to a number of $22.27{\pm}2.26$ $\Delta$, to 7.80 $\Delta$, at near, the fused cross cylinder test increased to an average of $0.55{\pm}0.09$ D, 0.40 D after training, showing a normal result. The value of negative relative accommodation after training had an average of $2.22{\pm}0.08$D, showing that 0.42 D had increased. The value of near point of convergence after training had an average of $6.13{\pm}0.53$ cm, showing that 2.80 cm had decreased. To patients who had convergence insufficiency or basic exophoria, the value of the near vision test that used the Fresnel prism lens which was able to expand BO positive fusional vergence had increased without phoria. Conclusions: The changes were tested and the effectiveness of the Fresnel prism lens, due to the nature of the lens itself, helped with both cosmetic effects and cost. It also allows good optical correction effects, in addition to these clinical effects indicated before. Therefore, it may be determined that the Fresnel prism lens binocular vision therapy for patients is more popular and highly recommended.

Change of Phoria and Subjective Symptoms after Watching 2D and 3D Image (2D와 3D 영상 시청 후 나타난 사위도 및 자각증상의 변화)

  • Purpose: The changes of phoria and subjective asthenopia before and after viewing were compared based on 2D image and two ways of 3D images, and presented for references of 3D image watching and production. Methods: Change in phoria was measured before and after watching 2D image, 3D-FPR and 3D-SG images for 30 minutes with a target of 41 university students at 20-30 years old (male 26, female 15). Paired t-test and Pearson correlation between changed phoria and subjective symptoms which were measured using questionnaires were evaluated by before and after watching each images. Results: Right after watching 2D image, exophoria was increased by 0.5 $\Delta$, in distance and near, but it was not a significant level. Right after watching 3D image, exophoria was increased by 1.0~1.5 $\Delta$, and 1.5~2.0 $\Delta$, in distance and near, respectively when compared with before watching. In the significant level, exophoria tended to increase. Changes in near was increased more by 0.5 $\Delta$, compared with those in distance. Changes based on way of 3D-FPR and 3D-SG image were less than 0.5 $\Delta$, and there was almost no difference. In terms of visual subjective symptoms, eye strain was increased in 3D image compared with that in 2D image. In addition, there was no difference depending on way of image. In terms of Pearson correlation between phoria change and eye strain, as exophoria was increased, eye strain was increased. Conclusions: Watching 3D image increased eye strain compared with watching 2D image, and accordingly exophoria tended to increase.
