Clinical Significance of Tumor Infiltration at the Resection Margin in Gastric Cancer Surgery (위암 수술 시 절제연 암침윤의 임상적 의미)

  • Purpose: Despite knowledge of the adverse effects of resection-line disease, surgeons continue to perform inadequate resections. This demonstrates the need for a more aggressive approach to assessment of resection margins at operation. Materials and Methods: Seven hundred fifteen gastric cancer patients who were operated on at our hospital from 1992 to 1998 were included in this analysis. Various clinicopathological factors, including resection-line involvement, were ascertained from the surgical and histopathological records. Results: Of the 715 evaluable patients, 27 patients ($3.8\%$) had involvement of one or both resection lines; in 10 patients the proximal resection line only, in 16 the distal resection line only, and 1 both resection lines were involved. Presence of resection-line involvement was significantly associated with T3 and T4 stage, N (+) stage, M (+) stage, type of operation (total gastrectomy), tumor location (entire stomach), size$\geq$11 cm), and gross type of tumor (Borrmann 4 type). When performing a distal subtotal gastrectomy, no involvement was found when the cranial and caudal distances between the lesion and the line of transection was equal to or greater than 2 cm and 3 cm, respectively, for early cancer and 7 cm and 3 cm, respectively, for advanced cancer. When performing a total gastrectomy for upper 1/3 or middle 1/3 gastric cancer, no involvement was found when the cranial distances between the lesion and the line of transection were equal to or greater than 3 cm and 4 cm, respectively, without distinction of the presence of serosal invasion. Conclusions: The difference in survival between positive and negative margin patients is limited to the group of patients with curative surgery. An important principle of treatment is that the entire tumor must be removed with a 3 cm distal margin and a 2- to 7 cm margin depending on the location and the depth of wall invasion of the tumor, to provide histologically negative margins.

  • This study analyzed the variations in the performance and operation of a 200 kW class micro gas turbine according to performance degradation of compressor and turbine. An in-house code, developed by the present authors and presented in the first part of these series of papers, were used for the analysis. The degradation of compressor and turbine were simulated by modifications in the their performance maps: mass flow rate, pressure ratio and efficiency were decreased from the reference values. Firstly, the variations in the operating conditions (air flow rate, pressure ratio) were predicted for the full load condition. Then, the same analysis were performed for a wide partial load operating range. The change in engine's performance (power output and efficiency) due to the component degradation was predicted. In addition, the change in the compressor surge margin, which is an important indicator for safe engine operation, was evaluated.

Impact of Using Intra-Operative Ultrasound Guided Breast-Conserving Surgery on Positive Margin and Re-Excision Rates in Breast Cancer Cases with Current SSO/ASTRO Guidelines

  • Purpose: To review the impact of using intra-operative ultrasound guided breast conserving surgery with frozen sections on final pathological margin outcome with the current guidelines set forth by the Society of Surgical Oncology (SSO) and the American Society of Surgical Oncology (ASTRO). Materials and Methods: A retrospective review including all cases of intra-operative ultrasound guided breast conserving surgery was performed at the National Cancer Institute Thailand between 2013 and 2016. Patient demographics, tumor variables, intraoperative frozen section and final pathological margin outcomes were collected. Factors for positive or close margins were analyzed. Results: A total of 86 patients aged between 27 and 75 years with intra-operative ultrasound guided breast conserving surgery were included. Three cases (3.5%) of positive margin were detected by intra-operative frozen section and 4 cases (4.7%) by final pathology reports. There were 18 cases (20.9%) with a close margin (<1 mm). Factors affecting this result comprised multi-foci, presence of invasive ductal carcinoma (IDC) combined with ductal carcinoma in situ (DCIS) and invasive lobular carcinoma (ILC). Conclusions: With the current SSO/ASTRO for adequate margin guidelines, using intra-operative ultrasound to locate the boundary for resection with breast conserving surgery provided a high success rate in obtaining final pathology free margin outcomes and minimizing re-operation risks especially when combined with intra-operative frozen section assessment. The chance of finding positive or close margins appears higher in cases of IDC combined with DCIS, ILC and with multi-foci cancers.

  • On-line dynamic security assessment is becoming more and more important for the stable operation of power systems as load level increases. The necessity is getting apparent under Electricity Market environments, as operation of power system is exposed to more various operating conditions. For on-line dynamic security assessment, fast transient stability analysis tool is required for contingency selection. The TEF(Transient Energy Function) method is a good candidate for this purpose. The clustering of critical generators is crucial for the precise and fast calculation of energy margin. In this paper, we propose a new method for fast decision of mode of instability by using stability indices. Case study shows very promising results.

  • This study analyzed the influence of firing biogas on the performance and operation of a gas turbine combined heat and power (CHP) system. A reference CHP system designed with natural gas fuel was set up and off-design simulation was made to investigate the impact of firing biogas in the system. Changes in critical operating parameters such as compressor surge margin and turbine blade temperature caused by firing biogas were examined, and a couple of operating schemes to mitigate their changes were simulated. Part load operation of the biogas-fired system was compared with that of natural-gas fired system, and it was found that as long as the two system produce the same electric power output, they exhibit nearly the same heat recovery.

  • The emergence of competitive power market makes dispatch algorithm with transient stability constraints increasingly important for the transparent power system operation. Heuristic and off-line evaluation for the operation point can produce a discrimination among market players in the deregulated power system. In this paper, a dispatch algorithm with transient stability constraints is proposed. Energy margin under the TEF(Transient Energy Function) structure is adopted as a measure for the stability index. Implementation issues and simulation results are discussed in the context of a 10-bus system

  • A Study of Improvement in Reactor Thermal Power Measurement Method using Kalman Filter. The objectives of the safety analysis of nuclear power plants are to maintain the surface temperature of fuel and fuel cladding within limit value in case of Loss of Coolant accident (LOCA) so that it ensures the safety and reliability of nuclear power plants. The new technique evaluating the reactor power and improvement of existing plant system increase the safety margin of nuclear power plant operation, and accordingly, economic effect will be anticipated. Hereby, 1 would like to introduce reactor power measurement method using Kalman filter that enables to calculate the reactor power more precisely combining the parameters, for example, turbine output as the 1 st stage pressure of high pressure turbine, and reactor power using energy equilibrium relation. It is expected that the new technique will enhance the accuracy of measurement of reactor power and maintain the reliability of nuclear power operation by increasing operational safety margin, and gain the economic benefit

A Study on the Enhancement of Westinghouse DNB Protection Logic

  • Since the conventional Westinghouse DNB (Departure from Nucleate Boiling) protection logic is implemented on analog circuits, the logic must be very simple. However, if the DNB protection logic is implemented in a digital processor, a little bit of complexity can be allowed to increase the thermal (or operation) margin. The Westinghouse OTΔT DNB protection logic heavily restricts the operation region by applying the same logic for a full range of pressure in order to maintain its simplicity. In this work, the different DNB protection logic is used for several regions of pressure. The proposed method is applied to Yonggwang 1&2 nuclear power plants and it is calculated that the improved OTΔT can have 5.07% percent more thermal margin than the conventional OTΔT trip logic.

  • In this study, I analyzed the effects on stabilizing the draft system, boiler efficiency when changing excess air ratio under 870 MW load limit operation condition in a large capacity coal fired power plant and decided optimum excess air ratio. It is positively necessary to choose adequate excess air ratio for stabilizing draft system because air pre-heater pressure drop and induced draft fan first stall margin are changing when adjusting excess air ratio. This study therefore, measured air pre-heater pressure drop, induced draft fan first stall margin, boiler efficiency, loss and has chosen adequate excess air ratio adjusting excess air ratio from 1.153 to 1.127. So it is recommended that the operation point needs to be changed to 1.127 that is not only to decrease air pre-heater pressure drop and to stabilize draft system and to secure stall margin but also to maintain boiler efficiency to equivalent level.

초 저 소비전력 및 저 전압 동작용 FULL CMOS SRAM CELL에 관한 연구

  • 0.4mm Resign Rule의 Super Low Power Dissipation, Low Voltage. Operation-5- Full CMOS SRAM Cell을 개발하였다. Retrograde Well과 PSL(Poly Spacer LOCOS) Isolation 공정을 사용하여 1.76mm의 n+/p+ Isolation을 구현하였으며 Ti/TiN Local Interconnection을 사용하여 Polycide수준의 Rs와 작은 Contact저항을 확보하였다. p-well내의 Boron이 Field oxide에 침적되어 n+/n-well Isolation이 취약해짐을 Simulation을 통해 확인할 수 있었으며, 기생 Lateral NPN Bipolar Transistor의 Latch Up 특성이 취약해 지는 n+/n-wellslze는 0.57mm이고, 기생 Vertical PNP Bipolar Transistor는 p+/p-well size 0.52mm까지 안정적인 Current Gain을 유지함을 알 수 있었다. Ti/TiN Local Interconnection의 Rs를 Polycide 수준으로 낮추는 것은 TiN deco시 Power를 증가시키고 Pressure를 감소시킴으로써 실현할 수 있었다. Static Noise Margin분석을 통해 Vcc 0.6V에서도 Cell의 동작 Margin이 있음을 확인할 수 있었으며, Load Device의 큰 전류로 Soft Error를 개선할수 있었다. 본 공정으로 제조한 1M Full CMOS SRAM에서 Low Vcc margin 1.0V, Stand-by current 1mA이하(Vcc=3.7V, 85℃기준) 를 얻을 수 있었다.
