포스트 코로나 시대 수술 로봇의 역할 및 발전 방향에 관한 전망 (A Perspective on Surgical Robotics and Its Future Directions for the Post-COVID-19 Era)

  • The COVID-19 pandemic has been reshaping the world by accelerating non-contact services and technologies in various domains. Hospitals as a healthcare system lie at the center of the dramatic change because of their fundamental roles: medical diagnosis and treatments. Leading experts in health, science, and technologies have predicted that robotics and artificial intelligence (AI) can drive such a hospital transformation. Accordingly, several government-led projects have been developed and started toward smarter hospitals, where robots and AI replace or support healthcare personnel, particularly in the diagnosis and non-surgical treatment procedures. This article inspects the remaining element of healthcare services, i.e., surgical treatment, focusing on evaluating whether or not currently available laparoscopic surgical robotic systems are sufficiently preparing for the era of post-COVID-19 when contactless is the new normal. Challenges and future directions towards an effective, fully non-contact surgery are identified and summarized, including remote surgery assistance, domain-expansion of robotic surgery, and seamless integration with smart operating rooms, followed by emphasis on robot tranining for surgical staff.

Three-Dimensional Video Microscopy: Potential for Improved Ergonomics without Increased Operative Time?

  • Three-dimensional (3D) video exoscopes are high-magnification stereo cameras that project onto monitors mounted in the operating room, viewable from different angles. Outside of plastic surgery, exoscopes have been shown to successfully improve the ergonomics of microsurgery, though sometimes with prolonged operating times. We compare a single surgeon's early experience performing free flap procedures from 2020 to 2021 using either a binocular microscope or a 3D video exoscope. Ten procedures were performed with the standard operating microscope and 8 procedures with the 3D exoscope. The microsurgeon, having minimal prior experience using an exoscope, reported less neck discomfort following the free flap procedures performed with the exoscope compared with the binocular surgical microscope. Total average operating time was comparable between the standard surgical microscope and the 3D exoscope (13.7 vs. 13.4 hours, p = 0.34). Our early experience using a 3D exoscope in place of a standard optical microscope demonstrated that the exoscope shows promise, offering an ergonomic alternative during microvascular reconstruction without increasing overall operating times. Future studies will compare free flap ischemia time between cases performed using the exoscope and the conventional binocular microscope. 수술실 간호사의 직업적 손상 경험 현황과 안전 분위기의 상관관계 (The Correlation between Occupational Injuries and Safety Climate among Operating Room Nurses)

  • Purpose: This study aimed to examine the occupational injuries experienced by operating room nurses and investigate the relationship between these injuries and safety climate. Methods: The study involved 198 operating room nurses working in general and tertiary hospitals with more than 300 beds. Results: Among the four sub-factors of occupational injuries experienced by operating room nurses, risks due to ergonomic hazards were the most prevalent and those due to physical hazards were the least. The scores related to occupational injury showed a significantly negative correlation with the safety climate. Conclusions: The findings indicate that higher operating room nurses' awareness of the safety climate is associated with lower rates of occupational injury. Therefore, organizational efforts and interventions are imperative to improve the safety climate within hospitals. Additionally, in order to investigate the safety of operating room nurses more in-depth, it is necessary to expand not only the sample size but also the geographic scope.

  • Objective: The aim of this study is to investigate the attributing factors influencing team performance. Background: Technically, it is necessary that operators adapt themselves to computerized and advanced techniques to operate the main control rooms safely in nuclear power plant in Korea. The more main control rooms are digitalized, the more important for operators to have high team performance it is. Method: This paper analyzes team process through literatures review and elicits team performance shaping factor. Especially, the objective of this research is to elicit communication using common team performance shaping factors. Results: This study has found communication through team performance shaping factors in Main Control Room of the SMART. Conclusion: This paper can offer a starting point for team communication, which can use team performance shaping factor framework that are emerging in these new nuclear power plant. Application: As a result, I expect that the evaluation communication for MCR operator's team performance will lead the operating techniques in nuclear power industry internationally.

  • Inside the factory buildings of aprecision industry (which produces semiconductors or TFT-LCD), thousands of vibration-sensitive precision equipments operate in the clean rooms. Various utility equipments that are installed around them for proper operation cause various sources of noise and of vibration to exist inside the clean rooms and thus deteriorate the noise and the vibration environment within. In this study, we check if the existing transient vibration phenomenon is caused by the vibrations that arise from operating the utility installed near these equipments; compare and evaluate the vibration criteria of precision equipments regarding the transient vibration phenomenon; and check the efficiency of these criteria.

  • We use floor radiant heating system in the house of commons in winter Floor radiant heating system, which transfer heat by radiation, is one of the energy efficient and comfortable systems that. Floor radiant heating system is configured to be controlled by the room for energy-saving. Proper flow rate to a comfortable heating in the room is important. However, Using a constant speed circulation pump in separate rooms, heating system may cause an imbalance because of the difference of length of coil when operating in the rooms. In this study, our Research team examined heating imbalance due to the variation length through the coil length changes and flow control of the circulation pump.

간호사의 방사선 방어 지식, 태도 및 행위에 관한 연구 : 수술실과 중환자실 중심으로 (A Study on Knowledge, Attitude and Behavior for Radiation Protection of Nurses : Focus on the Operating Rooms and Intensive Care Units)

  • The purpose of this study was to investigate the knowledge, attitudes and behaviors of radiation protection in operating room and intensive care unit nurses. The Participants for this study consisted of 240 nurses working in operating rooms and intensive care units of 7 general hospitals located in Busan, South Korea. The data were collected from Feb. 15 to Feb. 29, 2016. The study results were as follows. First, 72.9% of the nurses had never been educated for radiation safety. Second, the mean score of knowledge for radiation protection was 9.21. The knowledge score of radiation safety educated nurses is higher than uneducated of nurses(t=-5.67, p<.001). Third, The mean score of attitude towards radiation protection was 41.80. The attitude score of radiation safety educated nurses is lower than uneducated of nurses(t=0.02, p<.030). Fourth, the mean score of behavior for radiation protection was 32.57. The behavior score of radiation safety educated nurses is higher than uneducated of nurses(t=0.35, p=.001). Consequently, it was found that nurses had lack of the radiation safety education. Overall knowledge and behavior towards radiation protection was low. Therefore, it is necessary to raise awareness of radiation protection and to educate, manage, and improve processes to increasing compliance with protective measures against radiation.

수술실 의료진의 팀워크와 환자안전문화에 대한 인식이 수술환자안전 프로토콜 수행에 미치는 영향 (The Effect of Operating Room Nursing and Medical Staff Teamwork and Perception of Patient Safety Culture on the Performance of Surgical Patient Safety Protocol)

  • Purpose: The purpose of this study lies in investigating nursing and medical staff perceptions on the importance of surgical patient safety protocol, teamwork, and patient safety culture, and how their grasp of the factors affects the degree of their performance of the protocol. Methods: A survey was conducted on 249 nurses and medical staff participating in the operating rooms of one higher general hospital in Seoul, using a 5-point scale self-reported questionnaire. Logistic regression analyses were used. Results: Operating room nurses yielded the highest scores on both the importance of the patient safety protocol and its performance. In patient safety culture, the operating medical staff yielded significantly higher scores than those of operating room nurses. Perception of the importance of the patient safety protocol and teamwork had a significant effect on the nurses' complete performance of the protocol. Conclusion: It is important to create a safety culture, where all the staff can actively and freely communicate with one another through team-based training programs. By enhancing teamwork and patient safety culture, it will be possible to establish the surgical patient safety protocol and to improve the performance of the protocol by health professionals.

간호학생을 위한 수술간호 e-Learning 프로그램의 개발 및 효과 (Development and Effects of an e-Learning Program in Operating Room Nursing for Nursing Students)

Methods: Based on content and need analysis, 9 learning modules were developed for nursing care in operating rooms and with operating equipment. To verify the effects of the program, a quasi- experimental pretest-posttest control group design was employed. The participants in this study were 74 third-year nursing students (34 in the experimental and 40 in the control group) from a junior college in G-city, Korea, who were engaged in a one week clinical practicum in an operating unit. Frequencies, $X^2$-test and t-test with the SPSS program 17.0 were used to analyze the data. Results: Knowledge was significantly higher in the experimental group compared to the control group (p=.018). However, there was no significant difference between the two groups in self-directed learning. The experimental group had significantly higher motivation toward learning, which was examined posttest only (p=.027). Conclusion: These results indicate that the implementation of an e-Learning program needs to be continued as an effective educational tool, but more research on the best way to implement e-Learning in students' practicum is needed.

호텔의 건축적 속성이 운영 수익에 주는 영향에 관한 연구 - 서울지역 관광호텔을 중심으로 - (A Study on the Influence of Hotel's Architectural Attributes on Operating Revenue - Focused on Tourist Hotel in Seoul -)

  • The purpose of this study is to analyze the effect of architectural attributes on operating revenue by concentrating on the facility side and to investigate how much individual architectural factors can affect operating revenue. The time range of this study is limited to 2015, and the spatial range is limited to a tourist hotel in Seoul that can be easily surveyed and verified. The analysis method is to confirm the significance and influence of each attribute through regression analysis. According to the results, The room part such as number of rooms or area showed a relatively low correlation with "revenue per available room (Rev.PAR)" than "average daily rate (ADR)", but showed a high correlation with non - architectural attributes such as auxiliary facilities and chain. This shows that factors that influence the operating profit of hotel are not the part of the room but the other factors such as the auxiliary facilities work more. In addition, the presence of spa and the number of auxiliary facilities in the hotel have a positive effect on the operating profit of the hotel due to the non - architectural attributes. This is expected to provide a new direction for future research in studying hotel facilities.
