수술실간호사의 직무수행과 피로에 관한 연구 (A Study On the Operating Room Nurses' Performances of Duties and Their Fatigue)

  • The aim of this study is to present the basic datum for the promotion of effectiveness and improvement of nursery tasks, by grasping the situation of operating room nurses tasks and the degree of their fatigue. For these researches, 70 nurses out of the operating rooms of 3 university-affiliated hospitals-two in Seoul and one in Pusan, were chosen at random by a non-probability sampling method. These researches were done from April 14 to April 26 in 1997 by questionairing method. The questionaire was composed of 30 items, which asks the examinees of their physical, mental, and neurosensory symptoms, with 10 items respectively. The reliability of the research instrument was turned out very high with Cronbach's ${\alpha}=.9376$. The datum were electronically processed using Statistics Program for Social Sciences(SPSS). The analysis of datum in this study has a general character, in which the demosociological character and the special ex-officio character was calculated by frequency and percentage. The situation of tasks in operating rooms was calculated by frequency and percentage. The fatigue of operating room nurses was calculated by average and standard deviation. To compare the fatigue with regard to the character of operating room nurse's tasks, t-test and F-test(ANOVA) were used after the character of variations, and the variations at the level of P<.05 which might have some meaning was verified after the fact with Duncan's Multiple Range(DMR). The results of this research are as follows : 1) The nurses working in operating rooms show their fatigue in three fields-in Group I physical symptoms 3.28, in Group III neurosensory symptoms 2.85, and in Group II mental symptoms 2.73, which shows I Dominant type (general type). 2) They complain, in Group I they are "feeling the heavy legs," 3.28 and in Group II they "occasionally forget soon what to do," 3.09. and in Group III, they "feel lumbago," 3.47, which is the highest rate of the three. The highest rate results from the character of their tasks, in which they have to move rapidly the heavy appliances and do their jobs standing many hours, especially wearing heavy radiation protector. 3) As to transportation, subway using group feel the greatest fatigue, 3.18(F=4.315, P=.008). 4) As to department, nurses in the orthopedic's surgery part feel the greatest fatigue, 3.26(F=2.040, P=.050). 5) As to the change of physical symptoms, the group answering that they found physical abnormality after working in operating rooms show the greatest fatigue, 3.12(t=-3.13, P=.003). 6) At to the general circumstances, the group answering that they receive insufficient consideration on their physical abnormality in their department, show the greatest fatigue, 3.10(F=3.200, P=0.47). 7) As to the relation with superior officers, the group answering that their superior officer has an impetuous temperament, show great fatigue, 3.11(F=4.855, P=.011). 8) As to the time of feeling fatigue, the fatigue reaches the highest point 1~2 hours after operations, 3.04(F=2.703, P=.046). 9) When they feel fatigue after scrub nurse duties, they feel the greatest fatigue 2 hours after the duties, 3.09( F=2.841, P=.038). 10) As to the operation instruments, when they use complex instruments borrowing from the outside in addition to the basic instruments, their fatigue becomes the greatest, 3.09(F=7.831, P=.000). 11) As to the kind of operation, when they participate in orthopedic's surgery operations, they feel the greatest fatigue, 3.18(F=4.362, P=.000). With the above results, it is proved that the degree of operating room nurses' fatigue is considerably high. So it may be concluded that the measure for lessening the fatigue should be find immediately, not on the level of personal matters but on the level of hospital nursing administration.

'계획에 없던 재수술' 의 분석 (An analysis of unplanned reoperation)

  • Background: Clinical indicators are objective measures of process or outcome of patient care in quantitative terms. This study aims to review the medical records of patients who 'return to operating room during the same admission', which is one of the critical clinical outcomes, and describe the result by unplanned reoperation rate. Methods: Computerized patient registry was used for selecting subject conditions. For medical records retrieved, two nurse evaluators identified the presence of explicit reoperation planning in medical records. Results: Overall reoperation rate was 2.8% and unplanned reoperation rate 1.3%. The main category of reoperation cause was the postoperative bleeding. Duration of stay from previous operation to reoperation of the unplanned group, 12.7 days, was shorter than that of the planned(p< .05). The differences did not reach statistical significance in age, sex and length of stay. Conclusion: Results suggested that unplanned reoperation rate was lower than 'threshold' level other institutions had established. However, this result could become comparable only after management of medical records would be improved and risk adjusted.

수술실 간호사의 방사선 방어행위의 영향 요인 (Factors Affecting Radiation Protection Behaviors among Operating Room Nurses)

  • Purpose: This study was designed to identify knowledge, attitude, environment, and self-efficacy among perioperative nurses in terms of radiation protection and to describe those factors affecting radiation protection behaviors. Methods: The sample was comprised of 128 perioperative nurses who agreed to participate in this descriptive study. Data were collected from a self-administered questionnaire and were analyzed using the t-test, analysis of variance, Mann-whitney U test, Kruskal-Wallis test, Pearson's correlation, and multiple regression analyses. Results: Radiation protection behaviors were significantly different by participant's gender, length of career as a perioperative nurse, educational level and prior experience with radiation protection education. Radiation protection behaviors were significantly correlated with radiation protection environment and self-efficacy in terms of radiation protection behaviors. In the multiple linear regression models, radiation protection environment and self-efficacy on radiation protection behaviors were statistically significant predictors of radiation protection behaviors, which accounted for 50.3% of variance in dependent variable. Conclusion: This study concluded that radiation protective environment in operating room is important to promote radiation protective behaviors and radiation safety management program to enhance self-efficacy is highly recommended.

수술실 간호사의 직무 만족도 측정 도구 개발 연구 (Development and Validation of an Instrument to Measure the Job Satisfaction of Perioperative Nurses)

  • Purpose: The purpose of this study was to develop a valid and reliable instrument to explore and measure job satisfaction as perceived by perioperative nurses, specifically from the perspective of contemporary perioperative nursing in Korea. Method: Items (69) were developed from the responses to two open-ended questions by 168 perioperative nurses in one teaching hospital; (a) reasons that make work enjoyable, (b) reasons that make work unpleasant. Finally 55 items were selected, excluding items with lower correlation with the total scale. The instrument was completed by 885 perioperative nurses from 89 different hospitals nationwide who were attending the annual conference of the Korean Association Operating Room Nurses (KAORN), yielding a response rate of 95.6%. Results: Ten factors were extracted by factor analysis, which explained 60.0% of the total variance; Reliability and internal consistency were established (Cronbach's alpha=.95). A smaller sample of 39 perioperative nurses completed the instrument on two occasions, with a 2-week interval between. Mean scores were computed and two sets of scores were analyzed by Weighted Kappa (W-Kappa=0.680, p=.029). Conclusion: This instrument is a concise, highly practical and flexible tool, which can be used for the perioperative nurse in a wide range of hospital settings.

  • A surgical patient is identified by an operating surgeon or a nurse manually. There always exists a chance that he/she misses the necessary identification process. It can bring about serious and critical demage to the surgical patient. Thus, it is necessary to prevent this kind of mistake automatically. This study proposes a business model identifying surgical patients using RFID system. The paper presents a design of the system identifying patients at the entrance of a operating room. It gives also some user interfaces of the system.

수술실 간호사의 방사선 노출 경험: 현상학적 연구 (Paper Operration room nurses Experience in Using Radiation Suegery: Phenomenological Research)

  • 본 연구의 목적은 방사선 노출 경험을 참여자의 관점에서 확인함으로써 수술실 간호사의 방사선 노출 경험에 대한 구성요소를 확인하고 의미구조를 밝히는 데 있다. 연구의 참여자는 일 광역시 종합병원 수술실에서 근무하는 간호사로 수술실 경력이 6개월 이상인 자 9명을 대상으로 하였다. 심층면담으로 자료를 수집하였고, Giorgi(2004)의 현상학적 분석 방법으로 분석하였다. 연구 결과 수술실 간호사의 방사선 노출 경험은 "반복되는 방사선 노출로 질환 발생을 걱정함", "육체적, 정신적으로 소진됨", "방사선 방어를 통해 자기보호를 하고자 노력함", "내적 혼란과 갈등을 느낌", "근무환경 개선을 요구함", "현실을 받아들이고 적응함"의 6개 구성요소로 도출 되었다. 결론으로 이들의 경험은 방사선 노출 수술에 참여하면서 질환 발생을 걱정하고 방사선 피폭과 관련된 신체적, 정신적 소진을 경험하고 갈등을 느끼면서 이직을 고려하기도 하지만 자기보호를 하고자 노력하고 현실을 받아들이고 적응하는 과정을 보였다. 본 연구결과의 바탕으로 방사선 안전 환경조성을 위해서는 조직적 차원에서 체계적으로 구체적인 해결 방법을 모색하는 것이 바람직하다고 볼 수 있다.

수술환자의 욕창예측변수에 관한 연구 (Predicting Risk Factors for Pressure Sores in Patients Undergoing Operations ; A Prospective Study)

  • The purpose of this study was to identify risk factors predictive of alterations in skin integrity during the intraoperative period. The predictive risk factors were studied for intraoperative pressure sores from December 1998 through January 1999. A sample of 220 patients was selected from the operating room schedule of a University Hospital in Pusan. There were two criteria in including patients : the operation lasted longer than 2 hours and the absence of skin break-down according to NPUAP criteria. The data were analized by SPSS/PC, Stepwise multiple logistic regression was used to identify the variables which were predictive of alterations in skin integrity. Of the 220 patients studied, 41 patients (18.6%) developed stage 1 pressure sores in the immediate postoperative period. In relation to skin changes, three independent variables emerged from the stepwise multiple logistic regression as being significant (p<0.05). Factors predictive of pressure sore formation included low serum albumin(p=0.000), prone position while undergoing surgery(p=0.0004), time on the operating table(p=0.0165). Among the intrinsic factors, serum albumin was the most significant causal factor in pressure sores development in the intra-operative period. Pressure and shearing force were the most significant extrinsic factors in pressure sores development. From the results of this study we concluded that the primary nursing goal is the maintenance of the proper patient' position during the intraoperative period. Also imperative for sore prevention is the reduction of surgery time and improving preoperative nutritional status.

수술실 간호사의 병원감염관리에 대한 인지도와 실천정도에 관한 연구 (A Study on Levels of Awareness of Nosocomial Infection and Management Practices by Operating Room Nurses)

  • Purpose: This study was done to examine the awareness and management practice of operating room nurses, to prevent nosocomial infection, to identify problems and to propose solutions. Method: The research tools used were the hospital infection control guideline and a modified version of the measurment tools used in the study of Cho (1998). The questionnaire consisted of questions on hand washing, personal hygiene and clothes control, cleaning and environment control, sterilizing supplies and disposal of contaminated materials. The collected data were analysed with the SPSS program. Results: The mean score for domain-specific awareness of nosocomial infection control was 4.81 out of a possible 5 points. The highest score was for sterilizing supplies and disposal of contaminated materials. The mean score for domain-specific practices of nosocomial infection control was 4.40, out of a possible 5 points. Sterilizing supplies and disposal of contaminated materials had the highest scores. The mean score for awareness in all domains was higher than mean score for practice. Among the general characteristics of the nurses, high awareness was found only in the provision of infection control guidelines and it had shown statistically significant difference. Examination of relation of general characteristics to practice showed that for age, career, the provision of infection control guidelines, and experience in infection control education there were statistically significant differences in the scores. For the relation between awareness of nosocomial infection control and practice, positive correlation was found in all domains, thus high awareness leads to high practice. Conclusion: Considering the result of this research, plans are needed that promote virtual practice of hospital infection control.

경력개발시스템 경력단계별 수술실 간호사 업무수행능력 비교 (Comparison of Nursing Performance by Career Levels in a Nurse Career Ladder System)

  • Purpose: This study was to compare nursing performance by career levels in a career ladder system. Methods: This survey was conducted with 140 OR staff nurses. Ten unit managers evaluated their own staff with a performance evaluation tool composed of 51 items. Data were analyzed by frequency, t-test, one-way ANOVA, ${\chi}^2$-test, and ANCOVA. Results: In career level CN II, nursing performance showed differences by personal characteristics. However, there was no significant difference in level CN III. CN IIIs' nursing performance was significantly higher than CN IIs' in all domains. Nursing performance of CN III was significantly higher than CN IIs' in 5 to 10 years of clinical experience. Although there was no significant difference in other clinical experience groups, CN IIIs' scores were always higher than CN IIs' Conclusion: The results indicate that nursing performance improves with career ladder advancement. Organizations should encourage advancement to higher levels and eliminate obstacles.

대장암수술 시 장갑교환의 효과 (The Effect of Glove Changing during Surgery for Colon Cancer)

  • Purpose: The purpose of this study was to explore the effect of glove changing during surgery for colon cancer on reducing the degree of contamination of surgical gloves. Methods: The randomized posttest control group design was used. Total 72 surgeries of colon cancer in the K University Hospital in Seoul performed by the team of A-Surgeon and B-Scrub nurse were randomly assigned to one of the three groups. Glove changing with single-gloved state was implemented in the Experimental Group I and outer glove changing with double-gloved state was executed in the Experimental Group II. Single-gloved state was carried in the Control Group. Following the surgical procedure, specimens for bacterial culture were collected the from scrub nurse's gloves to compare the degree of contamination among the three groups. Data were analyzed with One-way ANOVA and the Scheff$\acute{e}$'s multiple comparison test. Results: The degrees of contamination of the Experimental Group I and II were significantly lower than that of the Control Group. There was no significant difference between Experimental Group I and II. Conclusion: Glove changing during surgery with either single or double-gloved state is effective in reducing the degree of contamination of surgical gloves, therefore these methods should be utilized in clinical practice.
