변형 Biemer 방법에 의한 요골부위 전박 유리피판을 이용한 음경재건 (Penile Reconstruction Using a Radial Forearm Free Flap with Modified Biemer's Method)

  • As the defects of the penis caused by trauma, surgical amputation, or congenital abnormality give the patients both psychological trauma and functional impairment, reconstruction of the penis is mandatory. Radial forearm free flap is reliable one-stage procedure, which can reconstruct both the phallus and the urethra. Chang and Whang's adaptation of the "tube-in-a-tube" concept and its incorporation into a free flap design represented a major advance in microsurgical phallic construction. Biemer described a modification of the radial forearm flap design in which the neourethra was centered over the radial artery, but the phallic shaft was separated into two paraurethral swatches. The authors have performed one-stage penile reconstruction in two patients since 1998, using a radial forearm free flap. Our present design incorporates the original Biemer triple skin island and includes a fourth distal island for neoglans. One case was the amputation of the penis from felonious assault and the other case was the iatrogenic penile amputation from repetitive urologic surgery for congenital hypospadia. All patients showed aesthetically acceptable results and good tactile sensory recovery. Severe complications such as necrosis, fistula, or urethral stricture were not occurred. Biemer's method modified by the authors is reliable one-stage penile reconstruction providing good aesthetic and functional results.

동종 아킬레스 건을 이용한 한 단계 전방 십자 인대 재 재건술 (One-stage Revision Anterior Cruciate Ligament Reconstruction Using Achilles Tendon Allograft)

  • 서론: 신선 동결된 동종 아킬레스건을 이용하여 실패한 전방 십자 인대 재 재건술의 술기 및 슬관절의 안정성 및 기능적 결과를 분석하고자 한다. 재료 및 방법: 실패한 전방 십자 인대 재건술에 대하여 동종 아킬레스건을 이용하여 재 재건술을 시행한 13예를 대상으로 하였다. 재 재건술까지의 평균 기간은 61.8개월 이었으며 평균 추시 기간은 38.4개월이었다. 술기는 아킬레스건에 부착된 종골을 골결손부에 이식하고 재확공을 하여 한 단계로 시행하였다. 일차 재건술의 실패 원인을 분석하였고 술 후 평가는 Lysholm knee score, Lachman 검사, pivot shift 검사와 KT-1000 arthrometer 검사를 이용하였다. 결과 실패의 원인은 부적절한 수술 수기가 10예로 가장 많았다. 최종 추시점에서 Lysholm knee score는 10(75.9%) 예에서 우수 또는 양호의 결과를 보였다. Lachman 검사는 12예(92.3%) 에서 음성 또는 경도의 전방전위를 보였으며 pivot shift 검사는 음성인 경우가 11(84.6%) 예이었다. KT-1000 검사는 최대 전방 전위차가 3 mm 미만이 9예였다. 결론: 전방 십자 인대 재 재건술시 동종 아킬레스건을 이용하면 한 단계로 골이식 및 재확공을 할 수 있고 안정성 면에서 좋은 이식물의 하나이며 추후 장기적인 추시가 필요하리라 생각된다.

Prophylactic Mastectomy and Implant-Based Breast Reconstruction of BRCA1/2 Mutation-Positive Patients in Korea

  • Purpose: Mastectomy is performed as a surgical treatment for patients with breast cancer who have the BRCA 1/2 mutation. In this study, we have reported the trends in Korea for both immediate breast reconstruction and prophylactic mastectomy. Methods: This retrospective study was conducted from 2019 to 2021. Both skin-sparing mastectomy and immediate implant-based breast reconstruction with prepectoral and/or subpectoral techniques were performed in five patients with BRCA 1/2 mutations. Data on age; body mass index; cancer stage; BRCA 1/2 mutation; estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2 expression; diagnosis; and complications were collected. Results: The average (±standard deviation [SD]) age was 44.0±6.48 years old; BMI 24.5±2.25 kg/m2; and breast volumes were 365.8±70.34 and 382.4±96.33 cc for right and left ones, respectively. The BRCA 1 and 2 were diagnosed in four and one patients, respectively. The estrogen and progesterone receptors and human epidermal growth factor receptor 2 were detected in one (20%), one (20%), and three (60%) patients, respectively. The applied implant-based breast reconstruction techniques for ten breasts were subpectoral technique (n=7, 70%) and prepectoral technique (n=3, 30%). For the cancer stage, those with I, II, and III stages were one (20%), two (40%), and one (20%), respectively. There were no major complications such as Infection, seroma. Conclusion: When mastectomy is performed as surgical treatment in BRCA 1/2 mutation positive breast cancer patients, it is possible to obtain a better outcome with both implant-based breast reconstruction and different circumstances between breast cancer and contralateral breast.

  • Scalp defect management is complicated secondary to reduced laxity in the scalp and forehead area. For reconstruction of larger defects with exposed bone and loss of the periosteal layer, free flap reconstruction is one option for single-stage surgery, although the procedure is lengthy and includes the possibility of flap loss. We successfully performed a single-stage reconstruction of a large scalp defect using a combination of artificial dermis, split-thickness skin graft, and full-thickness skin graft following wide excision of a cutaneous angiosarcoma, and present our method as one option for the treatment of large oncologic surgical defects in patients who are poor candidates for free flap surgery.

귓볼 하부 전위피판을 이용한 결손형 선천성 귓볼갈림증의 재건 (A One Stage Reconstruction of Defective Type Cleft Earlobe: Infra-auricular Transposition Flap)

  • Purpose: Reconstruction of the cleft earlobe is challenging. Several procedures are available to reconstruct congenital earlobe deformities. However, for large defective type, surgical procedures and designs are complex and tend to leave a visible scar. We present a simple method of reconstruction for defective type congenital cleft earlobe using a one stage technique with infraauricular transposition flap. This allows for easy and accurate size estimation and good aesthetic outcomes. Methods: A 4-year-old male patient has congenital cleft earlobe and antihelical deformity. Otoplasty for antihelical deformity correction and one stage infra-auricular transposition flap for earlobe reconstruction were performed. The flap was designed from the inferoanterior margin of the earlobe. The size of the flap was determined based on the normal side, and the width and length of the flap was 1 cm and 3 cm in size, respectively. An incision was made at the midline of the defective lobule. Further, the elevated flap was inserted. The elevated flap and the incision margins of the lobule were sutured together. Then, the donor site was closed primarily. Results: The volume and shape of the reconstructed earlobe were natural. There was no flap necrosis. The donor site had no morbidities and scar was not easily notable. Conclusion: Infra-auricular transposition flap can be designed easily and offer sufficient volume of earlobe. Furthermore, the scar is inconspicuous. In conclusion, infra-auricular transposition flap can be a good option for reconstructing a large defect type cleft earlobe.

부분 광배근 피판을 이용한 안면마비의 단단계 재건 (One Stage Reconstruction of Facial Palsy Using Segmental Latissimus Dorsi Muscle Free Flap)

  • The goal in facial paralysis treatment is to achieve the normal appearance of the face as well as to reconstruct the natural symmetrical smile. In cases of facial paralysis, a widely accepted procedure is the two stage method, which combines neurovascular free muscle transfer with cross face nerve grafting. Although the results are promising, the two operations of this method, which are about 1 year apart, impose an economic burden on the patients and require a lengthy period before results are obtained. In order to overcome these drawbacks, one stage method, using latissimus dorsi neurovascuular free muscle flap was introduced. Between January 2000 and January 2004, fifteen patients with long standing facial paralysis were treated in the Korea University Anam Hospital. The segmental latissimus dorsi with long nerve and pedicle was transferred to the paralyzed side of the face. The first postoperative movement of the transferred muscle was reported at 8.9 months, faster than that of the two stage method. During the next 24 months, a constant increase in the power of muscle contraction was observed. The fifteen cases were evaluated within an average of 31.7 months following the surgery and satisfactory results including muscle contraction were obtained in eleven of the cases but muscle contraction was not found in three cases.

A multi-institutional analysis of sternoclavicular joint coverage following osteomyelitis

  • Background Sternoclavicular joint (SCJ) osteomyelitis is a rare pathology requiring urgent intervention. Several operative approaches have been described with conflicting reports. Here, we present a multi-institutional study utilizing multiple surgical pathways for SCJ reconstruction. Methods A multi-institutional retrospective cohort study was conducted to identify patients who underwent surgical repair for sternoclavicular osteomyelitis between 2008 and 2019. Patients were stratified according to reconstruction approach: single-stage reconstruction with advancement flap and delayed-reconstruction with flap following initial debridement. Demographics, operative approach, type of reconstruction, and postoperative outcomes were analyzed. Results Thirty-two patients were identified. Mean patient age was 56.2±13.8 years and 68.8% were male. The average body mass index (BMI) was 30.0±8.8 kg/㎡. The most common infection etiologies were intravenous drug use and bacteremia (both 25%). Fourteen patients (43.8%) underwent one-stage reconstruction and 18 (56.2%) underwent delayed two-staged reconstruction. Both single and delayed-stage groups had comparable rates of reinfection (7.1% vs. 11.1%, respectively), surgical site complications (21.4% vs. 27.8%), readmissions (7.1% vs. 16.6%), and reoperations (7.1% vs. 5.6%; all P>0.05). The single-stage reconstruction group had a significantly lower BMI (26.2±5.7 kg/㎡ vs. 32.9±9.1 kg/㎡; P<0.05) and trended towards shorter hospital length of stay (11.3 days vs. 17.9 days; P=0.01). Conclusions Both single and delayed-stage approaches are appropriate methods with comparable outcomes for reconstruction for SCJ osteomyelitis. When clinically indicated, a single-stage reconstruction approach may be preferable in order to avoid a second operation as associated with the delayed phase, and possibly shortening total hospital length of stay.

A new flap combination for reconstruction of lower nasal dorsum and supra-tip skin defects

  • Repairing surgical defects of the nose is still challenging due to its tridimensional shape and its aesthetic concern. Difficulty in reconstructing nasal subunits lies in their contour, skin texture and limited availability of adjacent skin. For lower nasal dorsum and supra-tip regions, we design a new combined local flap as existing local flaps may give disappointing results. This combination flap was performed on two patients for reconstruction of the lower nasal dorsum area after basal cell carcinoma excision. Size of the excision ranged from 20 to 25 mm diameter and safe margins were obtained. The defects were reconstructed with a local flap that combined a rotation nasal flank flap and a V-Y advancement nasolabial flap. Excision and reconstruction were performed in a one-stage surgery under intravenous sedation and local anesthesia. There were no postoperative complications and no flap loss occurred. Aesthetic and functional results after 6 months postoperatively were satisfying without modification of nasal shape. This flap is reliable and offers interesting functional and aesthetic outcomes. It can be considered as a new reconstruction alternative for supra-tip and lower nasal dorsum skin defects performed in a one-stage procedure under local anesthesia.

Two-Stage Latissimus Dorsi Flap with Implant for Unilateral Breast Reconstruction: Getting the Size Right

  • Background The aim of unilateral breast reconstruction after mastectomy is to craft a natural-looking breast with symmetry. The latissimus dorsi (LD) flap with implant is an established technique for this purpose. However, it is challenging to obtain adequate volume and satisfactory aesthetic results using a one-stage operation when considering factors such as muscle atrophy, wound dehiscence and excessive scarring. The two-stage reconstruction addresses these difficulties by using a tissue expander to gradually enlarge the skin pocket which eventually holds an appropriately sized implant. Methods We analyzed nine patients who underwent unilateral two-stage LD reconstruction. In the first stage, an expander was placed along with the LD flap to reconstruct the mastectomy defect, followed by gradual tissue expansion to achieve overexpansion of the skin pocket. The final implant volume was determined by measuring the residual expander volume after aspirating the excess saline. Finally, the expander was replaced with the chosen implant. Results The average volume of tissue expansion was 460 mL. The resultant expansion allowed an implant ranging in volume from 255 to 420 mL to be placed alongside the LD muscle. Seven patients scored less than six on the relative breast retraction assessment formula for breast symmetry, indicating excellent breast symmetry. The remaining two patients scored between six and eight, indicating good symmetry. Conclusions This approach allows the size of the eventual implant to be estimated after the skin pocket has healed completely and the LD muscle has undergone natural atrophy. Optimal reconstruction results were achieved using this approach.

One-stage nipple and breast reconstruction using a deep inferior epigastric perforator flap after a skin-sparing mastectomy

  • Background Nipple reconstruction is usually performed as a delayed procedure in patients with breast cancer who undergo skin-sparing mastectomy and breast reconstruction surgery using a deep inferior epigastric perforator (DIEP) flap. The authors designed this study to evaluate the utility of breast reconstruction based on a DIEP flap and immediate nipple reconstruction. Methods A retrospective review was conducted of all patients who underwent breast reconstruction performed by a single plastic surgeon from October 2016 to June 2018. Through a questionnaire and chart review, we compared surgical results and complications in cases of single-stage nipple reconstruction after skin-sparing mastectomy (n=17) with patients who underwent delayed nipple reconstruction after skin-sparing mastectomy, modified radical mastectomy, or simple mastectomy (n=7). Results In a subjective analysis using clinical photos, the immediate nipple reconstruction group had higher scores than their counterparts in an evaluation of the nipple-areolar complex (NAC) (NAC placement, 3.34 vs. 3.04; nipple projection, 3.05 vs. 3.03; nipple size, 3.30 vs. 3.29). No significant differences between the groups were found in terms of complications. Conclusions Simultaneous nipple reconstruction is a reliable surgical method with economic advantages. No differences were found in terms of outcomes and complications in comparison to delayed reconstruction. Therefore, surgeons can consider simultaneous nipple reconstruction without particular concerns about asymmetry or necrosis.
