A Case Report of a Functional Dyspepsia Patient with Anxiety and Abdominal Pulsation Treated with Shihogayonggolmoryo-tang (불안, 복부동계를 동반한 기능성 소화불량 환자에 대한 시호가용골모려탕 치험 1례)

  • Objective: The aim of this case report is to report the effect of Korean medicine treatment, including Shihogayonggolmoryo-tang, on anxiety and abdominal pulsation in a patient diagnosed with functional dyspepsia. Method: The patient was treated with Shihogayonggolmoryo-tang, including acupuncture, moxibustion, and cupping therapy. Changes in symptoms were evaluated using a self-reported numerical rating scale (NRS) score every morning. The gastrointestinal symptom score (GIS) was checked on admission and discharge day. Results: After 6 days, the NRS scores for anxiety, abdominal pulsation, abdomen discomfort, and GIS all decreased. Conclusion: The results suggest that Shihogayonggolmoryo-tang with Korean medicine could be effective for treating anxiety, abdominal pulsation, and accompanying abdomen symptoms. However, further evaluation is needed to clarify the relationship between the treatment's effect and Shihogayonggolmoryo-tang.

Study on vasorelaxant activities of various Traditional Herbal Prescriptions in rat thoracic aortas (수종 전통 한약 처방에 대한 혈관 이완 활성 연구)

  • Objectives : High blood pressure (also called Hypertension), which is the blood pressure that is higher than normal, is a chronic disease and causes various complications. Historically, Traditional Herbal Prescriptions (THP) have treated many diseases. However, there are not many studies on the treatment of hypertension with THP, very few studies have investigated the interactions between the co-administration of synthetic anti-hypertensives and THP. Therefore, the objective of the present study was to investigate the vasorelaxant activities of 10 THP in rat thoracic aortas pre-contracted with potassium chloride (KCl). Methods : An electric extractor was used to extract THP in distilled water for 2h. Rat thoracic aortas were isolated and pre-contracted using KCl in organ chambers containing 10 ml Krebs Henseleit (KH) buffer. THP extracts were added in increasing concentrations (10-1000 ㎍/mL) to investigate vasorelaxant activities. The vasorelaxant activities induced by THP were expressed as a percentage in response to contraction generated by KCl. Results : Among the 10 THP, Dangguisu-san, Mahwang-tang, Bulwhangeumjeonggi-san, Jakyakgamcho-tang, and Hyangsapyeongwi-san showed significant vasorelaxant activities. Maekmundong-tang, Bojungikgi-tang, Samryeongbaekchul-san, Yukmijihwang-tang, and Insampaedok-san showed no significant effect. Also, in co-administration with amlodipine, Mahwang-tang showed higher vasorelaxant activities than amlodipine alone, and Hyangsapyeongwi-san showed greater vasorelaxant activities at low concentrations but inhibited amlodipine's vasorelaxant activities at high concentrations. Conclusion : The results of these experiments are expected to provide useful data to establish guidelines for THPs and co-administration with western antihypertensive drugs to treat hypertension.

The Weight Gain Effects of the Bofesungjangjeungbo-tang (Bǔfèichéngzhǎngzēngbǔ-tang) on Under-weight Korean Preschool Children with Frequent Common Cold or Chronic Rhinitis - Analysis of Medical Records - (잦은 감기나 만성 비염을 동반한 마른 체형 소아에 대한 보폐성장증보탕 (補肺成長增補湯)의 체중개선 효과 - 의무기록 분석 -)

  • Objectives: We investigated weight gain effects of the Bofesungjangjeungbo-tang ($B\check{u}f\grave{e}ich\acute{e}ngzh\check{a}ngz\bar{e}ngb\check{u}$-tang) on thin Korean preschool children with frequent common cold or chronic rhinitis Methods Bofesungjangjeungbo-tang was empirically composed of 14 herbs for treating indigestion, and common cold or chronic rhinitis. We analyzed 60 preschool children who have visited Kim Kijoon Oriental Clinic BOM from Jan. 1, 2006 to Aug. 31, 2011 with three criteria: 1) 2~5yrs of age and BMI<50th percentile 2) children those who had taken the Bofesungjangjeungbo-tang for 1~3 month (s), 3) Availability of records on before/after treatment within 3 months. Weight gain of the children after treatment was evaluated by the changes of BMI percentile compared to pre-treatment. Results Bofesungjangjeungbo-tang significantly improved BMI percentile in all groups (1 month: $21.33{\pm}10.04$ vs $26.43{\pm}13.16$, p<0.001; 2 months: $21.90{\pm}10.28$ vs $29.03{\pm}15.06$, p<0.001; 3 months: $19.06{\pm}10.04$ vs $35.71{\pm}17.40$, p=0.001). Gender, age and pre-treatment BMI had no significant effects on the effectiveness of Bofesungjangjeungbo-tang. Conclusions: Bofesungjangjeungbo-tang positively affected weight gain in 2~5yrs Korean preschool children with BMI below 50 percentile within 1~3 month (s) of treatment. However, additional studies on functional dyspepsia or the change of energy intake are needed to understand the factors related to weight gains.

A Case study of Taeumin Esophagus Cold Pattern Patients with Gastro-esophageal Reflux Disease(GERD) and Irritable Bowel Syndrome(IBS) (GERD와 IBS를 동반한 태음인 위완한증 환자 치험례)

  • Objectives Rate of gastroesophageal reflux disease accompanied by irritable bowel syndrome is on the increase. This case study reports significant improvement of patients with gastroesophageal reflux disease and irritable bowel syndrome who had suffered from chest pain, globus pharyngis and diarrhea after taking Sasang constitutional treatment. Methods This patient was diagnosed as Large Yin type Esophagus Cold pattern(Taeeumin Wiwanhanjeung). Herb medicine(Jowiseungcheong-tang) was taken by the patient, three or two times per day during treatment periods. We assessed the changes of the main symptoms such as chest pain, globus pharyngis, defecation discomfort, insomnia et al. using visual analogue scale(VAS). Results The symptoms of chest pain, globus pharyngis, defecation discomfort, insomnia decreased from VAS 7~8 to VAS 0~1 for about three months. Conclusions This case shows that Sasang constitutional medicine treatment can be effective treatment method for gastroesophageal reflux disease accompanied by irritable bowel syndrome.

Effect of Tongqiao-tang on OVA Induced Allergic Rhinitis Mouse Model

  • Objectives : Tongqiao-tang(TQT) has been commonly used for the treatment of common cold, rhinitis etc. Nowadays, TQT becomes one of the most frequently used medicines for allergic rhinitis, but the mechanism of TQT in vivo isn't investigated yet. This study was performed to investigate the effect of TQT on OVA-induced allergic rhinitis mouse model by calculating serum cytokines and IgE. Methods : 8 weeks aged male BALB/c mice were divided into three groups: the normal group, the control group and the medicated group (the TQT group). Each group was consisted of 15 mice. The TQT group was administered TQT extract orally one time a day (1g/kg) from the $1^{st}$ day of experiment till the $26^{th}$ day. The control group and the normal group were administered normal saline by the same method of the TQT group. To induce the allergic rhinitis in the control group and the TQT group, mice of each group were sensitized intraperitoneally with ovalbumin (OVA) solution at the $1^{st}$, the $7^{th}$ and the $14^{th}$ day. After then, intranasal sensitization was performed by dropping 0.1% OVA solution in nasal cavity at the $22^{th}$, the $24^{th}$ and the $26^{th}$ day. At the $27^{th}$ day, the mice were killed and the changes of interferon-${\gamma}$, interleukin-4, interleukin-5, total IgE and OVA-specific IgE were checked. Results : IFN-${\gamma}$ was increased 36% more in the TQT group than that in the control group. IL-4, IL-5, the total IgE and OVA-specific IgE were decreased in the TQT group as compared with the control group and these results were statistically significant. Conclusions : Considering the above experimental results, this study showed that TQT could reduce the allergic reaction in allergic rhinitis. Advanced studies are required to investigate the further mechanisms of TQT.

Sochungyong-tang, Tradititional Korean Medicine, Suppresses Th2 Lineage Development (소청룡탕(小靑龍湯)이 분화된 Th1 cell 및 Th2 cell cytokine profile에 미치는 영향)

  • In this study, the immunological effect of a traditional Korea herbal medicine, Sochungyong-tang (SCRT) that has been widely used for the treatment of various immunological disorders including allergic asthma in Korea, was examined in vitro. In our previous study demonstrated that SCRT decreases the expression of IL-4 mRNA, that plays pivotal role in Th2 cell development, while increases $IFN-{\gamma}{\tilde{a}}expression$, which is one of the key cytokines for Th1 lineage development in Th0 condition. That study strongly implies that SCRT can correct Th2 dominant condition directly affecting to the CD4+ T cell development. Present study designated to further evaluate the SCRT on helper T cell development by monitoring Th1/Th2 specific cytokine secretion patterns in artificially induced Th1 or Th2 polarized condition. The results demonstrated that Th2 cells were dramatically under-populated in Th2 driven condition with SCRT treatment, while Th1 cells were not altered in Th1 skewed condition. Furthermore, under Th2-skewed conditions the levels of and IL-4 were considerably decreased with SCRT treatment. However, the expression of GATA-3, a transcription factor that plays pivotal role in Th2 lineage programming, was not changed with SCRT, suggesting that the suppression of Th2 cell development by SCRT was not mediated by GATA-3. Present study implies that the effect on CD4+ T cell may be the one of key pharmacological effect point for treating IgE medicated allergic asthma by SCRT. These results also suggest that SCRT might be desirable agent for the correction of Th2 dominant pathological disorders.

A Review on Research of Korean Medicine Treatments for Infertile Women with Uterine Myoma (자궁근종을 동반한 난임 여성의 한의학적 치료에 대한 연구 동향)

  • Objectives: The purpose of this study is to review the research trends of infertile women with uterine myoma in Korean medicine and to recognize the efficacy of Korean medicine intervention. Methods: A search was conducted on November 18, 2022, using six databases, including Research Information Sharing Service (RISS), Oriental Medicine Advanced Searching Integrated System (OASIS), Cochrane Library Central, Pubmed, China National Knowledge Infrastructure (CNKI) and WangFang Med Online. We analyzed the studies using Korean medicine on women with uterine myoma preparing for pregnancy or complaining infertility. Results: Total 4 studies were selected and all of which were case reports. Among 6 cases, Korean medicine treatment including herbal medicine, acupuncture, electroacupuncture, moxibustion was used alone. Ikgibohyeol-tang-gami-bang, Gyejibokryeong-hwan, Saenghwa-tang-gami-bang were prescribed in 2 cases each. Angelicae Gigantis Radix and CV4 were most frequently used herb and acupoint. All subjects were successful in becoming pregnant. Conclusion: This review shows that Korean medicine treatment can be effective in infertile women with uterine myoma. However, the number of included studies is small, so the level of evidence needs to be raised through more clinical studies in the future.

  • Objectives: The aim of this trial is to evaluate the efficacy and safety of Yukwool-tang (Liuyu-tang) for the treatment of major depression in women by comparing the Yukwool-tang (Liuyu-tang)-treated group with the placebo-treated group and assessing the association of various biological factors related to depression through various outcome measures. Methods: This study is a single-center, randomized, double-blind, placebo-controlled, parallel-design clinical trial. The subjects to be selected are women between the ages of 19 and 65, and the registered subjects are to be randomly assigned to treatment with Yukwool-tang or the placebo control. The Yukwool-tang group will take 1 bottle of Yukwool-tang (30 mg) for 8 weeks, 3 times a day, before meals. The control group will take the placebo in the same way. The primary outcome to be examined will be the change between the total score after 8 weeks and the total score before the start of the study of the K-HDRS score. Secondary outcomes are assessed by the change in total score after 12 weeks of K-HDRS, K-HDRS remission rate, K-HDRS improvement rate, BDI-K, PITD, KSCL-95, ISI, STAI-K, EQ-5D, VAS, Emotional Stimulation Test, BDNF test, inflammatory cytokine and tumor necrosis factor test, intestinal microbiome test, dietary report and Beck's hopelessness scale. Results: This protocol has been approved by the IRB of Dunsan Korean Medicine Hospital of Daejeon University and is registered in the CRIS, and it is made public in advance to ensure transparency of the research process and conduct ethical clinical trials. Conclusions: Based on this protocol, when the trial is completed, its data can be used to access the validity and safety of Yukwool-tang for major depression in women, and it is also expected to be helpful in the study of the correlation between future treatment of Korean medicine for depression and related biological factors, and quality of life.

Case Study of Cerebellar Infarction Base on Oriental Medical Diagnosis of Dongeuibogam (동의보감 현훈, 운동실조의 내용을 중심으로 살펴본 소뇌경색 환자 치료경과 3례)

  • The cerebellum processes input from other areas of the brain, spinal cord and sensory receptors to provide precise timing for coordinated, smooth movements of the skeletal muscular system. A stroke affecting the cerebellum may cause vertigo, nausea, balance and coordination problems. The papers on central vertigo caused by cerebellar infarction were reported by Kim, Ku, Ko, Choi, and Han. Their cases were classed as Heo hoon, Dam hoon, and Sangchopungyeul yeul dam, and they treated with Jaeumkunbi-tang, Taeksa-tang, Cheonghunwhadam-tang gami, Cheonginwhadam-tang, Hachulbosim-tang. In this paper, three patients who suffered from vertigo were diagnosed as cerebellar infarction. They felt too giddy to walk without any help. After Oriental medical treatment based on Dongeuibogam, their sequelae of cerebellar infarction - vertigo, nausea, ataxia - had improved dramatically. So we report these cases, and suggest Oriental medical treatment based on Dongeuibogam have effect on cerebellar infarction. But more clinical case reports should be further examined.

Comparative Study of the Neuroprotective Effect of Sihogyeji-tang, Sihosogan-tang, and Sihocheonggan-tang on an MPTP-Induced Parkinson's Disease Mouse Model (MPTP로 유도된 파킨슨병 생쥐 모델에 대한 시호계지탕, 시호소간탕, 시호청간탕의 신경세포 보호 효과 비교 연구)

  • Objectives : Parkinson's disease (PD) is the second most common neurodegenerative disorder worldwide and is characterized by the loss of the dopaminergic neurons in the substantia nigra (SN). In a previous in vitro study, we demonstrated that Sihogyeji-tang (SG), Sihosogan-tang (SS), and Sihocheonggan-tang (SC) have the potential to be candidate medicines for PD. This study aimed to compare the neuroprotective effect of SG, SS, and SC using 1-methyl-4-phenyl-1,2,3,6-tetrahydrophridine (MPTP)-induced PD mouse model. Methods : Eight-week-old male C57BL/6 mice were intraperitoneally administered with 30 mg/kg of MPTP for 5 days and orally administered SG, SS and SC for 12 days from the first MPTP injection. Motor function was assessed using the pole test and the rotarod test. Dopaminergic neuronal survival in the SN and striatum was evaluated through tyrosine-hydroxylase immunohistochemistry. Results : MPTP administration resulted in behavioral impairment and dopaminergic neuronal death in the SN and striatum. In the pole test, treatment with SG, SS, and SC alleviated the MPTP-induced motor dysfunction on day 5 and 12. In the rotarod test, SS and SG alleviated the MPTP-induced motor dysfunction on day 5, while only SS showed improvement on day 12. SS and SG significantly protected dopaminergic neurons in the SN from MPTP toxicity, and all three compounds (SG, SS, and SC) showed significant protection in the striatum. Notably, SS demonstrated superior efficacy in suppressing MPTP-induced motor dysfunction and dopaminergic neuronal death compared to SG and SC. Conclusions : These findings suggest that SS is the most effective formula among SG, SS, and SC for PD, indicating its potential role in the treatment of PD.
