A Case of Persistent Hiccups of Non-Small Cell Lung Cancer Patient Treated with Korean Medicine (한방 치료로 지속성 딸꾹질이 호전된 비소세포폐암 환자 치험 1례)

  • Persistent hiccups persist for more than 48 hours. Those can often occur in cancer patients after chemotherapy or high dose of corticosteroids. A 52-year-old male patient undergoing Non-small cell lung cancer(NSCLC), recently suffered presistent hiccups and dyspnea. He was treated with herbal medicine for 56days and acupuncture for 9days. No side effects were observed during treatment. The severity of hiccups was measured in every hospital visit and psychometric symptoms were evaluated periodically through the Edmonton Symptom Assessment System(ESAS). The severity of hiccups and psychometric symptoms were alleviated and the effect persisted during the treatment. The patient took no other nervous system drugs in the end of the treatment. In conclusion, Korean medicine like Hyeongbangjihwang-tang and Jeongcheonhwadamgangki-tang can be effective for patients with persistent hiccups and dyspnea who cannot use alternative drugs or treatment because of other complications.

A Case of Combined Korean Medicine Treatment for Homonymous Hemianopia and Loss of Visual Acuity after hemorrhagic Stroke (동측 반맹 및 시력저하를 호소하는 후두엽 및 측두엽 뇌출혈 환자 한의복합치료 1례)

  • Background: Although not clarified, the overall prevalence of visual field loss after stroke was estimated at 52% of stroke survivors and for visual acuity loss, at 70% of them. Among visual field loss patients after stroke, homonymous hemianopia is the most common symptom. Visual defect degrades the quality of life by disrupting independent life and affecting the survival of the patients, but highly-evidenced treatment has not been found until now. ■Case report A 51-year-old female with left-side homonymous hemianopia and central vision loss by a right occipital-temporal hemorrhage was treated with Hyeolbuchukeo-tang, Ikgibohyeol-tang, acupuncture, electroacupuncture, and moxibustion. We used a confrontation visual field exam, automated perimetry and visual analog scale, and visual acuity test to observe changes in the patient's symptoms and evaluate the efficacy of the treatment. After 61 days of such interventions, the patient's subjective discomfort lessened, followed by positive changes in the scales of tests mentioned above. ■Conclusion This case suggests that combined Korean medicine treatment might be an effective tool for treating post-stroke homonymous hemianopia and central vision loss. More studies should be conducted to support the effectiveness of the treatment.

The Effects of Gudambope-tang on the Rat Model with Allergic Rhinitis (거담포폐탕(祛痰補肺湯)이 알레르기 비염 모델 흰쥐에 미치는 영향)

  • Objectives: Allergic rhinitis is an inflammation or irritation of the mucous membranes that line the nose. Common symptoms include sneezing; a stuffy or runny nose; itchy eyes, nose and throat; and watery eyes. We aimed to determine therapeutic effects of Gudambope-tang by observing changes in blood cells and the nasal mucosal tissue. Methods: Fifteen Spraque-Dawley rats were divided into three groups: normal, control, and sample group. Allergic rhinitis was induced in the control and sample group by intraperitoneal and intranasal sensitization with 0.1% ovalbumin solution. Then Gudambope-tang was orally administered only to the sample group for 28days, while the rats in the control group was given normal saline. We observed changes in the number of RBC and WBC; changes of neutrophil, Iymphocyte and monocyte proportions; total IgE level and AST & ALT changes; also, changes in the nasal mucosal tissue. We used ANOVA test statistically(p<0.05). Results: 1. Gudambope-tang showed an inhibitory effect on the process of allergic rhinitis. 2. Gudambope-tang inhibited the inflammatory reaction on the nasal mucosal tissue. 3. Gudambope-tang had no hepatoxicity. Conclusions: According to the above results. it is considered that Gudambope-tang has an inhibitory effect on the process of allergic rhinitis and it can be used in relieving symptoms of allergic rhinitis.

Case Report of Metabolic Dysfunction-Associated Fatty Liver Disease Treated with Saenggangunbi-tang (대사이상 관련 지방간질환의 생간건비탕 치험 1례)

  • Objective: This study identified the effects of Korean medicine treatment on a patient with metabolic dysfunction-associated fatty liver disease (MAFLD). Methods: A 43-year-old man with MAFLD was treated with Saenggangunbi-tang with regular exercise from August 13, 2022, to December 24, 2022, to reduce fatigue and dyspepsia and to improve laboratory findings, such as liver enzymes and lipid profiles. We observed changes in symptoms and laboratory findings during the approximately four-month treatment. Results: Treatment with Saenggangunbi-tang resulted in decreased serum levels of liver enzymes, triglycerides, hepatic steatosis index scores, and clinical symptoms. During the treatment, the patient performed regular exercise; however, there was no significant change in body weight until the end of the study. Conclusion: This study suggests the availability of Saenggangunbi-tang as a therapeutic option for managing MAFLD patients.

Case of an Iron Overload Patient Who Discontinued Iron Chelation Therapy and Was Treated with Saenggangunbi-tang (철 킬레이트화 치료를 중단한 철과잉증 환자의 생간건비탕을 사용한 치험 1례)

  • Objectives: This study identified the effect of Korean medicine treatment on a patient with iron overload who discontinued iron chelation therapy. Methods: A 64-year-old woman with iron overload was treated with Saenggangunbi-tang from November 14, 2022, to March 15, 2023, to reduce fatigue and improve laboratory findings. We observed changes in the symptoms and improvement of laboratory findings during the four-month treatment. Results: The approximately four-month treatment with Saenggangunbi-tang showed considerable improvement in laboratory findings and fatigue. In addition, no adverse effects, such as liver injury, were observed during Korean medicine treatment. Conclusion: This study suggests the availability of Saenggangunbi-tang as a therapeutic option for managing patients with iron overload.

Studies on the Effect of 'Gal Geun Tang' upon Stress Resistance of Rats (갈근탕(葛根湯) 투여가 흰 쥐의 스트레스 저항 반응에 미치는 영향에 관한 연구)

  • Oxygen consumption rate has been determined to study the effect of 'Gal Geun Tang' upon stress resistance of rats. The oxygen consumption rate of liver tissue increased slightly when normal rats were given the Gal Geun Tang infusion only. When rats were given the infusion after an injection of typhoid vaccine, the elevated boby temperature and oxygen consumption rate due to typhoid vaccine fell to normal level. From this result, it is concluded that 'Gal Geun Tang' has a function that may normalize abnormally strained liver metabolism. Oxygen consumption rate of the rats significantly increased when rats were exposed to cold condition. However, under the same condition it rather maintained the normal consumption rate when the rats were given previously "Gal Geun Tang". It is suggested that 'Gal Geun Tang' has an effects that impairs the immune response from cold stress. The rats that were given Cinnamomi Ramulus('Ge Ji Tang') showed less effect than the group of 'Gal Geun Tang'.

A Case of Urinary Tract Infection Treated with Gagamyookmijihwang-tang(Jia-Jian-Liu-Wei-De-Huang-Tang) in Stroke Patient (중풍환자의 급성요도염에 대하여 단독 한방처방 투여로 치료한 1례)

  • Urinary tract infection(UTI) is one of the common complications in stroke patients. As it has negative effect on the recovery of stroke, it should be cured out immediately. But antibiotics might cause some adverse reactions such as diarrhea, eruption, anorexia, nausea and vomiting. so there have been several reports about treating urinary tract infection with Traditional Korean Medication. We treated a 54-year-old male patient with cerebral hemorrhage, who had had neurogenic bladder after stroke and had been urinated by intermittent catheterization. About 10 days later, he could void by himself without catheter, but showing the symptoms of UTI; Voiding pain, hematuria and yellowish pus. The pus culture grew Staphylococcus spp., which was resistant to most of antibiotics except vancomycin and teicoplanin. Based on accompanying symptoms of intermittent dizziness, headache, insomnia, nocturnal sweating, weak pulse, red tongue and urinary problem, we differentiated him as the deficiency of Yin of the Kidney[腎陰虛] and treated with Gagamyookmijihwang-tang (Jia-Jian-Liu-Wei-De- Huang-Tang), which improved his urinary symptoms and other general conditions without any side effect. In next follow-up culture, there was no pathogen. We conclude that Traditional Korean Medicine based on differentiation is useful in the treatment of urinary tract infection.

Analysis of the Active Compounds and Therapeutic Mechanisms of Yijin-tang on Meniere's Disease Using Network Pharmacology(I) (네트워크 약리학을 활용한 메니에르병에 대한 이진탕(二陳湯)의 활성 성분과 치료 기전 연구(I))

  • Objectives : This study used a network pharmacology approach to explore the active compounds and therapeutic mechanisms of Yijin-tang on Meniere's disease. Methods : The active compounds of Yijin-tang were screened via the TCMSP database and their target proteins were screened via the STITCH database. The GeneCard was used to establish the Meniere's disease-related genes. The intersection targets were obtained through Venny 2.1.0. The related protein interaction network was constructed with the STRING database, and topology analysis was performed through CytoNCA. GO biological function analysis and KEGG enrichment analysis for core targets were performed through the ClueGO. Results : Network analysis identified 126 compounds in five herbal medicines of Yijin-tang. Among them, 15 compounds(naringenin, beta-sitosterol, stigmasterol, baicalein, baicalin, calycosin, dihydrocapsaicin, formononetin, glabridin, isorhamnetin, kaempferol, mairin, quercetin, sitosterol, nobiletin) were the key chemicals. The target proteins were 119, and 7 proteins(TNF, CASP9, PARP1, CCL2, CFTR, NOS2, NOS1) were linked to Meniere's disease-related genes. Core genes in this network were TNF, CASP9, and NOS2. GO/KEGG pathway analysis results indicate that these targets are primarily involved in regulating biological processes, such as excitotoxicity, oxidative stress, and apoptosis. Conclusion : Pharmacological network analysis can help to explain the applicability of Yijin-tang on Meniere's disease.

  • Although the 'Insamyangwee-tang' has been widely used in clinical purposes in the oriental medicine, its clinical efficacy is only documentated for the cases of gastritis, gastric ulcer and enteritis, but the experimental study on these has not been undertaken. So, to investigate the clinical efficacy of 'Insamyangwee-tang' comparing with animal experiments, This study was carried out. The results showed that relaxing action was shown on the isolated ileum in mice and that strong antagonistic actions were seen on $BaCl_2$, acetylcholine and histamine induced contraction of the ileum in mice, rats, rabbits and guinea-pigs that the relaxing effect of the intestinal smooth muscle was recognized. Inhibitory effects on transport rate in the small intestine of mice. Strong antagonistic actions were seen on acetylcholine induced contraction of duodenum in rats and remarkably inhibiting actions were seen of duodenum in rats. Inhibitory action on the secretion of gastric juice and pepsin, anti-ulceration effect was recognized.

  • In order to investigate the pharmacological activities of the combined preparation of crude drugs, Chunghyulgangki-Tang was studied. Chunghyulgangki-Tang has been widely used to cerebral apoploxy, hypertension and arteriosclerosis, etc. In this study, water extract of Chunghyulgangki-Tang was conducted in attempt to investigate for analgesic, sedative, antipyretic, isolated ileum and heart, blood vessels and blood pressure action in mice, frogs, rats, guinea-pigs and rabbits. The following results were obtained; Analgesic action by the acetic acid stimulating method in mice was recognized. Sedative activities by the wheel cage method and rotor rod method in mice were shown. Prolonged action against the hypnotic duration induced by thiopental-Na was significantly noted in mice. The effect of antipyretic in endotoxin febrile rats was significantly recognized. Spontaneous motility of the isolated ilem of mice was suppressed and contractions of the isolated ileum of mice and guinea-pigs induced by acetylcholine chloride, barium chloride and histamine were remarkably inhibited. Acceleration of isolated heart motility was shown in frogs. Vaso-dilating and hypotensive actions were recognized in rabbits. According to the above results, effects based on the oriental medicinal references were approximate to the actural experimental results.
