노인들의 체력 변화와 운동의 제언 (The changing of fitness and exercise guidelines for old adults)

  • The aging is related to numerous changes in our body system which declines functional ability. The functional ability in advanced age can be evaluated by the 4 components of fitness (cardiorespiratory endurance, muscular strength and endurance, flexibility and body composition). The understanding and knowledge of changing 4 components of fitness and the role of exercise on the process of aging is of paramount importance. Despite exercise and physical activity have been shown to delay the process of aging, the exercise guidelines and recommendations for old adults are not well published. This review is to presents the age-related changes in each component of fitness and to present current guidelines and recommendations for exercise in older adults.

The Mediating Effect of Acceptance Action in the Relationship between Diabetes Distress and Self-stigma among Old Adults with Diabetes in South Korea

  • Purpose: This study investigated the mediating effects of acceptance action on the relationship between diabetes distress and self-stigma in older adults with diabetes. Methods: A descriptive research approach was adopted using 187 patients diagnosed with diabetes mellitus by an endocrinology doctor. The data were collected from 26 to 31 March, 2020 and were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficient analysis, and hierarchical multiple regression. Results: The mean scores for diabetes distress, self-stigma, and acceptance action were 2.98±0.64, 2.54±0.74, and 4.16±0.35, respectively. Acceptance action partially mediated the relationship between diabetes distress and self-stigma (z=1.98, p=.024), with an explanatory power of 51.0%. Conclusion: To reduce diabetes self-stigma among old adults in diabetes distress situations, it is necessary to improve their acceptance action and develop step-by-step differentiated acceptance action enhancement programs through multidisciplinary collaborations.

청년 중년 노년세대별 노인에 대한 태도 (A Study on Attitudes toward Older Adults : Comparative Analysis of Young, Midlife, and Older Adults)

  • This study examined attitudes toward older adults among a sample of young (496 Participants), midlife (200 participants), and older adults (115 participants). The results indicate that attitudes of the three generations toward older adults were modest in score, being neither positive nor negative. The older adults' attitudes were the most positive regarding elderly people. In other words, each generation of people showed different attitudes toward older adults. There was also a significant interaction effect between generation and gender. Middle aged women were the most negative toward the elderly, yet older women were the most positive. In addition, there were differences among generations in the variables that are related with the attitudes toward older adults. The attitudes were positively correlated with relationship with their grandparents for young adults, whereas care giving stress was significantly related for midlife adults, and the level of preparedness for old age (physical, emotional and economic) was a strong predictor for older adults.

  • Purpose: The purpose of the present study was to investigate the prevalence and lifestyle factors related to the metabolic syndrome in Korean elderly. Methods: A total number of 122 elderly over 60 yr were recruited from a community elderly center. Elderly were classified into young old (60-74 yr) and old (75-84 yr). Metabolic syndrome was defined by third report of the national cholesterol education program (NCEP) expert panel on Adult Treatment Panel III (ATP III). Abdominal obesity was determined by Korean society for the study of Obesity criteria. The nutritional status was measured by mini nutritional assessment (MNA). Results: The prevalence of the metabolic syndrome was significantly higher in the old than young old elderly (p<0.05, respectively). Abdominal obesity and hypertension were the most common risk factors. The related factors to metabolic syndrome were high Body mass Index (BMI) and currently not participating in exercise. Abdominal obesity, low HDL cholesterol and hypertension were associated with specific nutrient intakes. Conclusion: A strategy to decrease the metabolic syndrome in the elderly is urgently needed particularly so for the old elderly. A nutritional and exercise program for young old elders may be helpful reducing the prevalence of metabolic syndrome in later stage.

Different Effects of Cognitive and Non-exercise Physical Leisure Activities on Cognitive Function by Age in Elderly Korean Individuals

  • Objectives: We aimed to examine the effects of various leisure activities on cognitive impairment in young-old (aged 65-74 years) and old-old (aged ${\geq}75$ years) adults. Methods: In total, 10,279 elderly Korean individuals from the 2014 Korean National Survey on Older Adults' cohort were enrolled in our study. Cognitive impairment was assessed using the standardized score of the Mini-Mental State Examination for Dementia Screening, whereas leisure activities were recorded via self-reporting of the extent and type of leisure activity the subjects involved in over the past year. Multivariate logistic regression was used to assess the effect of leisure activities on cognitive impairment, while controlling for potential covariates. Results: The subjects were more likely to participate in cognitive activities than in non-exercise physical activities. After controlling for selected covariates, involvement in cognitive activities was found to be a significant predictor of cognitive impairment in both the groups, whereas involvement in non-exercise physical activities was not a predictor of cognitive impairment in individuals aged ${\geq}75$ years. Moreover, depressive symptoms, rural residence, and hearing difficulties were common predictors of cognitive impairment among elderly-Korean-individuals. Conclusion: Leisure activity involvement may help delay cognitive impairment, which is often concomitant with aging. Hence, an early intervention service may significantly benefit both young-old and old-old individuals.

요양병원과 요양시설 노인의 건강상태, 사회적 지지 및 거주만족도 (Comparison of Perceived Health Status, Social Support and Residential Satisfaction in Longterm Care Hospital and Nursing Homes)

  • Purpose: The purpose of this study was to identify and compare the differences on perceived health status, Activities of Daily Livings (ADL), social support, and residential satisfaction between long-term care hospitals and nursing homes. Methods: Data was collected through questionnaires and interviews conducted from March 29 to April 22, 2011. The subjects were 66 old adults in 3 long-term care hospitals and 53 old adults in 6 nursing homes. Data were analyzed by Pearson's correlation analysis and t-tests. Results: ADL and subjective health status in nursing homes were worse than those in long-term hospitals, but it was not statistically significant (p>.05). Old adults in nursing homes received more emotional support from other residents and staff, and received more instrumental support from staff than those in long-term care hospitals (p<.001). The mean scores of resident satisfaction in long-term care hospitals and nursing homes were 3.53 ($SD={\pm}0.36$) and 3.97 ($SD={\pm}0.44$), respectively. Resident satisfaction in nursing homes significantly was higher than long-term care hospitals (p<.001). Conclusion: Health care personnels in long-term care hospitals should enhance resident satisfaction and social support and need to coordinate long-stay patients with nursing homes.

성인의 연령별 구강건강행위에 따른 건강관련 삶의 질에 관한 연구 (Health-related quality of life according to oral health behavior in adults)

  • This study identifies oral health behavior factors related to the health-related quality of life(EQ-5D) of adults. Three groups are analyzed, young adults(aged 19 to 39 years old), middle-aged(40 to 64 years old), and elderly(over 65 years old). By comparing the factors related to health-related quality of life by age, we will provide information for improving the overall quality of life, including oral health for current and future elderly. Using raw data from the 1st year(2016) of the National Health and Nutrition Survey, the oral health behaviors of the study subjects were compared. These included: the number of times teeth were brushed, the time brushing took place, the use of interdental care products, and whether they went for oral and examinations. There was a statistical significance in treatment experience and drinking habits. There was also a significant influence on the health-related quality of life across the age groups. Therefore, it is possible to improve the health-related quality of life, including oral health practices according to age. It will be necessary to develop and apply.

한국 노인의 식품안정성에 따른 인구사회학적 특성, 영양섭취상태 및 정신건강상태: 2008-2010년 국민건강영양조사 자료를 이용하여 (Socio-demographic Characteristics, Nutrient Intakes and Mental Health Status of Older Korean Adults depending on Household Food Security: Based on the 2008-2010 Korea National Health and Nutrition Examination Survey)

  • Objectives: This study was performed to investigate the associations of food security with socio-demographic characteristics, nutrient intakes and mental health status among older Korean adults. Methods: This study was conducted using data from the 2008-2010 Korea National Health and Nutrition Examination Survey (KNHANES). Subjects were 4,451 adults aged 65~98 years. Food security was measured using a self-reported question on food sufficiency of subjects' household. Based on the answers, study subjects were classified into secure, mildly insecure, moderately insecure, and severely insecure groups. Dietary intake was estimated by 24-hour dietary recall. Nutrient intake was assessed by dietary reference intakes (DRI). As for mental health status, the data on mental stress, depression, and suicide ideation were used. Results: Rate of food insecurity in older adults was 14.3%. Old age, being female, low education, low income level, living alone, and discomfort in daily living were more related to food insecurity. Means of nutrient intakes were significantly different according to food security status. Intakes of calcium, potassium, and vitamin $B_2$ were lower than recommended intakes in all groups. Consumption amounts of soy and soybean products, vegetables, mushrooms, fruits, vegetable oils, meats, eggs, seafood, and dairy products were lower in food insecure groups. Mental stress, depression, and suicide ideation were higher in food insecure groups independent of the gender and income level. Conclusions: These present findings suggested that food security is related to mental health as well as nutrition status in older Korean adults. A national system that include food and psychosocial support programs for the elderly should be considered in order to improve the overall health of older Korean adults.

한국 성인의 성별 및 연령에 따른 치주염과 비만, 건강행동의 연관성 (Association between periodontitis, obesity, and health behavior according to sex and age groups in Korean adults)

  • Objectives: The aim of this study was to examine the relationship between periodontitis, obesity, and health behavior according to sex and age in Korean adults. Methods: Data on 11,032 adults aged 19-79 years were obtained from the 7th Korea National Health and Nutrition Examination Survey (KNHNES). We used multivariate logistic regression analysis to test for associations. Results: The final model that was adjusted for demographic characteristics and health status, showed a higher risk of periodontitis with increasing body mass index (BMI), smoking, failure to use oral care products, and no dental check up. In the sub-group analysis, only smoking was significant in the 19 to 39-year-old age groups. In the 40 to 64-year-old age group, a BMI of 30 or higher, and smoking, use of oral care products, and dental check-up were significantly associated with periodontitis. In the female group, BMI, smoking, use of oral care products, and dental check-up were significantly related to periodontitis. However, in males, only smoking was significant. Conclusions: Obesity management can be helpful for periodontal health as periodontitis prevalence in adults increased as BMI increased. For periodontal health, an oral health program should include smoking cessation and the use of oral care products and dental check up as part of obesity management.

Comparisons of Spatial-Temporal Characteristics between Young and Old Adults While Walking: Factors Influencing the Likelihood of Slip-Initiation

  • A laboratory study was conducted to evaluate if two different age groups(young vs. old) had differences in walking velocity and heel contact velocity and, furthermore, if these gait characteristics could adversely influence initial friction demand characteristics(i.e. RCOF) and the likelihood of slip-initiation. Twenty eight(14 younger and 14 older adults) participated in the study. While wearing a safety harness, all participants walked at their preferred gait speed for approximately 20 minutes on the linear walking track(1.5m× 20m) consisting of two floor-mounted forced plates. During subsequent 20 cameras, respectively. The results indicated that older adults walked slower(i.e., slower whole body center-of-mass velocity), exhibited lower heel contact velocity, and produced lower initial friction demand characteristics (i.e. RCOF) in comparison to younger adults. However, ANCOVA indicated that the diferences in heel contact velocity between the two age groups were due to the effects of walking velocity. The bivariate analysis further suggested that walking velocity was correlated to RCOF and heel contact velocity, while heel contact velocity was not found to be correlated to RCOF. In conclusion, could be a better indicator for predicting initial friction demand characteristics(i.e. RCOF) not hel contact velocity.
