  • This study presents the cases of two patients suffering from chest pain as the chief complaint and refers to related literatures to reveal the psychoanalytic meaning of chest pain. In the first case, the patient who was balked of the desire to be loved complained of her pain in the chest She had been bereaved of her husband and felt deserted by someone on whom she depended, and these experiences caused the chest pain. The drive related to this chest pain in a dependent and aggressive one. The second case, the chest pain of a 28-year-old unmarried woman, resulted from Oedipal conflict Her Oedipal conflict did not resolve successfully for the exessive sexual stimulation in her childhood such as her experiences of witnessing the primal scenes sleeping with her parents in the same room, even under a same blanket In addition, there were some other traumas which prevented her from that conflict: Her father bathed her until her puberty: She saw her father's back view as he urinated in a jerry: She heard her parents' frequent quarrels. This patient felt guilty about desire of Oedipal incest, and chest pain seemed to occur as a kind of self-inflicted punishment.

  • A parosteal lipoma is a benign tumor containing mature adipose tissue and is intimately related to the adjacent periosteum. We experienced a very rare case of parosteal lipoma arising from the shaft of femur. A 46 years old lady visited the hospital with complaining of slowly growing mass in her thigh for 7 month. Initially, it was difficult to differentiate from osteochondroma, parosteal osteosarcoma or liposarcoma. Based on the assessment of plain radiogram and magnetic resonance imaging, it was suspected the parosteal lipoma or osteosarcoma. Marginal excision was performed, and it was confirmed to parosteal lipoma histologically. We present the case with review of literatures.

  • The most of discoid meniscus is located in lateral side. Medial discoid meniscus is rare. The medial discoid meniscus was reported by Cave and Staples at first in 1941 but rarely reported since then. A 8 year's old boy was diagnosed as having a complete medial discoid meniscus by means of MRI and arthroscopy. Authors performed the arthroscopic partial menisectomy for medial discoid meniscus. For confirmation of the bilaterality, arthroscopic examination on contralateral knee was performed but we did not find a medial discoid meniscus. We report 1 case of a medial discoid meniscus with a brief review of literatures.

  • In July, 2000, A 71-year-old man complained of these symptoms: frequent defecation, urgent need to defecate, fecal leakage, anal pain, flatus, and diarrhea after surgery for colorectal cancer. The symptoms are caused specifically by injury to the bowel and sphincter function. The patient suffered physically and psychologically; but, there was no special treatment in western medicine. Therefore, we treated him with electrical acupuncture for the management of sphincter tone and the alleviation of anal pain. We also treated him with herbal medicine for the recovery of strength and gastro-colic function. As a consequence of these treatments, the bowel disorders improved immediately and successfully, even though there were some limitations. So, We report this case with a brief review of related literatures.

The methods of this study are the study of academic literatures as well as practical study through expert investigation and analysis of case studies about actual works. Fashion trends since 2000 years are summarized classic, sporty, elegance, ethnic, natural, romantic, vintage, futurism by expert investigation and analysis of case studies about actual works. Orbital Cavernous Lymphangioma with Intracranial Extension - Case Report - (두개강내로 침윤을 보인 안와 해면상 림프관종 - 증 례 보 고 -)

  • Lymphangioma is a rare benign developmental vascular tumor that may be found in orbit, skull and elsewhere in head and neck. Few cases of extension of this benign but insidious tumor posteriorly out of the bony orbital cavity have been reported. The patient was 40-year-old man complaining of proptosis of right eye for one month. Physical examination revealed severe right exophthalmus, impairment of eyeball movement in all directions. Visual acuity was much impaired and he could percept only light with right eye. CT and MRI scans showed intraconal and extraconal involvement of ill-defined, heterogenous mass with extension of the tumor posteriorly beyond the orbital cavity involving right frontal and temporal lobe, skull and subcutaneous tissue. The tumor was subtotally removed via orbito-frontal approach without damaging vital neural and orbital component. Then, orbital roof reconstruction and cranioplasty were done with resin. Successful surgical removal of lymphangioma is very difficult due to its severe infiltration to surrounding tissue and tendency to bleed during debulking. We report a rare case of orbital cavernous lymphangioma with intracranial extension treated with surgical decompression, with review of literatures.

Calcified Chronic Subdural Hematoma : Late Sequele of Shunt Operation in a Child with Hydrocephalus - Case Report - (소아에서 뇌수두증의 단락술 이후 발생한 석회화된 만성 경막하 혈종 - 증례보고 -)

  • Post-shunting subdural hemorrhages are usually small and self-limited, and they can be recognized on the routine post-operative CT examinations. However, delayed subdural bleeding may occur without any clinical symptoms or signs. Thus the hematoma remains undetected, and it can be increased in size and sometimes become calcified with time. We experienced a case of 15-year-old male with a large calcified subdural hematoma who had undergone shunt operation 10 years previously. With pertinent review of the literatures, we discuss the possible mechanism of calcification and the proper way of treatment in calcified chronic subdural hematoma.

  • Purpose: Morel - Lavallee lesions is post - traumatic effusion at subcutaneous layer. Early management of Morel - Lavallee lesion is percutaneous drainage and compression. Surgical excision is recommended for cases that are chronic. Method: 41 - year old male patient had slow growing, painful mass on left thigh, which has been present for 3 years. He had a history of blunt trauma 3 years ago. Computed tomography presented $10{\times}10{\times}5cm$ sized cystic mass on left thigh, between subcutaneous fat layer and muscle fascia layer. The mass was surgically removed and biopsy was performed. Result: Histopathological examination shows chronic inflammation and fibrotic change. After 6 month follow - up period, postoperative course was uneventful. Conclusion: We performed surgical excision of Morel - Lavalle lesion in a patient who had trauma 3 years ago. We report a case of Morel - Lavallee lesion with the review of the literatures.

A Clinical Experience of Nasopalatine Duct Cyst with Bony Defect (골결손을 동반한 비구개관 낭종의 치험례)

  • The nasopalatine duct cyst, known as the incisive canal cyst, is the most common nonodontogenic cyst in the maxillofacial area. It is believed to arise from epithelial remnants of the embryonic nasopalatine duct. Nasopalatine duct cysts are most often detected in patients between forties and sixties. The trauma, bacterial infection, or mucous retention has been suggested as etiological factors. The cysts often present as asymptomatic swelling of the palate but can present with painful swelling or drainage. Radiologic findings include a well demarcated cystic structure in a round, ovoid or heart shape presenting with a well-defined bone defect in the anterior midline of the palate between and posterior to the central incisors. Most of them are less than 2cm in size. On MRI, the cyst is identified as a high-intensity, well-marginated lesion, which indicates that it contains proteinaceous material. We experienced a case of a 61-year-old female patient who had a $2.3{\times}2.6{\times}1.7cm$ sized nasopalatine duct cyst. The bony defect after a surgical extirpation was restored with hydroxyapatite. So we report a good results with some reviews of the literatures.

A Large Malignant Peripheral Nerve Sheath Tumor in the Neurofibromatosis Patient: A Case Report (신경섬유종증 환자에 발생한 거대 악성말초신경초종의 치험례)

  • Purpose: Malignant peripheral nerve sheath tumors most often arise from the anatomically discernible peripheral nerve or neurofibroma. Methods: A 55-year-old man had a rapidly growing pedunculated large mass on the sacrolumbar junction for 2 years. He has congenital neurofibromatosis type I. He had multiple caf-au-lait spots and multiple neurofibromas on the entire body. The mass developed from a subcutaneous nodule on the sacrolumbar junction and grew rapidly. The preoperative punch biopsy revealed a malignant peripheral nerve tumor. The mass was completely excised with 1 cm free margin above the deep fascial plane. Results: There was no evidence of recurrence of tumor for 19 months of follow-up examination. Conclusion: Malignant peripheral nerve sheath tumor is very rare and has unique feature. We report a successful case of malignant peripheral nerve sheath tumor with the review of the literatures.
