Posterior decentering of the humeral head in patients with arthroscopic rotator cuff repair

  • Background: In some patients with rotator cuff tear (RCT), the axial view of magnetic resonance imaging (MRI) shows subtle posterior decentering (PD) of the humeral head from the glenoid fossa. This is considered to result from a loss of centralization that is typically produced by rotator cuff function. There are few reports on PD in RCT despite the common occurrence of posterior subluxation in degenerative joint disease. In this study, we investigated the effect of PD in arthroscopic rotator cuff repair (ARCR). Methods: We conducted a retrospective study of consecutive patients who underwent ARCR at our institute and were followed-up for at least 1 year. PD was identified as a 2-mm posterior shift of the humeral head relative to the glenoid fossa in the axial MRI view preoperatively. The tear size and fatty degeneration (FD, Goutallier classification) were also evaluated using preoperative MRI. Retears were evaluated through MRI at 1 year postoperatively. Results: We included 135 shoulders in this study. Ten instances of PD (including seven retears) were observed preoperatively. Fifteen retears (three and 12 retears in the small/medium and large/massive tear groups, respectively) were observed postoperatively. PD was significantly correlated with tear size, FD, and retear occurrence (p<0.01 each). The odds ratio for PD in retears was 34.1, which was greater than that for tear size ≥3 cm and FD grade ≥3. Conclusions: We concluded that large tear size and FD contribute to the occurrence of PD. Furthermore, PD could be a predictor of retear after ARCR.

자살사고에 영향을 미치는 위험요인 분석 : 국민건강영양조사 자료를 중심으로 (Analysis of Risk Factors on Affecting Suicidal Thoughts : Focusing on Korean national health and nutritional examination survey 2017)

  • Purpose : This study examined the relationship between suicidal thoughts, hand grip strength, socioeconomic status, educational level, and disease occurrence. Methods : Korean national health and nutrition examination survey 2017 were used in this study. 5,449 were analysed. For comparison between groups, cross-tabulation analysis and mean comparison were performed. Logistic regression analysis were performed to analyze the influencing factors between grip strength and suicidal ideation. Results : Our results are consistent with the literature on the importance of socioeconomic status in health. The lower the level of education, the higher the suicidal thoughts. Being single or divorced was also significantly associated with suicidal ideation. Moreover, a lower income level was significantly associated with a higher suicide intention. Furthermore, older ages, lower educational levels, and lower income were significantly associated with a higher odds ratio of suicidal thoughts. Interestingly, suicidal thoughts were significantly lower among non-smokers. In contrast, suicide intention did not differ significantly according to gender, age, monthly drinking habit, aerobic physical activity, and disease occurrence. Suicidal thoughts decreased as grip strength increased and this was statistically significant. Socioeconomic status, disease occurrence, and handgrip strength level affected the security of an individual's livelihood and were significant risk factors for suicidal thoughts. These associations remained significant in multiple logistic regression even after controlling for all covariates. Conclusion : Future prevention intervention efforts to reduce suicide risks should consider handgrip strength. Studies to explore the possible proximal risk factors and mediators between handgrip strength and suicidal thoughts are also warranted.

의사결정나무모형을 이용한 급경사지재해 예측프로그램 개발 및 적용 (Development and its APPLIcation of Computer Program for Slope Hazards Prediction using Decision Tree Model)

  • 본 연구에서는 화강암, 편마암 등 결정질암 지역에서의 급경사지재해 발생지역 및 미발생지역에 대한 현장조사자료 및 토질시험자료를 토대로 의사결정나무모형을 이용한 급경사지재해 예측모델을 개발하였다. 선정된 급경사지재해 예측모델의 분리기준은 최상위부터 사면경사, 투수계수 및 간극비로 선정되었다. 그리고 이를 토대로 GIS기법을 이용한 국가 주요시설물 주변 급경사지 재해 예측프로그램 SHAPP ver 1.0을 개발하였다. 개발된 예측모델 및 예측프로그램을 검증하기 위하여 강릉시 주문진읍 일대의 현장조사결과와 대상현장에 대한 예측결과를 비교 검토하였다. 검토결과 실제 급경사지 재해가 발생된 구간과 급경사지재해 예측구간이 유사하게 일치하고 있는 것으로 나타났다. 추후 지속적인 연구를 통하여 급경사지재해 예측 결과에 대한 정확도를 높이고, 이를 실용화하여 범용적으로 사용이 가능하도록 할 예정이다.

Spalling Properties of High Strength Concrete Mixed with Various Mineral Admixtures Subjected to Fire

  • This study investigates the spalling properties of high strength concrete designed with various types of mineral admixture and diverse content ratios of polypropylene (PP) fiber. Experimental factors considered in series I are four pozzolan types of mineral admixture and series II consists of three shrinkage reducing types of mineral admixture. PP fiber was added 0.05, 0.10 and 0.15vol. % in each mixture of series I and series II, so that totally 27 specimens including control concretes in each series were prepared. Test results showed that the increase of fiber content decreased the slump flow of fresh concrete and increased or decreased the air content depending on the declining ratio of slump flow. For the properties of compressive strength, all specimens were indicated at around 50 MPa, which is high strength range; especially all specimens in series II were 60 MPa. Fire test was conducted in standard heating curve of ISO 834 with ${\phi}100{\times}200\;mm$ size of cylinder moulds for 1 hour. The specimens incorporating silica fume exhibited severe spalling and most specimens without the silica fume could be protected from the spalling occurrence in only 0.05vol % of PP fiber content. This fire test results demonstrated that the spalling occurrence in high strength concrete was not only affected by concrete strength related to the porosity of microstructure but also, even more influenced by micro pore structure induced by the mineral admixtures.

  • This experimental study concerns the characteristics of a transitional flow in a concentric annulus with a diameter ratio of 0.52, whose outer cylinder is stationary and inner one rotating. The pressure losses and skin-friction coefficients have been measured for the fully developed flow of a 0.2 % aqueous solution of sodium carbomethyl cellulose (CMC) at a inner cylinder rotational speed of $0{\sim}600$ rpm. The transitional flow has been examined by the measurement of pressure losses, to reveal the relation of the Reynolds numbers with the skin-friction coefficients, in the laminar and transitional flow regimes. The occurrence of transition has been checked by the gradient change of pressure losses and skin-friction coefficient with respect to the Reynolds numbers. The increasing rate of skin-friction coefficient due to the rotation is uniform for laminar flow regime, whereas it is suddenly reduced for transitional flow regime and, then, it is gradually declined for turbulent flow regime. Consequently, the critical(axial-flow) Reynolds number decrease as the rotational speed increases. Thus, the rotation of inner cylinder promotes the early occurrence of transition due to the onset of taylor vortices.

신생사업장 재해율 및 재해발생형태 분석 (Estimation of Occupational Accident Rate about Start-up Establishment and Analysis of Characteristics about Occurrence)

  • In order for start-up establishment to enter the business market, it has to get over the first mover advantage of the existing establishment. As a result, its working condition deteriorates and a great deal of occupational accidents take place. Based on 2009 standard, the occupational accident rate of start-up establishment is 1.28%, which is two times higher than existing establishment and the gap between manufacturing and service industry has been getting wide. For the characteristic of injured person in start-up establishment, it is showed that the occupancy rate of women and people with less than 30 age starting out in a career is high and the occupancy rate of occupational accident for holiday and night work is high. Therefore, the act for occupational accident prevention is needed in the early period of business and underprivileged group for occupational accident.

불확실한 온도 조건하의 모형 강 판형교의 진동기반 손상 모니터링 (Vibration-Based Damage Monitoring in Model Plate-Girder Bridges under Uncertain Temperature Conditions)

  • A vibration-based damage-monitoring scheme is proposed that would generate an alarm showing the occurrence and location of damage under temperature-induced uncertainty conditions. Experiments on a model plate-girder bridge are described, for which a set of modal parameters was measured under uncertain temperature conditions. A damage-alarming model is formulated to statistically identify the occurrence of damage by recognizing the patterns of damage-driven changes in the natural frequencies of the test structure and by distinguishing temperature-induced off-limits. A damage index method based on the concept of modal strain energy is implemented in the test structure to predict the location of damage. In order to adjust for the temperature-induced changes in the natural frequencies that are used for damage detection, a set of empirical frequency correction formulas is analyzed from the relationship between the temperature and frequency ratio.

The Occurrence of Griffithsia okiensis (Ceramiaceae, Rhodophyta) from Korea on the Basis of Morphology and Molecular Data

  • Despite continued studies on red algal flora in Korea, the taxonomy of the tiny ceramiaceous algae has received little attention. We report for the first time Griffithsia okiensis from Korea on the basis of morphology and molecular data. The species is small in thalli height (0.3-1.5 cm), and in diameter of vegetative cells (50-500 μm), and the ratio of cell length/breadth is 2-3 times. It has two carpogonial branches from the supporting cell of procarp. We generated psbA and rbcL sequences from ten specimens of G. okiensis isolated from Korea and Japan and from one G. japonica species isolated Japan. Eight specimens of G. okiensis from Korea were almost identical in both psbA and rbcL regions, nevertheless they differed from Japanese specimens by 4 ucleotides in psbA and 7 in rbcL. In all analyses of psbA, rbcL, and psbA + rbcL data sets, G. okiensis was determined to be a different species from G. japonica isolated from Japan, although both species showed a sister relationship. For all that extensive collection trips, we found no evidence for the occurrence of G. japonica in Korea.

The flow field of an annulus has been numerically solved using a finite volume method. The pressure losses and Skin-friction coefficients have been measured for the fully developed flow of water and $0.2{\%}$ aqueous solution of sodium carboximethy1 cellulose (CMC), respectively at inner cylinder rotational speed of $0{\~}600rpm$. The transitional flow has been examined by the measurement of pressure losses to reveal the relation of the Reynolds and Rossby numbers with the skin-friction coefficients. The occurrence of transition has been checked by the gradient changes of pressure losses and skin-friction coefficients with respect to the Reynolds numbers. Consequently the critical(axial-flow) Reynolds number decreases as the rotational speed increases. Thus, the rotation of inner cylinder promotes the early occurrence of transition due to the onset of Taylor vortices.

안쪽축이 회전하는 환형관내 헬리컬 유동장의 실험적 연구 (Experimental Study on the Helical Flow Field in a Concentric Annulus with Rotating Inner Cylinders)

The transitional flow has been examined by the measurement of pressure drops and the visualization of flow field, to reveal the relation of the Reynolds and Rossby numbers with the skin-friction coefficients and to understand the flow instability mechanism. The present results show that the skin-friction coefficients have the significant relation with the Rossby numbers, only for laminar regime. The occurrence of transition has been checked by the gradient changes of pressure drops and skin-friction coefficients with respect to the Reynolds numbers. The increasing rate of skin-friction coefficient due to the rotation is uniform for laminar flow regime, whereas it is suddenly reduced for transitional flow regime and, then, it is gradually declined for turbulent flow regime. Consequently, the critical (axial-flow) Reynolds number decreases as the rotational speed increases. Thus, the rotation of inner cylinder promotes the early occurrence of transition due to the excitation of taylor vortices.
