Background: Ovarian cancer is ranked as the fifth most common cause of cancer death in women. In Malaysia, it is the fourth most common cancer in females. CA125 has been the tumor marker of choice in ovarian cancer but its diagnostic specificity in early stages is only 50%. Hence, there is a critical need to identify an alternative tumor marker that is capable of detecting detect ovarian cancer at an early stage. HE4 is a new tumor marker proposed for the early diagnosis of ovarian cancer and disease recurrence. Currently, none of the normal ranges of HE4 quoted in the literature are based on data for a multiethnic Asian population. Therefore, the aim of this study was to determine reference intervals for HE4 in an Asian population presenting in University Malaya Medical Centre, a tertiary reference hospital. Materials and Methods: 300 healthy women were recruited comprising 150 premenopausal and 150 postmenopausal women, aged from 20-76 years. All women were subjected to a pelvic ultrasonograph and were confirmed to be free from ovarian pathology on recruitment. Serum HE4 levels were determined by chemiluminescent microparticle immunoassay (CMIA, Abbott Architect). The reference intervals were determined following CLSI guidelines (C28-A2) using a non-parametric method. Results: The upper limits of the $95^{th}$ percentile reference interval (90%CI) for all the women collectively were 64.6 pmol/L, and 58.4 pmol/L for premenopausal) and 69.0 pmol/L for postmenopausal. The concentration of HE4 was noted to increase with age especially in women who were more than 50 years old. We also noted that our proposed reference limit was lower compared to the level given by manufacturer Abbott Architect HE4 kit insert (58.4 vs 70 pmol/L for premenopausal group and 69.0 vs 140 pmol/L in the postmenopausal group). The study also showed a significant difference in HE4 concentrations between ethnic groups (Malays and Indians). The levels of HE4 in Indians appeared higher than in Malays (p<0.05), while no significant differences were noted between the Malays and Chinese ethnic groups. Conclusions: More data are needed to establish a reference interval that will better represent the multiethnic Malaysian population. Probably a larger sampling size of equal representation of the Malay, Chinese, Indians as well as the other native ethnic communities will give us a greater confidence on whether genetics plays a role in reference interval determination.
This study compared FTIR with XRD method for the analysis of quartz by % recovery, coefficient of variation (CV) and influence of the interference. the results were as the following. 1. In FTIR method, the coefficient of determination ($r^2$) was 0.9998 in a calibration curve of $695\;cm^{-1}$, and the limit of detection was $4.9{\mu}g/sample$. 2. The highest recovery was $799\;cm^{-1}$ (98.2%). 3. The CVpooled of the FTIR method was approximately 10% in three wave numbers. 4. The analysis of qualitative and quantitative for quartz is difficult with mixed cristobalite and iron oxide. 5. In XRD method with rotating sample holder and LynxEye detector, the coefficient of determination was 0.9996 in a calibration curve, and the limit of detection was $5.9{\mu}g/sample$. 6. The recovery and CV pooled were 104.3%, and 11 %, respectively. 7. In muffle furnace ashing, the quartz weight decreased to 34% when the maximum weight of the iron oxide was more than eight times. In conclusion, the accuracy (% recovery) and precision (CV) of FTIR and XRD method for analyzing $\alpha$-quartz were similar. FTIR method was a disadvantage for sample matrix because it indicates possibility of interference. However, XRD method distinguished specific crystalline forms of silica, and the majority of silicate minerals. In addition, XRD method recommend filter dissolution to pretreatment method.

Status of benzene exposure was evaluated upon petrochemical workers in the Yeosu Industrial Complex, based on questionnaire responses by workers, review of previous work environment assessment reports between 1996 and 2004, in addition to short-term exposure measurements for unit tasks in the field. Questionnaire results showed that workers in the field were suffering concern on and symptoms of respiratory diseases, cancers or other illnesses of unknown causes. In particular, workers were highly worried about the risk of exposure to chemical hazards including benzene, while conducting specific tasks (e.g. sampling, draining, gauging) among normal operations during which equipments are opened and contents are exposedin the air, as well as periodic turnaround (TA) task. However, the review of previous work environment assessment reports found out that short-term exposure assessment data for unit tasks accounted for less than 1% of total data, and most of them were on 8-hr average exposure level. It also turned out that benzene was not detected in 83% of total samples, suggesting routine but pointless sampling has been repeated. Short-term exposure level was assessed focusing on tasks with high level of complaints on the exposure risk, based on the questionnaire responses. As predicted, a maximum of 741 ppm benzene exposure was reached depending on types and conditions of operations involved. In conclusion, these findings suggest that the evaluation and management on the short-term high exposure tasks including turnaround are crucial in reducing benzene exposure and preventing haematopoietic cancer in workers in the petrochemical industry.
Background: As of now, the impact of low back pain (LBP) and its chronic state, chronic low back pain (CLBP), on mental health-related quality of life (HRQOL) has never been investigated among police officers. The present investigation aims at studying this relationship using a biopsychosocial model. Methods: Between May and October 2014, a Web-based cross-sectional study was conducted among Quebec police officers (Quebec, Canada). Mental HRQOL was measured using the role emotional (RE) and the mental health (MH) domains of the SF-12v2 Health Survey. The impact of CLBP on mental HRQOL (as opposed to acute/subacute LBP or no LBP) was studied with a multivariate linear regression model. Results: Of the 3,589 police officers who participated in the study, 1,013 (28.4%) reported CLBP. The mean age of respondents was $38.5{\pm}8.7years$, and 32.0% were females. The RE (44.1/100) and MH (49.0/100) mean scores of the CLBP group were comparable with the scores found in populations suffering from cancer or heart diseases. Compared to officers without LBP, the presence of CLBP was significantly associated with lower RE (${\beta}$: -0.068; p = 0.003) and MH (${\beta}$: -0.062; p = 0.002) scores. These relationships were not found in the acute/subacute LBP group. Conclusion: Our results underscore how frequent CLBP is among police officers and how burdensome it is. Considering the importance of good physical and mental health for this occupational population, police organizations should be aware of this issue and contribute to the efforts toward CLBP prevention and management in the workplace.
Objectives: In Korea, the system of management of medical waste largely relies on the incineration method. Our study aimed to identify any regional imbalances between the generation and treatment of medical waste, and provide reference data for future policy in Korea. Methods: We analyzed data on the amount of medical waste from 2,000 hospitals in 2012, 2013, and 2014 obtained from the Korea Environment Corporation. In the Arc GIS program (version 10.2.3), the addresses of hospitals and incinerators were transformed into map coordinates. The amount of waste generated by each hospital and the amount incinerated were represented by density and size of a triangle symbol, respectively. Results: As of 2014, the total amount of medical waste from the top 2,000 hospitals was 136,073 tons, out of which about half (49%) was generated in the capital area. Eleven incineration companies (three in the capital area, two in the Chungcheong Provinces area, one in the Jeolla Provinces area, and five in the Gyeongsang Provinces area) treated this waste. For the years 2012, 2013, and 2014, about 60% of the medical waste generated from the hospitals in the capital area was treated within the capital area and about 40% was transported to other areas, especially the Gyeongsang Provinces area, for treatment. On the other hand, about 90% of the medical waste incinerated in the capital area originated from the capital area. Conclusion: Our results suggest a spatial imbalance between the generation and treatment of medical waste in Korea and warrants multilateral policies, including the expansion of on-site treatment, strengthening regulation of the containment of medical wastes, promoting reductions in medical waste and more.

The objectives of this study were to evaluate the air quality surrounding an indoor swimming pool, to estimate the cancer risk based on the airborne exposure to trihalomethanes (THMs), and to examine the ventilation efficiency by Computational Fluid Dynamics (CFD). Chlorine and THMs were measured poolside, and in the staff room and reception area. The indoor swimming pool was modeled using the Airpak program, with ventilation drawings and actual survey data. Temperature, flow and mean age of the air were analyzed. Levels of chlorine poolside, and in the staff room, and reception area were $203\;{\mu}g/m^3$, $5\;{\mu}g/m^3$, and $10\;{\mu}g/m^3$, respectively. Chloroform was the dominant THM in all sampling sites and mean concentrations were $16.30\;{\mu}g/m^3$, $0.51\;{\mu}g/m^3$, and $0.06\;{\mu}g/m^3$ poolside, in the staff room and reception area, respectively. Bromodichloromethane and Dibromochloromethane levels were respectively estimated as $10.3\;{\mu}g/m^3$ and $1.7\;{\mu}g/m^3$ poolside, $1.3\;{\mu}g/m^3$ and $0.1\;{\mu}g/m^3$ in the staff room, and were not detected in the reception area. The cancer risks from inhalation exposure to THMs were estimated between $3.37{\times}10^{-7}$ and $1.84{\times}10^{-5}$. A short circulation phenomenon was observed from the supply air vents to the exhaust air vents located in the ceiling. A high temperature layer was formed within one meter of the ceiling, and a low temperature layer was formed under this layer due to the low velocity and high temperature of the supply air, and the improper locations of the supply air vents and exhaust air vents. The stagnation was evident at the above adult pool and the mean age of the air was 22 minutes. Disinfection by-products in the indoor swimming pool were present in higher concentrations than in the outdoor air. In order to increase the removal of pollutants, adjustment was required of the supply air volume and the supply/exhaust position.
Promoter hypermethylation of the $p16^{INK4a}$ gene was investigated in 52 sets of samples of tumor tissue and adjacent normal tissue from Korean patients with colorectal cancer, using the proposed modified the Real-time PCR/SYBR Green detection method presented in this study. In normal tissue, 29 of 52 patients (56%) were methylated and in tumor tissue, 23 of 52 patients (44%) were methylated. The 34 cases (65.4%) showed a concordant DNA methylation pattern in both normal tissue and tumor tissue. Analyzing the association between the clinicopathologic features and DNA methylation status of the $p16^{INK4a}$ gene, the DNA methylation status according to by Duke's stage was different while other clinicopathological characteristics, including the age, sex, tumor stage, and histologic type of the patient were not found to be correlated with $p16^{INK4a}$ methylation. With multivariate logistic regression, it was observed that the DNA methylation status of $p16^{INK4a}$ gene in normal tissue was correlated with the DNA methylation status of the $p16^{INK4a}$ gene in tumor tissue (P=0.026). According to a Kaplan-Meier survival analysis, a difference in the survival rate by DNA methylation status was found, but it was not significant.
The increased risk of cancer with exposure to low-dose radiation was estimated through longitudinal study for radiation workers at the nuclear power plants in Korea. The radiation dose data were collected from the Radiation Safety Management System(RSMS) of the Korea Hydro & Nuclear Power Co., Ltd(KHNP). The cancer risks with radiation exposure were evaluated in terms of relative mortality ratios(RMR) and relative incidence ratios(RIR) to the unexposed employees at the nuclear power plants, and of the standardized mortality ratios(SMR) and standardized incidence ratios(SIR). There were no significant increases of canters of all sites in the exposed group either in RIR[1.08, 95% confidence interval(CI) 0.74-1.58] or in RMR[1.21, CI 0.70-2.08]. Neither SIR[0.81, CI 0.28-0.96] nor SMR[0.86, CI 0.66-1.10] significantly deviated from 1.0 for cancers of all sites. The trend analysis did not identify evident dose-response relationship due to insufficient numbers of the cases. Consequently, it is concluded that increases in cancer risks in the radiation worker group exposed to low doses at the nuclear power plants in Korea are not identified at this time.
Background: Beedi rollers are exposed to unburnt tobacco dust through cutaneous and pharyngeal route and it is extremely harmful to the body since it is carcinogenic in nature and can cause cancer during long exposure. This indicates that occupational exposure to tobacco imposes considerable genotoxicity among beedi workers. Materials and Methods: In the present study, 27 beedi workers and age and sex matched controls were enrolled for clinical, cytogenetics and molecular analysis. Clinical features were recorded. The workers were in the age group of 28-67 years and were workers exposure from 8-60 years. Blood samples were collected from workers and control subjects and lymphocyte cultures were carried out by using standard technique, slides were prepared and 50 metaphases were scored for each sample to find the chromosomal abnormalities. For molecular analysis the genomic DNA was extracted from peripheral blood, to screen the variations in gene, the exon 1 of CYP1A1 gene was amplified by polymerase chain reaction (PCR) and then screened with Single Strand Conformation Polymorphism (SSCP) analysis. Results: A statistically significant increase was observed in the frequencies of chromosomal aberrations in exposed groups when compared to the respective controls and variations observed in Exon 1 of CYP1A1(Cytochrome P450, family 1, subfamily A, polypeptide 1) gene. Conclusions: This study shows that, the toxicants present in the beedi that enter into human body causes disturbance to normal state and behavior of the chromosomes which results in reshuffling of hereditary material causing chromosomal aberrations and genomic variations.

This study was performed to evaluate tannin exposure by wooddusts for workers in furniture factories and to investigate the relationship between tannin exposure and sino-nasal cancer risk. In order to explore possible cytological changes leading to nasal cancer, we have examined 50 male furniture workers and 50 matched controls using brush cytology. The results we have obtained in this study were as follows: 1. The tannin contents of woods used in woodworking factories have been measured and varied from 0.43 to 8.72 mg tannic acid equivalent per gram wood, for reconstituted softwood and turpentine (Syncarpia glomuliferia) respectively. 2. Airborne tannins in wooddusts were also determined by area and personal exposure. The values of mean exposures for both methods are ranged from 3.1 to $5.0{\mu}g/m^3$ and from 4.6 to $14.5{\mu}g/m^3$ in furniture manufactures. 3. Over nasal cytology scores 2, the scores of study group were slightly more than control group and this kind of metaplasias seemed to be occurred over $2mg/m^3$ wooddust and $6{\mu}g/m^3$ tannin exposure. Keratinising squamous metaplasia was investigated at nasal cytology score 3 and $10{\mu}g/m^3$ tannin exposure. The nasal cytology score 4 was seemed to be atypical squamous metaplasia. 4. To find out contributing factors to nasal cytology change, odds ratio that is one of fundmental biostatistics was applied. Actually the relationship between wooddust, tannin concentration and metaplasia were not meaningful, but the relationship between working experence more than 15 years and metaplasia was calculated as 1.83. This reveals that significant clinical abnormalities could be influenced from the years of woodworking experiences. However further research is required to evaluate the significance of the data, for the purposes of sino-nasal risk assessment, standard setting to prevent nasal cancer occurrences and possibility of changing workplace.
