A Study on System-Based Accident Analysis : An Accident at In-house Subcontractor of a Manufacturing Company (제조업 사업장 사내협력업체 사고사례의 시스템적 분석에 관한 연구)

  • In this study, an accident at an in-house maintenance subcontractor of a manufacturing company was analyzed using representative systemic analysis methods, and the results were compared to determine the socio-technical and organizational structure causal factors. Systemic accident analyses were performed using AcciMap, STAMP-CAST, and a method that utilizes work processing procedures. The causal factors derived from the three methods were classified according to HFACS classification criteria. AcciMap and STAMP-CAST analyses were able to derive legal problems and defects in organizational structure between the company and the subcontractors. The method that utilized the work processing procedures drew the most causal factors of the three methods but showed some limitations in deriving legal and facility-related problems. Most of the causal factors identified through the systemic methods could be classified according to the HFACS classification criteria, except for the legal and organizational structure matters. Socio-technical and organizational problems with a holistic perspective of the company and subcontractors could be found using systemic analysis methods. However, it is necessary to conduct analysis using various methods in order to derive more comprehensive measures to prevent accidents because each analysis method showed some limitations in the derivation or expression of some causal factors. The results of this study can be helpful in selecting and using an appropriate method for accident analysis.

A Case Study on the Human Error Analysis for the Prevention of Converter Furnace Accidents (전로사고 예방을 위한 인적오류 분석)

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.

  • This study delivers the actual condition of investment for industrial accident prevention based on survey of 500 construction sites from 'reports far industry safety and health' published by Korea Occupational Safety & Health Agency (KOSHA). The various research techniques were used such as technical statistic analysis for construction industry, cost comparison of industrial accident prevention and accident loss. A formula was deduced to calculate accident loss and accident frequency by accident prevention cost through regression analysis.

Analysis of Injury Characteristics of Elderly Workers in Small Manufacturing Factory (소규모 사업장의 고령자 재해특성에 대한 분석)

  • Korea is becoming an aged society as well developed country. Accident rates of elderly workers are increased. When elderly workers injured, elderly workers are more likely to die as a result, and they take longer to recover than young workers. Thus the cost to industry per individual accident gets higher for elderly workers. If information on the occupational accidents of elderly workers could be collected and analyzed for the purpose of preventing occupational accidents, we would be able to get rid of accidents of elderly workers. The accidents of elderly workers in small manufacturing factories were considered in this study. 97 accidents, which occurred in 1995-1998, were investigated. These accidents were analyzed in terms of sex, work period, cause of accident and form of accident. The accidents of elderly workers in small manufacturing factories are numerous and are often serious and worthy of greater attention than they have received. Successful strategies for accident prevention depend on effective analysis.

  • Even if several chemical accident prevention systems such as PSM(Process Safety Management), RMP(Risk Management Plan), etc. have been carried out, many chemical accidents have still occurred at industrial plants in Korea. We describe the status of chemical industry and the trend of occurrence of chemical accidents in Korea. And this paper analyzes the recent chemical accidents in eight ways. These ways include chemical accident forms, ignition sources, sources of chemical equipment, human vs equipment/material causes, worker's working situation, employee scale, hazardous substances, week & time, fatalities of manufacture & contractor's workers. Finally we proposes the four representative prevention measures brought to result of cause analysis by accident statistics.

  • After the financial crisis, the spread of indirect employment, which is an outsourcing method, is gradually becoming common in our society, and the risk of harm is relatively high or the proportion of simple jobs is high. Simple workers have a higher proportion of non-manufacturing industries such as the service industry, and production workers are intensively used in the manufacturing industry. In Article 63 of the Occupational Safety and Health Act, contractors must take necessary safety and health measures. In addition, due to the expansion of the scope of contracts under the Occupational Safety and Health Act, it is necessary to review the appropriateness of industrial accident prevention measures and implementation standards.

The Quantitative Assessment of Occupational Accident Reduction by the Injury Ratio Survey Regulations (건설업 재해율 조사의 정량적 산업재해 감소효과)

  • Injury Ratio Survey Regulations(IRS) was introduced to the construction industry in the Republic of Korea since 1992 and brought positive effect on occupational accidents reduction. There were tremendous decrease of injury ratios and enforcing of contractors' safety organizations from the beginning of IRS. In spite of these positive results, there were some negative effects such as contractors' shrinking injury reports to keep good injury ratios since these figures had a great impact on pre-qualification stage of bidding when general contractors were competing for new construction projects. Thus, this study aims to devote on lessening construction injury and elimination of above negative impacts through the quantitative statistic analysis of the effectiveness on the occupational accidents prevention of IRS. According to this assessment, there were decrease of from 6.37% to 44.34% in the accident ratios compared to those of non-IRS groups and decrease of from 3.32% to 83.51 % in the accident ratios compared to those of general industry including the unreported accidents.

  • Risk assessment techniques are processes used to evaluate hazardous risk factors in construction sites, facilities, raw materials, machinery, and equipment, and to estimate the size of risk that could lead to injury or disease, and establish countermeasures. The most important thing in assessing risk is calculating the size of the risk. If the size of the risk cannot be calculated objectively and quantitatively, all members who participated in the evaluation would passively engage in establishing and implementing appropriate measures. Therefore, this study focused on predicting accidents that are expected to occur in the future based on past occupational accident statistics, and quantifying the size of the risk in an overview. The technique employed in this study differs from other risk assessment techniques in that the subjective elements of evaluators were excluded as much as possible by utilizing past occupational accident statistics. This study aims to calculate the size of the risk, regardless of evaluators, such as a manager, supervisor, safety manager, or employee. The size of the risk is the combination of the likelihood and severity of an accident. In this study, the likelihood of an accident was evaluated using the theory of Bud Accident Chainability, and the severity of an accident was calculated using the occupational accident statistics over the past five years according to the accident classification by the International Labor Organization.

The Development of the Analysis Program for the Electric Shock Accidents Occurred in the TN and TT System (TN 및 TT 계통에서의 감전사고 분석 프로그램 개발)

  • According to the statistics from the Ministry of labor, 70~80 people died by work-related electric shock accidents in the workplace and this number is about 10~20 times higher than those of the foreign countries such as Japan and the United Kingdom in frequencies and rates per 100,000. The electrocution deaths had decreased from several hundred in 1990s to 67 in 2002, since then there has been little change over the last 5 years. To reduce this number of the electrocution death, it is necessary to analyze the sources of the accidents and to implement the suitable measures for preventing the similar accidents from occurring. But there is not any consensus standard method or procedures for accident investigation yet. And the results solely depend on the ability of the investigator. Because of the demand of eliminating technical barrier for free trading, the IEC 60364 is adopted as a regulation for low voltage electrical installations in Korea. The facilities designed and constructed by IEC 60364 are increasing. The facilities by IEC 60364 have different features in comparison with the facilities by Korea eletro technical regulation, and these features combined with parameters of electric shock accident can greatly affect the consequences of the electric accidents. The accident analysis program for electric shock injuries in Terra Neutral(TN) and Terra Terra(TT) system was developed in this paper, which was based on KOSHA Electric Shock Analysis Program(ESAP).
