Exploring the Contributory Factors of Confined Space Accidents Using Accident Investigation Reports and Semistructured Interviews

  • Background: The oil and gas industry is one of the riskiest industries for confined space injuries. This study aimed to understand an overall picture of the causal factors of confined space accidents through analyzing accident reports and the use of a qualitative approach. Methods: Twenty-one fatal occupational accidents were analyzed according to the Human Factors Analysis and Classification System approach. Furthermore, thirty-three semistructured interviews were conducted with employees in different roles to capture their experiences regarding the contributory factors. The content analyses of the interview transcripts were conducted using MAXQDA software. Results: Based on accident reports, the largest proportions of causal factors (77%) were attributed to the organizational and supervisory levels, with the predominant influence of the organizational process. We identified 25 contributory factors in confined space accidents that were causal factors outside of the original Human Factors Analysis and Classification System framework. Therefore, modifications were made to deal with factors outside the organization and newly explored causal factors at the organizational level. External Influences as the fifth level considered contributory factors beyond the organization including Laws, Regulations and Standards, Government Policies, Political Influences, and Economic Status categories. Moreover, Contracting/Contract Management and Emergency Management were two extra categories identified at the organizational level. Conclusions: Preventing confined space accidents requires addressing issues from the organizational to operator level and external influences beyond the organization. The recommended modifications provide a basis for accident investigation and risk analysis, which may be applicable across a broad range of industries and accident types.

  • The modem industrial society is developing while growing more diverse and gigantic. Accordingly, occupational injuries or accidents can be caused in various situations, not just in the limited range of workplaces but also in the surroundings, and interest has increased in the prevention of occupational accidents with respect to occupational health and safety, and environment. Thus, this thesis will consider 4MlE (Man, Machine, Method, Material, Environment) as the fundamental causes of accidents and introduce a model of system in which the output of the process control system is replaced by accidents with its input by 4M1E. Furthermore, it will demonstrate how occupational hazardousness can be measured, whereby it can also be rated, by examining the relationship between 4M1E and types of accident in terms of the categories of severity, frequency, and detectability, based on the application of the model to the framework of FMEA.

  • Automobile-related industries have been a few of leading ones among domestic industries reporting high rates of musculoskeletal disorders (WMSD's). In this paper, major ergonomic aspects of automobile-related works were reviewed with reference to WMSD's. According to the result, high repeatability of works with as short as 1 minute or less, awkward postures required, excessive forces, and vibrations due to power tools were drawn out as major physical factors. To eliminate or mitigate those factors - at least in automobile-related industries - ergonomic approach has tried for more than a decade. With all, however, ergonomists still seem to confront with several problems to be solved such as development of appropriate assessment tools, enhancement of work improvement activities, system establishment for continuous prevention and management of WMSD's. As lots of previous researches declared, it was concluded that ergonomic approach would collaborate with other approaches such as ndustrial medicine considering physical factors as well as psychosocial factors, and that the necessity of an integrated Occupational Safety and Health Management System(OHSMS) was mentioned.

제조업 사업장에서 위험성평가가 근로자 안전행동 수준에 미치는 영향 (The Effect of Risk Assessment on Employee Safety Behavior in Manufacturing Workplaces)

  • Objectives: This study was designed to examine the effect of risk assessment on employee safety behavior in manufacturing workplaces. In addition, this study attempted to explore factors related to the occupational safety and health system in the workplace affect the risk assessment of manufacturing sites. Methods: This research is a cross-sectional study using the Korea Occupational Safety and Health Agency's 2018 Occupational Safety and Health data. The sample for study is 1,967 manufacturing workplaces. Data were analyzed using descriptive statistics, t-test, ANOVA, chi-square test, and hierarchical multiple regression analysis using SPSS (ver.25.0). Results: As a result of the multiple hierarchical regression analysis, it was found that risk assessment had an effect on employee safety behavior (t=4.435, p=<.001). Furthermore, the size of the workplace affected employee safety behavior (t=2.494, p=<.001). In addition, the presence of safety and health management organizations affected employee safety behavior (t=4.301, p=<.001). The factors of the safety and health organization (𝑥2=35.245, p=<.001), the occupational safety and health committee (𝑥2=149.440, p=<.001), and the supervisor (𝑥2=16.472, p=<.001) were identified as factors that increased the possibility of risk assessment in the manufacturing workplaces. Conclusions: In this study, it was found that risk assessment is a factor that increases the level of workers' safety behavior in manufacturing workplaces. Therefore, it is necessary to provide institutional support for activating risk assessment at manufacturing workplaces.

AHP를 이용한 산업안전 교과목 선정에 대한 연구 (An Selection of Occupational Safety Curriculum using Analytic Hierarchy Process)

  • A significant number of all reported accidents in Korea involved education and training Based on the Labor Ministry data, statistics, show the education cause was directly involved in 38% of major accidents, Since safety managers have in charge of safety education at the factory, occupational safety curriculum must be studied to train safety manager effectively. In this study an analysis on relative importance of industrial safety curriculum for a good safety manager was performed by Analytic Hierarchy Process(AHP) The results showed that relative importance of industrial safety curriculum by AHP was safety management(w=0.240) ergonomics and system safety(w=0.201) construction safety(w=0.182) mechanic safety(w=0.139) electronic safety(w=0.134) and chemical safety(w=0.104) in order. The results could be used for industrial safety curriculum planning in university and safety organization such as Korea Occupational safety and Health Agency.

항공기의 소독작업 및 좌석커버 교체작업에서 1-브로모프로판 노출평가 사례 (Case Study of 1-bromopropane Exposure Assessment During Aircraft Disinfection and Seat Cover Replacement Work)

  • Objectives: To evaluate the levels of exposure to organic compounds during aircraft disinfection and seat cover replacement operations. Methods: According to the working schedule, organic compounds were collected using activated carbon tubes and then analyzed by GC/FID and GC/MSD. Results: In the disinfection task, the main substances listed in the material safety data sheet (MSDS) of the disinfectant were not detected. However, 1-bromopropane, which had been used in the previous task of replacing seat covers, was detected at a level of 2.37 ppm at the measurement time. During seat cover replacement, bonding workers were exposed to 2.48 ppm on an eight-hour time-weighted average, and seat cover replacement workers were exposed to 0.22 ppm. Conclusions: It is necessary to ensure the reliability of MSDS. A work environment management system is necessary when different companies alternate working in the same place.

건설공사 발주 공공기관의 안전점검 체계구축에 관한 연구 (Establishment of a Safety Inspection System for Public Institutions Ordered Construction Projects)

  • Public institutions have a responsibility to ensure the safety of their employees and the public. One way to do this is to implement a systematic safety inspection system based on risk assessments and continuous improvements. This study developed a systematic safety inspection system for public institutions that are ordered construction projects. The proposed system in this study consists of a three-step process: (1) developing safety grade evaluation tables, (2) preparing and conducting safety inspections, and (3) evaluating and improving safety management grades. The first step is to develop safety grade evaluation tables by analysis and diagnosis of the construction site's work type, disaster statistics, and related laws. The second step is to conduct safety inspections using the developed evaluation tables. The third step is to determine the safety management grade based on the results of the safety inspection, and to improve risk factors found during the safety evaluation. The proposed system was implemented in highway construction projects carried out by public institutions. The results showed that the proposed system has two major effects: (1) reducing accident-related deaths and injuries, (2) improving safety management levels by continuous evaluation and improvement. The proposed system can be utilized in construction projects ordered by public institutions to improve the level of occupational safety and health.

소규모 사업장 재난안전 프로그램 개발 및 산업안전보건법에의 도입방안: 화재, 폭발, 약품 누출을 중심으로 (Development of Disaster Safety Program for Small-sized Workplaces and Its Introduction to Occupational Safety&Health Act: Focusing on Fire, Explosion and Chemical Spill)

  • 국내 기업에서 발생하는 사고의 98.8%가 50인 미만의 소규모 사업장에 편중되어 있다. 따라서 본 연구에서는 산업안전 및 시설 관련 법령과 활발하게 활용되고 있는 안전관리시스템을 상호 비교분석하여 중소기업에서 이행할 수 있는 재난안전 프로그램을 개발하였다. 재난안전 프로그램에는 적용범위, 조직의 구성, 위험성평가, 비상대응체계, 교육 및 훈련, 안전용품 구비 및 프로그램 평가 등의 내용을 포함하였으며, 각 항목별로 작성방법과 절차를 상세하게 수록하였다. 또한 재난안전 프로그램 운용 시 유의해야 할 사항과 운영 전략을 제시하였다. 이후 연구결과를 산업안전보건법에 도입하는 방안을 제언하였다.

  • This study examined the certification effects of safety and health management system (SHMS) on the establishment level of SHMS and accident statistics in construction industry. This study obtained the establishment level of SHMS for 106 construction companies surveyed from our previous study. In addition, three major accident statistics (mortality rate, accidental mortality rate, and injury rate) for the companies were collected from the database in Korean Occupational Safety and Health Agency. The statistical analysis results revealed that the establishment level for SHMS certified companies was significantly higher than those for uncertified or certification preparing companies. Furthermore, SHMS certified companies showed significantly smaller accident statistics compared to uncertified or certification preparing companies. The results of this study support the positive effects of SHMS on reducing major industrial accidents in construction companies.

Social Distancing and Public Health Guidelines at Workplaces in Korea: Responses to Coronavirus Disease-19

  • Background: In the absence of a vaccine or treatment, the most pragmatic strategies against an infectious disease pandemic are extensive early detection testing and social distancing. This study aimed to summarize public and workplace responses to Coronavirus Disease-19 (COVID-19) and show how the Korean system has operated during the COVID-19 pandemic. Method: Daily briefings from the Korean Center for Disease Control and the Central Disaster Management Headquarters were assembled from January 20 to May 15, 2020. Results: By May 15, 2020, 11,018 COVID-19 cases were identified, of which 15.7% occurred in workplaces such as health-care facilities, call centers, sports clubs, coin karaoke, and nightlife destinations. When the first confirmed case was diagnosed, the Korean Center for Disease Control and Central Disaster Management Headquarters responded quickly, emphasizing early detection with numerous tests and a social distancing policy. This slowed the spread of infection without intensive containment, shut down, or mitigation interventions. After entering the public health blue alert level, a business continuity plan was distributed. After entering the orange level, the Ministry of Employment and Labor developed workplace guidelines for COVID-19 consisting of social distancing, flexible working schedules, early identification of workers with suspected infections, and disinfection of workplaces. Owing to the intensive workplace social distancing policy, workplaces remained safe with only small sporadic group infections. Conclusion: The workplace social distancing policy with timely implementation of specific guidelines was a key to preventing a large outbreak of COVID-19 in Korean workplaces. However, sporadic incidents of COVID-19 are still ongoing, and risk assessment in vulnerable workplaces should be continued.
