Conversion of implant overdenture to an implant assisted removable partial denture in maxilla: case report (상악 임플란트 피개의치에서 임플란트 보조 국소의치로의 전환: 증례보고)

  • The long-term use of an implant overdenture shows problems, such as wear of attachment, concentrated occlusal forces on the anterior teeth, fracture of artificial teeth and rotation of denture. By transitioning to an Implant-assisted removable partial denture (IARPD) using additional implant placement, the problems can be solved. In this case report, a transition was made from implant overdenture to IARPD utilizing surveyed crowns to distribute occlusal forces concentrated on anterior teeth and to prevent denture rotation in a skeletal Class III patient. Design of definitive prosthesis with appropriate function and aesthetics was determined through several stages of provisional restorations. In this case, appropriate posterior occlusion and maintenance of peri-implant bone level of definitive prosthesis were observed.

  • Excessive wear causes many complications when untreated, so that accurate diagnosis, analysis and predictive treatment plan should be made, and through evaluation of vertical dimension and stepwise treatment, a stable inter-arch relationship can be set. For the long-term success of implant treatment, ideal position and angle of implant is important, and its importance increases especially in multiple implant cases. Therefore, thorough diagnosis and planning, accurate surgery and prosthodontic procedures are significant. In this case, a 68-year-old male patient with a loss of vertical dimension due to multiple tooth loss and overall tooth wear was planned with systematic analyses from the pre-treatment stage to rehabilitate vertical dimension. Full-mouth fixed rehabilitation with computer tomography guided implant surgery was performed to the newly set vertical dimension and attained satisfactory outcomes both functionally and esthetically.

  • Severe tooth wear may lead to pathological changes of pulp, imbalance in occlusion as well as functional and esthetic problems. In this case, 34-year-old male came to the hospital because of generally worn dentition due to attrition and erosion. After evaluation, a full mouth restoration with elevation of the vertical dimension of occlusion was planned. After occlusion was stabilized by an occlusal stabilization appliance, centric relation position was recorded and subsequent provisional restorations were fabricated. After evaluation, a CAD-CAM (computer aided design-computer aided manufacturing) prosthetic restoration was carried out using monolithic zirconia. After 12 months of follow up observation, the patient was satisfied with function and esthetic appearance.

  • PURPOSE. The aim of this in vitro study was to investigate the fracture resistance under chewing simulation of implant-supported posterior restorations (crowns cemented to hybrid-abutments) made of different all-ceramic materials. MATERIALS AND METHODS. Monolithic zirconia (MZr) and monolithic lithium disilicate (MLD) crowns for mandibular first molar were fabricated using computer-aided design/computer-aided manufacturing technology and then cemented to zirconia hybrid-abutments (Ti-based). Each group was divided into two subgroups (n=10): (A) control group, crowns were subjected to single load to fracture; (B) test group, crowns underwent chewing simulation using multiple loads for 1.2 million cycles at 1.2 Hz with simultaneous thermocycling between $5^{\circ}C$ and $55^{\circ}C$. Data was statistically analyzed with one-way ANOVA and a Post-Hoc test. RESULTS. All tested crowns survived chewing simulation resulting in 100% survival rate. However, wear facets were observed on all the crowns at the occlusal contact point. Fracture load of monolithic lithium disilicate crowns was statistically significantly lower than that of monolithic zirconia crowns. Also, fracture load was significantly reduced in both of the all-ceramic materials after exposure to chewing simulation and thermocycling. Crowns of all test groups exhibited cohesive fracture within the monolithic crown structure only, and no abutment fractures or screw loosening were observed. CONCLUSION. When supported by implants, monolithic zirconia restorations cemented to hybrid abutments withstand masticatory forces. Also, fatigue loading accompanied by simultaneous thermocycling significantly reduces the strength of both of the all-ceramic materials. Moreover, further research is needed to define potentials, limits, and long-term serviceability of the materials and hybrid abutments.

  • 저작은 치아, 악골, 저작근 뿐만 아니라 근 신경계, 고위 중추까지 복합적으로 관여되는 기능적 행위이다. 저작 형태는 다양한 모양을 가지나 두 가지 전형적인 군, 즉, 전방에서 관찰 시 그 양상이 수직적이며 절단 (chopping) 운동을 하는 군과 주로 측방으로 이루어지며 연마 (grinding)를 하는 군으로 나눌 수 있다. 본 연구의 목적은 저작 형태의 차이가 교합면 마모에 미치는 영향을 조사하는 것이다. 두개 하악 관절과 저작 습관에 이상이 없으며 교합면에 수복물이 없는 치과 대학생으로 하악 운동 궤적 기록기를 이용하여 상기의 전형적인 2가지 저작 형태를 보이는 각 15명씩을 피검자로 선택하였다. 각 피검자에 대한 임상 검사를 통해 ordinal scale로 교합면 마모의 등급을 조사하여, 평균 치아 마모도와 부위에 따른 치아 마모도를 비교 조사하였다. 각 피검자에 대한 인상 채득 후 모형을 제작하고 arbitrary scale로 교합면 마모의 등급을 조사하여, 평균 치아 마모도와 부위에 따른 치아 마모도를 비교 조사하고 저작측과 비저작측 마모를 비교 조사하였으며 수평 마모면과 수직 마모면을 비교 조사하였다. 1. 평균 치아 마모도는 ordinal scale로 측정하였을 때, 절단형과 연마형간에 유의할 만한 차이가 없었다.(p >0.05) 2. 부위에 따른 치아의 마모도는 ordinal scale로 측정하였을 때, 절단형과 연마형간에 유의할 만한 차이가 없었다.(p >0.05) 3. 평균 치아 마모도는 arbitrary scale로 측정하였을 때, 절단형에 비교하여 연마형에서 높은 마모도를 보였다.(p<0.05). 4. 절단형에 비교하여 연마형은 arbitrary scale로 측정하였을 때, 구치부에서는 높은 마모도를 보였으며 전치부에서는 유의할 만한 차이가 없었다(p<0.05) 5. 구치 평균 치아 마모도와 부위에 따른 치아의 마모도는 균형측에서 절단형과 연마형간에 유의할 만한 차이를 보이지 않았다(p>0.05). 6. 전치 평균 치아 마모도와 부위에 따른 치아의 마모도는 수평 마모면과 수직 마모면 비교시, 절단형과 연마형간에 유의할 만한 차이를 보이지 않았다.(p>0.05)

  • Loss of posterior support may cause extrusion of antagonistic teeth and excessive wear of remaining teeth which often leads to the destruction of the occlusal plane. In such cases, it is critical to verify the need to increase vertical dimension of occlusion (VDO). Should you increase VDO, provisionalization is crucial in evaluating function and esthetics. Double scanning technique is a useful method when fabricating definitive restoration that mimic provisional restoration. In this case, a patient with apparently no loss of VDO and insufficient interocclusal space for dental materials due to loss of posterior support and extrusion of antagonistic teeth was rehabilitated using double scanning technique.

  • With the evolution of the computer-aided design/computer-aided manufacturing (CAD/CAM) technology, the intraoral scanners are playing an increasingly important role, as they are the first step towards a completely digital workflow. The CAD/CAM double scanning technique has been used to transfer the information from provisional restorations to definitive restorations. In this case, a 67-year-old male with esthetically compromised anterior teeth, generalized severe attrition of teeth, and reduced vertical dimension was treated with full mouth rehabilitation including a re-establishment of the lost vertical dimension of occlusion assisted by the crown lengthening procedure. The provisional restorations were fabricated using an intraoral scanner and the CAD/CAM double scanning technique. After the period of adaption, the definitive monolithic zirconia restorations were delivered. The CAD/CAM double scanning technique successfully transferred the occlusal and morphological characteristics, obtained from the provisional restorations, to the definitive restorations.

A STUDY ON MARGINAL ADAPTATION OF READY-MADE STAINLESS STEEL CROWN TO THE PRIMARY MOLAR (기성금관수복 유구치의 변연 적합도에 관한 연구)

  • In Class II amalgam restoration in deciduous molar, failure rate and incidence of recurrent caries are high as children become older. In order to preserve deciduous molars till the physiologic exfoliation time, stainless steel crown is a choice of the treatment. As a result of a careless treatment, such as overhanging margin, poor marginal adaptation, poor proximal contour and inadequate mesiodistal width give rise to interfering eruption of the adjacent teeth, recurrent caries and chronic gingival irritation and insufficient arch length respectively. In this study, 252 s.s. crowned teeth extracted due to physiologic exfoliation or periapical lesion. The purpose of this study is to analyze the marginal adaptation of stainless steel crown to the deciduous molar in order to obtain better clinical result. The results were as follows : 1. Between the length of s.s. crown and the marginal gap of crown, positive correlations were shown. 2. Largest amount of marginal gap was shown at buccal side in upper deciduous molars and lower first deciduous molar, lingual side in lower second deciduous molar. But no significant diffrence were found statistically compared to second most largest one. 3. Incidence of exposed restoration and recurrent caries were higher in proximal surface than buccal/lingual surface. And extension of restoration below the margin of s.s. crown gives rise to higher rate of recurrent caries. 4. Defect of contour was found in 34%, frequently found in lower 1st deciduous molar and upper 1st deciduous molar. 5. Marginal polishing defects were found in 23%. 6. Ledge was formed in 10% especially in lower 1st deciduous molar and lower 2nd deciduous molar. 7. 16% of the teeth had wear facet due to traumatic occlusion, 7% of them had occlusal perforation.

  • This clinical report focuses on the treatment of a patient with esthetic and functional impairments due to severe worn dentition. Absence of posterior support for a prolonged period require comprehensive prosthetic restoration. Accurate clinical and radiographic examinations, diagnostic wax-up, and occlusal vertical dimension evaluation were performed and the degree of patient adaptability was evaluated using an interim restoration. After 8 weeks of stabilization with interim restoration and confirmation of absence of any abnormal findings, definitive prostheses were fabricated. Satisfactory functional and esthetic outcomes were observed after 6 months of follow-up.

THE EFFECT OF VARIOUS DESENSITIZING AGENTS ON THE PERMEABILITY OF HUMAN DENTIN (수종의 desensitizing agent가 상아질 투과도에 미치는 영향)

  • The hydrodynamic theory of dentin sensitivity states that movement of tubular contents or tubular fluid, in either direction of dentinal tubule, causes dentin sensitivity. A corollary of that theory is that anything that can decrease dentinal fluid movement or dentin permeability should decrease dentin sensitivity. A wide variety of physicochemical methods have been used to reduce the permeability and sensitivity of exposed dentin. The purpose of this study was to evaluate the ability of 4 kinds of clinical desensitizing agents(2% NaF, 30% Potassium oxalate, MS Coat$^{(R)}$, Tubulitec system$^{(R)}$) to reduce the rate of fluid flow through dentin in vitro. Sixty coronal dentin discs, 1mm in thickness, were prepared from extracted third molars, free from decay and wear. Dentin discs were treated with 3% EDTA(Tubulicid Plus$^{(R)}$(Dental Therapeutics AB, Sweden)) to remove the smear layer and debris occluding the tubular orifices. After placing the discs in a split chamber device, the rate at which physiologic saline solution could filter across dentin under 150cm $H_2O$ hydrostatic pressure was measured. The occlusal side of the discs were then treated with MS Coat$^{(R)}$, 2% NaF, Tubulitec system$^{(R)}$, and 30% Potassium oxalate, and the filter ratio of the saline solution was measured again. The following conclusions were drawn : 1. Hydraulic conductance which was measured after the application of desensitizing agents was decreased in all the groups(p<0.05). 2. % change of hydraulic conductance was compared but no significant difference was found among the four desensitizing agents(p>0.05). 2% NaF, 30% Potassium oxalate, MS Coat$^{(R)}$ and Tubulitec system$^{(R)}$ decreased the permeability of dentin. It is considered that above four agents can be used in treating the hypersensitive teeth.
