한국중소기업의 전자무역시스템 활용상의 장애요인과 성과분석에 관한 실증적 연구 (A Empirical Study on the Obstructive Factors and Performance of Using e-Trade Service System for Korean Small and Medium Enterprises)

  • 본 연구는 지난 2009년부터 시행해 온 전자무역시스템 서비스의 활용상 장애요인과 성과를 한국 중소기업을 대상으로 실증적으로 분석한 연구이다. u-TradeHub 서비스는 시스템 구축 및 개통 이후 무역업체는 물론 무역업과 연계하여 업무를 이행하는 각 관세사 및 운송회사 등을 통해 이용되고 있는데, 아직까지 당초 기대효과에는 부합하지 못하고 있는 실정이다. 이에 본 연구에서는 중소기업을 대상으로 전자무역시스템 활용 현황에 대해 알아보고, 이용 상의 실무적 장애요인을 파악하여 궁극적으로는 전자무역시스템의 활용을 향상시키는 방안을 제시하고자 하였다. 본 연구 결과에 따르면 실제로 장애요인은 전자무역시스템 활용에 저해하는 요인으로 존재 및 작용하고 있으며, 그 중 시스템적 장애요인이 가장 큰 영향을 미치는 장애요인으로 작용하고 있어, 현 시스템의 개선안을 시급히 강구할 필요성이 있다.

  • Chronic obstructive pulmonary disease (COPD) is characterized by a diverse array of pulmonary and nonpulmonary manifestations, but our understanding of COPD pathogenesis and the factors that influence its heterogeneity in disease presentation is poor. Despite this heterogeneity, treatment algorithms are primarily driven by a single measurement, forced expiratory volume in 1 second ($FEV_1$) as a percentage of its predicted value ($FEV_1%$). In 2011, a major shift in Global Initiative for Chronic Obstructive Lung Disease (GOLD) treatment recommendations was proposed that stratifies patients with COPD on the basis of symptoms and exacerbation history. This article reviews the work reported in 2013 that enlightens our understanding of COPD with respect to COPD classification systems, phenotype, biomarker, exacerbation, and management for patients with COPD.

한국성인의 만성 폐쇄성 폐질환 유병률, 위험요인 및 삶의 질 (Chronic Obstructive Pulmonary Disease in Korea: Prevalence, Risk Factors, and Quality of Life)

  • Purpose: This study seeks to examine prevalence, risk factors, and quality of life of Korean adults with Chronic Obstructive Pulmonary Disease (COPD). Methods: From the database of the Fourth Korean National Health and Nutrition Examination Survey (KNHANES IV-1, 2008), the researchers selected 1,458 adults over the age of 45. The original study was a population-based epidemiological survey of health and nutrition with a stratified multistage clustered probability design. Prevalence of COPD was computed on the basis of the sampling weight. Data were analyzed using descriptive statistics, ${\chi}^2$ test, t-test and multiple logistic regression with the SPSS WIN 18.0 and SAS Ver. 9.1 program. Results: The prevalence of COPD was 18.0% among people older than 45 yr. The prevalence of current smokers was 19.7% in this population and 26.3% in individuals with COPD. Age, gender, education, and smoking levels were found to be risk factors for COPD. Significant difference in quality of life was founded between adults with COPD and the healthy controls. Conclusion: The results of this study indicate that COPD is a highly prevalent disease in Korea. To reduce the prevalence of COPD and improve health-related quality of life in patients with COPD, nursing interventions must focus on prevention of risk factors.

만성폐쇄성폐질환자의 건강 관련 삶의 질에 영향을 미치는 요인 (Factors Influencing Health-related Quality of Life of Patients with Chronic Obstructive Pulmonary Disease)

  • Purpose: The purpose of this study was to identify the factors influencing health-related quality of life (HRQOL) of patients with chronic obstructive pulmonary disease (COPD). Methods: A sample of 108 Patients diagnosed with COPD was recruited from outpatients, at a university hospital. Data were collected from February to April in 2016 using the St. George's Respiratory Questionnaire, the Hospital Anxiety-Depression Scale, and the COPD Self-efficacy scale. Results: The average score of HRQOL was 47.56 in patients with COPD. Results of the regression analysis showed that dyspnea phase (${\beta}$=.34, p<.001) and presence or absence of oxygen (${\beta}$=.20, p=.009), anxiety (${\beta}$=.29, p<.001), $FEV_1$ % predicted (${\beta}$=-.15, p=.014) were statistically significant in patients' HRQOL. Conclusion: The research findings show that dyspnea phase and anxiety among psychological factors were the most powerful factors that explain the HRQOL of COPD patients when the demographic and disease characteristics were controlled. Identification of quality of life-related factors should be used to inform targeted interventions, in order to improve HRQOL and help patients' cope.

만성호흡기질환자의 미세먼지에 대한 인식, 지식, 예방행위와 관련 요인 (Factors Affecting the Perception, Knowledge, and Preventive Behaviors of Chronic Pulmonary Disease Patients on Particulate Matter)

  • 이 연구의 목적은 천식과 만성폐쇄성폐질환 환자들의 미세먼지에 대한 인식, 지식, 예방행위 수준과, 예방행위에 영향을 미치는 요인을 확인하기 위한 것이다. 이 연구는 서술적 조사연구로 연구대상은 대구광역시 소재 일개 대학병원 호흡기전문질환센터를 방문한 만 19세 이상, 만 80세 이하의 만성호흡기질환자를 편의표본추출하였고, 2019년 12월부터 2020년 1월까지 구조화된 자기기입식 설문조사로 자료를 수집하여, 총 212부의 자료를 분석 대상으로 하였다. 연구대상자는 212명이었으며, 이중 천식 112명(52.8%), 만성폐쇄성폐질환 100명(47.2%)이었다. 미세먼지에 대한 인식, 지식, 예방행위의 평균 점수(10점 만점)는 천식 환자는 각각 7.92점, 6.99점, 7.10점이었고, 만성폐쇄성폐질환자는 각각 7.72점, 6.24점, 6.80점 이었다.미세먼지 지식 점수는 천식환자가 만성폐쇄성폐질환자보다 유의하게 높았다(p=0.007). 미세먼지 예방행위 수준에 영향을 미치는 요인은 천식은 인식 점수, 지식 점수, 연령이며, 만성폐쇄성폐질환은 인식 점수가 유의한 요인이었다. 이상의 결과 천식과 만성폐쇄성폐질환 환자의 예방행위에 영향을 주는 요인은 천식은 인식 점수, 지식 점수, 연령이며, 만성폐쇄성폐질환은 인식점수였다. 미세먼지 예방행위 수준을 높이는 것은 미세먼지로 인한 만성호흡기질환의 악화를 막을 수 있으므로, 질환에 따른 대상자의 특성을 고려한 중재 프로그램의 개발과 지속적이고 반복적인 교육이 요구된다.

사상체질과 만성폐쇄성호흡기질환의 상관성 (Correlation of Sasang Constitution and Chronic Obstructive Pulmonary Disease)

  • 1. Objectives: This study is to investigate the association of Sasang Constitution and chronic obstructive pulmonary disease(COPD). 2. Methods: One thousand five hundred forty five persons, more than 40 years old, participated in the community based cohort in Wonju City and Pyeongchang City of South Korea from October 29th in 2007 to February 26th in 2008. The diagnosis of COPD was confirmed by spirometry and based on the diagnostic criteria developed by GOLD (Global Initiative for Chronic Obstructive Lung Disease) standard. Relating items like height, weight, BMI(Body Mass Index), martial status, income, drinking, smoking and education were checked using questionnaires and Sasang Constitution was diagnosed by a specialist using PSSC(Phonetic System for Sasang Constitution), facial photos and check-up lists. 3. Results: There were 88 persons(5.7%) who had mild COPD. Old age(more than 60's) and male were significant risk factors of COPD. But smoking, drinking and Sasang Constitution were not risk factors of COPD. But there were many Soeumin who had mild COPD in terms of Sasang Constitution irrespective of sex. 4. Conclusions: Low BMI(<23kg/m2) and low income also were significant risk factors. And Sasang Constitution might be the variable to manage COPD patients, but more researches are needed.

한국어판 만성폐쇄성폐질환 자가간호와 자가간호 자기효능감 측정도구의 타당도와 신뢰도 (Validity and Reliability of Korean Version of Self-Care Chronic Obstructive Pulmonary Disease Inventory (SC-COPD) and Self-Care Self-Efficacy Scale (SCES-COPD))

  • Purpose: This study examined the validity and reliability of the Korean version of the Self-Care in Chronic Obstructive Pulmonary Disease Inventory (SC-COPDI) and the Chronic Obstructive Pulmonary Disease Self-Care Self-Efficacy Scale (SCES-COPD). The SC-COPDI consists of the Self-Care Maintenance Scale (SCMES), Self-Care Monitoring Scale (SCMOS), and Self-Care Management Scale (SCMAS). Methods: The original tool was translated using a back-translation process. Participants were 241 patients with COPD at the Chonnam National University Hospital in Korea. The construct validity was verified through confirmatory factor analysis, and reliability was verified using Cronbach's α. Results: The SCMES consisted of 10 items of three factors-one of four factors was deleted from the original tool. In the SCMOS, there were six items of two factors after two items were deleted from the original tool. The SCMAS consisted of the original 10 items of three factors. The SCES-COPD consisted of six items of two factors, with one item removed from the original tool. The model fit indices of all tools were good, and the construct validity was confirmed. Cronbach's α of SCMES was .72, SCMOS was .90, SCMAS was .81, and SCES-COPD was .85. Conclusion: The Korean version of SC-COPDI and SCES-COPD are valid and reliable instruments for measuring self-care in people with COPD. These instruments can be used in self-care studies of COPD patients in Korea.

  • Background: The Korean Obstructive Lung Disease (KOLD) Cohort Study is a prospective longitudinal study of patients with chronic obstructive pulmonary disease (COPD), asthma, or other unclassified obstructive lung diseases. It was designed to develop new classification models and biomarkers that predict clinically relevant outcomes for patients with obstructive lung diseases. Methods: Patients over 18 years old who have chronic respiratory symptoms and airflow limitations or bronchial hyper-responsiveness were enrolled at 17 centers in South Korea. After a baseline visit, the subjects were followed up every 3 months for various assessments. Results: From June 2005 to October 2013, a total of 477 subjects (433 [91%] males; 381 [80%] diagnosed with COPD) were enrolled. Analyses of the KOLD Cohort Study identified distinct phenotypes in patients with COPD, and predictors of therapeutic responses and exacerbations as well as the factors related to pulmonary hypertension in COPD. In addition, several genotypes were associated with radiological phenotypes and therapeutic responses among Korean COPD patients. Conclusion: The KOLD Cohort Study is one of the leading long-term prospective longitudinal studies investigating heterogeneity of the COPD and is expected to provide new insights for pathogenesis and the long-term progression of COPD.

소아수면과다증과 수면무호흡 (Childhood Hypersomnia and Sleep Apnea Syndrome)

  • Natural sleep pattern and its physiology in childhood are much different from those in adulthood. Several aspects of clinical evaluation for sleepiness in childhood are more difficult than in adulthood. These difficulties are due to several factors. First, excessive sleepiness in childhood do not always develop functional impairments. Second, objective test such as MSLT may not be reliable since it is hard to be certain that the child understand instructions. Third, sleepiness in children is often obscured by irritability. paradoxical hyperactivity, or behavioral disturbances. Anseguently, careful clinical evaluation is needed for the sleepy children. Usual causes of sleepiness in children are the disorders that induce insufficient sleep such as sleep apnea syndrome, schedule disorder, underlying medical and psychiatric disorder, and so forth. After excluding such factors, we can diagnose the hypersomnic disorders such as narcolepsy, Kleine-Levin syndrome, and idiopathic central nervous system hypersomnia. Among the variety of those causes of sleepiness, I reviewed the clinical difference of narcolepsy and obstructive sleep apnea syndrome in childhood compared with in adulthood. Recognition of the childhood narcolepsy is difficult because even severely sleepy children often do not develop pathognomic cataplexy and associated REM phenomena until much later. Since childhood narcolepsy give srise to many psychological, academical problem. Practicers should be concerned about these aspects. Childhood obstructive sleep apnea syndrome is different from adult obstructive sleep apnea syndrome too. Several aspects such as pathophysiology. clinical feature, diagnostic criteria, complication, management, and prognosis differ from those in the adult syndrome. An important feature of childhood obstructive sleep apnea syndrome is the variety of severe complications such as behavioral disorders, cognitive impairment, cardiovascular symptoms, developmental delay, and ever death. Fortunately, surgical interventions like adenotosillectomy or UPPP are more effective for Childhood OSA than adult form. CPAP is a "safe, effective, and well-tolerated" treatment modality too. So if early detection and proper management of childhood OSA were done, the severe complication would be prevented or ever cured.

조선소 근로자들의 폐쇄성 폐기능 저하와 관련된 요인 (Factors Associated With Obstructive Pattern Spirometry In Shipyard Workers)

  • Objectives: Alongside smoking, occupational exposure is an important risk factor for chronic obstructive pulmonary disease. The purpose of this study was to evaluate factors associated with occupational pulmonary function decline that can be used to create guidelines for the health management of shipyard workers Materials: This study analyzed spirometry from 10,597 male shipbuilding workers. Functional decline in spirometry was defined as FEV1/FVC <70% and logistic regression for work duration and occupational hazard exposure was performed Results: Among the subjects, 4.2% showed an obstructive pattern in pulmonary function. The odds ratios for hazard exposure were 1.67(indirect) and 3.54(direct), and for work duration 1.97(10-18 years), 2.29(19-27), and 5.02(28+). After adjusting for smoking and work-related factors, the odds ratios for work durations of over 10 years were 1.73(10-18 years), 1.99(19-27), and 4.09(28+), but for hazards exposure was 1.71(direct) alone after adjustment. Conclusions: Occupational COPD is insidious and chronic, and thus long-term hazard exposed(especially over 10 years) shipyard workers with functional decline in spirometry need to prevent and manage COPD. This study is important for establishing guidelines to manage hazard exposure among shipyard workers and prevent COPD.
