계지복령환(桂枝茯苓丸)의 배오(配伍)분석과 그 생리활성 (Analysis for Compatibility of Gyejibongnyeong-hwan and Its Biological Activities)

  • Objectives : We analysed Gyejibongnyeong-hwan's compatibility principle and investigated biological activities by categorizing with molecular level, cellular level, animal level and human level based on Korean study for this formula. Methods : Gyejibongnyeong-hwan's compatibiltity principle was examined by the system of chief, deputy, assistant, and envoy. We looked into studies that presented in Korea from 1956 to 2016 about Gyejibongnyeong-hwan through Korea Institute of Oriental Medicine, Korean medicine information system (OASIS). Then classify into molecular level, cellular level, animal level and human level to analyse. Results : According to the system of chief, deputy, assistant, and envoy, chief herb is Cinnamomi Ramulus, deputy herb is Persicae Semen, assistant herb is Moutan Cortex, Paeoniae Radix, Poria, and envoy herb is Mel. Biological activities can be detected in transcription factors, enzymes, and inflammatory mediators for molecular level. For cellular level, it can be determined in human uterine endometrial cancer cell, human hepatocarcinoma cell, and human platelets. In mouse and rats for animal level, in overian cystoma, menorrhalgia, quality of life improvement in postmenopausal women, and blood stasis with motor vehicle accident for human level, biological activities was caught. Conclusions : From above results, Gyejibongnyeong-hwan is composed in line with the system of chief, deputy, assistan, and envoy. Biological activities are effective to improvement of menorrhalgia, anti-cancer, anti-oxidative, anti-inflammation, improvement of atherosclerosis, analgesic, anti-convulsion, wound healing, and improvement of liver function.

A Nomogram for Predicting Non-Alcoholic Fatty Liver Disease in Obese Children

  • Purpose: Non-alcoholic fatty liver disease (NAFLD) ranges in severity from simple steatosis to steatohepatitis. Early detection of NAFLD is important for preventing the disease from progressing to become an irreversible end-stage liver disease. We developed a nomogram that allows for non-invasive screening for NAFLD in obese children. Methods: Anthropometric and laboratory data of 180 patients from our pediatric obesity clinic were collected. Diagnoses of NAFLD were based on abdominal ultrasonographic findings. The nomogram was constructed using predictors from a multivariate analysis of NAFLD risk factors. Results: The subjects were divided into non-NAFLD (n=67) and NAFLD groups (n=113). Factors, including sex, body mass index, abdominal circumference, blood pressure, insulin resistance, and levels of aspartate aminotransferase, alanine aminotransferase (ALT), γ-glutamyl transpeptidase (γGT), uric acid, triglycerides, and insulin, were significantly different between the two groups (all p<0.05) as determined using homeostatis model assessment of insulin resistance (HOMA-IR). In our multivariate logistic regression analysis, elevated serum ALT, γGT, and triglyceride levels were significantly related to NAFLD development. The nomogram was established using γGT, uric acid, triglycerides, HOMA-IR, and ALT as predictors of NAFLD probability. Conclusion: The newly developed nomogram may help predict NAFLD risk in obese children. The nomogram may also allow for early NAFLD diagnosis without the need for invasive liver biopsy or expensive liver imaging, and may also allow clinicians to intervene early to prevent the progression of NAFLD to become a more advanced liver disease.

가볍다차(茶)가 고지방식이로 유도된 비만 마우스에서 항염증에 미치는 효과 (The Anti-Inflammatory Effect of Gabyeobda Tea in High Fat Diet-Induced Obese Mice)

  • Objectives: The purpose of this study was to investigate the effects of Gabyeobda tea (GT) on anti-inflammation in ice induced high fat diet (HFD). Methods: The C57BL/6 mice fed HFD were administrated with GT once daily for 8 weeks. The changes of body weight, calorie intake levels were measured in mice. The level of serum total cholesterol, triglyceride, high density lipoprotein cholesterol, glutamic oxaloacetic transaminase (GOT), and glutamic pyruvic transaminase (GPT) were measured in mice by enzyme-based assay. It was also observed the histological changes of liver, and fat tissues with hematoxylin and eosin staining. Further real-time polymerase chain reaction (PCR) and enzyme-linked immunosorbent assay were employed to detect inflammatory cytokine levels such as tumor necrosis factor (TNF)-𝛼, interleukin (IL)-6, and IL-1𝛽. Results: HFD+GT group, which was administered with GT with HFD, showed no body weight gain compared with HFD group. However, levels of GOT, GPT, and inflammatory cytokines such as TNF-𝛼, IL-6, and IL-1𝛽 in the blood of HFD+GT group were significantly reduced compared with HFD group. In addition, the messenger RNA (mRNA) expression level of the IL-12 gene was significantly reduced and the mRNA expression level of the IL-10 was increased in the liver. Conclusions: It suggests that Gabyeobda tea can alleviate inflammatory responses induced by high fat diet by inhibiting inflammatory cytokines production.

지방흡입술 후 초기 대사성 변화 및 고찰 (Early Metabolic Changes and Its Considerations after Liposuction)

  • Purpose: Advanced techniques now make it possible to remove considerable amounts of subcutaneous adipose tissue more safely with minimum blood loss. However, few have analyzed the metabolic consequences of liposuction. The purpose of this study was to identify the early effects of the surgical removal of subcutaneous fat on metabolic changes in patients who have undergone liposuction. Methods: Nineteen patients were evaluated from June 2005 to December 2005. Preoperative body weight, serums levels of lipids, apolipoprotein A1, dehydroepiandrosterone(DHEA), uric acid, insulin, and glucose were evaluated. Insulin resistance was determined using the homeostasis model assessment of insulin resistance (HOMA-IR), which is based on fasting glucose and insulin concentrations. All of these data were remeasured in 1 and 4 weeks postoperatively. Tumescent fluid was infiltrated using the superwet technique. The liposuction device used was a $Liposlim^{(R)}$ power-assisted unit. Results: Average volumes of infiltrate and aspirate were 3,268mL and 2,892mL, respectively. Results in 1 week postoperatively demonstrated a significant difference in high-density lipoprotein(HDL) cholesterol, apolipoprotein A1, insulin, and HOMA-IR levels. However, all values were within normal limits and returned to baseline in 4 weeks postoperatively.Conclusion: This study provides little to support the presumed therapeutic effect of liposuction. And, it is unclear whether liposuction can prevent or be used to treat the metabolic complications of obesity. However, the results of the present study lead us to believe that liposuction is a metabolically safe procedure.

  • 보건소 '금연클리닉사업'은 흡연자들에게 6개월 동안 9회 이상의 금연상담서비스와 CO측정, 니코틴 패치, 니코틴 껌, 니코틴 사탕 등의 니코틴보조제를 제공한다. 또한 행동 강화 물품과 금연 성공 기념품을 제공하는 등, 금연 실천을 유도하고 흡연율을 감소시켜 지역주민의 건강증진을 도모한다. 흡연과 과음 등의 생활습관은 고지혈증 및 비만 등 만성질환을 유발시키는 주요인자이며, 경제적인 손실, 나아가 건전한 문화교양인으로서의 삶을 파괴하는 한 요인이 된다. 흡연, 과음 등의 생활습관이 건전한 삶이라는 문화적 가치와 더불어 의료비라는 경제적 가치와도 밀접한 관계가 있음을 고려해 볼 때, 흡연 예방 및 금연과 절주 등의 보건교육에 동참하는 사업장과 기관에 대한 제도적 지원이 병행되어야 할 것으로 사료된다.

Factors associated with paravertebral muscle cross-sectional area in patients with chronic low back pain

  • Background: This study was performed to reveal the relationships between the cross-sectional areas (CSAs) of the paraspinal muscles and the severity of low back pain (LBP), including the level of disability. Methods: This single-center cross-sectional study was conducted on 164 patients with chronic LBP. The effects of demographic characteristics, posture, level of physical activity, disc herniation type, and sarcopenia risk on the CSAs of paraspinal muscles were evaluated along with the relationship between the CSAs and severity of pain and disability in all patients. The CSAs of paraspinal muscles were evaluated using the software program Image J 1.53. Results: A negative significant correlation was found between age and the paraspinal muscle's CSA (P < 0.05), whereas a positive correlation was present between the level of physical activity and the CSA of the paraspinal muscle at the L2-3 and L3-4 levels. The CSAs of paraspinal muscles in patients with sarcopenia risk was significantly lower than those in patients without sarcopenia risk (P < 0.05). The CSAs of paraspinal muscles at the L2-3 and L3-4 levels in obese patients were significantly higher than those in overweight patients (P = 0.028, P = 0.026, respectively). There was no relationship between the CSAs of paraspinal muscles and pain intensity or disability. Conclusions: Although this study did not find a relationship between paraspinal CSAs and pain or disability, treatment regimens for preventing paraspinal muscles from atrophy may aid pain physicians in relieving pain, restoring function, and preventing recurrence in patients with chronic LBP.

비만 아동에 대한 설문지의 분석과 재구성 (Revision of a Questionnaire to Assess Health Behaviors in Obese Children)

  • 목적: 소아 비만은 급속하게 증가하고 있다. 소아 비만의 치료는 행동 수정을 하는 것이 중심이 되므로 생활 습관과 식습관을 파악하는 것이 중요하다. 본 저자들은 비만을 효율적으로 관리하기 위해 기존에 사용해오던 설문지를 보완하고자 하였다. 방법: 이대 목동병원 비만클리닉에 방문한 94명의 소아를 대상으로 기존에 개발되어 사용되고 있는 소아용과 부모용 설문지를 작성하게 하였다. 문항별 무응답률, 부모용과 소아용 설문지의 공통 항목에 대한 일치율(%)을 살펴보았다. 문항 분석을 통하여 추가 및 삭제할 부분을 검토하였다. 결과: 문항별 응답율은 매우 높았다. 부모용과 소아용 설문지의 공통 항목에 대하여 62.8~91.5%까지의 비교적 중등도 이상의 일치율을 보였다. 학교급식의 음식 선호도는 평상시 음식습관으로 파악할 수 있으며, 취사선택 할 수 없는 항목으로 판단하여 삭제하였다. 좌식생활 시간을 파악하기 위하여 TV 시청 시간외 컴퓨터나 비디오 게임 시간 항목, 간식의 종류로 인스턴트나 패스트푸드 외에 섬취한 음료의 종류와 양을 기록하는 항목, 어머니의 직장 항목을 추가하였다. 또한 부모와 아동의 답변이 일치하므로 일반적인 생활 습관은 부모가, 집 밖에서의 음식과 운동에 관한 항목은 비만 아동이 기록하도록 설문지를 재구성하였다. 결론: 비만 아동과 부모의 설문지를 분석한 결과 일치도가 높으므로 비슷한 항목을 중복되지 않게 재구성하였다. 비만아동의 습관파악을 위하여 생활습관은 부모가, 집밖에서의 음식섭취와 운동 습관은 아동이 직접 기록하도록 비만 설문지를 재구성하였다.

양극성 장애 외래환자에서 야간식이증후군의 임상적 특성 (Clinical Characteristics of Night Eating Syndrome in Outpatients with Bipolar Disorder)

  • 연구목적 이 연구의 목적은 양극성 장애로 진단받은 정신과 외래 환자에서 야간 식이습관의 임상적 특성을 조사하는데 있다. 방 법 정신건강의학과 외래를 방문한 84명의 양극성 장애 환자를 대상으로 14개 항목의 야간 식이증후군 설문지(NEQ)를 자가 보고로 실시하였다. 이들의 인구학적, 임상적 특성을 조사하고 신체 질량 지수(BMI), 주관석 기분과 수면, 폭식과 체중에 관련된 삶의 질에 대한 평가를 Beck Depression Inventory(BDI), Binge Eating Scale(BES), Korean version of Obesity-related Quality of Life scale(KOQoL), Pittsburg Sleep Quality Index(PSQI) 등을 포함한 설문을 통해 얻었다. 결 과 양극성 장애 외래 환자 중 야간 식이증후군의 유병률은 14.3%(84명 중 12명)이었다. NES와 Non-NES군 사이에서는 경제적 상태와 동반되는 내과 질환을 제외한 성별, 나이, 교육 년 수, 결혼 및 직업 상태, 흡연, 음주 등의 인구사회학적 특성에서 양군간에 유의한 차이를 보이지는 않았다. 하지만, Non-NES 군에 비해 NES군에서 폭식이 많았고 비만으로 인한 삶의 질 저하가 나타났다. 결 론 이번 연구는 야간식이를 보이는 양극성 장애 외래환자의 임상적 특성을 처음으로 기술하였다. 비록 양극성 장애 외래 환자의NES군에서 뚜렷한 상관 관계를 보이는 특성이 많지는 않았지만, 상대적으로 NES 유병률이 높은 것으로 볼 때 임상에서 양극성 장애 환자 평가 시 비만 여부에 관계 없이 NES를 염두에 두어야 할 것이다.

조현병 외래 환자에서 야간식이증후군의 유병률과 관련요인 (Prevalence and Its Correlates of Night Eating Syndrome in Schizophrenic Outpatients)

  • 연구목적 본 연구의 목적은 조현병 외래 환자를 대상으로 야간식이증후군(night eating syndrome : NES)의 유병률과 이와 관련된 임상적 요인들에 대해 살펴보는 것이다. 방 법 정신건강의학과 외래를 방문한 201명의 조현병 환자들을 14 문항의 자기보고 형식인 야간식이증후군 설문지(Night Eating Questionnaire : NEQ)로 평가하였다. 인구학적, 임상적 특징과 체 질량 지수(BMI)을 조사하였고, 주관적인 기분과 수면, 폭식과 체중 관련 삶의 질에 대해서는 각각 Beck's Depression Inventory(BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES)와 Korean version of Obesity-Related Quality of Life Scale(KOQoL)를 이용하여 평가하였다. 결 과 조현병 외래 환자에서 야간 식이 증후군의 유병률은 10.4%(201명 중 21명)이었다. NES 군과 non-NES 군간에 나이를 제외하면 인구사회학적 및 임상적 특성, BMI에서는 유의한 차이를 보이지 않았다. NES 군이 Non-NES 군에 비해 보다 우울했고, 수면과 폭식 양상, 체중 관련 삶의 질에 있어서 의미 있는 장애를 나타냈다. '아침 식욕부진'과 '지연된 아침식사'(NEQ에서 NES 핵심 항목 5개 중 2개)는 두 군간에 차이가 없었으나, '야간 섭식', '야간 식욕항진' 그리고 '기분/수면'은 NES 군에서 더 손상되어 있는 것으로 나타났다. 결 론 본 연구는 조현병 외래 환자를 대상으로 NES의 유병률과 관련 요인에 대해 기술한 최초의시도이다. 비록 NES와 비만과의 연관성은 밝히지 못했지만, 본 연구의 결과는 NES가 정신 건강에 부정적 영향을 준다는 사실을 보여주었다. 향후 이러한 결과를 뒷받침할 수 있는 추가 연구가 필요할 것이다.

CYP2W1, CYP4F11 and CYP8A1 Polymorphisms and Interaction of CYP2W1 Genotypes with Risk Factors in Mexican Women with Breast Cancer

  • Breast cancer (BCa) is the leading type of cancer in Mexican women. Genetic factors, such as single nucleotide polymorphisms (SNP) of P450 system, have been reported in BCa. In this report, and for the first time in the literature, we analyzed the rs3735684 (7021 G>A), rs11553651 (15016 G>T) and rs56195291 (60020 C>G) polymorphisms in the CYP2W1, 4F11 and 8A1 genes in patients with BCa and in healthy Mexican women to identify a potential association between these polymorphisms and BCa risk. Patients and controls were used for polymorphism analysis using an allelic discrimination assay with TaqMan probes and confirmed by DNA sequencing. Links with clinic-pathological characteristics were also analyzed. Statistical analysis was performed using the standard ${\chi}^2$ or Fisher exact test statistic. No significant differences were observed in the distributions of CYP2W1 (OR 8.6, 95%CI 0.43-172.5 P>0.05; OR 2.0, 95%CI 0.76-5.4, P>0.05) and CYP4F11 (OR 0.3, 95%CI 0.01-8.4 P>0.05) genotypes between the patients and controls. Only the CYP8A1 CC genotype was detected in patients with BCa and the controls. All polymorphism frequencies were in Hardy-Weinberg Equilibrium (HWE) in the controls (P>0.05). We found a significant association between BCa risk and smoking, use of oral contraceptives or hormonal replacement therapy (HRT), obesity, hyperglycemia, chronic diseases, family history of cancer and menopausal status in the population studied (P<0.05). Tobacco, oral contraceptive or HRT, chronic diseases and obesity or overweight were strongly associated with almost eight, thirty-five, nine and five-fold increased risk for BCa. Tobaco, obesity and hyperglycemia significantly increased the risk of BCa in the patients carrying variant genotypes of CYP2W1 (P<0.05). These results indicate that the CYP2W1 rs3735684, CYP4F11 rs11553651 and CYP8A1 rs56195291 SNPs are not a key risk factor for BCa in Mexican women. This study did not detect an association between the CYP2W1, 4F11 and 8A1 genes polymorphisms and BCa risk in a Mexican population. However, some clinico-pathological risk factors interact with CYP2W1 genotypes and modifies susceptibility to BCa.
