비만 환경 내 면역세포 활성화 표현형의 변화 (Phenotype Changes in Immune Cell Activation in Obesity)

  • 면역 체계와 대사 체계는 항상성을 유지하는데 중요한 요소이다. 면역 반응과 대사 조절은 연관성이 높아, 정상적인 대사가 교란되면 대사 질환이 발생하며, 면역 반응에도 변화가 발생하였다. 마찬가지로, 비만은 면역 반응과 높은 관련이 있다. 에너지 대사의 불균형으로 발생하는 비만은 인슐린 저항성, 제2형 당뇨병, 지방간 질환, 동맥경화증, 고혈압 등의 대사 질환과 관련이 있다. 알려진 바로는, 비만은 낮은 수준의 염증이 만성화된 상태가 특징이다. 비만 환경에서, 면역세포의 미세 환경은 대식세포, 자연살해세포, T세포 같은 면역세포의 독특한 활성화 표현형에 의해 염증성이 되었다. 또한, 면역 세포는 세포 간의 기전, 사이토카인을 매개하는 기전을 통해 상호작용하여 비만으로 인한 염증 반응을 강화한다. 이러한 현상은 기존의 췌장 리파아제나 알파-아밀라아제 같은 체내 효소의 억제나 지방전구세포의 분화를 억제를 표적으로 하는 일반적인 비만의 약리학적 치료 외에 면역세포 활성화 조절을 표적으로 하는 비만의 약리학적 치료 전략을 시사한다. 본 논문에서는 대식세포, 자연살해세포, T세포의 활성화 표현형과 비만 환경 내이들의 양상에 대해 정리하였다. 또한, 본 논문에서는 현재까지 확인된 면역세포의 활성화 조절을 통한 비만을 완화하는 약리학적 물질에 대해서 정리하였다.

농촌지역 초등학생의 1일 평균 보행수, 활동량 및 활동계수 평가 (Assessment of Daily Steps, Physical Activity and Activity Coefficient of the Elementary School Children in the Rural Area)

  • The purpose of this study was to assess the daily steps, physical activities and activity coefficient of the elementary school children in the rural area. Body weight, height and daily steps were measured and one-day activity diaries were collected by interviewing children. The average age of the subjects was $9.96{\pm}1.02$ years. Average height, weight, obesity index, body fat and muscle of subjects were $136.2{\pm}8.9cm,\;35.2{\pm}8.5kg,\;99.6{\pm}18.8%,\;22.9{\pm}8.5%\;and\;35.8{\pm}6.0%$, respectively. The average daily steps of the subjects was 17,584 and daily steps (19,314) of 3rd grade students was significantly higher than that (15,712) of 5th grade children. But there was no significant difference in daily steps and activity coefficients between boys and girls. Daily steps (23,347) of exercise group showed the significantly higher than that (16,144) of non-exercise group. Gender and grade of subjects did not have significant influence on activity coefficients, but there was a significant difference in activity coefficient on weekdays between the exercise group ($1.82{\pm}0.30$) and non-exercise group ($1.50{\pm}0.21$). Analysis of variance revealed stronger associations between daily steps and body fat (%) than between daily steps and BMI. Daily steps showed significant negative correlation with body fat (%) measured using two methods r=-0.321 and r=-0.365, respectively. Activity coefficient was significantly correlated (r=0.436) with daily steps, thus increasing daily steps cm prevent and treat childhood obesity by increasing the energy expenditure. The higher activity coefficients (weekday 1.56, weekend 1.53) of the subjects was caused by the fact that rural students spent more time outside and enjoyed stronger activities than urban students. The results of this study can be used to estimate energy requirements for Korean children based on exercise levels and living areas.

초등학교 비만아동을 위한 통합비만관리프로그램의 효과 (Effects of integrated obesity management program on obese elementary school children)

  • 본 연구는 초등학교 비만아동을 위한 통합비만관리프로그램을 개발·적용하여 비만지수, 생활습관 및 자기효능감에 미치는 영향을 파악하기 위한 비동등성 대조군 사전·사후설계이다. 연구대상자는 초등학교 4~6학년 아동으로 실험군 30명과 대조군 28이었다. 통합비만관리프로그램은 교육프로그램 주 1회, 운동프로그램 주 3회로 구성하여 8주간 실시되었다. 수집된 자료는 chi-square test, independent t-test, repeated measure ANOVA를 사용해 분석하였다. 연구결과 실험군에서 체질량지수는 25.83kg/㎡에서 24.82kg/㎡로 감소하였고(p<.05), 체지방률은 대조군에 비해 감소하였으며(p<.05), 비만도도 개선되었다(p<.01). 그러나 근육량은 유의한 변화가 없었다. 중재 후 생활습관(p<.05)), 식이 자기효능감(p<.001), 운동 자기효능감(p<.05)은 향상되었다. 이를 통해 본 연구의 통합비만관리프로그램은 초등학교 비만아동들의 비만도와 비만을 감소시키는 생활습관 개선 및 자기효능감 증진에 효과적인 간호 중재임이 확인되었다.

초등학교 정상체중아동과 비만아동의 건강증진 생활양식 (Health Promotion Life Profile of Normal-Weight and Obese Children in Elementary Schools)

  • Purpose: The purpose of this study was to identify the Health Promotion Life Profile of normal-weight and obese children, and factors related to obesity in elementary school children, and provide basic data for the development an obesity management program for these children. Method: The survey participants were 878 fourth to sixth grade elementary school children in Gwangju. $x^2$-test, t-test, ANOVA and multiple logistic regression with the SPSS program were used to analyze the collected data. Results: The Health Promotion Life Profile showed significant differences between normal-weight and obese groups according to food habits (t=3.305, p= .001) and exercise habits (t=-4.065, p=< .001). The Health Promotion Life Profile of obese children was also significantly different according to interest in weight control (F=3.044, p= .050), regular breakfast (F=3.699, p= .014), regular meals (F=6.868, p= .002). The multiple logistic regression analysis, showed that factors in obesity were significantly different according to gender, interest in weight control, fast eating, overeating and predilection for some food. Conclusion: The results of this study suggest that items on Health Promotion Life Profile such as food habits and exercise habits should be included in education programs to prevent obesity in elementary school children.

생체전기임피던스법을 이용한 체성분 분석에 의한 신체외형도(Somatotype Drawing)의 타당성 연구 - 비만평가도구로서의 타당도 평가 - (A Study on the Validation of Somatotype Drawing as a Instrument of Measuring Obesity Level by Body Component Analysis)

  • Purpose: Somatotype drawing developed by Sorensen et al.(1983) has been evaluated as a simple instrument of obesity level without real somatic measuring as height and weight. This study was designed to validate somatotype drawings for obesity assessment by bioelectrical impedance body component analysis. Method: At first questionnaire of somatotype drawing was done. Subjects were measured body component by bioelectrical impedance analysis as weight, BMI(body mass index), WHR(waist-hip ratio), body water, protein mass, mineral mass, body fat mass, skeletal muscle mass, soft lean mass, fat free mass and percent body fat. We evaluated correlations between these data and somatotype drawings and tried to grouping of somatotype drawings with the means of major body component value. Result: The data were collected from 205 college women whose height and weight were $161.2\pm4.8,\;55\pm8.3$. Spearman's correlation coefficients of somatotype drawing were 0.74 with BMI, 0.68 with weight 0.69 with body fat mass, 0.65 with WHR. 0.64 with percent body fat after adiusted age. The grade of somatotype drawings were grouped as 1-2, 3-4, 5-6. 7-9 by BMI, body fat mass, weight, 1, 2-4, 5-6, 7-9 by WHR and 1-2, 3-4, 5-9 by percent body fat(ANOVA and Duncan's method). Conclusion: So quick instrument using somatotype drawings were useful tools for evaluation of obesity level and is applicable to screen degree of body fat in self-administered questionnaire survey.

비만도에 따른 초등학생의 식습관과 식생활 태도 비교 연구 (A Comparative Study on Eating Habit and Attitude of Elementary School Student by Obesity Indices)

  • The aim of this study was to provide fundamental data by degree of obesity which was necessary to nutritional education for the formation of right eating and dietary guideline by researching food habits of children. In order to achieve this purpose, a survey was conducted on 3, 4, 5, 6th grade of students from 4 schools in S and G city in Jeolla-Namdo. The results was as follows. There were 384(44.4%) students were normal weight, 193(22.3) were overweight, and 287(33.2) were low weight by obesity indices. Comparing the student's subjective health condition among the degree of obesity, 40.4% of overweight recognized they were healthy, 30.1% recognized ordinary, and 57.1% of lean recognized they were healthy, 34.5% recognized ordinary. It was turned out that most students who were overweight and lean recognized healthy and normal. Family was reported to be the most impact factor on student's diet habits, and then Multimedia such as TV seemed to have a effect on overweight and lean students, but nutrition teachers had a direct influence on normal weight students. The most thing which eating habits for a reform was unbalanced diet. The students in this research have breakfast everyday and regularly. Regarding dietary attitudes, overweight group enjoyed eating healthy food more than normal and low body weight group did, and tried to eat what they have never eaten before. In conclusion, it's necessary for elementary school students that increase the control ability of self perceived health condition with education for good eating habit and grasp their condition themselves through the nutrition education in school.

Obesity and Obese-related Chronic Low-grade Inflammation in Promotion of Colorectal Cancer Development

  • Colorectal cancer (CRC) is a worldwide health problem, being the third most commonly detected cancer in males and the second in females. Rising CRC incidence trends are mainly regarded as a part of the rapid 'Westernization' of life-style and are associated with calorically excessive high-fat/low-fibre diet, consumption of refined products, lack of physical activity, and obesity. Most recent epidemiological and clinical investigations have consistently evidenced a significant relationship between obesity-driven inflammation in particular steps of colorectal cancer development, including initiation, promotion, progression, and metastasis. Inflammation in obesity occurs by several mechanisms. Roles of imbalanced metabolism (MetS), distinct immune cells, cytokines, and other immune mediators have been suggested in the inflammatory processes. Critical mechanisms are accounted to proinflammatory cytokines (e.g. IL-1, IL-6, IL-8) and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$). These molecules are secreted by macrophages and are considered as major agents in the transition between acute and chronic inflammation and inflammation-related CRC. The second factor promoting the CRC development in obese individuals is altered adipokine concentrations (leptin and adiponectin). The role of leptin and adiponectin in cancer cell proliferation, invasion, and metastasis is attributable to the activation of several signal transduction pathways (JAK/STAT, mitogen-activated protein kinase (MAPK), phosphatidylinositol 3 kinase (PI3K), mTOR, and 5'AMPK signaling pathways) and multiple dysregulation (COX-2 downregulation, mRNA expression).

신체활동 중재가 초등학생의 비만 및 대사증후군 위험인자와 스마트폰 게임 참여 시간에 미치는 영향 (Effects of Physical Activity Intervention on Obesity and Metabolic Syndrome Risk Factors and Smartphone Game Time in Children)

  • 본 연구는 초등학교 5학년 남학생을 대상으로 12주 신체활동 중재가 비만 및 대사증후군 위험인자와 스마트폰 게임 참여에 미치는 영향을 검증하였다. 연구대상자는 초등학교 5학년 남학생 27명으로 실험군 16명, 대조군 11명으로 나누어 실시하였다. 신체활동 중재는 런닝과 줄넘기를 1일 60분, 주당 3회 실시하였고, 운동 강도는 50%에서 시작하여 70%까지 증가시켰다. 집단간 통계적 검증은 이원변량분석(two-way RM ANOVA)을 실시하였다. 연구결과, 비만과 대사증후군 지표, 스마트폰 게임 참여시간에 있어 신체활동 중재에 여부에 따라 유의한 상호작용 효과를 나타났다. 결론적으로 12주 신체활동 중재는 비만을 비롯한 대사증후군에 긍정적인 개선효과를 가짐과 동시에 부가적으로 스마트폰 게임 참여와 같은 부정적인 생활습관을 개선하는 효과를 가지는 것으로 사료된다.

중학교 통합체육을 위한 지적장애학생과 비장애학생의 건강체력 및 비만 비교 연구 (A Comparative Study on the Physical Fitness and Obesity of Students with Intellectual Disability and Non-disabled Students for the Integrated Middle School Sports)

  • 본 연구는 지적장애학생 10명과 비장애학생 10명의 대상으로 경기도 안산시에 소재한 중학교의 1학년~3학년의 학생들로 연구 대상을 선정 하였다. 통계 프로그램으로는 SPSS Windows 25.0을 사용해서 자료를 처리하였으며, 연구에서 사용한 통계 방법은 지적장애 학생 과 비장애 학생의 건강체력과 비만 정도를 알기 위해서 체력요인과 신체질량 지수의 평균 및 표준편차를 산출하였으며, 지적장애학생과 비장애학생의 건강체력과 비만 차이의 여부를 확인하기 위해서 독립표본 t-test를 사용하여 실시하였다. 연구의 통계적 유의수준으로는 p<.05로 설정하였으며, 연구의 결과로는 중학교 지적장애학생과 비장애학생 간에 심폐지구력과 유연성, 근력, 근지구력 및 순발력에 있어서 각각 통계적으로 유의하게 차이가 있는 것으로 나타났으며, 지적장애학생과 비장애학생에 비만의 경우에서는 유의하게 차이가 없는 것으로 나타났다.

학령기아동의 체형분류법에 따른 체중, 키, 비만도 및 체질량지수 비교 (Comparison in Weight, Height, Degree of Obesity and Body Mass Index Among Different Methods for Body Shape Classification in School-Age Children)

  • Purpose: The study was conducted to describe body shapes of school age children using the degree of obesity index (DOI) and body mass index obesity index classified by the Ministry of Education, Science and Technology (M-BOI) and Seoul Metropolitan Office of Education (S-BOI). Methods: In this cross sectional descriptive study health screening data for school children collected in 2007 was used. Results: Data were analyzed for 2,193 4th-6th grade boys (52%) and girls who attended 4 schools in rural areas. DOI determined that only 44.3% of students had average weight. This proportion was much lower than the results of other methods (74.3-77.6%). All three methods defined girls (51.3-61.8%) as skinnier than boys. Skinny and average body shaped children classified by DOI and obese children classified by S-BOI were heavier and taller and presented higher degrees of obesity (DO) and BMI scores than by other methods. M-BOI and S-BOI presented statistically significant positive correlations with weight, height, DO and BMI, while DOI was not correlated with height. Conclusion: BMI based body shape classifications provide a more rigorous classification of body shape which are favorable for school health professionals with limited resources and policy makers for internationally comparable references.
