Association between Celiac Disease and Intussusceptions in Children: Two Case Reports and Literature Review

  • Association between celiac disease and intussusception has been reported in adult. Although intussusception is common in children, it rarely has been reported in association with celiac disease. Two children, 5 and 7 years old, with celiac disease are reported here, whose initial presentation was intussusception prior to investigation for concomitant failure to thrive. They presented with acute and severe abdominal distention with vomiting, and donuts and pseudo- kidney appearance in abdominal ultrasonography. One patient's intussusception had reducted spontaneously, however the other had required surgery. In investigation for concomitant failure to thrive, tissue transglutaminase levels were very high and duodenal biopsies revealed celiac disease. Thus celiac testing is recommended in children with intussusception and growth failure.

  • The Metabolic syndrome is a constellation of lipid and non-lipid factors of metabolic origin. The presence of any 3 of the following factors is considered sufficient for diagnosis : hypertension(BP <130/85mmHg), low HDL-cholesterol (<40mg% in men, <50mg% in woman), high triglyceride(>150mg%) and abdominal obesity(abdominal girth >102cm in men, 88cm in woman). The major adverse consequence of the metabolic syndrome is cardiovascular disease. Several studies have shown an association between metabolic syndrome and increased cardiovascular events. In Korea, the prevalence of the factor of metabolic syndrome has been increasing since 20 years previously when the Korean economy began to grow rapidly, with a resultant change in lifestyle, toward that of western countries. Thus, the management of the metabolic syndrome is an important social and medical issue in terms of the national health problem. This review will consider each factor in turn, providing insight for health care providers in an effort to prevention of cardiovascular events and maintenance of quality of life in persons with metabolic syndrome was discussed.

  • 최근 각종 질환의 원인이 되고 있는 비만은 심각한 사회문제로 대두되고 있으며, 이를 해결하기 위해 비만관리를 위한 측정 시스템의 필요성이 증가하고 있다. 본 논문은 비만관리를 위해 복부의 근전도 신호를 분석해서 언제 어디서든 본인의 건강상태를 체크하여 적절한 의료 서비스를 받을 수 있는 측정 시스템에 관한 연구이다. 복부 근전도 신호 분석을 위해서 에너지 검출, 신호 특징 추출, 상태 분류 및 인식 등을 위한 알고리즘을 제안한다. 이 신호 분석 알고리즘을 측정 시스템에 적용하여 복부의 비만도 및 복부의 근력을 평가하여 건강상태에 대한 적절한 평가를 제공하는 시스템을 제안한다.

  • Obejectives : Slow-motion strengthening trainings build up muscles better than any other type of training, these exercises focus on balancing and strengthening the muscles in the anatomical center of the body - the core - which includes the back, hip and abdominal muscles. Methods : We had a concept of 'core', and compared core muscles with 12 meridian tendino-musculature based on Thomas Myers' myofascial meridian. Results : It has been suggested that the core muscles correspond to meridians of foot- three Yin and foot- three Yang. The study of core muscles strengthening exercise and movement therapy is requred.

A Study for Effects of Metabolic Syndrome Care Program with Korean Medicine in Public Health Center (한의약건강증진 사업 중 한방대사증후군 프로그램의 효과에 관한 연구)

  • Objectives: The purpose of this study is to evaluate the effects of Metabolic Syndrome care program with Korean Medicine in public health center. Methods: The data were collected from 28 participations who joined Korean Medicine care program. The program was started from April of 2013 and finished November of 2013. The effect of Metabolic Syndrome were evaluated with blood pressure check and blood test, physical test. Results: The number of Metabolic Syndrome patients decreased after the program(-24.9%). Specially TG decreased from 83.321 to 80.036(p<0.05) and FBS also decreased 8.607(p<0.01) after the program. HDL increased from 47.821 to 53.000(p<0.01) but blood pressure didn't change. BMI decreased 0.789(p<0.01) and abdominal obesity decreased 2.607(p<0.01) after the program significantly. Conclusions: From these study, the Metabolic Syndrome care program with Korean Medicine is worthy for treating Metabolic Syndrome.

Gastric Ulceration and Bleeding with Hemodynamic Instability Caused by an Intragastric Balloon for Weight Loss

  • Obesity in the United States is a medical crisis with many people attempting to lose weight with caloric restriction. Some patients choose minimally invasive weight loss solutions, such as intragastric balloon systems. These balloon systems were approved by the Federal Drug Administration (FDA) in 2015-2016 and have been considered safe, with minimal side effects. We report a patient with a two-day history of melena, abdominal pain, hypotension, and syncope which developed five months after placement of an intragastric balloon. Esophagogastroduodenoscopy with balloon removal revealed a small 8-mm gastric ulcer in the incisura. This gastric ulcer probably developed secondary to mechanical compression of the stomach mucosa by the gastric balloon which contained 900 mL of saline. The FDA is now investigating five deaths since 2016 associated with these second-generation balloons. Clinicians should be aware of these complications when evaluating patients with gastrointestinal complications, such as bleeding.

The Comparison of Right and Left Abdominal Temperature of dysmenorrhea patients using DITI (DITI를 이용한 월경통 여성의 좌우 복부온도 비교)

  • Purpose: We aim to research the difference of abdominal temperature in dysmenorrhea patients comparing with that of normal person. Metholds: We selected the 49 dismenorrhea patients as study group and 54 normal persons as control group, excluding the patients who have dermatitis, ared over 70 or under 10. and have extreme obesity. We compared the 4 points(Rt and Lt Gimun(LR14) and Guirae(ST29)) in thd abdomen of both group, check the temperature of those points with DITI(Dorex, DTI-16UTI) to compare the temperature difference with each other. For statistics, we use independent T-test, SPSS 8.0 for windows. Results: The temperature difference between Rt and Lt Gimun which are in upper abdomen shows statistically significant result in study group. Conclusion: The dysmenorrhea patients show larger temperature difference between Rt and Lt points in upper abdomen than normal persons. In lower abdomen, the temperature between control and study group shows little difference.

A Study on Volume of Respiration of the Patients following Upper Abdominal Surgery (상복부 수술 환자의 수술후 호흡량에 관한 연구(일회 호흡량과 폐활량을 중심으로))

  • This study intended to investigate the volume of respiration according to the postoperative time and positions among the upper abdominal surgery patients. Tidal volume and vital capacity were measured in three positions-supine, left lateral and sitting position-at preoperatively, 12 hours postoperatively and 36 hours postoperatively. Thirteen male and seven female patients who were admitted for elective abdominal surgery under general anesthesia were the subjects of the study. Those patients with cardiopulmonary problems, obesity and smoking habit were excluded from the study. The study was conducted from March 15 to June 30, 1985 in Seoul National University Hospital. Tidal volume and vital capacity were measured by Wright spirometer in various positions at preoperatively, 12 hours postoperatively and 36 hours postoperatively. The results were as following: 1) Vital capacity was significantly decreased at 12 hours preperatively and 36 hours postoperatively than preoperatively. Vital capacity was not significantly different in Various positions, but sitting position revealed better than left lateral and supine position. Tidal volume was not significantly different in each position. 2) Male patients showed significantly higher than female patients in tidal volume and vital capacity. Vital Capacity was not significantly different by sex in each position, but vital capacity was higher in sitting position than in lateral and supire position. 3) There was not significantly different in tidal volume and vital capacity according to the type of incision and positions, vital capacity was higher in sitting positionthan in left lateral and supine position.

The Prevalence and the Related Factors of Metabolic Syndrome in Urban and Rural Community (도시와 농촌 지역의 대사증후군 유병률과 관련 요인)

  • Purpose: The purpose of this study is to investigate the prevalence of metabolic syndrome and its components and to examine the related factors in urban and rural community, South Korea. Methods: The data from the 5th Korea National Health and Nutrition Examination Survey (KNHANES-V) conducted in 2010 were utilized in this study. The subjects were 5,760 adults 20 years or over. The data were consisted with health survey questionnaire and health examination. $x^2$-test and logistic regression was used for the analysis SAS 9.3 applying sampling weights. Results: The prevalence of metabolic syndrome based on the latest definition proposed the joint scientific meeting was 23.4% and 29.0% respectively in urban and rural community. Rural community showed higher prevalence of abdominal obesity (29.9% vs 38.5%), elevated blood pressure (31.3% vs 38.5%), elevated blood glucose (24.3% vs 28.4%) than urban community. The related factors were age, marital status, high risk drinking, obesity in urban community and age, marital status high risk drinking, severe physical activity, obesity in rural community. Conclusion: This study showed there was difference in prevalence and the related factors of metabolic syndrome and these findings have important implications to prevent and manage metabolic syndrome in public health field.

Study of the Efficacy and Safety of "adenophorae radix extract" in the Obese Patients or Overweight (비만 및 과체중 환자에 대한 사삼추출물의 유효성 및 안전성 평가)

  • Objectives: The purpose of this study is to evaluate the efficacy and biosafety of Adenophorae Radix(AR) extract in obesity or overweight patient. Methods: This study is double-blind, randomized, placebo-controlled intervention Study. 30 patients with BMI $25{\leq}$ and 30> were allotted into two groups at random. In 0, $6^{th}$, $12^{th}$ week, we had checked body weight, waist line, hip line, body fat and abdominal CT scan. In 0, $12^{th}$ week, we also had checked lipid metabolism and biosafety with blood test. Results: AR treatment had a significant effect on suppressing body wight gain (p<0.01) and BMI index(p<0.01). AR treatment reduced plasma TG level but we couldn't find statistical significance. AR treatment had produced no adverse reactions. Conclusions: This study shows that Adenophorae Radix(AR) extract can reduce the weight, BMI. Adenophorae Radix(AR) extract can be used in obesity or overweight patient.
