학령 전 아동에서 식습관과 신체발달에 관한 연구 (Study on the Eating Habits and Growth Development in Korean Preschool Children)

  • The study was designed to observe the correlation between the eating habits and growth development in 1,574 children (3-6 years old) in Kyunggi-do and Seoul, Korea. The eating habits and nutrient intake were determined by mini dietary assessment and food frequency questionnaire (FFQ). 35.6% children had been practicing an unbalanced diets. Children with unbalanced diet consumed less amount of protein source foods (meat, fish, egg, soy products), vegetables and fruits and had irregular meal time, but more of them preferred sweet foods (ice creme, cookies) and carbonated drinks as snack compared with children in balanced diet. 24-35% children consumed energy, iron (Fe), calcium (Ca), Niacin, Zinc (Zn) less than 75% RDA. Energy intake of children with unbalanced diet was not significantly different from those with balanced diet, but they consumed less amount of niacin, Fe, Ca and Zn than those with balanced diet. By using the relative percentage of standard weight-length-index (WLI), 65.7% children was normal weight, 12.9% was underweight, 13.4% was overweight and 7.9% was obese. However, 11.9% of underweight and normal weight children was so called thin obese since their body fat content was greater than 20%. There were 16.8% underweight in children with unbalanced diet and 12.6% underweight in those with balanced diet. Compared to normal weight, the underweight children significantly consumed less amount of milk and its product, high protein foods, fried foods and fruits, but obese children consumed more high protein source foods (meat, fish, egg, etc) and sweet foods. In conclusion, the eating habits of unbalanced meal was prevalent problem in preschool children which resulting in their health risks. Therefore, it would be needed that new approach for nutrition education to improve eating habits in preschoolers.

비만아에서 혈청 호모시스테인, 인슐린, 비타민 B12, 엽산 농도에 대한 연구 (Homocysteine, insulin, vitamin B12 and folic acid levels in obese children)

  • 목 적 : 소아 비만의 빈도가 점점 증가하면서 비만으로 인한 고혈압, 관상동맥 질환, 고지혈증, 인슐린 저항, 당뇨병 등의 합병증이 문제가 되고 있다. 관상동맥 질환에서 호모시스테인 치의 증가, 비타민 $B_{12}$ 섭취의 감소가 중요한 역할을 하고 있음이 보고되고 있으나, 비만에서는 아직 호모시스테인, 인슐린, 비타민 $B_{12}$에 대한 연구가 많지 않은 실정이다. 저자들은 호모시스테인, 인슐린, 비타민 $B_{12}$, 엽산이 비만아에서 어떤 연관성을 갖고 있는지를 알아보기 위하여 본 연구를 실시하였다. 방 법 : 8세부터 11세 사이에 비만도가 130 이상인 중등도 비만아 27명을 대상군으로 하였고, 같은 연령의 30명의 정상 소아를 대조군으로 하였다. 신장, 체중을 이용하여 비만도와 체질량 지수를 산출하였고, 안정된 상태에서 수축기, 이완기 혈압을 측정하였다. 10시간 이상 금식 후 총 콜레스테롤, 중성지방, 호모시시테인, 인슐린, 비타민 $B_{12}$, 엽산을 측정하였다. 생체전기저항법을 이용하여 세포내액, 세포외액, 단백질, 무기질, 근육량, 체지방량, 체지방률, 복부지방률 등을 분석하였다. 결 과 : 호모시스테인은 비만군에서 $8.1{\pm}2.1{\mu}mol/mL$$4.9{\pm}1.0{\mu}mol/mL$인 정상군보다 유의하게 높았고, 인슐린도 비만군에서 $26.8{\pm}11.2{\mu}IU/mL$$12.5{\pm}5.24{\mu}IU/mL$인 정상군보다 유의하게 높았다. 비타민 $B_{12}$는 비만군에서 $798.6{\pm}174.3pg/mL$$967.8{\pm}405.0pg/mL$인 정상군보다 유의하게 낮았다. 그러나 엽산은 두 군 사이에 유의한 차이가 없었다. 비만군에서 인슐린은 수축기 혈압(r=0.535), 중성 지방(r=0.517), 총 콜레스테롤(r=0.408), 복부지방률(r=0.306)과 양의 상관관계가 있었다. 결 론 : 비만아에서 호모시스테인, 인슐린과 비타민 $B_{12}$가 의미있는 혈중 농도 차이를 보였으나 심혈관 질환 등의 성인병을 예방하는 예측 인자로 사용할 수 있는지 알기 위해 더 많은 연구가 필요하겠다.

Effectivenes of Weight Control Program for Obese Children in Chuncheon

  • The effects of a four-week weight control program including nutrition, exercise, behavior modification and meditation were studied in 15 obese children who resided in the Chuncheon area. There were no differences in anthropometric value, health perception, self-esteem and nutrition knowledge before and after the nutrition education program. Food behavior significantly improved after the program, especially in the area of binge eating (p〈0.05). Consumption of ramyon and fried chicken significantly decreased (p〈0.05). These results showed that short-term nutrition education programs did not do enough to change the anthroppometric value of study subject. These results suggest that it is necessary to include parents in nutrition education programs for greater effectiveness. And there is a need to develop an apply systematic nutrition education programs to reduce the weight of obese children.

학령기 비만아동의 건강증진행위 예측모형 (A Predictive Model of Health Promotion Behavior in Obese School-Age Children)

  • Purpose: The purpose of this study was to propose and to test a predictive model that could explain and predict the health promotion behavior of obese school-age children in Korea. Methods: Participants for this study were 365 students from 13 elementary schools located in Jeonbuk Province, Korea. The data were analyzed using SPSS 15.0 program and Amos 7.0 program. Results: The results verified the factors that influence health promotion behavior of the participants. Important direct factors were prior health-related behavior, perceived self-efficacy, and commitment to a plan of action and indirect factors were perceived barrier and activity-related effect. These factors explained 75.3% of variance in the participants' health promotion behavior. The proposed model was concise and extensive in predicting health promotion behavior of the participants. Conclusion: Findings may provide useful assistance in developing effective nursing interventions for maintaining and promoting health promotion behavior in obese school-age children.

국내 비만 초등학생들의 식이와 운동 중재 효과에 대한 체계적 문헌고찰 및 메타분석 (Systematic Review and Meta-analysis of Dietary and Exercise Intervention Effects of Obesity Elementary School Students in Korea)

  • Purpose: The purpose of this study was to identify the trends regarding diet and exercise intervention studies for Korean obese children between 2010 and 2017. Methods: This study was synthesized and reviewed systematically by meta-analysis. In addition, a total of thirty nine studies were investigated. The random effect model was used for meta-analysis. Results: Most studies used single interventions and that their theoretical frameworks still required improvement. In addition, on-line education programs still need to increase their number over that of off-line ones. Regarding the dependent variables for understandings the influences obesity may have on Korean children, most studies took advantage of biological indicators. In terms of the effects of obesity management programs, multiple interventions have gained a competitive edge over single ones for Korean obese children's diet and exercise. In a similar vein, healthy eating habits and adequate physical activities would have more positive effects on Korean children' obesity management programs. Conclusion: Further various studies will be needed for the early detection and prevention of obese children through varied interventions and qualitative improvement of studies.

편식아 및 비만아의 영양교육 효과 (A Nutrition Education Program for the Children of Obese or Unbalanced Dietary Habits)

  • A nutrition education program has been designed for the children of obese or unbalanced dietary habits. A total of 120 children(60-obese and 60-unbalanced dietary habits) who were in 5th-6th grade was chosen for the program and the effectiveness of the nutrition education was evaluated. The nutrition education program was organized into daily lessons(3 wks). A booklet was made for the education and used during the education period, dealing with 5-Basic Food Groups and their functions, excess or deficiency in a human body, food exchange list, importance of health, obesity, unbalance dietary habits and of school lunch program were emphasized in every day session. dietary recall records were collected to assess their daily food intake and the amounts of meals were discussed. During the education period, booklets, wall charts, photographs, food models, VTR films and slide films were utilized. Assessment of effects of the nutrition education program was carried out by a nutrition knowledge test, food habit records, anthropometric measurements and food preference test. The nutrition knowledge scores were significantly improved after the education and the scores were higher on the children in the school with school lunch program. The anthropometric measurements and food preference test revealed no significant influences of the education on the children, showing that the education period was too short to change their eating behavior any may need a long-term education program. Food habit scores were improved after the education in both experimental and control groups. The experimental groups showed higher scores than the control group.

중등도 이상의 소아 비만 치험 3례 보고 (Three Cases Report about Obese Children over the Middle Level)

  • Objective : As today obese children have increased rapidly in Korea, the interest has been much increased. The childhood obesity causes not only physical problems but also psychosocial problems and it is more difficult than that of adult to treat. That's why we started to intervene actively. Methods : In the Oriental Medical Center, several children were treated for obesity with aroma massage, low frequency wave acupuncture therapy. auricular acupuncture therapy and behavior modification and 3 children over the middle level are mentioned in this study with good results. Results : 1) After treatments, they became taller$(1.1667{\pm}0.8505cm)$. 2) After treatments, they lost their weights$(1.6333{\pm}0.7768kg)$. 3) After treatments, their BMI and OI were reduced$(1.4667{\pm}0.5774kg/m^2,\;9.8{\pm}1.7776%)$. Conclusion : We report the good results of oriental medicine on three cases of obese children over the middle level. And the further study is needed with more cases, longer duration and other tools.

비만아동의 성별에 따른 행동특성 및 인식에 대한 연구 (Gender Differences in Behavioral Characteristics and Their Awareness of Obese Children)

  • The purpose of this study was to investigate gender differences in behavioral characteristics and their awareness of obese school children. The results were as follows 1. In comparison with eating habits and food preference, the girls had more irregular breakfast and were likely to have vegetables / fruits than the boys. 2. In characteristics related to exercise and activities, boys did more exercise than girls. Particularly, in lunch free time, boys participated in playing various active excercise with friends, but girls participated in more non-active behaviors (such as, chattering, reading, or playing jack-stones). 3. In comparison with their awareness related to obese persons, about a half of boys had positive opinions (e. grong, healthy.), but about only 20% of girls had positive opinions.

Effects of Nutritional Education and Physical Exercise Program on Nutritional and Health Status of Obese Children in Busan Metropolitan City

  • The objective of this study was to investigate the effects of a nutritional education and physical exercise program on obese children (47 boys and 36 girls). The nutritional education and physical exercise program was provided for 6 weeks and dietary habits, nutrient intake and hematological profiles were measured before and after the program. The BMI (body mass index, $kg/m^2$) in male children was significantly reduced to $24.58{\pm}2.81$ from $25.89{\pm}3.27$ (p<0.01) and it was significantly reduced to $24.29{\pm}3.51$ from $25.21{\pm}3.59$ in female children after the completion of nutritional education and exercise program. Body fat in male children was remarkably reduced to $32.69{\pm}7.74%$ after education from $37.38{\pm}9.21%$ (p<0.001). After the program total cholesterol concentration in male children was reduced to $176.48{\pm}29.10\;mg/dL$ from $196.03{\pm}29.10\;mg/dL$, whereas there was no significant difference in females before and after the nutritional education and exercise program. The grasping power of male children (p<0.05) and backmuscle strength of male (p<0.05) and female children (p<0.001) were significantly increased after the exercise program. The nutrient intakes that were different before and after the nutrition education were calories (p<0.05) and carbohydrate (p<0.05) in male children, and a significant increase in intake of vitamin A (p<0.05) was found in female children. There was a significant difference in meal time regularity before and after program. The findings of this study showed that there was a significant decrease in both body fat and weight, in addition to normalization of hematological profiles after the completion of nutrition education and physical exercise programs.

초등학교 비만아동 및 정상체중아동의 식행동과 어머니의 식행동 지도 (The Dietary Behavior of Obese and Normal Weight Elementary School Children with Maternal Guidance for Their Dietary Behavior)

  • This study was conducted to investigate whether there were any differences in the body image perception, dietary behavior and maternal dietary guidance between an obese group (“OG”) and a normal weight group (“NWG”), and furthermore, to provide basic informations for comprehensive educational programs for obese children. For this purpose, the questionnaire method was used. The subjects were 1,501 elementary school children, in the 4th to the 6th grades in Seoul, Busan, Daegu, Gwangju, and Andong, and their mothers (1,459). The percentage of the children who misperceive their body image was higher among the girls (30.4%) than the boys (22.6%) The average scores for balanced dietary behavior as assessed by the children was higher in the OG than in NWG; the average scores for dietary control as assessed by their mothers was higher in the NWG than in the OG. The scores for dietary behavior as assessed by the children was different from those as assessed by their mothers. Assuming that the mothers would give more correct and balanced answers, the development of a reliable questionnaire for dietary behavior that could be more accurately answered by the children is necessary. Maternal dietary guidance for dietary control was more frequently given in the OG as compared with the NWG and more frequently in girls as compared with boys ; while that for balanced diet was more in the NWG as compared with the OG. The worse the mothers estimate of the dietary behavior for their children, the more they tried to guide their children in dietary behavior. It is recommended that the children should be taught to correctly recognize the degree of their obesity, and receive the appropriate educational program accordingly, including the maternal dietary guidance.
