The purpose of this study was to the determine the optimal torques values to tighten the retaining screw. 3-different implant system tested were as follows : Branemark implant system$(3.75mmD{\times}100mm)$, Steri-Oss implant system$(3.8mmD{\times}10mm)$. One fixtures of each implant system was mounted into the epoxy resin block and abutment/superstructure complex was constructed. Eighty dental college students(male : 40, female : 40) of Chosun University were selected and were asked to tighten the retaining screws. Abutment/superstructure complex of each implant system was tightened to the maximum torque by use of hand-held screw driver, and then torque value was measured with torque value was measured with torque driver(Tohnichi torque driver, model 20 FTD, Tohnichi MFG, Co., LTD., Tokyo, Japan). Abutment/superstructure complex of each implant system was titghtened to each torque of 10 N-cm, 20 N-cm and 30 N-cm, and then the dynamic load(vertical & diagonal load) was applied to the abutment / superstructure complex. The gap between abutment/superstrure in each implant system was measured with 3-dimensional measuring microscope(model No. 850, Germany). The results were as follow : 1. Torque values according to the individual subjects showed wide range. 2. Torque values according to sex showed statistical significant difference. Those are as follows : in case of male, $9.38{\pm}2.93$ N-cm ; incase of female, $7.80{\pm}2.25$ N-cm. 3. Torque values according t implant systems showed statistical significant difference. Those are as follows : in ase of Branemark implant system, $6.54{\pm}1.54$ N-cm : in ase of Steri-Oss implant system, $10.1{\pm}2.88$ N-cm ; in case of IMZ implant system, $9.18{\pm}2.17$ N-cm. 4. The more torque value of tightening screw was increased, the less the gap was after the vertical and diagonal loading. 5. The gap after the diagonal loading was greater than that after the vertical loading. 6. The magnitude of gap between abutment/superstructure in order of IMZ, Steri-Oss, Branemark implant system after the verical and diagonal loading. 7. The gap under the diagonal loading after applying 30 N-cm torque showed no statistical significant difference in cases of the Branemark system and the Steri-Oss implant system but it showed significant different in case of the IMZ implant system.
Background: Auricular acupressure is one of the traditional health care treatments in oriental medicine. Approximately, 30~40% of the cancer patients have said to be suffering from insomnia and half of them having chronic and severe insomnia at the same time. Insomnia caused cancer patients feel more pain, fatigue, depression and anxiety and it sometimes let the power to have the best of cancer pull down. Objective: To investigate how effective the auricular acupressure treatment to cancer patients suffering from insomnia. Methods: We recruited participants from East-West Cancer Center of Daejeon University. Finally, of the people whose age range from 20 to 75, 12 patients who got less than 40 points from the score of Oh's sleeping score (OSS) were recruited. Single-blind, randomized pilot study was performed. The treatment group received auricular acupressure treatment (AAT) on active points and the control group had received sham acupressure treatment (SAT) for five times. Sleep parameters were checked by using OSS and numeric rating scale (NRS). We checked the scale everytime, both before and after treatment. We analyzed the data statistically by using independent T-test, paired T-test and analysis of variance (ANOVA) test. (p<0.05) Results: Twelve cancer patients participated in this pilot study and there was no significant difference between control and treatment group. Only 7 of them had completed the whole treatment process, 4 patients of AAT group and 3 participants of SAT. The OSS of AAT group had increased from $34.0{\pm}4.3$ to $39.5{\pm}3.1$ and that of SAT group had increased from $38.3{\pm}3.5$ to $40.0{\pm}0.0$. There was no significant difference between them. The NRS of AAT group had increased from $6.3{\pm}2.9$, $04.8{\pm}2.1$ and that of SAT group had increased from $7.0{\pm}1.0$ to $5.0{\pm}2.6$. No significant difference was observed between them. Conclusion: Although both groups did not show significant differences, most of the experimental participants showed increasing OSS and NRS after treatments. Significant participants' number will be needed in the next study.
Purpose: This research sought to determine the resorption rate of bone grafted to the maxillary sinus according to the grafted material's type, patient's age, systemic disease, implant size, site of implant placement, and residual ridge height. Materials and Methods: This research targeted 24 patients who had immediate Osstem$^{(R)}$ implant (US Plus$^{(R)}$) placement after bone graft. The panorama was taken before the surgery, after the surgery, and 6 months after the surgery. Vertical height change and resorption rate of the grafted bone were measured with the same X-rays and compared. The influence of the following factors on the grafted bone material's resorption rate was evaluated: grafted material type, patient's age, systemic disease, implant size, site of implant placement, and residual ridge height. Results: Patients in their 40s had $34.0{\pm}21.1%$ resorption rate, which was significantly higher compared to the other age groups (P<0.05). There was no significant relationship between systemic disease and grafted bone resorption. There was no significant relationship between implant size (diameter, length) and grafted bone resorption. There was no significant relationship between the site of implant placement and grafted bone resorption. The ramal bone-grafted site was significantly more resorbed than the ramal bone/Bio-Oss$^{(R)}$-grafted site, maxillary tuberosity bone/Bio-Oss$^{(R)}$-grafted site, and ramal bone/maxillary tuberosity bone/Bio-Oss$^{(R)}$-grafted site (P<0.05). There was no significant difference in the grafted bone resorption rate in the sinus between more than 4 mm and less than 4 mm residual ridge heights. After an average of 6 months, a second surgery was done; given an average follow-up of 1.9 years, the success rate and survival rate of the implant were 96.9% and 98.4%, respectively. Conclusion: These results indicate that the bone resorption rate of grafted bone among patients in their 40s is higher compared to patients in their 50s and over, and that only autogenous bone (ramus) shows higher resorption rate than the mixed graft of autogenous bone and xenogenous graft (Bio-oss) after maxillary sinus graft.
The aim of this study was to achieve healing of Peri-implantitis defects and hard tissue augmentation using a bovine-derived bone mineral on the defect site. Two patients were treated with the surgical approach. With a full muco-periosteal flap elevation, the implant surfaces were exposed and granulation tissue removed around the implant and between the threads. Each surface of the contaminated implant was prepared with the air-abrasive device(PerioFlow$^{(R)}$) for decontamination. Bovine-derived bone mineral(Bio-Oss collagen$^{(R)}$) was then used to fill the defects and muco-periosteal flaps sutured to achieve transmucosal healing. Radiographs and clinical photographs were taken before and after 6 months of healing and an estimate of bone fill was assessed. Within the limits of the present case report, a surgical approach in treatment of peri-implantitis defects using a collagen form of bovine bone mineral was visited. Although limited, the two cases showed the stability and biocompatibility of a bovine-derived bone mineral and effectiveness of air-abrasive device(PerioFlow$^{(R)}$) as a decontamination method.

The purpose of this study is to evaluate the superficial bony healing after guided bony regeneration using a various bone grafts. Four types of bone grafts were performed by one oral and maxillofacial surgeon to restore the defects around endosseous implants. Group I included the allografts using $Regenaform^{(R)}$. Group 2 included the autograft. Group 3 included the combined grafts using with autogenous symphysis bone and xenograft($BioOss^{(R)}$). Group 4 included the xenograft($BioOss^{(R)}$). After some heling period, superficial bone biopsy was performed with the surgical blade(#15) during the second surgery. Histologic and histomorphmetric examination were carried out by one pathologist. There was the most new bone formation in the group 3, next group 2. However, there were no statistically significant differences. All group except for group 4 showed favorable bone formation and remodeling.
Bovine bone-derived bone substitutes are widely used for treatment of bone defects in dental and orthopedic regenerative surgery. The purpose of this study was to evaluate the basic characteristics of deproteinized bovine bone mineral as a bone graft substitute. Commercially available products from three different bovine bone minerals-Bio-Oss(GeistlichPharma, Switzerland), BBP(Oscotec. Korea), Osteograf/N-300(Dentsply Friadent Ceramed, USA) - were investigated. They were evaluated by scanning electron microscopy(SEM), energy dispersive X-ray spectrometer(EDS), surface area analysis(BET), and Kjeldahl protein analysis. Cell viability on different products was evaluated by 3-(4, 5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide(MTT) assay. The results of this study indicated that each bone substitute displayed distinct surface properties. Furthermore, Kjeldahl protein analysis indicated that residual crude proteins are present in deproteinized bovine bone mineral. BBP showed relatively large amount of residual protein, which indicated that the possibility of disease transmission can not be safely ruled out. Based on the results of this study, it is suggested that active quality management is strongly needed in operations that involve processing bovine bone tissue for medical use.
Background: The Friends and Family Test (FFT) developed by the UK National Health Service evaluates whether patients are satisfied with a service provided, where improvements are needed, and how likely patients are to recommend the intervention. Calculated from the FFT, the Net Promoter Score (NPS) creates a recommendation metric for treatment. The primary aim of this prospective study is to evaluate NPS for arthroscopic subacromial decompression (ASD) and rotator cuff repair (RCR). Secondary aims are to postoperatively evaluate 1-year changes in patients' Oxford Shoulder Scores (OSSs) in terms of the proportion of patients satisfied with their surgery and correlation with FFT. Methods: During a 2-year period, all patients undergoing ASD or RCR completed questionnaires prospectively. Collected preoperatively and postoperatively at 1 year. Results: NPSs were 31 for ASD (n=32) and 52 for RCR (n=39). OSSs increased by 4.3 and 6.9 for ASD and RCR, respectively (P<0.001). Overall, 75% of ASD and 77% of RCR patients were either "satisfied" or "very satisfied," respectively, with procedure outcomes. Scores from FFT had a positive correlation with improvement in OSS and satisfaction scores among patients undergoing arthroscopic shoulder surgeries (P<0.001). Conclusions: The current study shows positive NPS outcomes in patients with ASD and RCR. Scores from FFT correlate well with both satisfaction and OSS among patients. NPS can be an adjunct to traditional patient-reported outcome measures to provide global evaluation of patient experiences to aid in determining the clinical value of common procedures in shoulder orthopaedics. Purpose: This study aimed to develop a scale to measure hospital nurses' silence behavior and examine its validity and reliability. Methods: A total of 52 preliminary items on hospital nurses' silence behavior were selected using a content validity test by seven experts on 53 candidate items derived from a literature review and in-depth interviews with 14 nurses. A total of 405 hospital nurses participated in a psychometric testing. Data analysis comprised item analysis, exploratory and confirmatory factor analyses, and convergent and discriminant validity tests. Pearson's correlation coefficient was used for assessing concurrent validity, and Cronbach's alpha was used for the reliability test. Results: The final scale consisted of nine factors with 31 items, exhibiting acceptable model fit indices, convergent validity, and discriminant validity. The score of the entire scale was positively correlated with the 'Organizational Silence Scale (OSS)-the issues on which nurses remain silent' (r = .60, p < .001) and 'OSS-the reasons why nurses remain silent' (r = .68, p < .001). Cronbach's α of the scale was .92, and α of each subscale ranged from .71 to .90. Conclusion: The Hospital Nurses' Silence Behavior Scale is a useful tool for assessing multifaceted silence behavior among nurses. It can provide basic data for developing better communication strategies among nurses and other hospital staff.
제4차 산업혁명을 이끄는 주요 원동력 중 하나인 인공지능 기술은 이미지와 음성 인식 등 여러 분야에서 사람과 유사하거나 더 뛰어난 능력을 보이며, 사회 전반에 미치게 될 다양한 영향력으로 인하여 높은 주목을 받고 있다. 특히, 인공지능 기술은 의료, 금융, 제조, 서비스, 교육 등 광범위한 분야에서 활용이 가능하기 때문에, 현재의 기술 동향을 파악하고 발전 방향을 분석하기 위한 노력들 또한 활발히 이루어지고 있다. 한편, 이러한 인공지능 기술의 급속한 발전 배경에는 학습, 추론, 인식 등의 복잡한 인공지능 알고리즘을 개발할 수 있는 주요 플랫폼들이 오픈 소스로 공개되면서, 이를 활용한 기술과 서비스들의 개발이 비약적으로 증가하고 있는 것이 주요 요인 중 하나로 확인된다. 또한, 주요 글로벌 기업들이 개발한 자연어 인식, 음성 인식, 이미지 인식 기능 등의 인공지능 소프트웨어들이 오픈 소스 소프트웨어(OSS: Open Sources Software)로 무료로 공개되면서 기술확산에 크게 기여하고 있다. 이에 따라, 본 연구에서는 온라인상에서 다수의 협업을 통하여 개발이 이루어지고 있는 인공지능과 관련된 주요 오픈 소스 소프트웨어 프로젝트들을 분석하여, 인공지능 기술 개발 현황에 대한 보다 실질적인 동향을 파악하고자 한다. 이를 위하여 깃허브(Github) 상에서 2000년부터 2018년 7월까지 생성된 인공지능과 관련된 주요 프로젝트들의 목록을 검색 및 수집하였으며, 수집 된 프로젝트들의 특징과 기술 분야를 의미하는 토픽 정보들을 대상으로 텍스트 마이닝 기법을 적용하여 주요 기술들의 개발 동향을 연도별로 상세하게 확인하였다. 분석 결과, 인공지능과 관련된 오픈 소스 소프트웨어들은 2016년을 기준으로 급격하게 증가하는 추세이며, 토픽들의 관계 분석을 통하여 주요 기술 동향이 '알고리즘', '프로그래밍 언어', '응용분야', '개발 도구'의 범주로 구분하는 것이 가능함을 확인하였다. 이러한 분석 결과를 바탕으로, 향후 다양한 분야에서의 활용을 위해 개발되고 있는 인공지능 관련 기술들을 보다 상세하게 구분하여 확인하는 것이 가능할 것이며, 효과적인 발전 방향 모색과 변화 추이 분석에 활용이 가능할 것이다.
