Traditional Korean Medicine(TKM) Management for the Recovery after Laparoscopic Gynecological Surgery (부인과 복강경 수술 후 한의진료 방향에 대한 고찰)

  • Purpose: The purpose of this study is to report the TKM Management for the recovery after laparoscopic gynecological surgery. Methods: The basic informations about laparoscopy and report 3 laparoscopic postoperative patients were managed with TKM. Patients had taken the surgery for the different gynecologic diseases. Results: The 3 patients' symptoms were improved gradually. TKM management is good for the recovery of laparoscopic surgery. And the management need to reflect postoperative complications. the cause and part of operation. and postoperative common symptoms. Conclusion: The TKM managements are effective in the postoperative recovery after laparoscopic gynecological surgery. And more study is needed for developing the model.

Effect of Geijibokryung-hwan and Combination of Geijibokryung-hwan and Gangji-hwan on Obesity and Lipid Metabolism in Ob/Ob Mice (Ob/Ob 마우스에서 계지복령환(桂枝茯苓丸)과 계지복령환(桂枝茯苓丸) 합강지환(合降脂丸)이 비만 및 지방대사에 미치는 영향)

  • Objectives: This study was designed to investigate anti-obesity effects the improvement effects of Gyejibongnyeong-hwan and Gyejibongnyeong-hwan-Gangji-hwan (CIPPDF) in a ob/ob mouse model. Methods: Seven-week old mice (wild-type C57BL/6J and ob/ob) were used for all experiments. Wild-type C57BL/6J mice were used as normal group and obese ob/ob mice were randomly divided into 4 groups. a normal group given a standard diet, an obese control group given a standard diet with CIPP (300 mg/kg), CIPPDF (1) (300+300 mg/kg), CIPPDF (2) (300+600 mg/kg) respectively. After 10 weeks of treatment, body weight gain, feeding efficiency ratio, blood lipid markers, mRNA levels of genes involved in fatty acid ${\beta}-oxidation$ and lipogenesis in in-vivo, were examined. Results: 1. Body weight gain and Feeding efficiency ratio were significantly decreased in CIPPDF (1) compared with control. Fat mass was significantly decreased in CIPPDF (2) in EAT compared with control. 2. Consistent their effects on body weight gain and fat mass, circulating concentrations of LDL-cholesterol were decreased in CIPPDF (1), CIPPDF (2) groups compared with control. 3. MCAD mRNA levels of genes was increased in CIPPDF (1), CIPPDF (2) groups in the liver, epididymal adipose tissue compared with control. VLCAD mRNA levels of genes was increased in CIPPDF (1), CIPPDF (2) groups in the skeletal muscle compared with control. 4. $PPAR{\gamma}$ mRNA was decreased in CIPPDF (1) in the liver compared with control. SCD1 mRNA was decreased in CIPPDF (1), CIPPDF (2) groups in the epididymal adipose tissue compared with control. Conclusions: In conclusion, These results suggest that CIPPDF not only decrease feeding efficiency ratio, and LDL-cholesterol, but also reduce EAT fat mass contributing to the improvement of ovesity. CIPPDF also were increased in mRNA levels of genes involved in fatty acid ${\beta}-oxidation$ and decreased in mRNA levels of genes involved in lipogenesis.

  • Purpose : Because girls at puberty· are lack in sex ability, temporary menstruation disorder can be occured. This disorder is considered that will be disappeared as growing, so people used to leave the disease untreated and just watched. But clinically I frequently experience not to disappear. So I have carried out this study to investigate the actual condition of young girls's menstrual disorders. Methods : I researched 440 high school girls in Pusan by Menstruation Diary which I made about menstrual cycle, duration, amount and pain. The results were managed by the ststistics. Results :1. Menstrual cycle 1) In disorder of menstrual cycle, persons who have Bate menstruation are more than persons who have premature menstruation. 2) Persons who have normal menstrual cycle are in 124 persons(28.51%), the others who have severe premature menstruation or late menstruation more than one time for 4-7 months are in 311 Persons.(71.49%) 2. Menstrual duration and amount 1) Persons who have normal menstrual amount we in 66-89%, hypermenorrhea is in 1-11%, hypomenorrhea is in 5-21%. 2) In the study of menstrual duration, persons more than one thirds are irregular in thier menstrual amount every menstruation. 3) In the study of MMQ, persons who are irregular in thier menstrual amount every menstruation are in 125persons.(29.76%) 3. Menstrual pain 1) Persons who have slight menstrual pain are in 289 persons(65.98%), the middle is in 86 persons(19.63%) the severe is in 34 persons(7.76%) by MMP. 2) Persons who are irregular in thier menstrual pain every menstruation are in 145 persons.(33.11%) Conclusion : In menstrual cycle, there are more persons who have irregular menstrual cycle than normal.(71.49%) In menstrual duration and amount, more persons have normal menstrual amount.(66-89%) In menstrual pain, persons who have slight menstrual pain are the most.(65.98%)

Neuroprotective Effect of Yukul-tang against the Oxidative Stress (육울탕(六鬱湯)의 산화적 스트레스에 대한 뇌세포 보호효과)

  • Purpose: In this rapidly aging society, the research and development of traditional oriental medicine treatment is one of the critical factors to protect the increasing neuro-degenerative disorders. In this study, we wanted to verify the effect of Yukul-tang (YUT) on neuro-degenerative disease model by assessing the antioxidant and anti-inflammation effects. Methods: To assess the antioxidant effects of YUT, we carried out DPPH radical and ABTS radical scavenging assays and determined the total polyphenolic contents in YUT. To evaluate the neuro-protective effects of YUT, we performed the MTT and ROS assays and TH immunohistochemistry, NO and TNF-${\alpha}$ assays in SH-SY5Y or mesencephalic dopaminergic neurons damaged by 6-OHDA. Results: The treatment of YUT showed eliminating effects on DPPH radical and ABTS radical. it showed deterring effects on ROS, NO and TNF-${\alpha}$ and protecting effects on TH-positive cell in SH-SY5Y cells or mesencephalic dopaminergic neurons. Especially in the case of the treatment of YUT with 0.2ug/mL + 6-OHDA 10uM, the protective effect on dopaminergic neurons was most outstanding. Conclusion: In this study, we have demonstrated that YUT has an antioxidant effect and a neuro-protective effect on neuro-degenerative disease model caused by neurotoxin such as 6-OHDA. The results of our present study suggest that YUT can be useful agent to prevent and to treat neuro-degenerative diseases.

Effects of Panax Ginseng on the Sperm Motility and Spermatogenesis in the SD Rat

  • Objective : This study was conducted to investigate the effects of Panax Ginseng (인삼) on the sperm motility and spermatogenesis in the male rat. Methods : We used 8-week-old Sprague-Dawley rats, and administered the extract powder of Panax Ginseng to 5 rats (treated group) and normal saline (control group) once a day for 28 days. We isolated their testes surgically, then observed the change of the body weights before and after administration of Panax Ginseng extracts and normal saline. We observed the weight of the testes, epididymis, vascular gland, and prostate. Also, we examined the total, normal motile sperm concentration, and the concentration of testicular catalase and peroxidase. Results : We found that the concentration of normal, motile sperm in the testes of the Panax Ginseng group showed a significant difference compared with the control group. The angiogenesis of the seminiferous tubule was increased and the increasement of the number of spermatogonia, primary and secondary spermatocyte was observed in the Panax Ginseng group through a microscope. The body weight, the weight of the testes, epididymis, prostate and the concentration of testicular catalase and peroxidase were higher in the Panax Ginseng group but showed no significant difference. Conclusion : This study shows that Panax Ginseng may have an effect on the morphology and motility of sperm, the important factor in male fertility, and can promote the concentration of antioxidants, catalase and peroxidase, which is the important factor in spermatogenesis.

Review on RCT Research Papers Related Nausea and Vomiting in Pregnancy - Focusing on Acupoint Treatment - (임신오조(姙娠惡阻) 관련 무작위대조군연구(RCT)에 관한 고찰 - 경혈을 이용한 치료를 중심으로 -)

  • Objectives: To observe randomized controlled trials(RCTs) related to investigating the effects of acupoint treatments on nausea and vomiting in pregnancy to establish the basis of korean medical treatments, and suggest the problems of future clinical research. Methods: After searching for national and international papers published after 2000 and restricting the parameters to 'clinical trial', 'controlled clinical trial', 'randomized controlled trial', then comparing the subjects, methods, research tools, and results to understand the effects and re-comparing the conflicting results to assume the cause. Results: Of the 9 research papers that applied acupressure, four of them used control and experimental groups, and three(75%) research papers indicated significant reduction in vomiting and nausea, and of the seven research papers that utilized the experimental and placebo groups, five(71.4%) reported significant reducing effects. Of the four research papers the used acupuncture treatments, four research papers using control and placebo groups, two showed improvements in symptoms but did not have significant results compared to sham acupuncture groups, and two showed significant reduction of symptoms compared to the control groups. The one research paper that used electrical stimulation showed significant results in the reduction of vomiting and nausea compared to the control group. Conclusions: Acupressure and electrical stimulation showed efficacy reducing the symptoms. Acupuncture treatments did not indicate efficacy some experiments. Therefore it is thought that future acupuncture treatments should use methods that resemble actual clinical medicine to establish efficacy to produce significant results.

A Survey Study on Use Condition of Korean Medical Institution and Demand of Korean Medical Treatment Project of Infertility in Infertile Couple (난임부부의 한방의료기관 이용 실태 및 한방난임치료 지원사업 수요에 관한 설문조사연구)

  • Objectives: The purpose of this study is to investigate use condition of Korean medical institution and demand of Korean Medical Treatment Project of Infertility in infertile couple. Methods: Questionnaires were distributed to all members of the incorporated association AGAYA(http://www.agaya.org/) via e-mail and to infertile women who visited to Korean medicine OB & GY outpatient clinic of university Korean medicine hospitals of the country and an infertility specialized Korean medicine hospital. One hundred twenty four of completed questionnaires were analyzed. Results: 1. Korean medical institution utilization rate of infertile couple was 89.6% and was similar with western medical institution utilization rate of 87.1%. 2. 96.8% of infertile couple answered that Korean Medical Treatment Project of Infertility is necessary. And 90.3% of infertile couple answered that herbal medicines should be supported first of all. 3. 90.3% of infertile couple answered that they will participate if Korean Medical Treatment Project of Infertility go ahead. Conclusions: It was confirmed that most of the infertile couple have been received Korean medical treatment for infertility, and think that Korean Medical Treatment Project of Infertility is necessary.

A Study on Thermal Effect and Medication Compliance of Red Ginseng Extract (홍삼의 온열 효과와 복약 순응도에 관한 연구)

  • Purpose: To evaluate thermal effect and medication compliance of red ginseng extract. Methods: Randomized, double-blind, placebo-controlled, cross-over clinical study was performed. Twenty four healthy, married women aged 30-45 years with FSFI score below 25 were randomly divided into two groups; red ginseng group(N=12) and placebo group(N=12). During the first 6-week period (Study1), each group was dosed with red ginseng or placebo twice a day. Before starting the second 6-week period(Study2), a crossover design was chosen with a 2-week break(Washout period). Interchanging two groups after Washout period, red ginseng and placebo were dosed to each group. The efficacy of thermal effect was measured with subjective warm sensation scale and lower abdomen temperature by Digital Infrared Thermographic imaging(DITI) before and after each 6-week period. A medication compliance was assessed after each 6-week period and the correlation medication compliance between Sasang Constitution and subjective warm sensation was analyzed. Results: Overall 23 participants completed the study. In subjective warm sensation scale, after taking placebo, all participants exhibited an improving trend, but there was no significant difference. In lower abdomen temperature by DITI, statistically significant objective thermal effect of red ginseng was also not shown. A medication compliance was higher in Yin constitution(Taeumin, Soeumin), and showed an upward trend with decreasing subjective warm sensation. But no statistically significant difference was exhibited. Conclusion: Statistically significant thermal effect of red ginseng was not shown in this study. We anticipate if a long-term clinical trial is practiced, significant thermal effect of red ginseng will be shown.

A Case Report of Patient Treated after Stillbirth Caused by Oligohydramnios (양수과소증으로 인한 사산 후 산모 치험 1례)

  • Purpose: The purpose of this study is to report the effect of Traditional Korean Medicine therapy for patient in the state of post stillbirth by oligohydramnios Methods: The patient in this case was 33-year-old female who was pregnant by artificial insemination in 2010. Intrauterine growth restriction was discovered at 17 weeks after gestation. And oligohydramnios was found at 20 weeks after gestation. Amnioinfusion was done three times, amniotic fluid infection occurred in the process of amnioinfusion and she had a stillbirth. After stillbirth she came to our clinic with symptoms of abdominal tenderness, distention, mammalgia. The patient was treated by traditional herbal medicine therapy such as Saenghwa-tang(生化湯), Gungguijohyeol-eum(芎歸調血飮), Silso-san(失笑散) and Sasang constitution medicine(荊防地黃湯, 荊防敗毒散, 荊防瀉白散). The progresses of symptoms were evaluated by visual analogue scale. Results: After Traditional Korean Medicine therapy, the patient's symptoms caused by stillbirth was improved. Conclusion: This case study shows that the postpartum treatment in Traditional Korean Medicine therapy and Sasang constitution medicine is effective for the patient in the state of post stillbirth.
