A Study on Improvement of School Lunch Programs and Its Health and Education Effect in Korea (학교급식효과(學校給食效果)와 제도개선(制度改善)에 관(關)한 연구(硏究))

  • The purpose of school lunch service programs (SLPs) is to provide school children with nutritious meals each day, to assist them in improving their mental and physical development, to make them understand the basic knowledge needed for healthy daily living throughout their lives with sound nutritional habits, and further, to enhance the national health status as well as the individual. The major purpose of this study is to describe the history and current status and promotional plans in Korea's SLSPs to improve the nation's health status. The major findings of this study are as follow: 1) The progression of the SLSPs can be devided into four major periods: Relief (1953-1972), Self-supportive (1973-1977), System-establishment (1973-1983), System-development(1984- ). 2) As result of the implementation of SLSPs in Korea, many positive effects have been found in school education, notably in the field of physical growth, social development and personality cultivation. 3) This study deals with a more detailed assessment with regard to the effects of the program on school education activities and the improvement of nutritional habits in the family. Although it is limited and not yet complete, the program implementation has contributed to the improvement in children's food-intake habits, understanding of food, and manners, and increased sanitary consciousness. It has also affected of parent's concerns and attitudes towards the improvement of daily meals as well as national eating patterns, and eventually provided motivation for increased farm production of nutritious food stuffs and reasonable food consumption. 4) The SLSPs does have its stare of problem such as an unsatisfactory budget, lack of understanding of the program by school personnel and parents. However, there is no doubt that the SLSPs should be carried out effectively for better health, and especially for the growth and development of the children. Finally, the problems and promotive Plans of the SLSPs can be approached by aspects of the administrative, financial and operational systems. These system have to coordinate with relevant authorities to expand participation in the program for the successful implementation of the SLSPs. The eventual objective this study is to propose ways and means for mobilizing every available element in the effort to attain the goals of educating and training healthy and competent personnel manpower, stabilizing the demand and supply of food and the ensuring adequate coordination of national food production and consumption.

Physical Characteristics and Dietary Patterns of Strength Athletes; Bodybuilders, Weight Lifters (근력 종목선수들의 신체적 특성 및 식생활 패턴)

  • The purpose of this study was to provide a descriptive assessment of the nutritional habits of resistance trained males in relation to protein and carbohydrate intakes in comparison with the recommended values. Thirty-four male bodybuilders ($27.0{\pm}2.1years,\;173.5{\pm}5.0cm,\;8.3{\pm}0.61%$ body fat), twenty-four male weight lifters ($20.9{\pm}2.1years,\;171.8{\pm}6.9cm,\;7.6{\pm}0.98%$ body fat) and twenty-five male non-athletes ($26.9{\pm}2.5years,\;175.3{\pm}5.7cm,\;8.5{\pm}0.95%$ body fat) participated in the study. Participants completed a comprehensive survey by twenty-four hour dietary recall methods. All diets were analyzed using the Computer Aided Nutritional (Pro) analyzer by a registered dietician. Body fat was measured using skin fold thickness. The average time spent in resistance training was $18.0{\pm}1.7$ hrs/week for body builders and $14.6{\pm}8.7$ hrs/week for weight lifters. Total daily calories were $2583.6{\pm}874.8kcal\;(31.9{\pm}11.9kcal/kg)$ for bodybuilders, $3565.9{\pm}1281.8kcal\;(42.7{\pm}15.0kcal/kg)$ for weight lifters and $2016.0{\pm}955.3kcal\;(28.1{\pm}13.9kcal/kg)$ for non-athletes (p=0.001). Percent of calories from carbohydrate, protein, and fat 64.2%, 27.1%, and 8.7% for bodybuilders, and 66.3%, 18.6%, and 15.1% for weight lifters. The mean protein intake was $1.9{\pm}1.2g/kg$ of BW for bodybuilders, $1.6{\pm}0.6g/kg$ of BW for male weight lifters and $1.1{\pm}0.69g/kg$ of BW for non-athletes. Although dietary calcium (78.7%) and riboflavin (86.3%) intakes for bodybuilders were below the RDA, dietary intakes of micronutrients were not deficient. More research needs to be conducted to determine the optimal amounts of protein, carbohydrates, fats, and micronutrients for the resistance trained athletes.

Maternal Nutritional Status at the End of Pregnancy, and Correlation among Pregnancy Weight Gain, Birth Weight and Serum Leptin Levels (산모의 임신말기 영양상태와 임신 중 체중증가, 출생체중과 혈청 렙틴 농도와의 상관관계에 관한 연구)

  • The necessity of adequate pregnancy weight gain for optimal pregnancy outcome has been recognized. However, the specific components of pregnancy weight gain that might be critical for fetal growth and development have not been elucidated clearly. The purpose of this study was to investigate the correlation of pregnancy weight gain and birth weight with serum leptin levels in women delivered newborns. The subjects were recruited from K university hospital. The subject's characteristic data (age $32.1\;{\pm}\;4.3\;y$, gestational age $39.5\;{\pm}\;1.1wk$, pre-pregnancy weight $58.0\;{\pm}\;8.6\;kg$, pregnancy weight gain $12.7\;{\pm}\;5.5\;kg$, newborn's birth weight $3.5\;{\pm}\;0.5\;kg$) were gathered. Maternal dietary assessment was carried out at the end of pregnancy. After delivery, blood samples were collected from 20 mother-newborn pairs. Serum levels of various lipids and leptin were analyzed. Maternal daily consumption of iron, zinc, folate were lower than the RDA of each nutrient and index of nutritional quality was less than 1 showing that the quality of maternal diet was low. The levels of serum leptin of mothers and infants were $10.2\;{\pm}\;6.7\;ng/ml$ and $1.7\;{\pm}\;0.6\;ng/ml$, respectively. The serum leptin concentrations of male infants $(1.9\;{\pm}\;0.7\;ng\;ml)$ were not different from that of females $(1.7{\pm}0.5\;ng/ml)$. A negative correlation was found between the maternal pre-pregnant BMI and weight gain during pregnancy (r = -0.54, p < 0.05). There was a positive correlation between the pregnancy weight gain and the newborn's birth weight (r=0.59, p < 0.01 There were also positive correlation between newborn's birth weight and newborn's serum leptin levels (r = 0.57, p < 0.01). No correlations were found between maternal serum leptin levels and that of newborn's. Efforts should be made to attain adequate diet and weight gain during the pregnancy to reduce the likelihood of low or over birth weight of newborns.

A Study on Body Image Recognition, Food Habits, Food Behaviors and Nutrient Intake according to the Obesity Index of Elementary Children in Changwon (창원시 일부 초등학교 고학년의 비만도에 따른 체형인식도, 식습관, 식행동 및 영양섭취상태에 대한 비교연구)

  • This study was carried out to investigate the difference of the body image recognition, food habits, food behaviors and nutrient intake according to the obesity index(underweight-, normal-, obese group) in children(aged 10.8 ys, 47 boys, 33 girls). Subjects were eva1uated based an anthrofometric measurement and questionnaries including food record diary in Changwon. The results are summarized as follows. The weight, BMI, body fat, waist circumference, hip circumference and birth weight except height showed a significant difference between the three groups(p<0.001). The waist and hip circumference showed the possibility of being a predictor of obesity in children. The children's obesity was related to mother's weight(p<0.01), but not to father's. Ninety percent of the obese group, 47.8% of the normal group, and 37.8% of the underweight group preferred a thinner figure than their current status. The underweight group did more exercise regularly than the other groups. The underweight and obese groups had more irregular meal times and foster eating habits compared to the normal group. Most of subjects(89.9%) had an overeating habit, and tole ratio of the overeating habit increased with obesity. Fifty five percent of the subjects clad the habit of skipping meals, mainly breakfast. 46.8% of the subjects ate snacks more than twice per a day, the underweight and obese groups had snacks more of than the normal group. The preferred snack was 'cookies'and 'fruits & juices'. 53.8% of the subjects had a prejudice for special food(especially 'pulses'(37.9%) and 'vegetables'(31.0%)). The assessment scores of food behaviors was relatively low in most of the subjects. Most nutrient intakes, except vitamin B$_1$, C and phosphate, were lower than those of Korean RDA. The nutrient intake of the normal group was higher than the underweight and obese groups. The results of this study showed some nutritional problems, which indicates the need for nutritional management for the children. To educate children, who are able to change their food habits and lifestyle, each means to help healthy growth and to help them become healthy adults.

A Study on the Food Habit and Seasonal Difference of Nutrient Intake of Adult Working Women (일부 성인직장여성들의 식습관 및 계절별 영양소섭취상태 조사)

  • To assess the food habits and the seasonal differences of nutrient intakes and diet qualities of adult working women aged 30 - 49y in Busan, dietary survey was conducted in summer and in winter by a questionnaire and two-day food record. Anthropometric assessment was also investigated in two seasons. $91.8\%$ of those skipped breakfast in the main. $44.3\%$ had irregular meals. The mean daily energy intake was 1725.8 kcal with $63.3\%$ of energy intake being supplied by carbohydrates, $14.7\%$ by protein, $22.3\%$ by fat in summer and 1598.4 kcal with $62.1\%$ of energy intake being supplied by carbohydrates, $15.6\%$ by protein, $22.1\%$ by fat in winter. Over $70\%$ of iron intake came from plant origin in two seasons. The mean intakes of energy, calcium, iron and vitamin A in summer and energy, calcium, iron, vitamin A and vitamin $B_2$ in winter were below Recommended Dietary Allowance (RDA) for Koreans. As well as insufficiency in iron, the bioavailability of iron is considered to have been low because most of iron intake came from plant origin in two seasons. For calcium and iron in summer and calcium, iron, vitamin A and vitamin $B_2$ in winter, proportions of subjects with intake levels less than $75\%$ of RDA were over $40\%$ in summer and over $50\%$ in winter, respectively. The nutrient adequacy ratios (NAR) were below 0.75 for calcium and iron in summer and calcium, iron, vitamin A and vitamin $B_2$ in winter. NARs of iron (p<0.05), vitamin A (p<0.01) and vitamin $B_2$ (p<0.001) in winter were significantly lower than those in summer. The mean adequacy ratios (MAR), an index of overall dietary quality were 0.85 in summer and 0.80 in winter. The MAR in winter was significantly lower than that in summer (p < 0.05). The indexes of nutritional quality (INQ) were below 1 for calcium and iron in summer and calcium, iron, vitamin A and vitamin $B_2$ in winter. The intake (p<0.05) and NAR (p<0.05) of vitamin $B_2$ showed positive significant correlations with height in winter. In conclusion, nutrient intake and diet quality of adult working women were different between the summer and the winter. So nutritional education programs for summer and winter are needed for adult working women.

The Changes and Suggestions in Korean Dietary Guideline

  • The Recommended Dietary Allowances(RDAs, Nutrient standards), dietary guidelines, and food guides, each define aspects for a healthy diet in different ways. The RDA and food guide for Koreans were first established in 1962 by the Food and Nutrition Committee of the Korea FAO Association. The committee released the RDA and suggested ways to intake the recommended nutrients. Every five years, the committee has added more data and released revisions. The latest edition of the RDA is the 6th revision. In the beginning, the concept of basic food groups was emphasized as basic data for planning means based on RDA. In the 5th revision, the basic food groups and dietary guideline for public health from the Ministry of Health and Welfairs(December, 1990) suggests that, 1) Eat a variety of foods with a recommended fat intake equaling or less than 20% of total calories ; 2) Maintain ideal body weight and prevent obesity ; 3) Eat foods low in salt. Salt intake should not exceed 10g ; 4) Do not drink too much ; 5) Eat regularly and enjoy meals. After these guidelines were established, the first nutritonal education efforts guidelines were developed in 1984. Despite broad possibilities for application, they had limited use, mainly as a nutritional assessment and food balance sheet preparation. They were not well utilized in public nutritional education and nutritonal policy through the media because of the weakness of the government's food and nutriton policy. Also a lack of administrative support and dietitians in the health department and administrative organizations was partly to blame. In regard to public health and nutrition status, life expectancy has increased 10 years since the 70's and the elderly population increased threefold in 1995 compared to 1960. The common causes of death in 1996 by 19 Chapters classification, were first disease of the circulatory system ; the second, neoplasms ; the third, external causes fo mortality ; the forth, diseases of the digestive system ; and the fifth, respiratory system diseases, In food intake, grain and complex starch intake has decreased while fruit and animal foods have considerably increased. Therefore, energy from carbohydrates has decreased while energy from protein and fat has increased. Energy intakes from protein, fat and carbohydrates were respectively 12.5, 7.2 and 80.3% in 1969 but 16.1, 19.1 and 64.8% in 1995. 62.9% of the householes had the fat energy less than 20%, while 37.1% had the fat energy above 20%. The only intakes of vitamin A and calcium were below RDA levles. Therefore, nationwide attention should be focused on public nutriton education and public activities with supplementation of the RDAs, according to the food guide and the dietary guideline.

Assessment of Functional Food Consumption Among the Working Women in Korea (한국 직장 여성의 건강식품 섭취 실태에 관한 연구)

  • The aim of this study was to identify the status of health functional food consumption of working women. The 558 subjects of the study were aged between 10-60 years, and had previous experiences of consuming health functional foods. The majority of the subjects lived in Seoul, and some lived in the Gyeonggi Province. The most prevalent goals that the subjects hoped to achieve by consuming health functional foods were improvement of physical health status and reduction of exhaustion. Other goals included supplement of nutrition, and prevention or treatment of diseases. Those who were older or who possessed a high BMI spent more expenses on health functional foods, and showed a higher intake frequency as well as longer periods of consumption. Subjects placed in the older group showed a lower level of knowledge related to functional foods, which turned out to be statistically significant. Most of the respondents answered that their knowledge related to functional foods was picked up from listening to others. In conclusion, we were able to observe that groups with elder working women and high BMI show higher consumption and expense rates of health functional foods, as well as longer periods of consumption. However, they show a minimum level of knowledge related to functional foods. According to our results, these subjects should seek practical help in order to select functional foods that will aid their health, and learn the proper method of consumption. In order to reduce inaccurate consumption of health functional foods, it is imperative that more in-depth study is carried out in this field.

Relationships between the Nutrient Intake Status, Dietary Habits, Academic Stress and Academic Achievement in the Elementary School Children in Bucheon-si (부천시 일부 초등학생의 영양소 섭취상태, 식습관, 학업스트레스와 학업성취도와의 관련성 연구)

  • This study examined the influence of the nutrient intake status, dietary habit, and academic stress of elementary school children on their academic achievement. Two-hundred and twenty-four fifth-graders in Bucheon-si, Gyeonggido were surveyed. The study included the 24 hr-recall, anthropometric measurement, assessment of stress level and academic achievement. The subjects were normal in height, weight and Rohrer index, but higher percentage of underweight was seen in girls and vice versa in boys. The overall nutrient intake and dietary habits were fairly good, but Ca and folate intake was less than 75% KDRIs and dietary habits of boys were inferior. Academic stress level of all the subjects was not high. In terms of academic performance and its relations with nutrient intake, the more the amount of nutrient intake, the higher the academic performance. Especially, it was true for the energy, protein, phosphorus, potassium, zinc, polyunsaturated fatty acids, and n-6 fatty acid intakes (p < 0.05). The overall academic performance was higher for those who eating-out less frequently. Children with higher comprehensive dietary habit scores have shown better academic performance (p < 0.05). Less stress implied to those shown higher academic achievement while those with relatively poor academic performance showed high stress level since their grade did not improve as much as they anticipated. In conclusion, the academic achievement was higher for those who have a better nutritional status, better dietary habits, and lower stress levels. Therefore, it is critical for nutritionists, parents, and teachers to improve the nutritional status and dietary habits as well as to help them manage their stress levels, which will eventually contribute to an enhanced academic performance.

Change in Dietary Intake and Nutritional Status Using Mealworms as Hospital Meal in Postoperative Patie (갈색거저리를 이용한 식사 섭취에 따른 영양 섭취 및 영양상태 변화 : 수술 후 환자를 대상으로)

  • The purpose of this study was to verify the validity of Mealworms as a hospital meal with increased nutrition density. We provided a meal for postoperative patients and conducted analysis of dietary intake and nutritional status of patients and assessment of acceptability of the meal. This study was carried out as a randomized control trial. Patients were supplied either a hospital meal using Mealworms (Experimental group) or a regular hospital meal (Control group). We investigated the administration amounts of parenteral nutrition (PN) and food intake of patients after surgery and measured anthropometry, body composition, and blood tests before surgery and at hospital discharge. We included 34 postoperative patients who were admitted to Gangnam Severance Hospital from March to September. In the groups of patients not supplied with PN, the experimental group ($964.68{\pm}284.6kcal$, $38.82{\pm}12.9g$) had significantly higher dietary calorie and protein intake than the control group ($666.62{\pm}153.7kcal$, $24.47{\pm}4.9g$)(P<0.05). Additionally in the group of patients not supplied with PN, the experimental group (1.37%) showed a significantly higher increase in fat free mass index than the control group (-3.46%)(P<0.05). In all subjects, calorie density and protein density were significantly higher in the experimental group (P<0.001), and acceptability of calorie (P=0.036) and protein (P=0.001) was also significantly higher in the experimental group. Therefore, the results of this study support the validity of the introduction of hospital meals using Mealworms.

A Cross-sectional Study of Biochemical Analysis and Assessment of Iron Deficiency by Gestational Age(II) (임신 시기별 생화학적 철분 분석 및 철분 결핍상태에 대한 횡적 조사 연구(II))

  • The purpose of this research is to assess hematological and biochemical status and the prevalence of iron deficiency of pregnant women by gestational age to provide the primary data about iron nutritional status of pregnant women. Pregnant women visiting public health centers in Ulsan participated in study and were divided into 3 trimester by last menstrual period(LMP). Hemoglobin (Hgb), hematocrit(Hct)and mean corpuscular volume(MCV) among iron status indices were not statistically different from normal distribution, however total iron binding capacity(TIBC) and serum ferritin were skewed to left and serum iron and transferrin saturation(TS) were skewed to right. Hgb was positively correlated with Hct(r=0.93, p<0.001) but TIBC was negatively correlated with all indices. Serum ferritin was also correlated with all indices, especially in 3rd trimester but not reached to 1st trimester level. Mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), Red cell distribution width(RDW), serum iron and TS were not significantly different by trimester, however when serum serum iron was adjusted with hematocrit to correct the hemodilution, it significantly decreased in 2nd trimester. MCV increased in 2nd trimester and was maintained until late pregnancy, TIBC continued to increase throughout the trimester. The prevalence of anemic by CDC(Centers for Disease Control) Hgb criteria(Hgb <11.0g/dl in 1st and 3nd trimester, Hgb<10.5g/dl in 2nd trimester) was 2.8% in 1st trimester, 22.5% in 2nd trimester, 27.1% in 3rd trimester and was similar with prevalence by CDC Hct criteria(Hct < 33% in 1st and 3rd, Hct < 32% in 2nd). The prevalence of anemic of total subjects was 32.7% by WHO criteria(Hgb < 11.0g/dl). Although almost iron status indices increased in 3rd trimester, the prevalence of anemia by different criteria of all indices increased throughout the trimester, so iron nutritional status was considered as serious during late pregnancy. However, since factors other than iron deficiency, such as infection, infection, inflammation, other nutrient deficiency may also play a significant role, to differentiate the anemia due to mainly iron deficiency from the anemia due to other factors, serum ferritin is among the more useful indices in distinguishing the two conditions because it is depressed only in iron deficiency. Hgb<11.0g/dl and serum ferritin<12.0ug/L as the criteria of iron deficiency was suggested by CDC. 17.8% of all subjects were classified as iron deficient anemia, 14.9% as anemic from other reasons, 21.2% as iron deficiency any only 46.2% were in normal iron status.
