A Caspase Inducing Inhibitor Isolated from Forsythiae fructus (연교(Forsythiae fructus)로부터 분리한 caspase 유도 저해물질)

  • During the screening of inhibitors of caspase-3 induction in U937 human monocytic leukemia cells from natural sources, Forsythiae fructus, which showed a high level of inhibition, was selected. And then, the compound was purified from the methanol extract using silica gel column chromatography and HPLC. The inhibitor was identified as rengyolone, by spectroscophic methods of ESI-MS, $^1H-NMR$, $^{13}C-NMR$, DEPT, and HMBC. Rengyolone showed inhibitory activity of caspase-3 induction, a major protease of apoptosis cascade, with an $IC_{50}$ value of $6.25\;{\mu}g/mL$ after 7 h of treatment in U937 cells. It also showed inhibitory activity of caspace-1 induction, with an $IC_{50}$ value of $7.50\;{\mu}g/mL$ after 40 h of treatment in D10S cells. In addition, it showed protective effect against cell death with an $IC_{50}$ value of $11\;{\mu}g/mL$ on U937 cells induced by etoposide after 24 h of treatment, but did not show any cytotoxicity at the same condition without etoposide, a caspase 3 inducing agent.

A Survey of Urban Middle-Aged Women's Transition (중년여성의 전환상태 실태조사)

  • The purpose of this study was to identify transition conditions, health behavior and indicators of healthy transitions among middle-aged women based on Schumacher & Meleis(1994) transition model for developing intervention program for their health promotion. A convenience sample of 221 women aged 40-60 was obtained in Jung-Gu, Seoul and they were asked to complete the questionnaires, which consisted of modified health-promoting lifestyle profile(HPLP), knowledge of menopause, physical well-being, emotional well-being, modified women's role integration protocol (WRIP), Beck's Depression Inventory(BDI), and indicators of healthy transition with subcategories such as subjective well-being, role mastery and well-being of interrelationship. The results were as follows: 1. Women had a mean age of 47.53 years. More than half(53.39%) of the women had jobs and 88.69 % had their spouses, Of 221 women, 51.13 % were premenopausal, 19.91% were perimenopausal, and 28.96% were postmenopausal. 2. Women scored lower on health responsibility and exercise than on self actualization, nutrition and interpersonal support among subcategories of health behavior measured the modified HPLP. Only 11.98% of respondents had breast-self examination and 42.66% had pap smear for screening cancer. 3. In transition conditions, women had poor knowledge about menopause and median level of physical well-being, emotional well-being and stress. 15.45% of the women had clinical depression. 4. As for the outcome index of the transition model, the mean of indicators of healthy transition was 3.69(possible range 1-5). 5. The levels of education and economic and the menstrual status were significantly related to physical well-being, depression and stress in the categories of transition conditions. The total score of health behavior correlated negatively with depression. The total score of indicators of healthy transition correlated with physical well-being, emotional well-being, stress, and depression in the categories of transition conditions. In conclusion, these findings suggested a profile of fragile middle-aged women and contributed to developing the community-based intervention program for health promotion.

In vitro Screening of Essential Oil Active Compounds for Manipulation of Rumen Fermentation and Methane Mitigation

  • The objective of this study was to investigate the effects of 11 active compounds of essential oils (ACEO) on rumen fermentation characteristics and methane production. Two trials were conducted. In trial 1, ACEO (eugenol, carvacrol, citral, limonene, 1,4-cineole, p-cymene, linalool, bornyl acetate, ${\alpha}$-pinene, and ${\beta}$-pinene) at a dose of $1,000{\mu}L/L$ were incubated for 24 h in diluted rumen fluid with a 70:30 forage:concentrate substrate (16.2% crude protein; 36.6% neutral detergent fiber). Three fistulated Holstein cows were used as donors of rumen fluid. The reduction in methane production was observed with nine ACEO (up to 86% reduction) compared with the control (p<0.05). Among these, only limonene, 1,4-cineole, bornyl acetate, and ${\alpha}$-pinene did not inhibit volatile fatty acid (VFA) production, and only bornyl acetate produced less methane per mol of VFA compared with the control (p<0.05). In a subsequent trial, the effects on rumen fermentation and methane production of two concentrations (500 and $2,000{\mu}L/L$) of bornyl acetate, the most promising ACEO from the first trial, were evaluated using the same in vitro incubation method that was used in the first trial. In trial 2, monensin was used as a positive control. Both doses of bornyl acetate decreased (p<0.05) methane production and did not inhibit VFA production. Positive effects of bornyl acetate on methane and VFA production were more pronounced than the effects of monensin. These results confirm the ability of bornyl acetate to decrease methane production, which may help to improve the efficiency of energy use in the rumen.

Research for Health Examinations for Infants and Children by Korean Medicine (한방 영유아 건강검진 도입 필요성 연구)

  • Objectives This study is to analyze current guidelines for health check-ups in infants and children, and to develop better guidelines based on oriental medicine. Methods We analyzed The Manual of 2015's Health Examination for Infants and Children which is a Korean Developmental Screening Test for Infants and Children (K-DST). The statistics from the test was collected from 2010 to 2014 to figure out the problems of Health Examinations for Infants and Children. 20 articles from 2000 to 2014 from RISS, NDSL were also analyzed. Results The current guidelines for health examinations in infants and children didn't include major pediatric diseases such as allergy and asthma. Also, the pediatrics health education materials were mainly focused on hypernutrition but not so much on nutrition deficiency. The number of the centers for Health Examinations for Infants and Children was 52.2% of the number of NIP-Participating medical institutions for infants and children. Conclusions Oriental medicine can be useful to prevent major pediatric diseases and to promote general health in pediatrics. By legislating 'Geon-a-beop', which is a law for infants and children, we can increase the number of medical centers for infants and children to get health check-ups. Currently, there are 9,769 Korean medical institutions, and the rate of traditional medical doctors of public health doctors was 25.5%. Weak Children Questionnaire is developed, considered to be more useful when it is developed with 5-point scale rather than 2.

Continentalic acid, isolated from Aralia continentalis, inhibits cariogenic properties (독활로부터 분리한 continentalic acid의 항치아우식 활성)

  • In the present study, we investigated the anticariogenic compounds from Aralia continertalis (A. continnentalis) has been isolated and identified by MS, $^1H-NMR$ and $^{13}C-NMR$. The MeOH extract was suspended in $H_2O$ and sequentially Partitioned with $CHCl_3$, EtOAc, and n-BuOH. The $CHCl_3$ fraction showed remarkable antibacterial activity against S. mutans. The antibacterial activity compounds against 5. mutans by MIC was isolated successively through the screening system and various chromatography methods. From this active chloroform subfraction, isolation and identification finally gave continentalic acid by spectroscopic methods (MS, $^1H-NMR$ and $^{13}C-NMR$) as an active principle. The compound, continentalic acid, showed significant growth, acid production, adhesion and water-insoluble glucan synthesis inhibitory effect against 5. mutars. These results suggest that continentalic acid from A. continentalis may inhibit cariogenic properties of S. mutans and these properties may provide some scientific rationales that the local inhabitants used the extracts for treatment of dental diseases.

Concentration of Serum and Urine Creatinine in Children and Adolescents

  • Creatinine is a general marker as a screening test for renal disease. This study was conducted to provide basic data about pediatric concentration for serum and urine creatinine. The data from the 2011 Korean National Health and Nutrition Examination Survey was used. Analysis was done on 488 Children and Adolescents (boys 278, girls 210) aged 10 to 19, the Jaffe reaction method was used to gather the data. The highest serum creatinine levels were found in boys aged 19 to 20 years, the mean being 0.97 mg/dL (min 0.81 mg/dL, max 1.14 mg/dL). The levels showed increase over those in the 10 year old group. The highest urine creatinine levels were found aged 19 to 20 years, and the mean was 222.68 mg/dL (min 133 mg/dL, max 324 mg/dL). In the case of girls, the highest serum creatinine levels were found with those aged between 18 and 19, the mean being 0.71 mg/dL (min 0.49 mg/dL, max 0.84 mg/dL). The levels showed increase over the 11 to 18 year old group. The highest urine creatinine levels were found aged 14 to 15 years, and the mean was 218.44 mg/dL (min 131 mg/dL, max 321 mg/dL). The mean difference in serum creatinine in all age groups was statistically significant except for those aged 10 to 14, that of urine creatinine in all age groups was not significant statistically except for those aged 12 to 13, 17 to 18. Therefore, it is suggested that reference values for children and adolescents should be divided into different groups according to gender, further studies are needed using complementing data of the pediatrics.

Validity for Use of Non-HDL Cholesterol Rather than LDL Cholesterol

  • NonHDL cholesterol values have been suggested as a risk marker for cardiovascular disease. NonHDL cholesterol values were calculated, using a very simple measurement [nonHDL cholesterol=serum total cholesterol-HDL cholesterol]. This formula is very useful as a screening tool for identifying dyslipoproteinemias, risk assessment, and assessing the results of hypolipidemic therapy. The data from the 2009 Korean National Health and Nutrition Examination Survey were used. Analysis was done for 1,992 subjects with lipid panels (Cholesterol, HDL, LDLdirect and Triglycerides) results. We studied the relationship between nonHDL cholesterol and LDL cholesterol. As a result, nonHDL cholesterol values were plotted against the LDL direct and calculated values. The linear regression equation for nonHDL cholesterol and direct LDL cholesterol was $nonHDLchol=23.60+1.03{\times}LDLdirect$ (p<0.0001, $r^2=0.80$) in all subjects. The subjects were classified into triglyceride values. When triglycerides are below 400 mg/dL, the linear fit to LDL direct is found to be $[nonHDLchol=17.34+1.07{\times}LDLdirect]$ (p<0.0001, $r^2=0.88$) and to the Friedewald LDL calculation is $[nonHDLchol=23.10+1.02{\times}LDLcalc]$ (p<0.0001, $r^2=0.82$). For triglycerides above 400 mg/dL, the linear fit equation is $[nonHDLchol=87.57+0.92{\times}LDLdirect]$ (p<0.0001, $r^2=0.50$) and to the LDL calculated, it is $[nonHDLchol=142.70+0.50{\times}LDLcalc]$ (p<0.0001, $r^2=0.32$). This study provides examples of the utility of nonHDL cholesterol concentrations in clinical medicine.

Studies on Screening of Paraquat Toxicity Reducing Agent and its Inhibition Mechanism (Paraquat 독성 경감제 검색 및 그 억제 기전에 관한 연구)

  • In this study, we intended to evaluate the modulatory effects of natural products, ${\beta}-carotene$, aloesin and semiessential amino acid, taurine on the toxicitiy of paraquat. In the taurine treated groups, serem glutamic oxaloacetic transaminase (s-GOT), serem glutamic pyruvic transaminase (s-GPT). blood urea nitrogen (BUN), creatinine, malondialdehyde (MDA), alkaline phosphatase (ALP) activity in serum and MDA, ALP activity, collagen in lung tissue were decreased to the normal values. In the aloesin treated groups, s-GPT, BUN, creatinine, MDA level in serum were decreased to the normal values significantly. In the ${\beta}-carotene$ treated group, only s-GPTactivity was reduced to the normal values. In the lung tissue of taurine treated groups, MDA value, G-6-phosphatase activity and collagen synthesis were recovered to the normal valuse and ALP activity was increase about 40%. From these results, we concluded that taurine is an effective agent to inhibit the pulmonary and internal organs toxicities induced by paraquat and the inhibition effects of taurine are due to remove free radicals directly.

Path Analysis for Delirium on Patient Prognosis in Intensive Care Units (섬망이 중환자실 환자결과에 미치는 영향: 경로 분석)

  • Purpose: This study was conducted to investigate relationship between delirium, risk factors on delirium, and patient prognosis based on Donabedian's structure-process-outcome model. Methods: This study utilized a path analysis design. We extracted data from the electronic medical records containing delirium screening data. Each five hundred data in a delirium and a non-delirium group were randomly selected from electronic medical records of medical and surgical intensive care patients. Data were analyzed using SPSS 20 and AMOS 24. Results: In the final model, admission via emergency department (Β=.06, p=.019), age over 65 years (Β=.11, p=.001), unconsciousness (Β=.18, p=.001), dependent activities (Β=.12, p=.001), abnormal vital signs (Β=.12, p=.001), pressure ulcer risk (Β=.12, p=.001), enteral nutrition (Β=.12, p=.001), and use of restraint (Β=.30, p=.001) directly affecting delirium accounted for 56.0% of delirium cases. Delirium had a direct effect on hospital mortality (Β=.06, p=.038), hospital length of stay (Β=5.06, p=.010), and discharge to another facility (not home) (Β=.12, p=.001), also risk factors on delirium indirectly affected patient prognosis through delirium. Conclusion: The use of interventions to reduce delirium may improve patient prognosis. To improve the dependency activities and risk of pressure ulcers that directly affect delirium, early ambulation is encouraged, and treatment and nursing interventions to remove the ventilator and drainage tube quickly must be provided to minimize the application of restraint. Further, delirium can be prevented and patient prognosis improved through continuous intervention to stimulate cognitive awareness and monitoring of the onset of delirium. This study also discussed the effects of delirium intervention on the prognosis of patients with delirium and future research in this area.

Fermentation Property by Novel Cellulolytic Lactic Acid Bacteria Enterococcus sp. TO-94 on Omija (Schizandra chinensis Baillon) (신규 섬유질 분해성 젖산균 Enterococcus sp. TO-94를 이용한 오미자의 발효 특성)

  • The use of cellulolytic lactic acid bacteria in new method to prepare high nutrition complementary foods was investigated. For the screening of cellulolytic lactic acid bacteria, more than 1,150 bacterial colony were isolated from diluted infant feces samples. A typical strain which appeared the most excellent cellulolytic activities was identified novel acidophilic Enterococcus sp. TO-94 through the results of morphological, biochemical and chemotaxonomic characteristics and 16S rDNA sequencing. The optimal lactic acid fermentation conditions of Omija(Schizandra chinensis Baillon) by Enterococcus sp. TO-94 were as follows: pH and temperature were 3.0 and $37^{\circ}C$, respectively, and fermentation time was 20hrs. The fructose and glucose were major free sugar and the contents were 5.83 and 4.30 mg/g after fermentation, respectively. The contents of lactic acid and acetic acid were 9.84 mg/g and 2.08 mg/g after fermentation, respectively. The vitamin $B_1$, $B_2$, niacin, folic acid and C were major vitamin in the fermented broth, the contents were 1.5~3 times higher than those of initial fermentation time. Also, the contents of polyphenol and anthocyanine were 3.8 and 1.2 times higher than those of initial fermentation time.
