대입준비생들의 치과위생사에 관한 인식도 조사 (Recognition of college-preparatory students for dental hygienist)

  • Objectives : The purpose of this study was to survey the recognition of college-preparatory students who interested and prevented themselves in oral health diseases for dental hygienist in Yeungnam South Korea. Methods : The subjects in this study were 328 students who was seriously worried for career and entrance into a school. The survey period was from August 16th 2011 to November 26th 2011, and the data was collected from the high school students in Yeungnam and the preparatory students who visited S college in Gyeongju. This study was conducted by using the self-reported questionnaire. Results : The result of this study are as follows. 87.2% of the subjects have recognized about the name of dental hygienist, 51.5% have distinguished dental hygienist in the clinic, and 67.4% have recognized about education of dental hygienist that was more than a college graduate. The received services from dental hygienist was scalling(15.6%), assistant of treatment(14.7%), tooth brushing instrument(11.5%), Reception & receipt(11.0%), and etc. The majority of the subjects have recognized that dental hygienist was the professional(4.82 points), the important role in oral health(4.88 points), and the intermediary between patients and dentists(4.60 points). And they were friendly(4.64 points), responsible(4.81 points), and worthwhile job(5.01 points). However their appellation has not been established yet(4.00 points), they was difficult to distinguish from nursing assistants(4.37 points), and their task did not differs from AN(4.53 points). Also they compared to other job was not stressful job(3.58 points). Conclusions : Refer to these results, those will be the basis for professional development and status establish of clinical dental hygienists and for the upright role as a professional medical personnel of dental hygiene students. In addition, those will help to patients in the future, and become an effective instrument to learn knowledge, skill, and service that needed to conduct business.

일부 농촌 지역에서 회합 급식을 하는 여자 노인의 우울 정도, 신체계측 및 영양섭취실태 조사 (Depression Symptom, Anthropometric Assessment, and Nutrient Intake of Elderly Females Who Eat Congregate Meals at Lunch in Rural Area)

  • This study was carried out to determine the depression symptoms and nutritional status of elderly females that ate congregate meals at lunch in rural Pocheon. The subjects were 18 elderly females aged over 65 ($75.7{\pm}4.6$ years) and information on smoking, subjective health status, depression symptoms and malnutrition risk were collected using a questionnaire administered with the help of trained research assistants. Measurements included mid-arm circumference and calf circumference. Food consumption for 3 days during breakfast and supper were determined by 24-hour recall and food consumption at lunch was determined as the difference between the amounts served and the plate waste. All data were compared between two age groups (${\leq}75$ and > 75). The subjects had the lowest nutrient adequacy ratio (NAR) for vitamin $B_6$ (0.523), followed by calcium and niacin and the lowest index of nutrient quality (INQ) for calcium (0.738). The older elderly had significantly lower NAR for vitamin $B_1$, vitamin $B_2$, vitamin $B_6$ and niacin as well as significantly lower mean adequacy ratio (MAR) compared to the young elderly. They also had significantly lower INQ for vitamin $B_1$ and vitamin C and significantly thinner arm circumference (28.3 cm vs. 31.3 cm). They consumed more than 40% of their daily intake for each nutrient at lunch. Higher proportions of nutrient intake from lunch provided evidence of the importance of congregate meals, suggesting that the government and society should support congregate meals to improve nutritional status.

보건소 금연클리닉 금연성공자의 1년내 재흡연과 관련요인 (Smoking Relapse and Related Factors Within One Year Among Successes of the Smoking Cessation Clinics of Public Health Centers)

  • Objectives: This study aimed to investigate smoking relapse and the related factors within 1 year after discharge from the smoking cessation clinics (SCCs) of public health centers (PHCs). Methods: Data were collected with a structured questionnaire from 395 people who success fully stopped smoking at 4 SCCs in Busan between May and June 2009, and this data were analyzed by Kaplan-Meier survival curves and the Cox proportional hazard model. Results: The rate of smoking relapse within 1 year after discharge from SCCs was 39.2% and this decreased rapidly over 6 months after discharge. The factors related to smoking relapse within 1 year after discharge from SCCs were being female (HR, 2.11; 95% CI, 1.17 to 3.82), a trial of smoking cessation with any assistants (HR, 1.95; 95% CI, 1.19 to 3.19), more than 7 ppm of exhaled CO2 on the SCCs' registration (HR, 1.81; 95% CI, 1.24 to 2.64), use of pharmacotherapy after discharge from SCCs (HR, 2.00; 95% CI, 1.36 to 2.93), alcohol drinking more than once a week after discharge from SCCs (HR, 3.32; 95% CI, 2.15 to 6.78), and a perceived barrier (HR, 1.21; 95% CI, 1.14 to 1.30) after discharge from the SCCs. Conclusions: According to the results, at least 6 months follow-up after discharge from SCCs of public health centers is recommended to reduce the rate smoking relapse. It is also recommended to strengthen the education on how to overcome barriers such as drinking in the course of smoking cessation clinics.

경남지역 간호사의 영양지식, 식생활태도 및 영양소 섭취상태 평가 (Evaluation of Nutrition Knowledge, Dietary Attitudes and Nutrient Intakes of Nurses Working in Kyungnam Area)

  • This study was performed to investigate the dietary attitudes and nutrient intakes of nurses. A total of 291 nurses working at hospitals in the Kyungnam area participated in the study. The general characteristics and dietary attitudes of the subjects were surveyed using a self-administered questionnaire, and nutrient intakes was examined using one-day 24-hour recall method. The results were as follows : 67.7 percent of the subjects were nurses, 32.3% were nursing assistants. Average age of the subjects was 26.9 years old, average nursing experience was 5.7 years, and 70.7% of the subjects graduated from junior college. The rates of shift work (45.4%) and non-shift work (54.6%) were similar, and 91.5% of subjects worked on a three-shift a day schedule. The average nutrition knowledge score was 14.3 $\pm$ 2.5 out of a possible 20 points. Most of the nurses had experienced dietary problems such as skipping meals and overeating. Sixty-eight point three percent of the nurses had breakfast less than 3 times a week, and the main reason for skipping meals was a lack of time. Most of the nurses (74.1%) didn't exercise regularly. The average score on dietary habits was 55.6 out of 100, and most of the subjects belonged to the‘fair’group in terms of dietary habits. Married nurses, and those over 31 years of age, scored significantly higher on dietary habits than unmarried nurses, and those under 25 years of age (p < 0.001), and the scores significantly increased with increasing age. Mean daily carbohydrate and fat intake was 231.5 $\pm$ 66.1 g, 41.1 $\pm$ 18.8 g, and cholesterol and crude fiber intake was 238.2 $\pm$ 184.6 mg, 5.2 $\pm$ 2.0 g. respectively. Compared with the recommended allowances, the energy (1560.0 $\pm$ 448.5 kcal), calcium (453.8 $\pm$ 222.3 mg) iron (10.6 $\pm$ 7.6 mg), vitamin A (658.5 $\pm$ 538.1 R.E) and vitamin B$_2$ (0.96 $\pm$ 0.5 mg) figures were below the Korean RDA, and the protein (65.9 $\pm$ 31.0 g), vitamin B, (1.14 $\pm$ 0.5 mg), vitamin C (143.2 $\pm$ 119.6 mg), niacin (14.4 $\pm$ 6.3 mg) and phosphorus (958.1 $\pm$ 352.1 mg) figures were above the Korean RDA. The average carbohydrate, protein, fat ratio of energy intake was 60:17:23. The intake ratio of Ca and P showed an unbalance of 1:2. The dietary habits had a positive correlation (p < 0.001) with nutrition intake except vitamin A and vitamin C. Therefore, In order to improve overall dietary behavior and nutritional status, systematic nutrition education programs should be developed.

간호사의 전문적 업무에 대한 국가 간 비교법적 연구: 법적 근거와 업무 범위 표준화를 중심으로 (A Comparative Law Study on the Professional Work of Nurses: Focusing on Legal Basis and Standardization)

  • 본 연구는 간호사의 전문적 업무에 대한 범규범과 임상 현실의 실제적 차이에서 오는 법적 혼란 문제를 외국의 사례와 비교함으로써 이를 비판적으로 고찰하는 것을 그 목적으로 한다. 간호사의 업무 확대의 역사적 배경을 비교하고, 주요 국가의 전문간호사 등의 자격 및 업무에 대한 법적 근거 등을 살펴본 후, 이를 기초로 우리 법제의 발전 방향을 모색하고자 한다. 연구 방법은 비교 제도론적 연구 방법을 적용하였다. OECD 국가 중 미국, 호주, 일본을 선정하여 그 전문간호사 등에 관한 법제를 비교하였다. 미국, 호주, 일본 등에서 간호사의 전문적인 업무는 고령화 사회, 만성 질환, 의사 부족 등으로 인한 의료환경의 변화에 의해 출현하였다. 이들의 전문적인 업무에 관한 법적 근거, 자격인정 및 자격인정제도의 발전과 그 업무 범위 등을 비교하여 살펴보았다. 한국은 미국 등 외국과 비교하여 아직 법적 근거가 부재하며, 전문간호사 등의 업무 범위에 관한 법원의 입장과 실제 업무 간의 격차를 좁히지 못하고 있다. 비교법적 검토를 통해 전문간호사 등에 관하여 한국 실정에 맞는 법안 마련과 업무 범위 개발에 대한 시사점을 줄 수 있을 것이라 기대한다.

일 의료원의 통합 고충처리센터 접수 내용과 이에 대한 해결방안 분석 (Analysis of the Issues received by Quality Improvement Department and their Management in a Medical Center)

  • Background : A continuous healthcare quality improvement is needed to provide high quality healthcare service as well as to maintain trust in terms of satisfying the needs of the patients. Recently it also became an essential issue. in hospital management, recognized for it's competitive potentiality among healthcare organization groups. This study was conducted to analyze patient complaints and issues received by the Quality Improvement Department. Its purpose is to improve healthcare qualities within the hospital, as well as establish policies and appropriate strategies in hospital management. Method : From July 1st to September 30th of the year 1999, we analyzed all complaints and issues made by various patients and their families, which were received through 24 hour phone consultation, numerous suggestion boxes, letters and E-mails, The issues were classified into 16 different categories based on a Patient Satisfaction Assessment Tool. All data were segregated according to the departmental frequencies and their contents. To come up with for environmental and patient satisfaction improvement, all complaints or issues were communicated with hospital administrators, medical and nursing staff and employees. Comprehensive customer satisfaction activities including improving phone etiquette were discussed in Customer Satisfaction Team, CQI Team and each Department. All opportunities for improvement were implemented. Feedback actions were discussed. Results : A total of 317 cases were collected. Issues regarding parking and other accommodation facilities were most common complaints that were 14.5% of total. Issues regarding admission rooms (10.7%), admission procedures (10.7%), waiting room environment (8.8%), nurses and nurse assistants (7.6%), physicians (6.6%) and others (23%) followed. Thirteen of 45 departments received more than 8 complaints. The Nursing Department had the most complaint, receiving 9.8% of total complaints. Complaints regarding the Nursing Department were predominantly related to the environment of patient rooms. The Department of Psychiatry for phone etiquette (4.7%), Department of Otolaryngology for the nursing staff's attitude and phone etiquette (4.4%), and the Admission Department followed. As a part of efforts to improve patient satisfaction, a new parking structure was built and reallocation of the parking space was done. Renovation of other accommodation facilities were carried out by hospital administration, Monthly phone call and answering attitude survey was done by QI Department. Based on this survey we made a phone etiquette manual and distributed throughout the hospital. Compare to the last year, Patient Satisfaction Index measured by Korea Productivity Center using National Customer Satisfaction Index was improved 7 points. According to our organization's own study, we confirmed the phone etiquette was improved 11% than last year. Conclusions : Issues related to parking and other accommodation facilities ranked first followed by complaints made regarding the patient care area, the admission and cashier process, and nurses' and doctors' attitude. The Nursing and Psychiatry Departments need improvement regarding phone etiquette. Results were shared and played a vital role in policymaking and strategic planning of the hospital. It is imperative that we keep our database updated by listening to and solving the needs of each patient. The CQI activities can be achieved only by full commitment of the hospital top management supported by related personal.

한의원의 경영 현황과 정책 인식도에 대한 조사 연구 (Survey on the current status of the management of traditional Korean medical clinics and perception of the policies)

  • Objectives The purpose of this study is to help improve the management of traditional Korean medical(TKM) clinics and the policies for supporting them, by conducting a survey of the practitioners of TKM. Methods Stratified sampling was conducted based on regional location, and 700 samples were selected in a random manner from the membership list. The questionnaire was delivered and returned by mail. The survey was conducted between 20 July and 31 August 2010. A total of 177 questionnaires (recovery rate: 25.28%) were recovered and analyzed for the study. Results 1. The the survey indicate that the overall size of TKM clinics has fallen compared with previous survey, while the average number of beds per clinic has remained unchanged at 7.9. The sale of medicine as a proportion of total monthly sales has increased. 2. There has been no change in the composition of clinical staff as there are three nursing assistants. Although the average daily number of patients to clinics has remained at around 33.90 compared with ten years ago, the number of patients requiring seeking acupuncture treatment has increased while the number of those treated with medication has decreased. 3. Clinicians in TKM have indicated their preference for a binary system that separates TKM from western medicine (57.4%). The respondents do not favor the separation of dispensary from medical practice (81.5%), marks of origin for medicinal herbs (68.9%), disclosure of the prescriptions (67.2%), and the overseas expansion of Korean medical services (70.4%). However, they indicated that they are very much in favor of being granted the authority to employ and give orders to medical technicians (96.0%). 4. The respondents selected Korea as the country that maintains a proper academic system for traditional medicine (45.5%), and are not in favor of opening Korea's traditional medicine market under an Free Trade Agreement(FTA) with China (72.7%). Conclusion The overall status of the management of TKM clinics has declined compared with the preceding decade. There has been only a slight change in clinicians' opinion of the related policies and regulatory issues.

노인들의 건강문제와 컴퓨터 게임 요구도의 상관성 분석 (Analysis for the Correlations between health Problems and Computer Game Needs in the Elderly)

  • 만성질환 및 비만 관리와 낙상예방 등을 위한 간호중재의 하나로 규칙적인 신체활동의 필요성이 대두되고 있는데, 운동게임 등의 기능성 게임을 질병예방 교육과 치료보조, 재활 등에 응용하는 방안은 점차 중요영역이 될 것으로 보인다. 본 연구는 건강문제와 컴퓨터 게임요구도의 상관관계를 알아보기 위해 설문지를 이용하여 실시한 횡단적 조사 연구이다. 훈련된 연구보조원들의 면접조사와 전문조사기관의 온라인조사 결과 778명(평균 연령: $61.4\;{\pm}\;5.6$)의 대상자가 참여하였는데, 그 중 68.6%가 남자였다. 분석결과 남성(55.4%), 고졸이하(66.2%), 2명 이하의 가족수(32.5%), 월평균 35만원 이상(40.1%), 경증 우울증(51.7%), 온라인 응답자(68%) 그룹에서 게임 요구도가 높게 나타났다. 특히 게임을 통해 신체적 제약을 극복하고 싶어했다. 상관성 분석에서는 교육수준이 높을수록, 컴퓨터 이용 경험이 있을수록, 컴퓨터/인터넷 활용이 능숙할수록 운동게임이나 기능성 게임에 대한 요구도가 높았다. 결론적으로, 노인그룹을 위한 여가활용 차원에서의 즐거움과 건강증진의 효과를 모두 가져올 수 있는 노인용 기능성 게임이 개발된다면 노인의 건강증진과 삶의 질 향상에 크게 기여하리라 사료된다.

노인장기요양기관의 윤리경영이 조직투명성 및 경영성과에 미치는 영향 연구: 조직문화의 매개효과를 중심으로 (A Study on the Effect of Ethical Management on Organizational Transparency and Performance in Long-Term Care Institutes: With a Focus on the Mediation Effect of Organizational Culture)

  • 본 연구자는 연구결과를 근간으로 노인장기요양기관의 윤리경영, 조직문화, 조직투명성, 경영성과의 관계를 규명하되, 요양보호사의 윤리경영, 조직문화, 조직투명성, 경영성과의 수준은 어느 정도인지를 검증하고, 연구가설을 설정하여 윤리경영이 조직투명성, 경영성과에 어떠한 영향력을 미치는지, 또한 이들 간의 관계가 조직문화에 매개하는지 검증하였다. 검증결과, 경기지역 요양보호사의 윤리경영 수준은 5점 척도 기준에서 평균 2.929점, 조직문화 수준의 경우, 평균 3.393점이고 수준이었다. 조직투명성 수준은 평균이 2.925점, 경영성과 수준은 3.185점으로 나타났다. 본 연구에서 요양보호사의 윤리경영, 조직문화, 조직투명성, 경영성과의 수준이 유사기관의 생활복지사, 사회복지사, 간호조무사의 수준과 비교했을 때 낮은 수준이었는데, 이러한 결과는 요양보호사들의 학력, 급여, 근무환경 등이 일반 사회복지조직 종사자의 수준보다 낮는 결과의 산물이다. 본 연구의 주요 변수인 윤리경영, 경영성과, 조직투명성, 조직문화간의 상관관계에서 변수 간의 유의미한 관계성이 있는 것으로 파악되었다. 둘째, 본 연구가설 검증결과 윤리경영이 조직투명성, 경영성과, 조직문화에 모두 정적 영향을 미치는 것으로 가설이 모두 채택되었다 셋째, 조직문화가 조직투명성에 직접적인 영향을 미쳤으며, 또한 조직문화가 윤리경영과 조직투명성, 경영성과 간의 관계에서 매개효과로 유의한 영향력을 미치는 것으로 파악되었다. 이와 같은 연구결과를 근간으로 다음과 같이 제언하고자 한다. 첫째, 윤리경영 중심에 있는 관리자의 윤리적 리더십이다. 둘째, 조직투명성에 대한 관심도이다. 셋째, 경영성과의 균형이다. 넷째, 윤리경영 환경에 능동적으로 대처할 수 있는 유연한 조직문화가 구축되어야 한다.

재활병원 종사자들의 스포츠 참여 활동과 직무만족도에 관한 연구 (The Effects of Participation in Sporting Activities on Job Satisfaction in Rehabilitation Hospital Workers)

  • 본 연구는 재활병원에 종사하는 종사자를 대상으로 직무만족도 분석을 하여 스포츠 활동의 참여가 직무에 어떠한 영향을 미치는지 알아보기 위한 것이다. 이를 위해 2015년 4월부터 5월까지 8개의 부산광역시 재활병원소속 간호사, 간호조무사, 물리치료사, 작업치료사, 행정직으로 종사자를 선정 후, 400부의 설문지를 배포하여 분석하였다. 분석방법으로 t-검정, 빈도분석(frequency analysis), 탐색적 요인분석(exploratory factor analysis)과 Cronbach's ${\alpha}$분석, 일원배치 분산분석(one-way ANOVA), 사후검증(Duncan)을 실시하였고, 상관관계(correlation analysis)와 회귀분석(regression analysis)을 이용하였다. 연구결과는 다음과 같다. 스포츠 활동 참여 형태에 따른 직무만족도 차이 검정 결과 운동시간, 운동빈도, 운동종목 그리고 운동기간 등 모든 요소에서 유의한 차이가 나타났고 운동시간과 운동빈도가 많고 운동경력이 높을수록, 운동종목의 경우 아쿠아로빅과 에어로빅 종목을 참여하였을 때 직무만족도가 높아진 것으로 분석되었고, 회귀분석 결과에서는 직무만족도 하위 5개 요인 중 자기평가성, 이직성향, 직무몰입 그리고 직무만족에서 정의 영향을 미치는 것으로 나타났고 참여빈도에서는 유의한 차이가 나타나지 않았으며 참여빈도에서는 부의 영향을 미치는 것으로 나타났다. 결론적으로 스포츠 활동참여가 재활병원 종사자들의 직무만족도에 긍정적인 영향을 미치는 것으로 나타났고, 본 연구를 종합적으로 분석해 볼 때, 재활병원 내 직원 구성원들의 스포츠 동호회 활동과 체육시설 설치 그리고 운동시간에 대한 할애 등 지원을 통해 업무능률 향상과 활기찬 직장 분위기를 만들 수 있으므로 적극적으로 스포츠 활동을 장려해야 할 것으로 사료된다.
