신체적 억제대 지침 개발 및 사용 효과 (Development of physical restraints guidelines and use effect)

  • Objective: The objective of this research was to develop a guideline for more effective use of physical restraint on patients in the intensive care unit and training the nurses on it and applying it on clinical practice to assess its effectiveness. Method: This research analyzed the before and after effect of the development of a guideline for physical restraint by dividing the category into nurse and patient. In the case of nurse, a comparison of knowledge and nursing service regarding the use of physical restraint from before the training on physical restraint guideline(Jan. 2011) and after the training on physical restraint guideline(Dec. 2011) was made. In the case of patient, a comparison of physical restraint usage rate and average usage time, the number of unplanned extubation cases were compared from before the use of physical restraint (Jan.~Apr. 2011) and after the use of physical restraint (Sep.~Dec. 2011) were made. Result: After the training on the physical restraint guideline, the knowledge of the nurse and the nursing practice showed notable improvement by (p<0.000) and (p<0.048) respectively and in patient, physical restraint usage rate and average time of usage decreased by (p<0.001) and (p<0.001) respectively. And despite the decrease in the number of cases in which the physical restraint was used, the number of unplanned extubation cases remained the same. Conclusion: Physical restraint guideline training and guideline usage can be stated to have brought out positive effect in both the nurse and patient. In order to maintain such positive effects, continuous training is necessary and continuous revaluation is necessary, regarding knowledge and nursing practices.

액션러닝 교수설계에 의한 창의적 문제해결 교과의 학습성과 (Effects of an Action Learning based Creative Problem-Solving Course for Nursing Students)

  • Purpose:This study was conducted to identify the effects of an action learning based creative problem-solving (CPS) course on problem solving, creativity and team-member exchange in nursing students. Methods: A quasi-experimental study applying a non-equivalent control group pre-post design was employed. Sophomore nursing students (32 in the experimental group and 33 in the control group) were recruited from a university in G-city, Korea. Problem solving, creativity and team-member exchange were measured for the pretest and posttest using self-report questionnaires. Kolmogorov-Smirnov test, Chi-square, Fisher's exact test, t-test, and ANCOVA with SPSS/Win 20.0 program were used to analyze the data. Results: The scores for problem solving, creativity and team-member exchange in the experimental group were significantly higher than those of the control group. Conclusion: Results of this study indicate that an action learning based CPS course is an effective teaching method to improve nursing students' competencies. In the future longitudinal studies are needed to assess the long term effects of the course.

팀 기반 시뮬레이션 실습교육이 간호학생의 환자안전 자신감과 간호역량에 미치는 효과 (Effects of Team-based Simulation Training on Patient Safety Confidence and Nursing Competency among Nursing Students)

  • Purpose: This study aimed to evaluate the effects of team-based simulation training (TBST) on patient safety confidence and nursing competency among nursing students. Methods: This study used a one group pretest-posttest design. Among the senior nursing students enrolled in an academic course in one nursing school, the convenience sample of 114 students was recruited. Participants received 15-session TBST (2 hours per session) over 8 weeks and completed the patient safety confidence and nursing competency scales, and a questionnaire of participants' responses to TBST. Data were analyzed using a descriptive statistics test, a paired t-test, and Pearson correlation coefficients. Results: The results of this study showed that patient safety confidence and nursing competency significantly improved after TBST and there was a positive correlation between these variables. Conclusion: The findings indicate that TBST is effective for improving nursing students' patient safety confidence and nursing competency, and thus, team-based strategies can be useful to attain the goals of nursing education. Furthermore, the results of this study suggest further research to refine the relations between patient safety and nursing competency.

간호학생의 팀기반 시뮬레이션 학습경험 (Nursing Student's Experiences in Team Based Simulation Learning)

  • Purpose: The purpose of this study was to explore nursing students experiences in team based simulation learning. This study was based on a phenomenological research methodology. Methods: Fourteen nursing students participated in this study. The students gave in depth interviews and wrote reflective journals after participating in a team based simulation learning program, which was provided at C University in Gwangju for 8 weeks, 2011. These data were analyzed using Van Kaam's method. Results: 21 sub-themes and 9 themes were elicited from 55 significant statements. They were classified into 3 categories such as : 'First exposed to lessons on how Burden', 'the enjoyment of team based simulation learning', 'expansion of the clinical competence'. Conclusion: Teaching and learning strategy that combines a variety of simulation training to nursing students to develop critical skills to improve their care to give and take advantage of simulation training supplement is advised.

캡스톤 디자인 간호연구 수업의 팀워크역량과 팀활동 경험 (Teamwork Competency and Team Activity Experiences in Capstone Design Nursing Research Course)

  • 본 연구는 캡스톤 디자인을 활용한 간호연구 수업의 팀워크역량 효과와 팀활동 경험의 내용을 분석하고자 양적연구와 질적연구를 혼합한 통합연구(Triangulation)이다. 연구대상자는 D대학교 캡스톤 디자인 간호연구를 수강한 간호대학생 99명이었다. 간호대학생의 수업 전, 후의 팀워크역량을 비교하였으며, 팀별 성찰일지의 서술형 자료를 통해 캡스톤 디자인 간호연구 수업에서의 팀활동 경험을 질적내용 분석하였다. 연구 결과, 과제수행역량이 수업 후 유의하게 높아 졌으며(t=-2.166 p=.033), 과제수행역량 중 목표실행능력이 수업 후 통계적으로 유의하게 향상되었다(t=-2.224, p=.028). 대인관계역량 중 리더십이 수업 후 유의하게 향상되었다(t=-2.085, p=.040). 팀활동 경험 내용은 '자원관리의 어려움', '개인의 노력과 역량 발휘의 중요성 인식', '효과적인 역할 분배를 위한 노력', '의사소통의 중요성을 깨달음', '임상실무에 필요한 역량을 강화함'의 5개의 주제가 도출되었다. 본 연구를 통해 캡스톤 디자인 간호연구 수업이 간호대학생의 팀워크역량을 향상시키는데 효과적이며, 개인의 노력과 역량 발휘, 효과적인 역할 분배와 의사소통을 통해 임상실무에 필요한 역량을 강화시킬 수 있는 긍정적인 팀활동 경험을 제공했음을 확인할 수 있었다. 본 연구 결과를 바탕으로, 팀워크역량 강화를 통해 간호연구역량 향상을 도모하는 캡스톤 디자인 간호연구 수업에 활용해야할 것이다.

팀기반학습(Team-Based Learning)이 간호학생의 문제해결능력과 비판적사고 및 자기주도학습에 미치는 효과 (The Effects of Team-Based Learning on Problem Solving Ability, Critical Thinking Disposition and Self-Directed Learning in Undergraduate Nursing Students)

  • Purpose: The purpose of this study was to examine the effects of team-based learning (TBL) program on problem solving ability, critical thinking disposition and self-directed learning in junior nursing students caring for patients with respiratory diseases. Methods: One-group pretest-posttest design was used. TBL program was carried out for 2 hours a week for 6 weeks. Data were collected by questionnaires from 167 nursing students from March 6 to June 5, 2013. Data were analyzed by paired t-test and Pearson correlation coefficient. Results: At the completion of TBL program, significant improvement was found in problem solving ability (t=-6.04, p<.001), critical thinking disposition (t=5.02, p<.001) and self-directed learning (t=5.96, p<.001). There was a significant positive correlation among problem solving ability, critical thinking disposition and self-directed learning. Conclusion: In conclusion, TBL is a useful teaching and learning method on nursing students. We suggest that it is needed to measure the educational effects of TBL against other teaching methods in the future studies.

정신건강간호의 실체규명을 위한 업무경험 탐색 (An Exploration of Essentials of Psychiatric and Mental Health Nursing)

  • Purpose: This study aimed to explore the essentials of psychiatric and mental health nursing by identifying the care experience of the psychiatric nurses. Methods: Eleven psychiatric nurses were recruited across five geographical areas in the Korea. Data were collected using semi-structured interviews from two focus groups, which were analyzed with an inductive content analysis. Results: Participants constructed two categories including 'nursing tasks for the client' and 'nursing tasks for the organization'. Subcategories of nursing tasks for the client are 'staying alert on crisis prevention', 'leading them to be real with eager and persistent will', 'nursing beyond role boundaries', 'tuning the needs of clients' family', 'taking journey with clients' personal life history', and 'doing invisible but recognizable nursing'. Subcategories of nursing tasks for the organization are 'working as a responsible team member', 'integrating team competencies at the center of team', and 'balancing protection and control'. Conclusion: Psychiatric nurses played professional and holistic care roles for the clients, and managerial roles for the organizations. This study provides essential data for defining psychiatric nursing and estimating the appropriate ratio of psychiatric nursing staffing.

병원조직에서 간호팀장의 팀제 경험 (Team Managers' Experience With a Team System in the Hospital)

  • Purpose: The purpose of the study was to understand and describe the team manager's experience in relation to the team system in the hospital organization. Methods: In-depth individual interviews for collection of data were conducted from October to November 2011. Participants were chosen purposely among team managers who had worked as team managers under the team manager system in Seoul for more than three years. Content of all interviews was recorded and transcribed according to information collected by interviewers. Data were analyzed using Colaizzi's analysis. Results: Six categories were derived from the analysis: 1) Strengthening of team manager's authority; 2) Sharing of multifaceted leadership role; 3) Demonstrating people-oriented leadership; 4) Complete interpersonal relationship and communication; 5) Self-empowering with web-like plans and practices; and 6) Going through rigorous commitment for team performance. Conclusion: Results of the study demonstrate various positive factors in association with the team system in spite of the short period of time since its introduction in the hospital organization; however, some negative aspects of the team system need to be reevaluated.
