Prediction Models of Mild Cognitive Impairment Using the Korea Longitudinal Study of Ageing (고령화연구패널조사를 이용한 경도인지장애 예측모형)

  • Purpose: The purpose of this study was to compare sociodemographic characteristics of a normal cognitive group and mild cognitive impairment group, and establish prediction models of Mild Cognitive Impairment (MCI). Methods: This study was a secondary data analysis research using data from "the 4th Korea Longitudinal Study of Ageing" of the Korea Employment Information Service. A total of 6,405 individuals, including 1,329 individuals with MCI and 5,076 individuals with normal cognitive abilities, were part of the study. Based on the panel survey items, the research used 28 variables. The methods of analysis included a χ2-test, logistic regression analysis, decision tree analysis, predicted error rate, and an ROC curve calculated using SPSS 23.0 and SAS 13.2. Results: In the MCI group, the mean age was 71.4 and 65.8% of the participants was women. There were statistically significant differences in gender, age, and education in both groups. Predictors of MCI determined by using a logistic regression analysis were gender, age, education, instrumental activity of daily living (IADL), perceived health status, participation group, cultural activities, and life satisfaction. Decision tree analysis of predictors of MCI identified education, age, life satisfaction, and IADL as predictors. Conclusion: The accuracy of logistic regression model for MCI is slightly higher than that of decision tree model. The implementation of the prediction model for MCI established in this study may be utilized to identify middle-aged and elderly people with risks of MCI. Therefore, this study may contribute to the prevention and reduction of dementia.

Evaluation Study of Home Health Care Program Operated in G Public Health Center (인천광역시 일 보건소 대도시 방문보건사업의 평가 연구)

  • Purpose: The purpose of this study was to evaluate the achievements of the project, and also to find out its strengths and weaknesses. Method: This evaluative study employed system theory and analytic techniques by using criteria which were relevance, appropriateness, adequacy, progress, effectiveness and satisfaction for input, process, output, and outcome of program. Study subjects were participants in the home health care program implemented in G public health center in Inchon metropolitan city in 2003. Results: Input factors including recruitment personnel, and support organization development were not adequately met for the program. However. the goal and objectives of the project were really appropriate for the community needs as well as government's policy. For the Process evaluation, home health care record form and computer data base had not progressed as scheduled, but overall program activities were finished on time. However, cooperation between organizations in the health center during service activity were not supportive. Managerial ability of program charged personnel about coordination and integration of team members was not affirmative. Output and outcome evaluation showed that people improved self care ability were 221 (17.5%). and 71 (5.62%) of subjects were moved into category of possible self-care. Client's satisfaction for this project showed a high degree. Conclusions: Based on the above results support organization and staff personnel for this program should be developed. Also, a community network of resources should be established and case management services should be focused continuously in community based home health care.

Development and Evaluation of an Evidence-based Nursing Protocol for Postoperative Nausea and Vomiting (근거기반의 수술 후 오심·구토 관리 프로토콜의 개발 및 임상적 평가)

  • Purpose: Postoperative nausea and vomiting (PONV) is very common among postoperative orthopedic surgical patients with patient-controlled analgesia (PCA), especially for narcotics. Therefore, the purpose of this study was to investigate the effects of an evidence-based PONV management protocol on nursing and patient outcomes. Methods: A methodological study was conducted to develop PONV protocol and a quasi-experimental study to evaluate the effectiveness of protocol. The preliminary PONV protocol was drawn by conducting a systematic review and by reviewing clinical guidelines and best practice recommendations. Validation of the content was done by expert clinicians, and the clinical applicability was evaluated by staff nurses and patients. The effect was evaluated in clinical outcomes associated with PONV and nursing outcomes. Results: In the experimental group, the occurrence of vomiting (z= 2.147) was significantly decreased, the maintenance PCA (χ2=4.212) and the satisfaction of patients (z= 5.007) were significantly higher. In the outcomes of nurses, the PONV knowledge of nursing care (z = 3.791), awareness (z = 2.982) and self-efficacy (z= 2.745) were higher in the experimental group. The attitude towards evidence-based nursing practice (z= 2.446) was significantly positive. Conclusion: The results show that an evidence-based approach to the implementation of PONV care is effective in improving patient clinical outcomes and quality of care.

An Exploration of Factors Influencing College Students' Academic and Social Adjustment

  • The purpose of this study is to identify the characteristics of school life and to examine the influencing factors of academic adjustment and social adjustment in university students. This cross-sectional descriptive study by using secondary data from 2,064 subjects who responded to a self-rated student adjustment test. Descriptive, t-test, ANOVA, Pearson correlation and multiple regression analyses were conducted. Satisfaction with college selection, expectation for college, satisfaction with college service, emotional stability, and relationships with faculty were significantly higher among male students, whereas confidence in major, support from others were significantly higher among the female students. There was a significant positive relationship between academic adjustment and confidence in major, and between social adjustment and expectation for college. Confidence in major, relationships with faculty, emotional stability, expectation for college, satisfaction with college selection, and commitment to college life accounted for 31.8% of the variance in academic adjustment. In addition expectation for college, emotional stability, support from others, gender, commitment to college life, and satisfaction with college services accounted for 44.7% of the variance in social adjustment for university students. Overall, the results of this study suggest that understanding the levels of expectation for college, commitment to college life, and levels of emotional stability may be important for facilitating their transition and adjustment to university life.

A Study on Job Satisfaction by Medical Information System Accomplishment

  • Purpose. The purpose of this study is to investigate the success model related to the hospital information system accomplishment. It is important to examine the success model of the hospital information system and to analyze the factors affecting the job satisfaction accomplishment. Methods. The method of this study is to 150 copies of the entire survey data were distributed and 135 copies were collected, showing a collection rate of 90%. In order to ensure the reliability of the questionnaire items, Cronbach's Alpha was used to test reliability, and exploratory factor analysis was conducted to determine the convergence of various items. In order to grasp the convergence of various items, exploratory factor analysis was performed. The results of exploratory factor analysis were used to analyze the correlations between variables that were proven to have a single dimensionality before calculating factor loadings and regression analysis by Orthogonal Rotation by Varimax method Results. The results of this study, first, the system quality of the hospital information system has a statistically significant effect on user satisfaction. Second, the information quality of hospital information system is statistically significant for user satisfaction, indicating that information quality improves user satisfaction. Third, service quality of hospital information system was statistically significant in user satisfaction. Finally, the higher the satisfaction of the users who use the hospital information system, the higher the accomplishment of the organization Conclusions. This study is based on the successful model of D & M information system. In addition, the hospital information system, the user satisfaction, and the organizational accomplishment in connection with it can be found significant.

Factors Affecting the Oral Health Status in Vulnerable Elders (취약계층 노인의 구강건강상태에 영향을 미치는 요인)

  • Propose: This study was to identify the factors affecting the oral health status in vulnerable elders receiving home visiting health care service. Methods: A total of 444 elderly people over the age of 65 at 33 public health centers in Daegu and Gyungbuk areas were included in this survey. The data were collected by personal interview with nurses using oral health knowledge, behavior, and status questionnaires from the 10th to 22nd of July, 2012. The data were analyzed with t-test, one-way ANOVA, Scheffˊe test, and stepwise multiple regression, using SPSS program. Results: Factors affecting the oral health status were chewing satisfaction with denture 14.5% (β=.391, p<.001), denture care (wash with water) 3.2% (β=.187, p<.01), and types of health insurance (medical aid type1, 2) 1% (β=.111, p<.05). The factors explained 18.7% of variance in the oral health status of vulnerable elderly subjects. Conclusion: The results of this study indicate that in order to improve the oral health status of vulnerable elders, it is necessary to provide oral health education that includes the proper usage of denture to elders who receive home-visiting care.

Health Status and Health Care System of Homeless Shelter Residents (쉼터거주자의 건강관리실태 및 대안)

  • Purpose: The purpose of this study was to explore how homeless shelter worker and public health nurses perceive health status and health care of homeless shelter residents (HSRs). Methods: Data collected through focus group interviews. In Focus group, in-depth discussions were between 150 to 160 minutes. Data analyzed using Krueger (1998) step analysis. Participants were seven experienced clinical social workers, nurses working homeless shelters, and public health center nurses for dosshouse people. Results: The results were 4 themes and 15 sub-themes: Characteristics of HSRs, perception of health and health problem of HSRs (alcohol related disease, hypertension Diabetics, gastro-intestinal disease, dental disease and infectios disease such as Tuberculosis, musculo-skeletal disease, prostate problem), health care status of HSRs(insufficient health care service, discrimination of medical staff, lack of health care management, low satisfaction community health care services), and the health care proposal of HSRs(nurse in homeless shelter, integrated health care system, understanding of homeless) Conclusion: Based on the findings of this study, health care programs focusing on understanding of HSRs and chronic diseases of HSRs increasing steadily although the management system is limited. Therefore, more systemized health care plan and health referral system for homeless people.

Comparison of the Effects of Internet Diabetes Education by Gender in Type 2 Diabetic Patients (제 2형 당뇨병환자 성별에 따른 인터넷 당뇨 교육 효과 비교)

  • Purpose: The purpose of this study was to evaluate whether the effect of Intenet diabetes education varied by gender. Method: An experimental group assessed pre- and post intervention was used to assess the effectiveness of diabetes education by nurses. Forty patients separated into two groups by gender partook in the study. The goal of the intervention was to keep blood glucose concentrations close to normal range. The intervention was applied weekly for 3 months. Participants were requested to input their blood glucose level, diet, and exercise diary everyday at http://www.biodang.com by cellular phone or wire Internet. The researcher sent optimal recommendations to each patient using the short message service of cellular phones and wire Internet. All medication adjustments were communicated to the subjects' doctors. The plasma glucose levels, serum lipids, and care satisfaction were measured before and after the intervention. Result: Glycosylated hemoglobin (HbA1c) decreased 1.6% and 0.8% in male and female patients respectively after 3 months of education. Total cholesterol decreased 37.2mg/dl in male patients but increased 80.5mg/dl in female patients. Conclusion: These findings indicated that Internet diabetes education could improve HbA1c and total cholesterol in male patients.

Influencing Factors of Empathic Competence and Occupational Stress of Nurses on Their Ego-resilience (간호사의 공감역량과 직무스트레스가 자아탄력성에 미치는 영향)

  • Purpose: This descriptive survey study assesses the extent of empathic competence, occupational stress, and ego-resilience of nurses, and confirms factors that affect ego-resilience in order to provide basic data for the competency development program for nurses. Methods: Data obtained from the questionnaire survey conducted with 193 nurses were analyzed by means of t-test, ANOVA, and Pearson correlation coefficient and Multiple Regression. Results: Empathic competence, in accordance with the general characteristics of nurses, displayed statistically significant differences in terms of final academic background. Further, occupational stress and ego-resilience displayed statistically significant differences related to the nurses' department of employment, age, and total clinical experiences, respectively. Empathic competence and ego-resilience have a significant positive correlation, and age and empathic competence were found to be the factors that impart influence on ego-resilience. Conclusion: The enhancement of empathic competence of nurses has affirmative effects on the satisfaction of patients with their nurses, and it also improves the ego-resilience of nurses. Therefore, it is necessary to develop and apply career training program aimed at enhancing the empathic competence of nurses as an in-service educational program.

A Study on the Expressed Desire at Discharge of Patients to Use Home Nursing and Affecting Factors of the Desire (퇴원환자의 가정간호 이용의사와 관련 요인)

  • The purpose of this study is to investigate factors related to the intent of using home nursing of chronic disease patients who got out of a university hospital. For the purpose, the study selected 153 patients who were hospitalized and left K university hospital with diagnoses of cancer, hypertension, diabetes and cerebral vascular accident and ordered to be discharged and performed interviews with them and surveys on their medical records to obtain the following results. For this study a direct-interview survey and medical record review was conducted from June 28 to Aug. 30, 1998. The frequency and mean values were computed to find the characteristics of the study subjects, and X2-test, t-test, factor analysis and multiple logistic regession analysis were applied for the analysis of the data. The following results were obtained. 1) When characteristics of the subjects were examined, men and women occupied for 58.8% and 41.2%, respectively. The subjects were 41.3 years old in aver age and had the monthly aver age earning of 0.99 million won or below, which was the most out of the total subjects at 34.6%. Among the total, 87.6% resided in cities and 12.4 in counties. The most left the hospital with diagnosis of cancer at 51.6%, followed by hyper tension at 24.2%, diabetes at 13.7% and cerebral vascular accident at 7.2%. 2) 93.5% of the selected patients had the intent of using home nursing and 6.5%, didn't. Among those patients having the intent, 85.6% had the intent of paying for home nursing and 14.4%, didn't. The subjects expected that the nursing would be paid 9,143 won in aver age and 47.7% of them preferred national authorities as the main servers. 86.3% of the subjects thought that home nursing business had the main advantage of making it possible to learn nursing methods at home and thereby contributing to improving the ability of patients and their facilities to solve health problems. 3) Relations between the intent of use and characteristics of the subjects such as demography-related social, home environment, disease and physical function characteristics did not show statistically significant differences among one another. Compared to those who had no intent of using home nursing, the group having the intent had more cases of male patients, the age of 39 or below, residence in cities, 5 family member s or more, no existence of home nursing servers, leaving the hospital from a non-hospitalized building, disease development for five months or below, hospitalization for ten days or more, non-hospitalization with in the recent one month, two times or over of hospitalization, leaving the hospital with no demand of special treatment, operation underwent, poor results of treatment, leaving the hospital with demand of rehabilitation services, physical disablement and high evaluation point of daily life. 4) Among those patients having the intent of using home nursing, 47.6% demanded technical nursing and 55.9%, supportive nursing. As technical nursing,' inject into a blood vessel ' and 'treat pustule and teach basic prevention methods occupied for 57.4%, respectively, topping the list. Among demands of supportive nursing, 'observe patients 'status and refer them to hospitals or community resources as available, if necessary' was the most with percent age point of 59.5. Regarding the intent of paying for home nursing, 39.2% of those patients wishing to use the nursing responded paying for technical services and 20.2, supportive services. In detail, 70.0% wanted to pay for a service stated as 'inject into a blood vessel', highest among the former services and 30.7%, a service referred to as 'teaching exercises needed to make the body of patients move', highest among the latter. When this was analyzed in terms of a relation between the need(the need for home nursing) and the demand(the intent of paying for home nursing), The rate of the need to the demand was found two or three times higher in technical nursing(0.82) than in supportive nursing(0.35). In aspects of tech ical nursing, muscle injection(1.26, the 1st rank) was highest in the rate while among aspects of supportive nursing, a service referred to as 'teach exercises needed for making patients move their bodies normally'(0.58, the 1st rank). 5) factors I(satisfaction with hospital services), II(recognition of disease state), III(economy) and IV(period of disease) occupied for 34.4, 13.8, 11.9 and 9.2 percents, respectively among factors related to the intent by the subjects of using home nursing, totaled 59.3%. In conclusion, most of chronic disease patients have the intent of using hospital-based home nursing and satisfaction with hospital services is a factor affecting the intent most. Thus a post-management system is needed to continue providing health management to those patients after they leave the hospital. Further, supportive services should be provided in order that those who are satisfied with hospital services return to their community and live their in dependent lives. Based on these results, the researcher would make the following recommendation. 1) Because home nursing becomes more and more needed due to a sharp increase in chronic disease patients and elderly people, related rules and regulations should be made and implemented. 2) Hospital nurses specializing in home nursing should be cultivated.
