Development of Nursing Center for Elderlies and the Disabled (노인 및 장애자를 위한 건강간호센타 운영모형 개발 - 대학 건강간호센타를 중심으로 -)

  • Nursing centers are nurse-managed organizations that give the client direct access to professional nursing services. Academic nursing centers are faculty-created and -organized nursing centers integrated into nursing school or cooperated with community nursing center. Academic nursing centers are needed for providing services to the forgotten or underserved populations in the community, providing learning opportunities for nursing students and practice opportunities for faculties. The intent of this study is to identify the elements needed in developing process and operations of acedemic nursing center for elderlies and the disabled, and to present the desired model for academic nursing center. The processes of my study were : 1) The articles of the academic nursing centers in U. S. were reviewed and analysed. 2) The academic nursing center for elderlies and the disabled was developed and operated in my paper. 3) Desired model for academic nursing center was presented in my paper. The followings are the results of my study: 1. Elements needed in developing process of academic nursing center were philosophy and goals, the community support, assessment of the validity of the service and health needs, identification of the service contents, roles and responsibilities, communication lines, finances for facilities and operations, cooperation with resources, and developing record system. 2. Elements needed in operations of academic nursing center were the structural organizations, realization of the above philosophy and goals, development of policy and nursing standards, faculty participation, continuity of services, and financial solutions. 3. The desired model was presented according to the process and operations.

Status and Trend of Home Health Nursing for Long-Term Care Insurance Beneficiaries (노인장기요양 방문간호 현황 및 추이)

  • Purpose: This study aims to provide basic data for the development of measures and promoting home health nursing by examining the current status and trends in home health nursing for long-term care (LTC) insurance beneficiaries. Methods: Secondary data, including annual LTC insurance statistics reports for 2010-2017 and LTC manpower data, were used to compute current status and trends in the provision of home health nursing. Results: Beneficiaries of home health nursing under LTC insurance, insurance-covered costs for home health nursing, home health nursing provider, and home health nursing providing institution only accounted for 3% of all insurance-covered home care services, and were on a consistent decline since 2010. In particular, vulnerable rural regions with high proportion of individuals had poor infrastructure in terms of home health nursing institutions and manpower, but had a higher home health nursing utilization rate compared to urban regions. Conclusion: In addition to measures to support home health nursing service beneficiaries, policy measures are needed to support home health nursing service personnel and institutions. Furthermore, programs to cultivate the expertise of home health nurses and improve quality of home health nursing services should be developed in order to promote home health nursing utilization in vulnerable rural regions.

An Analysis of Pediatric Emergency Nursing Practice and Nursing Competence among Emergency Department Nurses (응급실 간호사의 소아응급간호 업무 및 간호수행자신감 분석)

  • Purpose: The purpose of this study was to examine the frequency, perceived importance and competence of pediatric emergency nursing practice (PENP) in nurses who cared for pediatric patients in the emergency department. Methods: This cross-sectional descriptive study analyzed 175 emergency department nurses caring for pediatric patients at 7 university hospitals with more than 500 beds, located in Seoul and Gyeonggi Province. The measurement tool was modified from the Classification of Standard Nursing Activities to measure the frequency, importance of PENP, and nursing competence. It comprised 143 items in 16 domains of PENP. Results: The most frequent nursing practice was the domain of 'nursing records and environmental management' and the least frequent practice was the 'research and consulting' domain. The nursing care domain perceived to be most important by nurses was 'specialized intensive nursing care'. The nursing care domain with the highest level of nursing competence was 'hygiene care', and the domain with the least level of nursing competence was the 'research and consulting'. Conclusion: These results will be utilized as basic data for future pediatric emergency nursing education initiatives and for establishing priorities of nursing policy to improve health care for children admitted to the emergency department.

Agenda and Alternatives for Home Health Care Policy in Low Fertility and High Aged Society of Korea (저 출산 고령 사회에 대비한 재가 간호사업 정책 과제와 대안)

  • The purpose of this study was to describe the policy agenda and alternatives for the home health care system in Korea. The home health care system development was not fully integrated while the medical laws were established in 2000, community health law in 1995, and elderly long-term health insurance law in 2007. Because of the increasing population of people over the age of 65 and dramatically decreasing fertility rate, the burden of various health-care expenses has become a great obstacle for the Korean government. Under these circumstances, the home of home health care system in has taken on an important role under the mandate of the national health care system. The types of home health care system in Korean shows a greater contrast from those utilized in other more industrialized countries, such as, U.S. or Japan. In conclusion, the strategy in overcoming the obstacles to enhance home health care system under the national health system would be developing it as a comprehensive and exchangeable consumer-focused organization.

  • Purpose : this study was to project demand of home nursing care in Korea for contributing toward the distribution of home nursing care resource. Method : demand of home nursing care is projected according to income level and needed service level of each person. in addition, demand sufficiency of home nursing care is taken by estimating for supply level available. Result : home nursing care service is scarecely provided to people who belong to second area, that is, only $1.6{\sim}2.3%$ out of them are provided and also, supply system comes short of meeting demand of the other people(who belong to one, third area). Conclusion : therefore, for proper provision of home nursing care, different supply system and policy direction for establishment and expansion of home nursing care is to be developed.

A Survey for Developing Strategies to Improve the Fees for Nursing Care (건강보험 간호관리료 수가체계 개선의견 조사분석)

  • Purpose: To examine the current fees for nursing care and propose the strategies for improvement. Method: The number of subjects for this study was 86, including 36 chief executives of the nursing department, 14 of the health insurance department and 33 nursing managers. Data were analyzed by SPSS WIN 12.0 program. A researcher-developed questionnaire with 30 items was utilized. Results: 61% hospital had improved the nursing management fee grade after adoption of the differentiated nursing management fee schedules. After grade improvement, the time for direct nursing care increased. Also, the patient health outcome, nurse's job satisfaction were improved and more nurses were employed in general nursing units. Many subjects addressed that ICU and more nursing units were needed to adopt the differentiated nursing management fee schedules and "bed to nurse ratio" needed to be changed to "patients to nurse ratio" and specialized by the nursing units. Conclusion: The health policy in reference to fees for nursing care needs to get improved further in order to provide the quality-assured nursing care.

Relationships among Emotional intelligence, Ego-resilience and Communication competence in Nursing Students (간호대학생의 감성지능, 자아탄력성과 의사소통능력과의 관계)

  • Purpose. The purpose of this study was to investigate the influence of emotional intelligence, ego-resilience and communication competence in nursing students. Methods. Data was collected from a group of 146 nursing students by means of self reported questionnaires about emotional intelligence, ego-resilience and communication competence in nursing students from the period between February 18, 2017 to March 19, 2017. Results. There were positive correlations between emotional intelligence, ego-resilience and communication competence. Factors influencing the communication competence of nursing students were ego-resilience (${\beta}=.42$, p<.001), Academic achievement (${\beta}=.38$, p=.003)and emotional intelligence (${\beta}=.24$, p=.026). These factors explained 54.8% of the variance in the participants' communication competence (F=60.15, p<.001). Conclusions. In conclusion, strategies for increasing emotional intelligence and ego-resilience of nursing students' ego-resilience, should be developed to improve the communication competence of nursing students.

The Critical Success Factors of Nursing Organizations Perceived by Nurses in a Hospital (간호사가 인지하는 병원 간호조직의 핵심성공요인)

  • Purpose: This study was designed to describe the Critical Success Factors (CSF) of nursing organization perceived by hospital nurses. Method: Data was collected from 12 nurses who were working in the university affiliated medical center in Seoul using interview. To analyze data, content analysis was done. Results: The Critical Success Factors (CSF) of nursing organizations perceived by hospital nurses was classified into 5 categories. The 5 categories (the expectation of nurses' clients, The awareness economic value that nurses perform work, efficient nurses' work process, attractive nurses' work conditions, nurses' grow into professional) include 21 themes and 53 significant statements. Conclusions: The result of this study may be helpful in the strategic performance management of nursing organizations.

Effects of Abdominal Breathing on Anxiety, Blood Pressure, Peripheral Skin Temperature and Saturation Oxygen of Pregnant Women in Preterm Labor (복식호흡이 조기진통 임부의 불안, 혈압, 말초 피부온도와 산소 포화도에 미치는 효과)

  • Purpose: This study was done to examine the effects of abdominal breathing on VAS-Anxiety (VAS-A), blood pressure, peripheral skin temperature and saturation oxygen in pregnant women in preterm labor. Method: The study design was a matched control group interrupted time series. Forty-six women matched to gestational age were assigned to either the experimental group (26) or control group (20). Data were collected between March 2007 and May 2008. For the experimental treatment the women performed abdominal breathing 30 times, which took 5 minutes, and did one set of 5-minute abdominal breathing daily for three days. Data collection was done before and after the abdominal breathing to measure VAS-A, blood pressure, peripheral skin temperature and oxygen saturation. Descriptive, $X^2$, Mann-Whitney U tests were used to analyze the data with the SPSS/PC+Win 15.0 program. Result: For the experimental group there were significant decreases in VAS-A (Z=-4.37, p=.00), systolic blood pressure (Z=-3.38, p=.00), and an increase in skin temperature (Z=-4.50, p=.00) and oxygen saturation (Z=-3.66, p=.00). Conclusion: These findings suggest that abdominal breathing in pregnant women in preterm labor results in decreases in anxiety(VAS-A) including biological evidences such as systolic blood pressure, and increases in peripheral skin temperature and oxygen saturation. Further longitudinal study is needed on the lasting effects and obstetric and neonatal outcomes following abdominal breathing.

Influence of Self Efficacy, Social Support and Sense of Community on Health-related Quality of Life for Middle-aged and Elderly Residents Living in a Rural Community (농촌지역 중년과 노인의 자기효능감, 사회적 지지와 지역사회 공동체의식이 건강관련 삶의 질에 미치는 영향)

  • Purpose: The purpose of this study was to examine the relationship between self-efficacy, social support, sense of community and health-related quality of life (HRQoL), including the direct and indirect effects of the variables on HRQoL. Methods: A cross-sectional survey was conducted with a convenience sample of 249 middle-aged and elderly residents living in a rural community in A-County, K Province. The structured questionnaire included 4 scales from the Euro Quality of life-5 Dimensions (mobility, self-care, usual activities, pain/discomfort, anxiety/depression), and measures of General Self-Efficacy, Social Support, and Sense of Community. Data were analyzed using SPSS WIN 20.0 and AMOS 21.0 program. Results: The mean HRQoL score for the participants was $0.87{\pm}0.13$. Self-efficacy (${\beta}$=.13, p=.039) and age (${\beta}$= -.38, p<.001) were significantly associated with HRQoL, explaining 21% of the variance. In the path analysis, self-efficacy showed a significant direct effect on HRQoL (${\beta}$=.14, p=.040) and significantly mediating relationships between both social support (${\beta}$=.05, p=.030) and sense of community (${\beta}$=.02, p=.025) and HRQoL. Conclusion: Although self-efficacy was found to be the main predictor for HRQoL, the findings imply that social environmental factors such as social support and sense of community need to be considered when developing interventions to increase HRQoL in middle-aged and elderly residents in rural communities.
