Worker's Health Management and Health Promotion of General Hospitals in Korea (우리나라 종합병원 사업장의 근로자 건강관리 현황 분석)

  • Purpose: This study was conducted to understand the situation of general hospital worker's health management and health promotion. Methods: To investigate the current situation of health management in the hospital, structured questionnaires were sent to 122 occupational health providers by post. About 79% hospitals returned questionnaires. The data were analyzed using descriptive analysis, ${\chi}^2$-test by SPSS 12.0 program. Results: A quarter hospitals responded set up separated health care office for workers, 87.5% provided health educations, and 56.5% operated health promotion projects. In the contents of health promotion program embraced both health behavior practice and disease prevention, musculoskeletal disease control, infection control, smoking cessation, and exercise program were most commonly provided to the workers in order. Occupational health care provider chose the item such as budget limitation, manager's apathy, lack of employee's participation, cooperation provider, and so on as the reason of difficulty to run health promotion program in the hospital setting. Conclusion: Hospital managers need to construct infra to manage and promote worker's health. For example, establishing Industrial safety and health committee in hospital and arranging nurses who being fully responsible to worker's health. And occupational health care provider should advertise health promotion projects both managers and workers actively.

Analysis of Research on Nursing Home in Korea - Focused on Theses and Journal Papers since 1990 - (노인요양시설에 관한 국내연구 분석 - 1990년 이후 학위논문과 학술지 게재논문을 대상으로 -)

  • The tendency of having fewer or no children and increasing number of single household will make the protection of the elderly with geriatric disease even harder in the future. Nursing home can provide long-term care for those who can-not be cared by the family. Korean government has been changing welfare policy for the elderly to cope with the changing needs of them regarding nursing home and geriatric disease. Researchers from various field also showed great interest in nursing home. The purpose of this study is to analyze the number and contents of theses and papers regard-ing nursing home in the 12 journals, to find out overall the research streaming of the theme, method, subject, and district of the papers in the journals. Data were collected by using keyword through library database. 246 thesis and 116 papers published from 1990 to 2006 were analyzed in 7 categories, and 66 papers in the journals were used on the content analysis for lading out research streaming. As results, research streaming according to 5 periods were identified. The most frequent subjects of theses regarding nursing home are nursing home systems and policies. Otherwise, the various subjects of papers in the journals have been deal with, especially plans of nursing home and spacial behavior of the elderly and staffs' satisfaction. In the future, the research subjects related to the nursing home should deal with total space plan as living environment for the elderly and work environment for the staffs according to privacy, spatial use and behavior, culture and lifestyle in Korea, and so on.

Obesity related Factors in Middle Aged Women: Comparison of Body Mass Index and/or Waist Circumference as Measures of Obesity (지역사회 중년여성의 특성 및 건강행위 실천과 비만: 비만 지표에 따른 비교)

  • Purpose: The study examined obesity conditions and related factors in middle aged Korean women who had been diagnosed as obese based body mass index (BMI) or waist circumference (WC) measurement. Presently, BMI alone, WC alone, or BMI + WC were used as obesity measures. Methods: Subjects were 488 women 40~64 years of age living in one city, Korea. Structured questionnaires concerning general characteristics, health behavior practices, and chronic disease history were completed by each subject. Results: The proportions of obesity were 42.4% by BMI, 39.3% by WC, and 51.3% by the combination of both measurements. Chi-square test revealed significant associations of obesity with age, education level, menopausal status, frequency of excessive eating, and history of diabetes mellitus or hypertension. These associations were similar for the three measurement schemes. There was significant positive relationship between BMI and WC, whereas the relationship between BMI and WHR (waist-hip ratio) was not significant. Conclusions: The combination of BMI and WC measurement may be a valid way to evaluate the obesity status in middle-aged women. Developing an obesity management program that includes control of excessive eating, and consideration of menopausal status and history of chronic diseases would be prudent.

Priorities of Nursing Research Areas in Korea (우리나라에서의 간호연구 우선순위)

  • This study was to identify priority areas of nursing research in Korea. Method: A data analysis was done for the PhD subjects (n=133) who responded at the national level two-rounds delphi survey. Subjects were asked to assign a score from 1 to 7 for 29 nursing research areas regarding 3 aspects: the degree of nurses research leadership; the degree of contribution to nursing professions; and the degree of contribution to health and welfare of patients/clients. Result: For the overall three aspects, research area on clinical nursing practice was identified as the first priority, followed by nursing education, home health nursing, nursing research, and chronic disease. For nursing research leadership, research area on clinical nursing practice, nursing education, home health nursing, nursing research, and nursing diagnosis were identified. Concerning contribution to the nursing profession, the first priority was nursing education, followed by clinical nursing practice, nursing research, nursing policy, and home health nursing. Concerning contribution to the health/welfare of patients, clinical nursing practice, home health care, geriatric nursing, chronic disease, and symptom management were ranked as top 5 research priorities. Conclusion: The findings of this national survey will assist in building insights into the research needs of nurses practicing in Korea.

Trends in Health-related Research on Foreign Migrants Living in Korea: Review of Published Studies during the Past 10 Years (2000-2009) (외국인 이주자 건강관련 연구 동향: 최근 10년간(2000-2009년) 연구에 대한 고찰)

  • Objectives: The purpose of this study was to investigate the current state and trends of health-related research on foreign migrants living in Korea. Methods: Among 85 articles identified via PubMed and RISS and additional 7 articles by ancestry search, 28 were selected for the review. Two authors independently reviewed the articles using a literature review matrix and examined together the three aspects of the studies: research methods, research topics, and research results. Results: Most of the articles were published between 2005-2009, and only 3 studies used an interdisciplinary approach. Most were exploratory and there were 4 intervention studies: three of them examined the effect of an art therapy on acculturative stress and mental health among marriage migrant women. The lack of information on informed consent, approval of institutional review board, and the validity/reliability of the translated instruments were noted. Conclusions: Research is needed to examine migrants' health behaviors, the prevalence of chronic illness among migrants, and the influence of acculturation on the use of health resources, health behaviors, and health status. Researchers should be aware of ethnic issues when they work with foreign migrants.

A study of Family Function and Social Support toward Perceived Quality of Life in Adolescents (청소년이 지각한 가족기능, 사회적 지지 및 삶의 질에 관한 연구)

  • Purpose: The purpose of this study was to explore family function, social support, perceived quality of life(QOL) in adolescent period and to find out the correlations between family function, social support, perceived QOL in adolescents. Method: The questionnaire to get some information for this study consisted of adolescents'general characteristics, 20 questions regarding family function, 11 questions regarding social support, 8 questions regarding perceived QOL in adolescents. The subjects of this study consisted of 128 adolescents who were living in S area. The data were analyzed by percentage, mean, t-test, ANOVA and Pearson's correlation using the WINSPSS program. Result: The mean score for family function was 3.07(max. : 5) and there was significant difference according to middle-high school student. The mean score for social support was 2.86(max : 5) and there was no significant difference. The mean score for perceived QOL was 4.02(max score: 7) and there was significant difference according to disease condition. There was a positive correlation between family function, social support, perceived QOL in adolescents. Conclusion: Consequently, the development of program, policy assistance and policy implementation to elevate juveniles' perceived quality of life is strongly required.

Health Status and Health Service Utilization: Barriers and Facilitators for Korea Medicaid Beneficiaries

  • Purpose: This study compares beneficiaries of Korean Medicaid with those under the National Health Insurance program, seeking to understand how each group utilizes their healthcare. Methods: Data were obtained from a Health Promotion Survey in 2005. Health status was measured by the respondents' perception of health. Health service utilization included the availability of healthcare services, the type of healthcare institution, and intent to revisit. Predisposing and enabling factors, as well as health care needs were used for this study. Results: Compared to National Health Insurance beneficiaries, Medicaid beneficiaries reported lower levels of health status and fewer enabling factors. They had more chronic diseases and disabilities. Education level, existence of chronic diseases, exercise patterns, and disabilities were associated with health status. Conclusion: We found that Medicaid beneficiaries had fewer resources and higher levels of health needs. As Medicaid is reformed, policy makers and administrators should understand healthcare utilization behaviors of Medicaid beneficiaries and the factors hindering access to care.

The Effects of Violent Experiences and Coping on Job Stress of Caregivers in Long-Term Care Facilities (요양보호사의 폭력경험과 대처방법이 직무스트레스에 미치는 영향)

  • Purpose: This study was conducted to investigate factors that affect job stress among caregivers in long-term care facilities. Methods: Data were collected from 125 caregivers through a structured questionnaire targeting general characteristics, violent experiences, coping, and job stress from February 2 to March 10, 2015. Data were analyzed by t-test, ANOVA, and multiple regression using SPSS 21.0. Results: The rate of violence experienced by caregivers was 99.2% for verbal abuse, 99.2% for physical violence, 95.2% for physical threatening, and 62.3% for sexual abuse. The mean score for coping and job stress was above average. Job stress significantly differed by marital status, education level, facility size, and policy on workplace violence. The significant predictors of job stress included policy on workplace violence, violent experiences, and education level. The regression model explained 28.0% of the variance in job stress. Conclusion: To improve job stress of caregivers, it is necessary to establish policies for violence within the organization and to develop and apply various programs that allow caregivers to work safe from violence both physically and psychologically.

Nursing Students' Practice Experience on Community Visiting Nursing (간호대학생의 지역사회 방문건강관리 실습경험)

  • Purpose: The purpose of this study was to explore the experiences of nursing students' practice on community visiting nursing. Methods: For this study, in-depth interviews were conducted with a total of 12 nursing students, and data were collected through individual in depth interviews from September to December 2018. The data were analyzed using the phenomenological analysis method suggested by Colaizzi. Results: The experience of visiting nursing practice consisted of four themes. Four themes are 'Understanding visiting nursing work: Tight time and problems to overcome', 'Understanding visiting nursing object: People waiting for a visiting nurse', 'Visiting nursing practice experience: Sometimes to get reprimanded but rewarding practice', and 'Expectations for visiting nursing business: Hopes for development'. Conclusion: Findings from this study presented vivid experiences of nursing students who on community home visit nursing. Community visiting nursing practice became an opportunity for students to understand visiting nursing and target people and to think about the necessity of visiting nursing and future development direction. It is necessary to develop an institutional and policy basis with regard to the increasing number and role expansion of community visit nurses.

Study on Patient Outcomes through the Construction of Korean Nursing Minimum Data Set (NMDS) (한국형 Nursing Minimum Data Set(NMDS)구축을 통한 환자결과에 대한 연구)

  • Purpose: The purpose of this study is developing the nursing information system which contains the core elements of nursing practice, the Nursing Minimum Data Set (NMDS) that should be collected and documented all the settings in which nursing care is provided. Method: The program was developed under the hospital information system by TCP/IP protocol and used NANDA, Nursing Interventions Classification (NIC), and Nursing Outcomes Classification (NOC) to fill out the elements of NMDS. The Oracle was used as DBMS under the Windows 98 environment and Power Builder 5.0 was used as a program language. Results: This study developed linkage among the NANDA-NOC-NIC to facilitate choosing correct nursing diagnosis, interventions, and outcomes and stimulate nurses' critical thinking. Also the system developed includes nursing care sensitive patient outcomes, so nurses can actively involve in nursing effectiveness research by analyzing the data stored in the database or by making relational databases with other health care related databases. Conclusion: The program developed in this study ultimately can be used for the nursing research, policy development, reimbursement of nursing care, and calculating staffing and nursing skill mix by providing tool to describe and organize nursing practice and measure the nursing care effectiveness.
