Differences in physical function, self-efficacy, and health-related quality of life by disease severity in community-dwelling patients with chronic obstructive pulmonary disease

  • Purpose: This study investigated the differences in physical function, self-efficacy (SE), and health-related quality of life (HRQoL) categorized by disease severity in community-dwelling patients with chronic obstructive pulmonary disease (COPD). Methods: This cross-sectional study included 182 patients with COPD selected from the pulmonology outpatient department of a tertiary hospital. Disease severity was measured using forced expiratory volume in 1 second (FEV1). Physical function, SE, and HRQoL were measured with the six-minute walking distance, Pulmonary Rehabilitation Adapted Index of Self-Efficacy (PRAISE), and St. George's Respiratory Questionnaire (SGRQ). Disease duration, FEV1, and 12-month history of exacerbations were obtained from medical records. Patients were categorized by Global Initiative for Chronic Obstructive Lung Disease (GOLD) category. Data were analyzed using the χ2 test, and one-way ANOVA. Results: Most of the participants were male and nonsmokers. The disease duration was 10.76 ± 10.03 years, the mean FEV1% was 62.13 ± 22.80, and 70.3% of the participants were in GOLD category 2 (moderate) or milder. Half of the participants reported modified Medical Research Council scores ≥ 2. Patients in GOLD categories 1 and 3 (mild and severe) exhibited significantly higher PRAISE scores than those in the other groups (F = 8.23, p < .001). The total SGRQ scores were highest in GOLD 4 (very severe), indicating the lowest HRQoL. Significant differences were identified among GOLD 1, GOLD 2 and 3, and GOLD 4 (F = 9.92, p < .001). Conclusion: We identified potentially useful variables to comprehensively assess disease severity and tailor management strategies, including airflow limitation, and to determine the consequences of COPD from patients' perspectives.

소셜미디어 내 의료소비자의 환자안전 관심에 대한 구조적 토픽 모델링 분석 (Structural Topic Modeling Analysis of Patient Safety Interest among Health Consumers in Social Media)

  • Purpose: This study aimed to investigate healthcare consumers' interest in patient safety on social media using structural topic modeling (STM) and to identify changes in interest over time. Methods: Analyzing 105,727 posts from Naver news comments, blogs, internet cafés, and Twitter between 2010 and 2022, this study deployed a Python script for data collection and preprocessing. STM analysis was conducted using R, with the documents' publication years serving as metadata to trace the evolution of discussions on patient safety. Results: The analysis identified a total of 13 distinct topics, organized into three primary communities: (1) "Demand for systemic improvement of medical accidents," underscoring the need for legal and regulatory reform to enhance accountability; (2) "Efforts of the government and organizations for safety management," highlighting proactive risk mitigation strategies; and (3) "Medical accidents exposed in the media," reflecting widespread concerns over medical negligence and its repercussions. These findings indicate pervasive concerns regarding medical accountability and transparency among healthcare consumers. Conclusion: The findings emphasize the importance of transparent healthcare policies and practices that openly address patient safety incidents. There is clear advocacy for policy reforms aimed at increasing the accountability and transparency of healthcare providers. Moreover, this study highlights the significance of educational and engagement initiatives involving healthcare consumers in fostering a culture of patient safety. Integrating consumer perspectives into patient safety strategies is crucial for developing a robust safety culture in healthcare.

The status of intimate partner violence against pregnant women in contemporary China: a scoping review

  • Purpose: This review explored the status of publications on intimate partner violence (IPV) against pregnant women in contemporary China. Methods: The PubMed, Cochrane Library, Embase, CINAHL, and PsycInfo databases were searched using the terms "IPV," "pregnant woman," "Chinese," and synonyms in English, along with related keywords for Chinese publications. All literature pertaining to IPV during pregnancy, conducted in China, and published between 1987 and September 2023 was included. Results: A total of 37 articles from 30 studies were selected. The prevalence of IPV during pregnancy ranged from 2.5% to 31.3%, with psychological violence being the most common form. Frequently identified risk factors included unintended pregnancy, poor family economic conditions, male partners engaging in health risk behaviors, poor employment status of women or their partners, low education levels among women, physical or mental health issues, strained couple relationships, and in-law conflicts. IPV during pregnancy primarily led to mental health problems for the victims and could result in adverse obstetric outcomes, as well as negative effects on the temperament and development of the offspring. Victims in China demonstrated a low willingness to seek help from professionals. Furthermore, relevant research in mainland China is scarce, with a limited number of studies and non-standardized research methodologies. Conclusion: Future research should investigate IPV in pregnancy from various perspectives, identify factors unique to IPV during pregnancy, and focus on high-risk groups. Considering the conditions in China, there is a pressing need to increase public awareness of IPV and to investigate interventions aimed at addressing this issue.

폐암 환자의 활동상태, 불안, 우울, 사회적 지지, 증상경험과 기능적 상태의 관계: 불쾌감이론 기반으로 (Relationships among Activity Status, Anxiety, Depression, Social Support, Symptom Experience, and Functional Status in Lung Cancer Patients based on the Theory of Unpleasant Symptoms)

  • Purpose: The purpose of this study was to investigate the relationships of activity status, anxiety, depression, social support, symptom experience, and functional status in patients with lung cancer based on the theory of unpleasant symptoms. Methods: The participants for this study were 101 lung cancer patients who visited the out-patient department for treatment or follow-up at one hospital in Seoul. Data were collected from January 1 to February 8, 2013 using self-reported questionnaires and clinical records. To measure variables, the functional scale and symptom scale of the European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire-Core 30, EORTC QLQ-Lung Cancer 13, Eastern Cooperative Oncology Group Performance Status, Hospital Anxiety and Depression Scale, and Multidimensional Scale of Perceived Social Support were used in this study. The data were analyzed using SPSS 19.0 software for Windows. Results: The symptom experience showed more severity in patients with lower activity status, higher anxiety and depression. With lower activity status and social support, functional status was lower. When anxiety, depression, and symptom experience were higher, functional status was also lower. The significant factors predicting symptom experience were depression, anxiety, activity status, and social support, which explained 57.8% of the variance. Conclusion: These results suggest that psychological factors such as anxiety and depression had a negative influence on the symptom experience of lung cancer patients. Therefore, providing emotional support based on the patients' needs prior to providing symptom management could be a useful strategy for improving symptom experience and functional status.

자녀를 둔 어머니의 출산과정 경험 (An Ethnographic Research Study on Childbearing Process of Mother with Children in Korea)

  • The childbearing process is a sociocultural phenomenon of a woman who gives birth to a child as well as a biological phenomenon. The purpose of this ethnographic research study was to explore the experience of childbearing process of mothers with children from pregnancy to the 3 months postpartum in Korea and to understand deeply the perspectives of childbearing women reflected on Korean sociocultural values. A convenient sample of 10 childbearing women were observed from January to October 2000 through field work in Seoul, Korea. Data analysis was accomplished under ongoing process. The results of this study were as follows : The mothers with children experienced self-reflection, family relation, and physical adaptation during pregnancy. In self-reflection, all mothers experienced universality and diversity in their self-discovering process. The universal experiences were maturation, life with family and priority on maternal value between being a mother and a woman. The diverse experiences were taking a dual role of working mother, emotional drift of a resigned mother, and disheartened life of a mother who has two daughters. In family relation, the foundation of the new marital relationship were attained during childbearing process and sexual life were changed for the benefit of a healthy mother and a healthy baby. All mothers established friendly relations with their mothers, but established friendly or conflicting or constraining relations with their mother-in-laws due to husband based family culture. In physical adaptation, the informants endured well the physical discomfort and recognized general appearance change. Also maternal-fetal interaction occurred and mothers realistically felt motherhood and accepted themselves as mother-to-be. The mothers prepared for the best delivery, look for a safe childbirth center, newborn goods, endorsed family coping during hospitalization and responded labor pain to make it more endurable, less painful, fast passed owing to labor recognition of the natural process to be a mother. After childbirth, they felt emancipation, satisfaction, accomplishment, more easiness, actually feeling as mother-to-be, emptiness, and showed response to the sex of newborn. Their Sanhujori practice was different according to the Sanhujori environment including provider, place, time in postpartum and reflected on Sanhubyung. The mothers felt actually mother-to-be and happiness during lactation regardless of feeding pattern. These mothers had a different maternal image about rearing subjecthood through their child-rearing experience. But all mothers felt need for family support and social support. The universal rearing response were actual feeling of mother-to-be, a strenuous experience, a pride on child-rearing, confusion, reflecting marital relationship, and wondering rivalry among children. In conclusion, mother of all with children went through self-discovery, self-reflection and made connections with the family as a mother and as a woman simultaneously during the childbearing process. Therefore it is suggested when harmony and balance between a mother and a woman is accomplished, the woman will lead a healthy and high quality of life. Also, this study sought to confirm the sociocultural factors affecting the childbearing process from the perspectives of the women with children. Therefore health care providers must understand deeply the childbearing women with children based on this finding of and try a integrative approach with new ideology of maternity with biocultural perspectives in a clinical setting.

중년기 여성의 폐경경험 (Korean Urban Woman's Experience of Menopause : Newlife)

  • What is the meaning of menopause experienced by urban Korean women? Nurses need an under standing of menopause as it is experienced by women themselves. Nursing needs to build knowledge of womens' health experiences. This phenomenological study examined what menopause means to modern Korean woman to build a structure of knowledge useful for practice to enhance the quality of life of women throughout this experience. Traditional definition of menopause according to physiological changes, as illness and more recently as psychosociocultural phenomena were examined along with the folk lore information generally available in the society A review of the research and scientific literature was done from the perspectives of four models including the medical model of menopause as disease, the psychosocial model as positive and negative behavioral responses to menopause, a feminist model of menopause as a time of rebirth and a nursing model of the changing patterns of meaning, rythms and transformation women experience through menopause. Van Kaam's method was used to analyse data audio-recorded during interviews by the investigator with 65 women, 40 to 60 years of agey whose confidentility was assured. Interpretation of the data was enhanced luther by consultation with professional colleugues and with informants. Four rhythmical patterns of process emerged : from suffering to comfort, from oppression to freedom from being a good wife and wise mother to becoming a woman and from a hard life to an abundant life. The detailed common elements making up each of the four patterns and definitions of each pattern were presented. Each pattern was discussed critically from the point of view of medical, psychosociocultural, womens' and nursing models. The structural definition of the synthesis of the four process patterns was stated as : in spite of suffering the middle-aged urban Korean woman find she is able to help herself to feel comfortable and to realize release as she moves from oppression to liberation and freedom from being a good wife and wise mother she experiences rebirth as a woman : she begins to live a profitable and valuable life : her life becomes one of transformed abundant living. The definition transcends the medical and phychosociocultural model to embody a nursing model. The analysis was critiqued by using Parse' Human Becomming theory of nursing because the emerging themes were process patterns. Parse' theory provides and explanation of the experience of menopause consistant with the data which enhances nursing understanding of womens' experience of menopause. Parse' practice methodology provide guidance for promoting womens' quality of life throughout the experience of menopause. Feminist analysis contributes valuable critique to nursing research, richly expanding the perspective from traditional approaches to promote understanding of the meaning of womens' health experiences.

결혼이주 일본여성의 만성질환 시부모 돌봄 경험 (The Lived Experience of Japanese Marriage-Immigrants' Caring for Parents-in-law with Chronic Illness)

  • Purpose: The study was done to explore lived experience of Japanese women taking care of chronically ill parents-in-law at home under the Korean family culture emphasizing filial piety. Method: van Manen's Hermeneutic Phenomenology research method was applied to conduct in-depth analysis on the meaning and nature of those experiences. This method derives the universal and fundamental aspect of human being. Results: There were 8 essential themes derived from the Japanese marriage-immigrants' experiences taking care of parents-in-law with chronic diseases as followings: 'stuck in a situation in which I am obligated to take care of the parent-in-law', 'hard to be the only person responsible to take care of the parent-in-law', 'unaccustomed to the Korean filial duty culture', 'the adverse effect of Korean-Japan history on taking care of the parent-in-law', 'refusing to let the taking care of the parent-in-law to be a shackle of my life', 'starting to be willing to take care of the parent-in-law', 'following through with my own way of taking care of the parent-in-law without being swayed by what others say', 'growing in the process of taking care of the parent-in-law'. Conclusion: The study results broadened ground to understand the experiences taking care of chronically ill parent-in-law from Japanese marriage-immigrants' social and cultural perspectives.

위내시경 검사 목적에 따른 검사 전 불안 정도와 불안 경험: 혼합연구방법의 적용 (Mixed Method Study on Patients' Level and Experience of Anxiety before Undergoing Esophagogastroduodenoscopy)

  • Purpose: This mixed method study aimed to investigate patients' level of anxiety and their experience of this before undergoing esophagogastroduodenoscopy (EGD). Methods: A total of 125 patients answered a questionnaire assessing their pre-EGD level of anxiety, and a total of 17 patients participated in individual interviews regarding their experience of EGD-related anxiety. The SPSS Software program was used for survey data analysis and content analysis was used for qualitative data. Results: The mean anxiety score was 40.00±9.86 and the factors related to anxiety levels were being female (p<.001) and being a relatively young age (20-30 years old, p=.004). There were no significant differences shown in level of anxiety according to the purpose EGD was performed for: screening, diagnosis, or disease follow-up. In the qualitative analysis, however, patients who underwent a follow-up procedure as a result of stomach cancer exhibited more complicated feelings of anxiety and helplessness than others. For those who underwent EGD for diagnostic reasons, they worried about the test results and were concerned about possible life changes after diagnosis. Those for whom EGD was performed for screening purposes expressed concern only in the event of EGD complications. Conclusion: This study explored the nature of pre-EGD anxiety according to the purpose of the procedure. Nurses and doctors should be aware that patients undergoing EGD may have different levels and experiences of pre-procedure anxiety.

노인병원 간호사의 노인에 대한 지식과 태도 및 부정적 행동통제기술의 사용정도 간의 관계 (Relationships among Knowledge, Attitude, and Use of Negative Control Maintenance Techniques Toward Older Adults among Nurses Working in Geriatric Hospitals)

  • Purpose: The purpose of this study was to examine the relationships among knowledge, attitudes, and the use of negative control maintenance techniques toward older adults among nurses working in geriatric hospitals (n=86). Methods: Data were collected from April 4th to May 30th in 2006 by using questionnaires of Fact on Aging Quiz Part I, Aging Semantic Differential Scaling, and Control Maintenance Techniques. Results: The findings showed that the nurses displayed lack of knowledge concerning older adults and their average rate of correct answer was 45.6%. They also hold some negative bias and attitudes, especially in psychological and social aspects. The level of using negative control maintenance techniques in managing older patients' problematic behaviors was reported as the mean of 5.65 with a possible range of 0-40. Nurse's knowledge of older adults was positively associated with their attitudes toward older adults, but their knowledge and attitudes were not associated with the use of negative control maintenance techniques. Conclusion: It is necessary to provide educational programs for nurses that focus on a comprehensive understanding of aging with lifetime developmental perspectives. Further studies are needed to understand the factors associated with using negative control maintenance techniques and to evaluate the intervention programs in reducing the use of negative control maintenance techniques.

정신장애인을 위한 권익옹호 프로그램이 인권인식 수준과 자아존중감에 미치는 영향 (The Effects of a Rights Advocacy Program for Mentally Ill Persons)

  • Purpose: The purpose of this study was to examine the effects of a program for rights advocacy on the level of human rights perception and self-esteem for those who are mentally ill. Methods: A quasi-experimental study using a nonequivalent control group pre-post test design was used. The data were collected from January 20 to March. 17, 2010. Forty one (23 in the experimental group and 19 in the control group) individuals participated in this study. The program was developed based on the education program for human rights developed by the Gyeonggi-do community mental health center in 2009. The program consisted of 8 sessions lasting 8 weeks. Results: There were no statistically significant differences in the demographic variables or the outcome variables between the two groups before the intervention. The level of human rights perception and self-esteem increased after the program in the experimental group but not significantly (t=1.87, p=.07; t=0.88, p=.384). Conclusion: Despite the fact that the program was not effective in increasing the level of human rights perception and self-esteem, the study was timely in that it suggests directions for those who develop rights advocacy programs for the mentally ill.
