Purpose: The purpose of this study was to analyze nursing records for pain management in intensive care unit (ICU) patients. Methods: Nursing process for pain management were analyzed retrospectively by 180 ICU patients' nursing records. Instruments consisted of 3 questionnaires (pain assessment, intervention, and evaluation). Results: For assessment, there was different pain intensity between cancer patients (7.95) and non-cancer patients (7.20). Also pain intensity was lower in PCA group (5.08) than in PCA with PRN group (8.27). Common pain site was surgical areas, along with 17 kinds of words expressed for pain, and mean of pain intensity was 7.47 by numeric rating scales (NRS). For intervention, the patients received pharmacologic interventions (99.4%) such as narcotic analgesics (38.3%) intermittently (70.5%) without side effects (94.4%). For evaluation, mean of pain intensity was decreased to 3.14, but a few patients (12.8%) experienced pain over 5 points despite the intervention. Nurses evaluated the degree of pain relief after the intervention in 87.2% of patients. Conclusion: Nurses do assess patients' pain by using objective tool, intervene, and evaluate for effective pain management. Nurses should make an individual approach and record all nursing activities for pain management.
Purpose: This study compared factors related to fear of falling among women in the early phase of old age based on hand grip strength. Methods: Researchers obtained data of 1,071 women who had participated in the 4th Korean Longitudinal Study of Aging. Data were analyzed using multiple regression analysis. Results: In both groups, fear of falling was positively correlated with falling experience within two years, osteoarthritis-related leg pain, depression, and instrumental daily life abilities; however, self-rated health was negatively correlated. In the low hand grip strength group, self-rated health, osteoarthritis-related leg pain, and hand grip strength accounted for 21.3% of the variance regarding fear of falling. In the normal hand grip strength group, age, self-rated health, fall experience within two years, osteoarthritis-related leg pain, and depression significantly accounted for 16.6% of the variance regarding fear of falling. Conclusion: In applying interventions to prevent falling in women in the early phase of old age, health care workers must verify if women have low hand grip strength and determine whether to focus on fitness improvement through exercise interventions or consider psychological interventions for depression.
Purpose: This study was done to identify the Domains, Classes, labels and nursing activities of nursing interventions used with 117 patients who were admitted to orthopedic nursing units. Method: Data were collected in January and February, 2004 using a computerized nursing process program that contained nursing diagnosis-outcome-intervention (NNN) linkages. The program was developed by the researcher. Frequencies and percentages were used in the analysis. Results: Sixty-five nursing intervention labels were identified. The Domains of the nursing interventions showed higher percentages for 'physiological: basic' (75.9%), 'physiological: complex' (12.9%), 'behavioral'(7.8%) 'family'(1.3%), and 'safety'(1.1%). The Classes of nursing interventions showed higher percentages for 'activity and exercise management'(30.8%), 'physical comfort promotion'(19.3%), 'immobility management'(14.5%), 'drug management'(8.1%), and 'coping assistance'(5.6%). Nursing intervention labels showed higher percentages for 'pain management'(14.7%), 'body mechanics promotion'(8.0%), 'exercise therapy : ambulation'(7.2%), 'splinting'(5.4%), and 'positioning'(5.1%). In the comparison of numbers between performed nursing activities and nursing activities of NIC according to nursing intervention label, the mean of combined rate was 52.3%. Conclusion: These findings will help in building of a standardized language for orthopedic nursing units and enhance the quality of nursing care.
Purpose: More than 60% of patients with advanced cancer experience pain, and uncontrolled pain reduces the quality of life. Nurses are the closest healthcare providers to the patient and are suitable for managing cancer pain using pharmacological and non-pharmacological interventions. This study aimed to identify factors affecting the performance of cancer pain management among nurses. Methods: This study was conducted among 155 participating nurses working at a tertiary hospital who had experience with cancer pain management. Data collection was performed between October 18, 2021 and October 25, 2021. Data analysis was conducted using descriptive statistics, the independent-sample t-test, one-way analysis of variance, and hierarchical regression analysis. Results: There were 110 subjects (71.0%) who had no experience of cancer pain management education. The results of regression analysis indicated that barriers included medical staff, patients, and the hospital system for cancer pain management (𝛽=0.28, P<0.001). The performance of cancer pain management was also affected by experience of cancer pain management training (𝛽=0.22, P=0.007), and cancer pain management knowledge (𝛽=0.21, P=0.006). The explanatory power of the variable was 16.6%. Conclusion: It is crucial to assess system-related obstacles, as well as patients and medical staff, in order to improve nurses' cancer pain management performance. A systematic approach incorporating multidisciplinary interventions from interprofessional teams is required for effective pain management. Furthermore, pain management education is required both for cancer ward nurses and nurses in other wards.
Purpose: The purpose of the study was to compare symptoms, medical therapies, and nursing interventions with terminal cancer patients during the last four weeks of their lives in a hospice unit and general units. Method: For the descriptive survey study, data were collected by reviewing the medical records of 243 patients who died of terminal cancer at K hospital in Seoul. The data was analyzed by using Chi-square test and t-test. Result: The study findings are summarized as follows: There were higher frequencies in physical symptoms of constipation, itching sensation, pain, sleeping disturbance, soreness and dysuria for those patients in the hospice unit than those patient in general units. All emotional symptoms were recorded significantly higher for those patients in the hospice unit than those in general units. Regarding the major medical interventions, pain management was used more significantly for those patients in the hospice unit, but antibiotic therapy and resuscitation were used more significantly for those patients in general units. Conclusion: The hospice unit provided more comprehensive nursing interventions including psychological, spiritual, and family cares as well as physiological care for terminal cancer patients. The facts showed that those patients who would need hospice care in general units should be referred to the hospice unit at an appropriate time.
Purpose: The aim of this study was to investigate the status of visiting nurses assessment and management of chronic musculoskeletal pain in older adults. Methods: The subjects of this study were 285 visiting nurses working at 24 public health centers located in Seoul, Gyeonggi-do, and Gangwon-do. Data were collected using a structured questionnaire from June to July, 2008. The SPSS/WIN 13.0 program was used for data analysis. Results: Of the visiting nurses, 195 (77.4%) reported lack of pain education for nurses. The education of exercise (83.3%) was used most frequently by nurses as a pain intervention. Barriers to effective pain management were inadequate pain management guidelines and lack of pain education for nurses. Conclusion: It is necessary to develop evidence-based nursing interventions for pain management and to develop systematic educational programs for visiting nurses management of chronic musculoskeletal pain.
Purpose: This study aimed to evaluate the effectiveness of exercise in patients with hemophilia. Methods: We carried out a systematic review and meta-analysis in accordance with the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guidelines. A literature search was conducted of published randomized controlled trials for exercise interventions from January 1, 2014 to March 15, 2023. To estimate the size of the effects of exercise, a meta-analysis was performed using the R package "meta." Results: Five databases were searched to obtain articles published in Korean or English. Of 1,150 articles reviewed, 13 were included in the systematic review and 9 in the meta-analysis. The risk of bias was assessed using RoB 2.0. The overall effect sizes of exercise interventions, calculated as the standardized mean difference, were -0.11 (95% confidence interval [CI] = -1.41 to -1.20) for pain, -2.13 (95% CI = -3.33 to -0.93) for joint health, 9.96 (95% CI = 7.51 to 12.28) for physical activity, and 0.59 (95% CI = -0.39 to -1.56) for quality of life. Conclusion: These findings suggest that exercise is useful for improving the joint health and physical activity of patients with hemophilia. Thus, it is necessary to develop and apply exercise interventions for patients with hemophilia to reduce their pain and improve their quality of life.
Purpose: The purpose of this study was to identify factors that have an influence on the pain which patients recognize when receiving an intravenous injection. Method: Participants were 111 patients who were admitting to C University Hospital. The collected data were analyzed using the SPSS 11.0 program. Results: For the participants, factors related to administration of medication were the following: Phlebitis scale, who administers the IV injection, failure to complete the IV injection, explaination of the IV injection and rate of injection. Level of pain, depression and anxiety in the participants was as follows: Pain ($4.86{\pm}2.09$), depression ($35.76{\pm}9.91$), anxiety ($32.34{\pm}8.87$). There were positive correlations between pain and depression (r= .437, p< .000), between pain and anxiety (r= .478, p< .000), and between depression and anxiety (r= .544, p< .000). Conclusion: Such findings provide new insights into the dynamic relationships between depression, anxiety and pain which patients recognize when having an intravenous injection. Further studies should be conducted to establish the causal relation between depression, anxiety and pain and to determine appropriate nursing interventions.
Purpose: This study aimed to compare the effects of three interventions on pain, blood pressure, and pulse rate during infiltration anesthesia in patients about to undergo gamma knife surgeries. Methods: The three interventions employed in a university-affiliated Hospital in J City, South Korea were as follows: EMLA cream plus Vapocoolant spray (Vapocoolant, n=30), EMLA cream plus 10.0% Lidocaine spray (Lidocaine, n=30), and EMLA cream only (EMLA, n=30). The equivalent control-group pre test - post test study design was used. Pain was assessed subjectively using the numeric rating scale (NRS) and objectively using a Galvanic Skin Response (GSR) tester. NRS scores were assessed after infiltration anesthesia and the GSR was assessed during infiltration anesthesia. Blood pressure and pulse rate were assessed twice: before and after infiltration anesthesia. Data were collected between August 3, 2016 and March 24, 2017. Results: NRS scores after infiltration anesthesia and the GSR during infiltration anesthesia were significantly lower in the Vapocoolant group than in the Lidocaine and EMLA groups (F=13.56, p<.001 and F=14.43, p<.001, respectively). The increase in systolic blood pressure (F=4.77, p=.011) and in pulse rates (F=4.78, p=.011) before and after infiltration anesthesia were significantly smaller in the Vapocoolant group than in the Lidocaine and EMLA groups; however, no significant differences were observed in diastolic blood pressures (F=1.51, p=.227). Conclusion: EMLA cream plus Vapocoolant spray was the most effective intervention to relieve pain and to lower increase in systolic blood pressure and pulse rate caused by infiltration anesthesia for stereotactic frame fixation. Thus, application of Vapocoolant spray in addition to EMLA cream is highly recommended as a nursing intervention for patients undergoing gamma knife surgeries.
Purpose: This study was done to identify factors (pain stress, perceived stress, pain disability, fatigue, depression) strategies used by elders to cope with pain based on their type of pain belief. Method: Data were collected from 314 elders in community settings in Seoul from September to December of 2007. Cluster analysis, t-test, and ANOVA were used to analyze data. Result: The types of pain belief were classified as the following groups: Self-blame, Enduring & Mysterious, and Short-term & Understandable. Perceived stress (t=2.36, p=.02), social support (t=2.24, p=.03), extent of pain relief (t=2.39, p=.02), and duration of pain relief (t=2.09, p=.04) were important factors for active and passive coping in the Self-blame group. Pain stress (t=2.39, p=.01) and depression (t=-3.99, p=.00) were significantly related to the active coping in the Enduring & Mysterious group. Perceived stress (t=2.55, p=.01) was an important factor in the passive coping in the Short-term & Understandable group. Conclusion: Considering different types of pain belief in elders and factors that are significantly related to different coping strategies, future nursing interventions should be population specific to encourage active coping strategies and to decrease passive coping strategies.
