Purpose: The objective of this study was to examine patient safety culture (PSC) and patient safety initiatives (PSI) according to IT-based medication errors prevention system which is constructed in this study, and to identify the relationships among system construction, perception to the usage, PSC and PSI. Methods: The subjects were 180 nurses who work at 12 different hospitals with over 300 beds. The questionnaire included the characteristics of participants, a system construction status, the perception to the usage using electric pharmacopoeia (EP), a drug dose calculation system (DDCS), a patient safety reporting system (PSRS) and a bar-code system (BS). The data were collected from July 2011 to August 2011. Descriptive statistics, ANOVA, Pearson correlation and MANOVA were used for data analysis. Results: Systems were constructed in participating hospitals; For EP and PSRS, 83.9%, DDCS, 50%, and BS, 18.3%. The perceptions on the usage of the system were marked highest in BS as 4.54 followed by EP as 3.85. There were significant positive correlations between PSI and EP construction (r=.17, p=.028); PSRS (r=.17, p=.028) and DDCS (r=.23, p=.002). Conclusion: The developed system for improving the user experiences and reducing medication errors was found out well accepted. It is hoped that the system is helpful for PSC and PSI improvement in clinical settings.
Objectives: This study was conducted to determine the effects of Geupunggibodan on brain damage in transient focal cerebral ischemia in rats. Methods: Rats were used for testing in the following three models: Morris water maze, eight-ann radial maze, and histochemistry. Results: In the Morris water maze model, the Geupunggibodan group showed significant decrease in the 3rd, 4th and 6th training sessions compared with the ischemia, group. A retention test in the Morris water maze model was performed on the 7th day without the escape platform. The Geupunggibodan group showed significant increase compared to the ischemia group. In the eight-ann radial maze model, the Geupunggibodan group showed significant decrease in the error rate compared to the ischemia group. In the density of hippocampal CA1 cell of the cresyl violet-stained section, the Geupunggibodan group showed significant increase compared to the ischemia group. Conclusions: These results suggest that Geupunggibodan may have a significant protective effect on brain damage and cognitive dysfunction in transient focal cerebral ischemia.
It is urgently needed to develop programs supporting lifelong education for nurses and students of nursing, which are not restricted by time or space and use personal computers or smartphone. The purpose of this study is to develop CCC(Clinical Care Classification) System into a education program and provides guideline to support clinical tasks for students of nursing. Comparing the search times of the book guideline and the web guideline developed, this study found that it was over 3.5 times faster. And its error rate was over four times lower. This result shows that it can provide accurate intervention for patients since it approaches to intervention and evaluation guideline fast and precisely in the actual tasks of nursing.
Purpose: The purpose of this study was to examine the impact of strategies to promote reporting of errors on nurses' attitude to reporting errors, organizational culture related to patient safety, intention to report and reporting rate in hospital nurses. Methods: A nonequivalent control group non-synchronized design was used for this study. The program was developed and then administered to the experimental group for 12 weeks. Data were analyzed using descriptive analysis, $\chi^2$-test, t-test, and ANCOVA with the SPSS 12.0 program. Results: After the intervention, the experimental group showed significantly higher scores for nurses' attitude to reporting errors (experimental: 20.73 vs control: 20.52, F=5.483, p=.021) and reporting rate (experimental: 3.40 vs control: 1.33, F=1998.083, p<.001). There was no significant difference in some categories for organizational culture and intention to report. Conclusion: The study findings indicate that strategies that promote reporting of errors play an important role in producing positive attitudes to reporting errors and improving behavior of reporting. Further advanced strategies for reporting errors that can lead to improved patient safety should be developed and applied in a broad range of hospitals.
Purpose: This study was done to develop and evaluate a smartphone application for the medication confirmation of high-alert medications. Methods: A nonequivalent control group non-synchronized design was used for this study. Participants in the treatment group used the application for four weeks. Data were analyzed using descriptive analysis, ${\chi}^2$-test, and t-test for the homogeneity of participants, and a paired t-test for effectiveness in each group with the SPSS 18.0. Results: Stability of medication administration was estimated by knowledge and certainty, ranged from a score of one to three. A correct answer with high certainty was coded as high stability, low certainty regardless of correct answer was coded as a moderate stability, and incorrect answers with high certainty were rated as low stability. There were no differences in 'knowledge of high alert medication', 'Certainty of knowledge', 'stability of medication administration', 'confidence of single checking medication', and 'medication safety activities' between the treatment group and the comparison group. The treatment group reported a greater difference between pretest and post-test in 'certainty of medication knowledge' (t=3.51, p=.001) than the comparison group. Conclusion: Smartphone application for medication confirmation of high-alert medications will provide an important platform for reducing medication errors risk.
Purpose : This study aims to investigate the differences in fatigue, sleepiness, and near-miss according to sleep disturbance among shift nurses in intensive care units (ICUs). Methods : A cross-sectional study in a tertiary hospital was performed. A total of 122 shift nurses working in the six ICUs were recruited. They completed self-reported questionnaires about sleep disturbance, fatigue, sleepiness, and near-miss in the past two weeks. Results : The prevalence of reported sleep disturbance was 30.3% (37 out of 122 subjects). Compared to the non-sleep disturbance group, the sleep disturbance group reported significantly more sleepiness (11.46 vs. 8.86) and higher fatigue (82.62 vs. 69.39). The sleep disturbance group showed higher rates of near-miss (78.4 vs. 57.6%) and a higher frequency of them (4.49 vs. 2.11/2weeks) compared to the non-sleep disturbance group. Medication error was the most common type of near-miss. Conclusions : This study suggests that sleep disturbances could increase fatigue, sleepiness, and near-miss among ICU shift nurses. Personal and organizational programs should be developed to support the sleep of ICU nurses.

Purpose: The objectives of this study were to understand and compare perception and experience between clinical staffs(nurses and pharmacists) and Quality Improvement managers. Method: A qualitative study was conducted with 14 clinical staffs and QI managers who are working at tertiary hospitals in Korea. Interviews were recorded and transcribed for systematic analyses of qualitative data. Results: Most critically, while QI managers acknowledged that establishment of the patient safety culture and reduction of medical errors are urgent tasks for QI effort, clinical staffs don't seem to share such perceptions. All participants agree that staff shortage and no compliance to safety procedures were major reasons for medical error occurrences. Many suggested that an organizational culture where errors were perceived as a systematic problems rather than individual failures or carelessness should be formed to promote voluntary reporting of medical errors. Conclusion: A more systematic effort and attention at the hospital leadership and public policy level should be promoted to constitute societal consensus on the urgence of promoting patient safety culture and more specific approaches to tackle the patient safety problems.
Purpose: This descriptive research study attempted to determine how general hospital nurses' awareness of the importance of patient safety management and patient safety competency affected patient safety management activities. Methods: From September 13 to 26, 2022, a survey was administered to 230 ward nurses who provided direct care to patients at five non-accredited general hospitals being evaluated for accreditation located in metropolitan cities. The collected data were analyzed with descriptive statistics, the t-test, one-way analysis, the Scheffé test, Pearson correlation coefficients, and hierarchical regression using SPSS for Windows version 26.0. Results: In total, 221 (96.1%) respondents were female. The average age was 32.2 years, and the average clinical experience was 3.5 years; 196 participants (85.2%) were general nurses. Patient safety competency (β = .44, p < .001), awareness of the importance of safety management (β = .31, p < .001), and medication error experience (β = -.15, p = .002) all had statistically significant associations with patient safety management activities. The explanatory power of these variables for patient safety management activities was 50.7%. Conclusion: This study confirmed that patient safety competency, awareness of the importance of patient safety management, and experience with medication errors significantly influenced patient safety management activities.
Purpose: The purpose of this study was to develop an eye-health program based on social learning theory (EPST) of preschoolers and evaluate its effectiveness. Methods: A nonequivalent control group pre-post test design was utilized and 141 six-year-old preschoolers and their parents participated (experimental group=69, control group=72) in the study. The EPST in this study included eye-health education and eye exercises. Attention, memory, replay, motivation, reinforcement, and self-efficacy were used as interventional strategies. To examine the effectiveness of EPST, proficiency in eye-health activities, refractive power, and visual acuity were measured before and after the intervention. Data were analyzed with SPSS WIN 21.0 using the Shapiro-Wilk test, ${\chi}^2$-test, Mann-Whitney U test and Wilcoxon signed rank test. Results: Following the intervention, eye-health activities, refractive power, and visual acuity significantly improved in the experimental group compared to the control group. Conclusion: The results of this study suggest that EPST is effective in improving eye-health activities, refractive power, and visual acuity in preschoolers, and its wider implementation in educational institutions will promise improved eye-health among preschoolers.

Purpose: The purpose of this study was to examine the actor and partner effects of self efficacy, marital adjustment and family support on Yangseang in infertile couples using the actor-partner interdependence model. Methods: The participants were 108 infertile couples. Data were collected using the Yangsaeng Scale, Infertility Self Efficacy Scale, Revised Dyadic Adjustment Scale, and Perceived Social Support Scale from March to April in 2016. The Actor-Partner Interdependence Model was used to analyze the actor and partner effects of self-efficacy, marital adjustment and family support on Yangsaeng. Results: The fit indices for the model were Comparative Fit Index (CFI) = 0.99, Normed Fit Index (NFI) = 0.99, Tucker-Lewis Index (TLI) = 0.90, and Root Mean Square Error of Approximation (RMSEA) = 0.01, which met the criteria. We found that only self efficacy had an actor effect on Yangsaeng. Marital adjustment and family support of infertile husbands had an actor effect on the Yangsaeng. In addition, marital adjustment showed an interaction effect. Conclusions: The findings of this study indicate that the level of Yangseang in infertile couples was positively associated with the levels of self efficacy and martial adjustment. Partner effects may be hidden due to much stronger actor effects on Yangseang.
