The Effects of Organizational Commitment and Perceived Patient Safety Culture on Patient Safety Nursing Activities among Nurses in Comprehensive Nursing Care Units (간호·간병통합서비스 병동 간호사의 조직몰입과 환자안전문화인식이 환자안전간호활동에 미치는 영향)

  • Purpose: The purpose of this study is to identify the effects of organizational commitment (OC) and perceived patient safety culture (PPSC) on patient safety nursing activities (PSNA) among nurses in comprehensive nursing care units. Methods: Participants were 173 nurses working at five general hospitals in Chungcheong area. Data were analyzed using descriptive statistics, 𝑥2 test, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis with SPSS/WIN 23.0 programs. Results: The mean scores of the OC and PPSC were 3.28±0.50 and 3.85±0.35, respectively. The mean score of PSNA was 4.55±0.41, and PSNA was significantly different by the experience of participating in hospital's safety culture campaigns (t=2.70, p=.008). The results of the multiple regression analysis showed that 'patient safety knowledge and attitudes' (β=.27, p=.006) and 'unpunished environment to error' (β=.22, p=.004) as the sub-categories of PPSC were affecting factors on PSNA with an explanatory power of 26.0% (F=6.40, p<.001). Conclusion: The results of this study suggest that in order to promote PSNA among nurses in comprehensive nursing care units, it is necessary to develop a program to enhance patient safety-related knowledge and attitudes. In addition, the hospital's organizational efforts such as operating safety campaigns and creating an unpunished environment to error should be needed.

Assay Error for Improved Pharmacokinetic Modeling and Simulation of Vancomycin (반코마이신의 약물동태학적 모델링과 시뮬레이션의 향상을 위한 분석오차)

  • The purpose of this study was to determine the influence of assay error for improved pharmacokinetic modeling and simulation of vancomycin on the Bayesian and nonlinear least squares regression analysis in 24 Korean gastric cancer patients. Vancomycin 1.0 g was administered intravenously over 1 hr every 12 hr. Three specimens were collected at 72 hr after the first dose from all patients at the following times, at 0.5 hr before regularly scheduled infusion, at 0.5 hr and 2 hr after the end of 1 hr infusion. Serum vancomycin levels were analyzed by fluorescence polarization immunoassay technique with TDX-FLX. The standard deviation (SD) of the assay over its working range had been determined at the serum vancomycin concentrations of 0, 20, 40, 60, 80 and $120{\mu}g/ml$ in quadruplicate. The polynomial equation of vancomycin assay error was found to be SD $({\mu}g/ml)=0.0224+0.0540C+0.00173C^2$ ($R^2=0.935$). There were differences in the influence of weight with vancomycin assay error on pharmacokinetic parameters of vancomycin using the nonlinear least squares regression analysis but there were no differences on the Bayesian analysis. This polynomial equation can be used to improve the precision of fitting of pharmacokinetic models to optimize the process of model simulation both for population and for individualized pharmacokinetic models. The result suggests the improvement of dosage regimens for the better and safer care of patients receiving vancomycin.

Clinical Evaluation of the Accuracy of Electronic Home Blood Pressure Measuring Devices (가정용 전자식 혈압계의 정확도에 대한 임상적 평가)

  • Purpose: This study evaluated the accuracy of electronic devices compared with mercury sphygmomanometer. Of 132 outpatients with electronic devices. 77 who possessed oscillometric cuff devices participated in this study. Method: When the blood pressure was measured, all electronic devices were connected by means of a Y-connector to a mercury sphygmomanometer. Using the simultaneous same arm approach, each comparison was carried out three times at 2-minute intervals. Results: Compared with the mercury sphygmomanometer, the electronic devices underestimated systolic and diastolic blood pressure, respectively by $4.0{\pm}5.8mmHg$ and $2.5{\pm}67mmHg$, which satisfies the standard error range of the American Association for the Advancement of Medical Instrumentation. According to the British Hypertension Society criteria, these differences achieved C grade for both systolic and diastolic pressure. For the graphical analysis, the differences had a tendency to fulfill the permitted error limits in both diastolic and systolic blood pressure. Moreover, the purchase duration was not correlated with the accuracy of electronic devices. Conclusion: These results indicate that difference in blood pressure between electronic devices and mercury sphygmomanometer is within the standard error. Therefore, electronic blood pressure measuring devices may be useful for therapeutic self-management of hypertension.

A Study on the Types and Causes of Medication Errors and Related Drugs - by Analyzing AJNs Medication Error 73 Cases - (임상에서의 투약오류원인과 관련 의약품 분석 - AJN에 기고된 Medication Error 기사의 73사례를 중심으로 -)

  • The purpose of this study were to illustrate the various medication error types and causes and identified to related drugs to provide basic data for preventing nurses' medication error by analysing 73 cases of AJN 'medication Error' column(1993, Oct -2000, Nov). Nurses' types of medication error were classified into 7 types. The most frequent error types are wrong medication$(21.9\%)$ and the wrong dose$(21.9\%)$ together. The others are wrong $time(4.1\%)$, $omission(2.7\%)$, mechanical $error(2.7\%)$, incorrect IV $rate(1.4\%)$. wrong route $administration(1.4\%)$ in order. Nurses' causes of medication error were 9 kinds. The most frequent type is confusing between similar drug shape, color, size, name, injection devices and patient's $name(43.9\%)$ and the others are lack of knowledge about $drugs(26.8\%),\; slips(7.3\%),\; miscalculating\;dose(4.9\%)$, incorrect adjusts $devices(4.9\%)$, difficulty to read or illegible decimal $point(4.9\%),$ $abbreviation(2.4\%)$, fatigue with $overwork(2.4\%)$ and no communication with $patient(2.4\%)$ in order. Related drugs with medication error are as follows. - dose unit(IU. minims. mcg/min. mEq) : Heparin. insulin. synthetic calcitonin, some enzymes and hormones, vitamins, some antibiotics, tuberculin injection. MgSO4 injection. nitroglycerin - similar size, color and shape drug : $0.9\%$ N/S and acetic acid $0.25\%$ for irrigation. premixed 2mg lidocaine sol. and $0.9\%$ N/S, gentamycin 20mg/2mL for children and 80mg/2mL for adult, dextroamphetamine 5mg and 10mg capsule. sedatives chloral hydrate 250mg/5mL and 500mg/5mL - similar name :Aredia(pamidronate disodium) and Adriamycin(doxorubicin), Lamictal (lamotrigine) and Lamisil 250mg. Perception and Experience of Medication Errors in Nurses with tess than One Year Job Experience (신규 간호사의 투약오류 인지 및 경험에 대한 조사 연구)

  • Purpose: This study was carried out to investigate perception and experience of medication errors by nurses. Method: Data collection through a survey was performed using structured questionnaires over the period of September 1 to October 15, 2004. Questionnaire were delivered to 222 nurses from 15 hospitals; thereafter, 205 questionnaires were responded (i.e., 92% response rate). The subject in the study was a nurse who had been working in the hospital for less than one year. Results: The average perception rate was 87.5%. The perception rates of subjects in medication errors from four areas are 62% in wrong dosage form for drug administration, 61.5% in air into an IV set, 63% in crystals in an IV lines, and 83.5% in wrong time. The experience rates of subjects in medication errors from four areas are 85.5% in wrong time, 39.5% in wrong injection site, 34.5% in omission error, and 28% in wrong patient. Conclusion: The average perception rate and experience rates of medication errors were 87.5% and 23.5%, respectively. Education about the Five right in medication and knowledges about drugs would improve the perception of medication errors of nurses whose work experience is less than one year, and prevent them from medication errors.

Type of Statistical Methods and Errors in the Journal of Korean Academy of Fundamentals of Nursing (기본간호학회지 게재 논문의 통계학적 방법 유형과 오류)

  • Purpose: In nursing research, studies using statistical methods are required and have increased. In this study, some statistical methods using in nursing study are summarized and appropriate usage is proposed. Methods: Twenty-five original articles from the Journal of Korean Academy of Fundamentals Nursing were reviewed. Statistical methods used in the Journal of Fundamentals Nursing were classified and common errors were presented. Results: Seventy-six statistical analysis were performed in the 25 studies. Among the articles, 28 cases contained errors. Most errors occurred in linear regression analysis and nonparametric analysis. Conclusion: When the use of statistical method is applied inappropriately, the result bring out a serious error. In order to ensure reliability and validity of study, researchers should recognize clear application and usage of statistical methods.

Accuracy and Satisfaction with IVIC300 (Intravenous infusion controller) (IVIC300(Intravenous infusion controller)의 정확성과 만족도)

  • Purpose: The purpose of this study was to determine the accuracy and satisfaction with IVIC300 (Intravenous infusion controller). Method: The subjects consisted of 90 nurses and 100 patients. Data collected during July 2013 were analyzed using descriptive statistics, t-test, ANOVA, and $X^2$-test (Chi-Square). Result: The error value of IVIC300 was less than that of Dosi-flow. Differences of error values according to variable of fluid infusion (needle's gauge, patient's moving, patient's calls, and nurse's arabitary calls) were not significant. The mean scores for satisfaction with IVIC300 of nurses ($3.67{\pm}.76$), patients ($3.75{\pm}.63$) were above average. Satisfaction with IVIC300 of nurses differed significantly according to age, work unit, and clinical career. Satisfaction with IVIC300 of patients was not significantly different according to general characteristics. Conclusion: These findings indicate that it is proven the accuracy of IVIC300, and is considered individual characteristics in use IVIC300.

A Structural Model on the Nursing Competencies of Nursing Simulation Learners (간호시뮬레이션 학습자의 간호역량에 관한 구조모형)

  • Purpose: The purpose of this study was to test a model of nursing competencies of nursing simulation learners. The conceptual model was based on the theory of Jeffries's simulaton theory. Methods: Data collection was conducted in October 2017 for 310 students from two nursing universities in Kyungbuk area for 20 days. Data analysis methods were covariance structure analysis using SPSS 21.0 and AMOS 22.0 statistical programs. Results: The hypothetical model was a good fit for the data. The model fit indices were comparative fit index=.97, normed fit index=.94, Tucker-Lewis Index=.97, root mean square error of approximation=.44, and standardized root mean square residual=.04. Teacher factors were directly related to simulation design characteristics, and it was confirmed that the curriculum, classroom operation and teaching method of the instructors were important factors. Learner factors were found to have a direct effect on nursing competence, self-confidence, and clinical performance that belong to nursing capacity. In particular, the results of this study indicate that the simulation design characteristics have a partial mediating effect on learner factors and clinical performance, and a complete mediating effect on learner factors and clinical judgment ability. Conclusion: In order to improve the learner's clinical performance and clinical judgment ability, it is necessary to conduct practical training through nursing simulation besides preparing the learner and the educator.

Students' college life adaptation experiences in the accelerated second-degree bachelor of science in nursing program in South Korea

  • Purpose: South Korea has expanded its accelerated second-degree Bachelor of Science in Nursing program to resolve its supply-demand imbalance in nursing, although how these students are adapting to and performing in college after admission remains unclear. Therefore, this study aimed to conduct an in-depth exploration of how Korean accelerated second-degree Bachelor of Science nursing students interact with society and adapt to college life for establishing further supportive educational policies and programs. Methods: Participants comprised 20 South Korean accelerated second-degree Bachelor of Science in Nursing students. The data were collected via online or offline in-depth interviews and analyzed using Strauss and Corbin's grounded theory and by applying a constant comparative method to achieve theoretical saturation. Results: The core category, derived using open, axial, and selective coding, was "process of the uncharted journey toward a new dream." The participants' college adaptation processes were found to involve three phases: trial and error, adjustment, and acceptance and integration. Conclusion: These findings can be used to develop, apply, test, and improve various support programs for accelerated second-degree Bachelor of Science in Nursing students and to advance the nursing profession by nurturing a workforce with diverse academic backgrounds and faster college life adaptation.

A structural model of nursing students' performing communication skills (간호대학생의 의사소통기술 수행 구조모형)

  • Purpose: The purpose of this study was to construct and test a structural model of nursing students' performing communication skills. Methods: The data collection was conducted from October 13 to October 20, 2020. The participants were 286 students from nursing colleges located in three cities. The data analysis method was a covariance structure analysis with using IBM SPSS statistics version 23.0 and AMOS 21.0 statistical programs. Results: The hypothetical model showed a proper fit with the data: root mean square error of approximation=.08, standardized root mean square residual=.06, adjusted goodness of fit=.85, normed fit index=.91, and comparative fit index=.94. The model fit indices were normed to fit index=2.96. Statistically significant explanatory variables for the performing communication skills of nursing students were peer support, emotional intelligence, ethical sensitivity, and communication self-efficacy. The variables accounted for 66.1% of the performing communication skills of nursing students. Conclusion: Based on the above results, it appears necessary to develop strategies for improving the performing communication skills of nursing students, and having positive effects on health outcomes of the subjects by considering the variables of peer support, emotional intelligence, ethical sensitivity, and communication self-efficacy. Such strategies could potentially have positive effects on the health outcomes of the patients.
