성별에 따른 대학생의 건강관련 삶의 질 차이 : 건강행위 중심으로 (The Differences of Quality of Life by Gender in Undergraduate : focused on health practice)

  • 대학생의 건강행위를 중심으로 건강관련 삶의 질 및 관련요인에 있어 성별에 따른 차이를 알아보고자 본 연구를 시도하였다. 삶의 질 측정도구로는 문화적 배경을 반영하여 개발된 KQOLS(Korean health related Qualtiy of Life Scale)l을 이용하였으며 D지역 남녀대학생 115명을 대상으로 자기기입식 설문조사를 시행하였다. 연구 결과 남성의 삶의 질이 여성에 비해 높았으나 통계적으로 유의한 차이는 없었고, 삶의 질 하부요인인 신체적 기능, 활력, 정신적 건강, 영적 건강은 남성이 여성에 비해 통계적으로 유의하게 높게 나타났다. 성별에 따른 삶의 질 영향요인 차이 분석에서는 남성의 경우 삶의 질 영향요인으로 질병유무(t=-2.118, p<.05), 규칙적 운동(t=2.346, p<.05), 정상 비만도(t=2.274, p<.05)가 포함되었으며, 이들 변수는 남자 대학생의 삶의 질을 약 25.1% 설명하였다. 반면에 여성에서는 건강행위가 삶의 질을 설명하지 못하는 것으로 나타났다.

보건소 중심 호스피스 운영모델 개발 - 부산지역 일개 보건소 시범사업을 중심으로 - (Development of Community Health Center-Based Hospice Management Model: Pilot Project at a Community Health Center in Busan)

  • 목적: 호스피스 서비스의 원칙, 일선 보건소의 특수성과 지역사회 가용자원을 고려한 보건소 중심 호스피스 운영모델을 개발하기 위하여 이루어졌다. 방법: 호스피스 관련 선행연구, 문헌고찰, 관할지역 내 호스피스 실태조사 및 시범운영 평가를 통해 보건소 중심 호스피스 운영모델을 개발하는 연구이다. 2008년 1월부터 12월까지 부산광역시 1개 보건소와 부산지역 말기암환자 의료기관 및 호스피스를 전공하는 간호대학이 연구팀을 구성하여 호스피스 시범사업 운영체계 확립, 호스피스 서비스 전달체계 구성 및 제공 그리고 시범운영 평가를 통한 보건소 중심 호스피스 운영모델 개발의 3단계 추진과정을 거쳐 이루어졌다. 결과: '보건소 중심 호스피스 운영모델'은 보건소의 특수성과 해당 지역사회가 가지고 있는 자원간의 연계를 통한 총체적 서비스 제공이다. 지역암센터는 관할지역 보건소에 재정적, 행정적인 부분을 지원해 주고, 보건소는 호스피스사업 수행을 지원할 수 있는 협력대학에 사업을 위탁하여 전체 사업운영에 대한 기획을 위임하였다. 또한 사업지원단과 사업자문단을 통하여 호스피스 운영과 관련된 제반문제를 지원받는 체계를 구성하였다. 방문간호 팀으로부터 재가 말기암환자를 의뢰받은 호스피스 담당간호사는 환자를 등록시키고 초기사정을 거친 후 호스피스 팀 회의를 거쳐 서비스 우선순위를 정한 다음, 필요한 서비스와 함께 자원봉사 파견을 통한 총체적 서비스를 제공하였다. 이러한 운영모델은 재가 암환자를 중심으로 한 보건소 중심 호스피스사업을 실시할 수 있는 가능성을 제시한다. 결론: 보건소가 가지고 있는 지역사회 가용자원을 최대한 활용하는 '보건소 중심 호스피스 운영모델'은 의료시각지대에 있는 재가 암환자와 가족의 삶의 질 증진을 통해 지역 보건복지 정책의 질적 향상을 유도하게 될 것이다.

병원종사자들의 관계갈등 및 스트레스 요인이 이직의향에 미치는 영향 (Effect of Relationship Conflict and Stress Factors on the Current Turnover Intention in Hospital Employees)

  • 본 연구는 병원종사자들의 이직의도 결정시 가장 큰 원인이 될 수 있는 관계갈등 종류를 살펴보고, 이직의도를 완화시킬 수 있는 관계갈등 요인과 스트레스 요인을 파악하여 병원조직의 안정적인 인력관리를 위한 유용한 정보제공을 목적으로 실시하였다. 연구방법은 병원 및 종합병원에 근무하고 있는 행정직과 간호직 종사자들을 대상으로 2019년 9월 16일부터 9월 30일까지 구조화된 설문자료를 수집하여, t-test, 분산분석, 카이제곱 검정, 회귀분석을 실시하였다. 분석결과, 이직결정시 가장 큰 원인이 될 수 있는 갈등종류는 병원에서 얻을 것이 없는 경우에 이직할 것이라는 응답이 과반수이상이었다. 행정직은 상사나 선임자의 자기중심적 언행으로 인한 관계갈등이 높을수록 그리고 불명확한 업무지시로 인한 스트레스가 높을수록 현재 이직의향이 높았으며, 간호직은 상사나 선임자의 자기중심적 언행으로 인한 관계갈등이 높을수록 그리고 무시하는 행동 및 불쾌한 언행으로 인한 스트레스가 높을수록 현재 이직의향이 높은 것으로 확인되었다. 따라서 병원조직이 안정적인 인력관리를 위해서는 조직차원에서의 조직문화와 부서차원에서의 조직문화가 조직구성원들이 직무현장에서 실질적으로 체감할 수 있도록 검토할 필요성이 제기되었다.

리더십유형이 병원 조직구성원의 직무만족, 조직몰입 및 이직의도에 미치는 영향 (Influences of Type of Leadership on Hospital Organizational Members' Job Satisfaction, Organizational commitment, and Turnover Intentions)

  • It is important how the leader leads the organizational members with leadership appropriate to environment changes as the leader has great influence on the management of hospital organization. These data were collected from the questionnaire surveyed, from February 14 through February 28, 2003, to 218 people among those in the field of doctorship, management administration, nursing, technology, medical engineering, management engineering, and general affairs, working for three hospitals under S medical foundation in Pusan. The principal findings of this research are as below : 1. Type of leadership and job satisfaction had positive correlation in terms of both transformational and transactional leadership, with former having more relation than the latter. 2. Type of leadership and organizational commitment had positive correlation in terms of intellectual stimulus and situational rewards, having little relation statistically. 3. Type of leadership and turnover intentions had negative correlation only in individual consideration, situational rewards, and exceptional management, showing that transactional leadership had more relation than transformational leadership. 4. In terms of leadership and its effect on the organizational performance and turnover intentions, transformational leadership had more correlation than transactional leadership. But transactional leadership also turned out to have effect on organizational performance. When transformational leadership used, turnover intentions were lower than when transactional leadership used. However, both transformational and transactional leadership had negative correlation to turnover intentions. 5. Effect that type of leadership according to job classification, status, age, and academic career has on organizational performance and turnover intentions was differed by those factors; especially, in the occupation of doctor, both transformational and transactional leadership had positive correlation to turnover intentions. 6. Regression analysis on the factors of organizational performance and turnover intentions showed that higher age had positive correlation to the organizational performance in terms of socio-demographical features and that the more leadership was used for charismatic and situational rewards and the less leadership was used for intellectual stimuli, the higher organizational performance was likely. In short, hospital operators and managers should examine each manager's qualification, type, and style of leadership in the hospital organization, and they must develop lasting programs for effective education and training for developing characteristics of leadership are in needed to have managers with appropriate managerial mind to respective post and status. It should be kept in mind that manager's leadership especially has great effect on members' goal-setting, goal-achievement, organizational performance, and turnover intentions as it is a decisive factor of organizational members' attitude and motivation.

암생존자의 암검진 미수검 관련 요인분석: 국민건강영양조사(2007-2012년) 자료 이용 (The Factors Related to the Non-Practice of Cancer Screening in Cancer Survivors: Based on the 2007-2012 Korean National Health and Nutrition Examination Survey)

  • Background: The aim of the current study was to investigate the factors related to the non-practice of cancer screening in cancer survivors, who are at high risk of developing second cancers. Methods: This study is a cross-sectional analysis of 1,125 cancer survivors ${\geq}19$ years old who participated in the Korean National Health and Nutrition Examination Surveys IV and V (2007-2012). A Rao-scott chi-square test and a survey logistic regression analysis were employed respectively to analyze the difference of cancer survivors in cancer screening by each characteristic and the factors related to the non-practice of cancer screening in cancer survivors. Results: Among total subjects, 33.5% did not participate in cancer screening in the last two years. Results from a fully adjusted logistic model showed that the non-practice of cancer screening in cancer survivors was significantly associated with variables such as sex, age, marital status, education level, monthly income, and drinking a alcoholic beverage Specifically, the odds ratio of non-practice of cancer screening was higher in males than in females, in the younger group than in older group, in the group with no spouse than in the group with a spouse; in a group with a low level of education than in a group with a high level of education; in a group with the lowest income level than in a group with the other levels of income; or in non-drinkers than in drinkers. Conclusion: Health policies to reduce the non-practice rate of cancer screening in cancer survivors should be designed and implemented with close attention to cancer survivors' socio-economic characteristics such as sex, age, marital status, education, and income, along with a health behavioral characteristic as drinking.

대학생들의 긍정심리자본과 교수지지가 진로준비행동에 미치는 영향 (The Effects of Positive Psychological Capital and Professor Support on Career Preparation Behavior in University Students)

  • 본 연구 목적은 대학생들의 긍정심리자본, 교수지지 및 진로준비행동과의 관련성을 확인하고, 진로준비행동에 영향을 미치는 요인을 파악하고자 하였다. 본 연구는 2020년 8월 30일부터 10월 30일까지 편의 표집한 대학생 180명을 대상으로 하였으며, 자료는 구조화된 설문지를 이용하여 수집하였다. 자료분석은 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, Scheffé test, Pearson's Correlation Coefficients, 다중회귀분석을 시행하였다. 연구 결과는 다음과 같다. 긍정심리자본과 교수지지는 성별, 진로결정, 경제수준에 따라 유의한 차이를 보였다. 진로준비행동은 성별(t=3.52, p<.001), 전공(F=10.85, p<.001), 진로결정(t=4.98, p<.001)에 따라 유의한 차이를 보였다. 긍정심리자본(r=.45, p<.001)과 교수지지(r=.40, p<.001)는 진로준비행동과 양의 상관관계를 보였다. 진로준비행동에 영향을 미치는 요인으로는 전공, 진로결정유무, 긍정심리자본으로 나타났으며, 이들 요인에 의한 설명력은 32.5%였다. 이러한 결과를 근거로 전공별로 대학생들의 긍정심리자본을 높이기 위한 효과적인 진로 및 상담지도를 계획하여야 한다. 또한, 대학생 시기에 자신에게 맞는 진로나 직업을 탐색하고 목표를 세우기 위해 다양한 진로탐색의 기회를 제공하고, 긍정적인 동기부여를 할 수 있는 환경을 제공하여야 한다.

국군간호사관학교 교육과정 개선을 위한 기초 연구 (A Study on Curriculum Improvement of the Korea Army Nursing Academy)

  • 1. Need for and Purpose of the Study. There is an increasing demand for curriculum improvement of the Korean Army Nursing Academy (KANA), since it was upgraded into 4-year institution of higher learning from 3-year one. In particular, it is strongly advocated that the KANA needs the outside expertise for its curriculum improvement-namely not only from the internal military view of points but also from the viewpoints of professional educational society, In line with such a necessity for the study, this study was aimed at 1) analyzing the current actual practices of KANA'S curriculum, 2) investigating the desired practices of KANA'S curriculum, and 3) identifying the discrepancy between the actual and desired practices of curriculum. 2. Problems for the Study This study had 4 problems to be answeared as follows: 1) What are the actual curriculum practices of KANA? 2) What are the desired curriculum practices of KANA? 3) How are the extents of perception of actual and desired curriculum different in four groups (student, faculty & administrator, nurse, and medical doctor in militay hospital) ? 4) What are the restraining forces that impede the change from actual to desired curriculum practices? 5) What are the relationships of curriculum component,』 in actual and desired curriculum practices? 3. Methods and Procedures This study was conducted by means of document analysis in addition to literature review and by means of needs assessment questionnaire which was developed by the researcher. The questionnaire included 62 statments with 7 questions for demographic data collection. The needs assessment questionnaire was managed to a total of 243 subjects (100 students, 46 faculty & administrators, 55 nurses, and 42 medical doctors), The collected data were treated using SPSS computer system so as to calculate mean scores, standard deviations, and correlation coefficients. The significance test was made through t-test and one-way ANOVA. The statistical significance level was set at both .05 and .01 level. 4. Major findings The major findings in this study are as follows: 1) The score of desired practices was significantly greater than that of actual practices, representing a strong need for curriculum betterment. 2) There were significant differences in the perceptions of actual practices as well as desired practices among four groups (student, faculty & administrater, nurse, and medical doctor). 3) The most frequently selected restraining forces were army's inherent character, economical limitation, and educational expertise limitations. 4) Such variables as sex, position attachment to the KANA and grade made a statistically significant effect on the perception of desired curriculum practice, while the variables like marrige, position, and military class made it on the perception of actual curriculum practice. 5) The coefficients among the curriculum components were lower in perception of the actual curriculum practices than those in the desired practices. 5. Conclusions The conclusions based on the major findings of this study are as follows: 1) The current curriculum development procedure of the KANA is not consistent with the theoretical frame of systematic development sarategy of curriculum. 2) There are wide conflicts among the groups who are supposed to participate in curriculnm development, concerning the actual and desired practices of KANN'S curriculum. 3) A great deal of need for curriculum improvement for the KANA is clearly felt, and in particular, in the process of teaching and learning. 4) Each component of curriculum is not intergrated into a whole development procedure, being segregated each other. 5) For better curriculum improvement, such restraining forces as financial and professional limitations should be eliminated. 6. Recommendations 1) For Further Research a. There is a need to replicate this study after in-depth statistical analysis of each item of need assessment questionnaire, and with more representative subjects. b. A study should be conducted which. has its focus on the analysis of restraining forces for the change from actual to desired curriculum practices of the KANA. 2) For KANA'S Curriculum Improvement a. There is a need to promote the professional expertise of the participants in curriculum development and the communication among them. b. It is desirable to establish an institution or section of administration, which is soley in charge of curriculum development. c. To better develop KANA's curriculum not only faculty and administrators but also students should be encouraged to participate in development process, while the military medical doctors' participation should be carefully considered.

지방의료원의 재정 및 운영효율성에 영향을 미치는 요인 (An Analysis of Factors Affecting Financial and Operating Efficiency at Regional Public Hospital)

  • Background: Financial efficiency in monetary units and operational efficiency in non-monetary units are separately classified and evaluated. This is done to prevent the duplication of monetary units and non-monetary units in inputs and outputs. In addition, analyses are conducted to determine the factors that affect each aspect of efficiency. To prevent duplication of monetary and non-monetary units in inputs and outputs, financial efficiency, consisting of monetary units, and operational efficiency, comprising non-monetary units, are separately classified and evaluated. Furthermore, an analysis is conducted to identify the factors that affect each aspect of efficiency. Methods: This study conducted a panel analysis of 34 regional public hospitals and influencing factors on efficiency for 5 years from 2015 to 2019. Financial efficiency and operational efficiency were calculated through data envelopment analysis. Moreover, multiple regression analysis was conducted to identify the factors that influence both financial efficiency and operational efficiency. Results: The factors that affect financial efficiency include the number of medical institutions within the treatment area and the ratio of patients receiving medical care. Additionally, operational efficiency is influenced by the type of medical institution, the number of medical institutions within the treatment area, and the number of nursing positions per 100 beds. Conclusion: In order for regional public hospitals to faithfully fulfill their functions and roles as regional base public hospitals, several measures are necessary. Firstly, continuous monitoring and reasonable support are required to ensure efficient operation and performance. Secondly, a financial support plan tailored to the characteristics of local medical centers is needed. Additionally, local medical centers should strive to enhance their own efficiency.

수술실간호사의 직무수행과 피로에 관한 연구 (A Study On the Operating Room Nurses' Performances of Duties and Their Fatigue)

  • The aim of this study is to present the basic datum for the promotion of effectiveness and improvement of nursery tasks, by grasping the situation of operating room nurses tasks and the degree of their fatigue. For these researches, 70 nurses out of the operating rooms of 3 university-affiliated hospitals-two in Seoul and one in Pusan, were chosen at random by a non-probability sampling method. These researches were done from April 14 to April 26 in 1997 by questionairing method. The questionaire was composed of 30 items, which asks the examinees of their physical, mental, and neurosensory symptoms, with 10 items respectively. The reliability of the research instrument was turned out very high with Cronbach's ${\alpha}=.9376$. The datum were electronically processed using Statistics Program for Social Sciences(SPSS). The analysis of datum in this study has a general character, in which the demosociological character and the special ex-officio character was calculated by frequency and percentage. The situation of tasks in operating rooms was calculated by frequency and percentage. The fatigue of operating room nurses was calculated by average and standard deviation. To compare the fatigue with regard to the character of operating room nurse's tasks, t-test and F-test(ANOVA) were used after the character of variations, and the variations at the level of P<.05 which might have some meaning was verified after the fact with Duncan's Multiple Range(DMR). The results of this research are as follows : 1) The nurses working in operating rooms show their fatigue in three fields-in Group I physical symptoms 3.28, in Group III neurosensory symptoms 2.85, and in Group II mental symptoms 2.73, which shows I Dominant type (general type). 2) They complain, in Group I they are "feeling the heavy legs," 3.28 and in Group II they "occasionally forget soon what to do," 3.09. and in Group III, they "feel lumbago," 3.47, which is the highest rate of the three. The highest rate results from the character of their tasks, in which they have to move rapidly the heavy appliances and do their jobs standing many hours, especially wearing heavy radiation protector. 3) As to transportation, subway using group feel the greatest fatigue, 3.18(F=4.315, P=.008). 4) As to department, nurses in the orthopedic's surgery part feel the greatest fatigue, 3.26(F=2.040, P=.050). 5) As to the change of physical symptoms, the group answering that they found physical abnormality after working in operating rooms show the greatest fatigue, 3.12(t=-3.13, P=.003). 6) At to the general circumstances, the group answering that they receive insufficient consideration on their physical abnormality in their department, show the greatest fatigue, 3.10(F=3.200, P=0.47). 7) As to the relation with superior officers, the group answering that their superior officer has an impetuous temperament, show great fatigue, 3.11(F=4.855, P=.011). 8) As to the time of feeling fatigue, the fatigue reaches the highest point 1~2 hours after operations, 3.04(F=2.703, P=.046). 9) When they feel fatigue after scrub nurse duties, they feel the greatest fatigue 2 hours after the duties, 3.09( F=2.841, P=.038). 10) As to the operation instruments, when they use complex instruments borrowing from the outside in addition to the basic instruments, their fatigue becomes the greatest, 3.09(F=7.831, P=.000). 11) As to the kind of operation, when they participate in orthopedic's surgery operations, they feel the greatest fatigue, 3.18(F=4.362, P=.000). With the above results, it is proved that the degree of operating room nurses' fatigue is considerably high. So it may be concluded that the measure for lessening the fatigue should be find immediately, not on the level of personal matters but on the level of hospital nursing administration.

남자대학생의 데이트폭력 인식이 데이트폭력 가해행동 및 피해행동에 미치는 영향 (The Effects on Harassment Behavior and Damage Behavior of Perception of Dating Violence of Male University Students)

  • 본 연구의 목적은 남자대학생의 데이트폭력 인식이 데이트폭력 가해행동 및 피해행동에 미치는 영향을 분석하는 것이다. 연구대상은 충북에 위치한 U대학의 6개 학과의 재학생은 총 233명이었다. 연구도구로 데이트폭력 인식은 정하윤(2013)의 도구를 사용하였고, 데이트폭력 가해행동 및 피해행동은 CTS2를 김정란(1999)이 번안한 도구를 적용하였다. 자료분석은 데이트폭력 인식이 데이트폭력 가해행동 및 피해행동 간의 상관분석을 실시하였고, 데이트폭력 인식이 데이트 폭력 가해행동 및 피해행동에 미치는 영향은 회귀분석을 실시하였다. 연구결과, 첫째, 남자대학생의 데이트폭력 인식과 데이트폭력 가해행동 및 피해행동 간에 유의한 부적 상관을 보이는 것으로 나타났다. 둘째, 남자 대학생의 데이트폭력 인식이 데이트폭력 가해행동 및 피해행동에 유의한 부적 영향을 미치는 것으로 나타났다. 본 연구결과는 대학생을 위한 데이트폭력 예방프로그램을 개발하는데 기초자료로 제공될 수 있을 것이다.
