동종 조혈모세포 이식 전후 영양상태 평가 (Pre- and Post-Transplant Nutritional Assessment in Patients Undergoing Allogeneic Hematopoietic Stem Cell Transplantation)

  • Purpose: This study was performed to identify the pre-and post-transplant nutritional assessment for patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). Methods: The subjects of this study were 25 patients undergoing allogeneic HSCT. The data collection was performed from January 31st to March 31st, 2011. The Patient-Generated Subjective Global Assessment (PG-SGA), anthropometrics and biochemical test were collected from the time they entered the isolation unit until they left. Results: Pre-transplant nutritional assessment status indicated moderate malnutrition which scored $7.32{\pm}1.68$ in PG-SGA. There were 22 patients (88.0%) with moderate malnutrition and 3 patients (12.0%) with severe malnutrition. Post-transplant nutritional assessment indicated severe malnutrition status which scored $11.92{\pm}3.26$ in PG-SGA. Pre-and post-transplant nutritional assessment displayed significant differences (p<.001) in PG-SGA score. Hematopoietic stem cell transplantation led to a deterioration of patients' nutritional status. Pre-transplant patients were already in malnutrition status and patients undergoing allogeneic HSCT were at risk for malnutrition. Conclusion: Pre-and post-transplant patients were categorized as having undernutritional and malnutritional status. Pre-transplant nutrition status impacted on post-transplant nutritional status. Health care personnel should pay attention to patient's nutrition status when undergoing allogeneic HSCT with appropriate nutritional assessment tools.

  • Purpose: The purpose of this study was to determine the needs for children's health promotion education programs as perceived by child care center teachers and mothers. Methods: This study conducted a survey of 88 child care center teachers and 70 mothers of preschool children in Seoul from January 4 to February 5, 2016. This study aimed to characterize the current conditions of health promotion education for preschool children and the needs for health promotion education as perceived by child care center teachers and mothers of preschool children. Results: Areas of high need for health promotion education included lifestyle improvements for preventing diseases in children and awareness of the importance of health in educational objectives, standardized educational manuals, health educators as educators, child care centers as educational places, local health centers as educational support organizations, regular class hours as educational time, role play for training, and actual models in the educational medium. The educational subjects for which a high need was reported included safety and accident prevention, the role of smartphones and TV watching in mental health, and personal hygiene and disease prevention. Conclusion: These findings suggest that it is necessary to develop a health promotion education program for preschool children.

목회간호사의 업무활동분석 (Analysis of Church based parish nursing activities in Teagu city)

  • The concept of parish nursing began in the late 1960s in the United States when increasing numbers of churches employed registered nurses (RNs) to provide holistic, preventive health care to the members of their congregations. Parish nursing role was developed in 1983 by Lutheran chaplain Granger Westberg, and provides care to a variety of church congregation of various denominations. The parish nurse functions as health educator, counselor, group facilitator, client advocate, and liaison to community resources. Since these activities are complementary to the population-focused practice of community health' CNSs, parish nurses either have a strong public health background or work directly with both baccalaureate-prepared public health nurses and CNSs. In a Midwest community in U.S.A., the Healthy People 2000(1991) objectives are being addressed in health ministries through a coalition between public health nurses and parish nurses. Parish nursing is in the beginning state in Korea and up untill now, there has been no research was conducted on concrete role of korean parish nurses. The main purpose of this study was to identify, classify and analyze activities of parish nurses. The other important objective of this study was to establish an effective approach and direction for parish nursing and provide a database for korean parish nursing model through analysis and' classification of the content of the nursing record which included nursing activities. This study was a descriptive survey research. The parish nurses were working in churches where the demonstration project developed on parish nursing. The study was done on all nursing records which were working in churches where the demonstration project developed on parish nursing. The study was done on all nursing records which were documented by parish nurses in three churches from March, 1995 to February, 1996. Namsan, Taegu Jeei and Nedang presbyterian churches in Taegu and Keimyung nursing college incooperated together for the parish nursing demonstration project. The data analysis procedure was as follows: First, a record analysis tool was developed and second, the data was collected, coded and analyzed, the classification for nursing activities was developed through a literature review, from which the basic analysis tool was produced and cotent validity review was also done. The classification of the activities of parish nurses showed 7 activitity categories. 7 activity categories consisted of visitation nursing, health check-ups, health education, referring, attending staff meetings, attending inservices and seminar, volunteers coordinating. The percentage of activities were as follows: Visitation nursing(A: 51.6%, B: 55%, C: 42.6%) Health check-ups(A: 13.5%, B: 12.1%, C: 22.3%) Health education(A: 13.5%, B: 13.2%, C: 18.2%) Referring(A: 1.4%, B: 4.2%, C: 2.4%) Attending staff meeting(A: 18.8%, B: 13.0%, C: 12.2%) Attending inservices and seminar(A: 1.5%, B: 2.2%, C: 2.1%) Volunteers coordinating(A: 0.3%, B: 0.4%, C: 0.0%) To establish and develope parish nursing delivery network in Korea, parish nurses role, activities and boundaries of practice should be continuously monitored and refined every 2 years. Also, It is needed to develope effective nursing recording system based on the need assessment research data of various congregation members. role, activities and boundaries of practice and arrangement of the working structure, continuing education, cooperation with community resources and structuring and organizing parish nursing delivery network. 학교 보건 실습 경험에 관한 현상학적 연구 (The Phenomenological Study of School health practice experience of Nursing Students)

  • This study was conducted to have better understanding of the students' experience in field practice by school health practice experience. The study subjects were 40 nursing students working in department of nursing K Univ. in C city. This study was approached by phenomenological method. Collected data were analyzed by Colaizzi's method. The results were from the protocol 980 significant statements and organized into 240 formulated meanings. From formulated meaning 89 themes were identified, organized into 18 them clusters, and then into 16 categories. The nursing students took part in the practice with (expectation and readiness) different from those of the clinical practice, expressed wonder at the school which had progressed much more than their primary schools used to be. They said that they began to feel (Fatigue and stress), and that experienced tension for the lack of nursing knowledge and skill during the health education and clinical treatment activity. In addition, they experienced 'ambivalence of satisfaction and something wanted', that is to say, they could have done better by means of video education and health education. The 10-day-school health practice brought about the change in( the image of teacher) and (cognition about the nursing teacher's role), made the students have(love to the client) and (desire to be nursing teacher), and then turned out experience benefical enough to be expressed 'satisfaction' However, they pointed out many problems in (School Eniviroment), (Clinic), (Physical assessment), (Recording and reporting), so they had a chance to apply the school nursing process to the school. The professor should play the role of promoting the learning through the field practice and providing the stimulant of learning to help the learner get as much from the field situation as they could. Therefore, I suggest that the students always have a chance to exchange actual affairs and educational study, and that the concrete discussion and continuous cooperation be done. The professors should keep doing their best to find the way to professors should keep doing their best to find the way to promote the ability of thinking through the process the learners experienced themselves.

일개 지역사회 중심 가정간호시범사업소의 성과평가 연구 (A Study for Evaluating the Performance of a Community-based Home Care Services Model)

  • Purpose: This study was aimed to evaluate the performance of a community-based home care services model. Method: The subjects were 138 patients who have been enrolled during a 6 month's period, nursing records and nursing service bills. The data was collected by self report and chart review. The mean cost per visit was compared with those of hospitalization & clinic visit in the data from National Health Insurance Corporation. Result: A significant number of patients were bed ridden(63.8%) and unconscious (27.5%), and most of the patients had complex chronic diseases. Except nursing assessment, bed sore care was the most frequent nursing treatment(25.1%) in home care services. The mean cost per visit of home care services was 34,665 won, which was lower than those of hospitalization & clinic visit for medical aid. The patients were highly satisfied with the services by visiting nurse specialists. Conclusion: Community-based home care services provided cost-effective and satisfied services. Community-based home care services needs systematic supports to expand it's domain for promoting community health.

아동간호학 임상실습교육 현황 (An Analysis on Clinical Education of Pediatric Nursing)

  • This study was conducted to analyse the current clinical education of pediatric nursing in baccalaurate nursing program, then to give basic data for enhancing the quality of future clinical education of pediatric nursing. Data were collected through self-reported questionnaire by mail from December 2001 to February 2002. The subjects were 29 schools of 50 baccalaurate nursing education programs. The data were analysed by double raters, researcher and assistant researcher. The results were summarized as follows: 1. Twenty-eight schools had the objectives of the clinical education of pediatric nursing, and 28 schools in pediatric ward, 23 schools in nursery, 22 schools in neonatal intensive care unit(NICU), 15 schools in objectives related to profession by clinical site. 2. Credits on clinical education of pediatric nursing were most 15 schools of 3 credits. 3. The clinical sites were mainly the hospital that sick children were admitted in. 4. The clinical teacher were 9 types including pediatric professor and field nurse. 5. On teacher's role, the professor instructed the case study and conference, and field nurse instructed the patient assignment and nursing procedures. 6. All of schools used explanation and conference as a method of clinical education, 1 or 2 schools used PBL or role play or field study. 7. On clinical education content, most of school included Apgar scoring system, physical examination in newborn assessment, respira- tion maintenance, temperature maintenance, infection prevention, nutrition, and bath in newborn care. 8. On clinical education content, most of school included care of incubator, phototheraty, infusion, gavage feeding and how to use the instruments in NICU. Eighteen schools included attachment promotion, and 20 schools case study. 9. On clinical education content, most of school included a checklist of nursing procedures, case study, assessment of growth and development in pediatric ward and other sites. 10.There were various evaluation types in scores, measuring items. In conclusion, the results of this study revealed that there were some discrepancy in the objectives and contents, clinical sites on hospital focused, teacher's role, and diversity of measurement items and ratings in clinical education of pediatric nursing. There is a need for a standardization of content, clinical site, and evaluation tool to improve a quality of clinical education of pediatric nursing based on this study.

Effects of a Self-Care Reinforcement Program for Socially Vulnerable Elderly Women with Metabolic Syndrome in Korea

  • Purpose: This study evaluates the efficacy of a Self-Care Reinforcement Program (SCRP) based on the Selection Optimization Compensation (SOC) model, in socially vulnerable elderly women with metabolic syndrome. Methods: This study adopts a pretest-posttest nonequivalent control group design. The participants were 64 socially vulnerable elderly Korean women with metabolic syndrome (experimental group: 31, control group: 33). Participants' body composition analysis, nutrient intake, risk factors of metabolic syndrome, depressive symptoms, and social network were measured. Data were analyzed with an independent t-test; statistical significance levels were set at p<.05. The SCRP, including metabolic syndrome education, nutritional education, exercise, and social network, was performed three times a week for 8 weeks. Results: There were statistically significant differences between the experimental and control groups in terms of systolic blood pressure, diastolic pressure, fasting blood sugar, triglycerides, sodium intake, depressive symptoms, and social networks. Conclusion: The SCRP is effective and can be recommended as a community health nursing intervention for socially vulnerable elderly women with metabolic syndrome.

Psychometric validation of the Child Healthy Lifestyle Profile in South Korea: a cross-sectional study

  • Purpose: This study aimed to validate the Child Healthy Lifestyle Profile as an instrument for screening healthy behaviors in school-aged children in South Korea. Methods: Self-reported questionnaires were administered to 454 students, comprising elementary-school students (n=221) and child cancer survivors (n=233). Reliability and validity were assessed using Cronbach's α, exploratory factor analysis (EFA), and confirmatory factor analysis (CFA). Results: Cronbach's α, as a reliability test, was 0.87. Varimax rotation yielded nine factors with eigenvalues greater than 1 in the EFA, which explained 61.0% of the total variance. In the CFA, both convergent and discriminant validities were acceptable. Therefore, the Child Healthy Lifestyle Profile was validated as an assessment tool for Korean school-aged children. Conclusion: Based on the results of this study, the Child Healthy Lifestyle Profile was identified as a reliable and valid instrument for assessing healthy lifestyles in elementary-school children in South Korea.

영적 간호가 말기 암환자의 외로움과 영적 안녕에 미치는 효과 (Effects of Spiritual Nursing Care on Loneliness and Spiritual Well-Being of Terminal Cancer Patients)

  • Purpose: This study utilized a non-equivalent control group pre-post design to assess the effects of spiritual nursing care on loneliness and spiritual well-being of terminal cancer patients. Method: Forty-one terminal cancer patients in a general hospital, were divided into an experimental group of 20 patients and a control group of 21 patients. Those in the experimental group received four weeks of treatment three times a week for about 45 minutes each session. The treatment included nurses' spiritual care involving five instruments of spiritual nursing intervention, each of which was used according to the six types of spiritual need assessment. Data was analyzed with descriptive statistics including real number, percentage, $X^2$-test, t-test, and ANCOVA. Result: There were significant differences between the experimental and control groups in the level of loneliness and spiritual well-being. Conclusion: Spiritual nursing care was verified as an effective program that can lessen the loneliness and improve the spiritual well-being of patients with terminal cancer.

  • Purpose: The aim of this study was to analyze the needs of HIV/AIDS care and to develop the job description of HIV/AIDS Counselling nurse in Korea. Methods: The needs assessment was done by focus group interview with HIV/AIDS care recipients who are 16 persons living with HIV/AIDS and four their family members, seven persons from HIV/AIDS high risk group, and five nurses working on HIV/AIDS clinics. Based on the result of needs assessment, job description was developed using the DACUM. Mail survey was done to identify the frequency, importance, and difficulty of duties, tasks, and task elements. Results: The job description was classified under 8 duties, 36 tasks, and 290 task elements. Duties were categorized as needs assessment, health promotion and quality of life, improvement of treatment compliance, symptom care, health education, resource network, administrative activity, and career development. The importance of all duties and tasks showed high score from the survey, but the frequency and the performance level were middle range. Conclusion: It is suggested that the special training program based on the job description needs to be developed. In the political aspects, the introduction of HIV/AIDS nurse specialist certification could be considered.
