The Use of Nursing Diagnosis in Practice (간호진단의 임상적용 현황, 문제점 및 개선방안)

  • This study was conducted to investigate the present situation and problems related to the use of nursing diagnosis in practice. The data were obtained from 332 subjects (27 director of nursing service, 302 staff nurses) who worked in university hospitals in Korea from July through August 1988 using a mailed questionnaire. Data were analyzed by frequency, X$^2$ test and t-test. The findings were as follows ; 1, Clinical use of nursing diagnosis by directors of nursing service and staff nurses. 1) The majority of the nursing departments (88.9%) conducted group education on nursing diagnosis during the last 5 years and 81.5% of them kept a record format for nursing diagnosis : 88.9% of them had had prior experience with the nursing diagnosis. 2) Most of nurses (97.0%) had received education on nursing diagnosis. 2. Factors related to the clinical use of nursing diagnosis in nursing service departments and by staff nurses. 1) The one factor related to the use of nursing diagnosis in the nursing service department was the existence of a record. 2) Factors related to the use of nursing diagnosis by the staff nurses were the organization style of the nursing service department, group education during the last 5 years, existence of a record, the attitude of the director of nursing service, and prior experience of the use of the nursing diagnosis as characteristics of nursing service department and educational experience of nursing diagnosis as a character of nurse. 3. Problems with the use of nursing diagnosis. 1) The primary problem was the lack of time and personnel (mean : 3.757) ; the second problem was the lack of knowledge and will to use nursing diagnosis in practice by the staff nurses(mean : 3.546). 2) There was no significant difference in problems expressed by the director of nursing services and the nurses. The majority of nurses who worked in the university hospitals expressed interest in and concern about the use of nursing diagnosis. Most of the nurses had had education about on nursing diagnosis but use in practice was limited. The primary problem was lack of time and manpower. Strategies for improving use of the nursing diagnosis in practice : 1) Strengthening the education about nursing diagnosis and a holistic approach to understanding human beings. 2) Develop protocols for the use of nursing diagnosis. 3) Eliminate the language barrier regarding nursing diagnosis by translation into in Korean. 4) Decentralization of the nursing service to promote accountability by individual nurses for use of nursing diagnosis.

  • Nursing diagnosis classification is needed to define nursing phenomena and set up nursing plans. The purpose of this study is to develope common nursing diagnosis by comparing and analysing nursing diagnosis classification systems and assessment tools in home care. Development of Computerized Program for Nursing diagnosis and Intervention linked to Medical Diagnosis (의학진단과 연계된 간호진단 및 중재 프로그램 개발)

  • Purpose: The actual nursing processes have been performed by individual nurses' judgment without any supporting programs in Korea. It is not easy for novice nurses to make accurate diagnoses and provide proper nursing interventions to patients. Therefore, we propose a computerized program for nursing diagnosis and intervention linked to medical diagnosis. Method: For the program, we have linked standardized nursing diagnosis and intervention classifications with medical diagnosis. It is premised that the program is connected to order communication system(OCS) in hospitals. Result: We provide a nursing information system with standardized database for nursing diagnosis and interventions so that nurses can make more accurate diagnosis and perform more adequate interventions. Conclusion: It is expected that the program will help the nurses perform their nursing processes more efficiently. And we expect the system can be used in many hospitals efficiently in the future after pilot operations are completed in some hospitals.

  • Purpose : to survey about satisfaction of nurses of NANDA nursing diagnosis and NIC nursing interventions and system's usefulness of information system forusing 10 medical diagnosis. Method : nurses learned about this system and used this system for 4 or 8 weeks. After that survey about satisfaction and usefulness of this system. Result : The good points of the nursing diagnosis systems are a rapid selection, accuracy, convenience of the using system. The good points of the nursing intervention system are same as the nursing diagnosis system. About the good points of the general system are easiness, improvement of nursing knowledge, convenience, etc. However, further studies for pilot operations of the system are mandatory. Conclusion : We expect this system can be used in many hospitals efficiently in the future after pilot operations are completed in some hospitals. After verifying the usefulness of the system through pilot operations, the further analysis on the relationship between medical diagnosis and NANDA nursing diagnosis is also necessary for the generalization.

Development of a Conceptual Framework of Nursing from Selected Concepts of Nursing Diagnoses (간호진단 분류체계에 근거한 간호개념틀 개발)

  • For the purpose of integrating nursing diagnosis into the nursing curriculum, a descriptive survey research was done using the inductive method with questionnaires and a literature review. Research subjects included nurse educators, textbooks of adult nursing published in Korea, and the course outline for adult nursing used in one college of nursing. The Results show that there was common agreement on 39 nursing diagnosis which should be in cluded in the adult nursing curriculum, textbooks of adult nursing, and patient care on the medical-surgical units. The two existing nursing diagnosis classification systems(NANDA and Gordon's Human Response Patterns) show different basic frameworks and difficulties were discovered in integration of nursing diagnosis into the curriculum. To develop a conceptual framework for a nursing diagnosis classification system, diagnosis were classified into three categories ; health promotion, high risk problem, and actual problem on the basis of the framework used in adult nursing textbooks and Gordon's 11 Functional Health Patterns. Subconcepts for actual problems were classified as ; activity and rest, nutrition and elimination, perception and coordination, stress and coping. Progress in this study supports further development of a conceptual framework of nursing based on a nursing diagnosis classification system, from which improvement in nursing education and clinical practice can be expected.

  • Purpose: This is the descriptive research project of which purpose is to acquire the practice, research, and educational data by establishing the database after confirming, classifying, and relating the nursing diagnosis, nursing intervention, and nursing outcome of Breast cancer patients by using the Yoo Hyung-sook's(2001) related 3N database model as the tool. Method : The Nursing Data occurring on Breast cancer patients nursing process was mapped to nursing diagnosis of NANDA, nursing interventions of NIC, nursing outcomes of NOC the 3N database linkage database which is related with the nursing process that was developed by using Yoo Hyung-sook's(2001). Result : 1. The nursing diagnosis were totally 505, and 26 articles of the nursing diagnosis were applied among 149 nursing diagnosis classification systems. 2. As for the nursing intervention, 250 articles(5l.4%) of nursing intervention were applied among 486 nursing intervention classification systems. 3. Regarding the nursing outcome, 28 articles(1l.2%l of the nursing outcome were applied among 250 nursing outcome classification systems. Conclusion: The result of this research in which the relating among the nursing diagnosis, nursing intervention, and nursing outcome of Breast cancer patients by using 3N nursing database was established is thought to be applied in the research and practice as well as to be utilized in the lecture or practice of the nursing process.

The Validity of Nursing Diagnosis in Rehabilitation Nursing (재활간호단위에 적용되는 간호진단의 타당도)

  • This study was conducted to investigate the nursing diagnosis applying to rehabilitation unit. The subject of this study was consisted of 101 nurses who have been working over 1 year in rehabilitation unit. The clssification of nursing diagnosis used NANDA's system and analysis of the validity was based on Fehring's model. In summury of this study. some dirrerences were found in related factors in nursing diagnosis between rehabilitation and general unit. Major related factors of nursing diagnosis were physical factors associated with neuromuscular disorder. Valid related factors in altered nutrition more than body requirements was not found for rehabilitation unit. It is helpful for the nurses who work at rehabilitation unit to apply the nursing diagnosis validated in this study. This finding can be used as the database for accomplished nursing diagnosis appropriate for improving the rehabilitation nursing practice.

A Validation Study of Nursing Diagnosis in Emergency Care Unit (응급간호단위에 적용되는 간호진단의 타당도 연구)

  • Purpose: Related factors for 24 nursing diagnoses frequently used in the emergency care unit were validated in this study. Method: A convenience sample of 65 registered nurses who had worked for 2 years or more in emergency care units and received instruction on nursing diagnosis was used for the study. The classification of nursing diagnoses was based on NANDA (1996) and validation, on Fehring (1987)'s DCV model. Result: Differences were found between emergency and general care units for related factors for nursing diagnosis. Newly reported related factors were not found for emergency care units. Conclusion: It is helpful for nurses who work in emergency care to be able to apply the nursing diagnosis validated in this study. These findings can be used as the database to provide a nursing diagnosis system appropriate to improving the emergency nursing practice.

  • The purpose of this study was to develop a standardized Nursing Diagnosis/ Intervention Protocol through a literature review and validity testing. Seven nursing diagnoses were selected as major nursing diagnosis in the field of Nutritional and Eliminative problem. The nursing intervention list was made by an expert group's review of Nursing Intervention Classification(NIC) suggested nursing interventions. Nursing activities which were included in each nursing intervention were sorted to follow the nursing intervention process after review and revision. The expert group's validity testing was done twice using the Likert scale. As a result the Nursing Diagnosis/ Intervention Protocol for Nutritional and Eliminative Problems was made to include 7 Nursing Diagnoses, 51 Nursing Interventions and 631 Nursing Activities.

Validity Testing Study for Related Factors and Characteristics of Nursing Diagnosis (간호진단별 관련요인 및 특성에 관한 타당도 연구)

  • This study was conducted to test validity of related factors and characteristics of 98 Nursing Diagnosis identified in a previous study by the Korean Nurses Association. Data for this study was collected from 892 nurses in eight teaching hospitals located in Seoul using a cross sectional survey method. Each participating hospital was asked to produce at least 10 cases for every nursing diagnosis. There were 7,422 responses out of a possible 7,840. Out of the 7,422 responses 26 were discarded due to incompleteness. Data were analyzed using SAS. The result of the study shows that most of the related factors and characteristics for each of the 98 nursing diagnosis were ranked at more than 3.5 point out of 5 point Likert scale in terms of significance. Through this study the related factors and characteristics of the 98 nursing diagnosis identificance. Through this study the related factors and characteristics of the 98 nursing diagnosis identified through literature review were validated by experts in nursing diagnosis. These validated related factors and characteristics will be utilized for computerization of the nursing diagnosis process.
