요양병원 요양보호사 이직의도와 관련변인 (Factors Influencing the Turnover Intention in Long-Term Care Hospital Care Workers)

  • 본 연구는 요양병원에 근무하는 요양보호사의 이직 및 예측요인을 파악하고자 시도 되었다. 연구대상자는 K시와 B시에 소재하는 3개 요양병원의 150명이었다. 자료수집을 위한 조사는 구조화된 설문지에 의해 수집되었다. 자료분석은 SPSS/WIN 18.0 프로그램을 이용하여 t-test, One way -ANOVA와 multiple regression을 분석하였다. 연구결과 이직과 자기존중감, 직무 만족도 사이에 유의한 음의 상관관계가 있는 것으로 나타났으며, 요양보호사 이직에 영향을 미치는 요인은 조직몰입(${\beta}=.59$, p<.001), 사회적 지지(${\beta}=.32$, p<.001)가 이직의도의 40% 설명하였다. 본 연구에 의하면 요양병원에서 이직율 감소를 고려한 효율적인 교육프로그램 개발이 요구된다.

소규모 사업장 보건관리 모델개발에 관한 연구 (Occupational Health Care Management Model in Small Scale Enterprises)

  • Forming health care management model in small-scale enterprises was the purpose of this study. For the purpose, we tried to investigate the characteristics of small-scale enterprises and analyzed the pattern of their health care management. The results are as follow: 1. The strength of health managing agency and technical supporting program lies in team approach by specialized manpower. However, if the liaison between each part of the organization is not smooth, the overall management will be very difficult. 2. Small scale enterprises are characterized by their short life after the establishment, use of rental building, lack of welfare facilities, weakness in sanitary management and aggregation of factories of similar type of industry. Because of these characteristics, it is very difficult to solve problem basically, such as improvement of working environment. Therefore, it is important to focus on health education and community based approach. 3. Many workers in small-scale factories are in middle and old age. They have health problems mainly related to personal habits. Implementation of an appropriate health promotion program is needed. 4. The number of workplaces, which should be managed by health managing agent. is increasing rapidly. But the number of health managing agent is limited. In the aspect of the requirement of manpower and equipment, training personal agent is more urgent than founding institutional agent. 5. The uniform method of health management hampers the choice of employer and workers. The types of provision of health management should be diversified. 6. For an efficient management, a frequent visit of personal agent and the following referral to a specialist should be done. The specialists in charge of secondary management are from the field of occupational medicine, occupational hygiene, ergonomics, etc. 7. The health management of small-scale facilities should have six components. They are community-based approach, multi-disciplinary cooperative system, program based on the need of recipient, forming partnership of employer and worker, change of lifestyle, and evidence-based program.

ICT 시대에서 노인요양시설의 구강건강 인식에 관한 연구 (Study on The Oral Health Awareness of Long-term Care Facility Among the ICT Age)

  • 본 연구는 서울소재의 요양시설 간호제공자들의 구강건강 인식을 조사하고, 이에 영향을 미치는 요인을 파악하고자 시행되었다. 연구대상은 92명의 간호제공자들을 대상으로 선정하였고, 설문지를 사용하여 자료를 수집하였다. 다중회귀분석결과, 근무경력과 구강건강에 대한 교육경험 유무 요인에서 유의한 영향을 미쳤다. 결론적으로, 요양시설 노인들의 구강건강 향상을 위해서는 이들을 간호하는 간호사나 사회복지사와 같은 간호인력들에게 구강보건교육 프로그램의 홍보 및 교육이 절실히 요구된다.

35세 이상 고령 임산부 진료실적 추이에 관한 연구 (The trend of national health insurance service use among pregnant and postpartum women aged 35 years and older)

  • This study was conducted to examine the trend of national health insurance service use with relation to pregnancy, childbirth, and the puerperium among pregnant and postpartum women older than 35 over the last decade. A descriptive analysis was conducted, using the data which were drawn from the "nationwide claim database of Korean National Health Insurance Corporation(NHIC)". Data were composed of the total cases related to pregnancy, childbirth, and the puerperium (International Classification of Disease, $10^{th}$revision [ICD-10] codes O00-O99) from 2001 to 2008. During 2001-2008, the number of pregnant and postpartum women older than 35 had continuously increased and the percentage of them also had increased in both hospital and ambulatory care. There are similar trends in their total use of national health insurance service and total expenditure. According to demographic characteristics, there was the biggest increase of the percentage in residents in large cities, self-employed workers, ones in the highest income level. According to ICD-10 codes, there was the biggest increase of the percentage in O10-O16 (oedema, proteinuria and hypertensive disorders in pregnancy, childbirth and the puerperium). According to the major prevalent disease, there was the biggest increase of the percentage in O60 (preterm labor and delivery). Throughout the past decade, the necessity has been emphasized of supporting pregnant and postpartum women older than 35. But in maternal and child health care, they are in an early stage of development. The findings of this study would be helpful in developing the support programs for the aged pregnant and postpartum women.

계획된 행동 이론을 적용한 화재안전교육이 요양보호사 교육생들의 화재안전행동에 미치는 효과 (Effect of Fire Safety Education Based on the Theory of Planned Behavior on the Fire Safety Behavior of Care Worker Trainees)

  • 본 연구는 요양보호사 교육생들을 대상으로 화재안전교육의 효과를 규명하기 위한 유사실험연구로 비동등성 대조군 전후시차설계(Non-equivalent control group no-synchronized design)이다. 연구대상자는 S요양보호사 교육원의 교육생으로 실험군 28명, 대조군 29명으로 총 57명 이었다. 실험처치기간은 2018년 5월 21일부터 6월 14일까지 주 1회씩 총 4회로 구성하여 화재안전교육을 실시하였다. 자료분석은 ${\chi}^2-test$, t-test로 분석하였다. 연구결과 화재안전교육은 요양보호사 교육생들의 화재안전에 대한 지식, 화재안전에 대한 태도, 화재안전에 대한 지각된 행동통제, 화재안전 행동의도, 화재안전 행동을 증진시키는데 효과적인 교육으로 일상생활에서 화재안전행동실천에 유용하게 활용될 수 있을 것이라 기대된다. 반면 화재안전에 대한 주관적 규범은 유의한 차이가 없는 것으로 나타나 화재안전에 대한 주관적 규범의 효과를 검증하는 후속연구를 제언한다.

근로자의 산업보건관리에 대한 지식, 태도, 실천 조사연구 -광주, 전남지역- (A Study on the Knowledge, Attitude and Practice about Industrial Health Care Services of workers -Kwang-ju City and Chonnam Province-)

  • The purpose of this study was to identify the degree and the relationship of the Knowledge, Attitude and Practice of industrial health care services of workers. This information will provide useful data for an effective industrial health care service. The was collected from 600 data employees in Kwang-ju city & Chonnam province. The data was gathered by questionnaire from Dec 20, 1992 to Jan 20, 1993. The questionnaire was developed by the Academic Affairs of Community Health Nursing Academy. Data was analyzed by using the statistical computer package, SPSS to manipulate the data along with percentages, means, standard deviations, modes, t-test and ANOVA. The results in this study were summarized as follows: 1. General characteristics of workers: Distributions of workers was laborer(76.2%). in the 25-34 age group(50.2%), married(63.3%), males comprised(77.5%), the educational level with the highest percentage was high school graduates (71.1%), with a monthly income of 300-700 thousand won(43.8%), and has been working in the present career over seven years (31.5%). 2. The Knowledge, Attitude and Practice levels of employees about industrial health care services The levels were measured according to a five point scale. The total mean score of knowledge was 2.92 points out of 4. The following are the scores of Knowledge of special medical examination and location of industrial clinic(3.48), periodic medical examination (3.18), occupational disease(3.08), personal protective equipment (2.92), and health education(1.37). The total mean score of Attitude was 2.77 point out of 4. The following are the scores of Attitude in order of working environment (3.35), necessity of periodic medical examination and health education(3.15), the worker's perception influence on the working environment to health was high(3.11). But, the level of satisfaction in the content of periodic medical examination was low(2.19). The total mean score of Practice was 2.70 points out of 4. The scores of Practice in order were, practice of periodic medical examination(3.70), utilization of industrial clinic(2.92), and to participate in health education(1.47). 3. The relationship of general characteristics to Knowledge, Attitude, and Practice of workers: Knowledge had a significant difference by sex (P<.01), marital status(P<.01), education level (P<.05) and monthly income(P<.01). Attitude was significantly different with sex(P<.05) and Practice was significantly related to monthly income (P<.01).

장기요양기관 요양보호사의 직무스트레스가 서비스 질에 미치는 영향에서 직무만족도의 매개융합효과 (Mediating Fusion Effect of Job Satisfaction on the Impact of Long-term Nursing Home Care Workers' Job Stress on their Service Quality)

  • 이 연구는 장기요양기관 요양보호사의 서비스 질을 높이기 위한 방안을 제시하기 위하여 직무스트레스와 직무만족도에 초점을 맞추었다. 연구대상자는 P시에 거주하면서 현재 장기요양기관에서 요양보호사로 활동하고 있는 자를 임의적 표집으로 자기기입식 설문조사를 실시하여 2021년 5월 한 달 동안 자료를 수집하였다. 최종적으로 자료분석에 활용된 자료는 130부 설문지이며, 자료분석방법으로 SPSS와 PROCESS macro(model 4) 통계프로그램을 활용하여 분석 결과를 도출하였다. 주요 연구결과는 장기요양기관 요양보호사의 직무스트레스는 서비스 질에 직접적인 영향을 미치지 못하였으나, 직무만족도는 간접적인 영향을 미치는 것으로 나타났다. 이는 장기요양기관 요양보호사의 직무스트레스가 높으면 직무만족도를 감소시켜 서비스 질에 부정적인 영향을 미칠 수 있음을 의미한다. 연구결과를 통해 장기요양기관 요양보호사의 서비스 질을 높이기 위한 제언이 이루어졌다.

간호대학생의 웰다잉에 대한 인식과 연명치료중단과 안락사에 대한 태도 (Nursing students' Awareness of Well-dying, Attitude toward Withdrawal of Life-Sustaining Treatment and Attitude toward Euthanasia)

  • 본 연구는 간호대학생을 대상으로 웰다잉에 대한 인식과 연명치료중단의 태도 및 안락사에 대한 태도 간의 관계를 알아보고자 시도되었다. 연구 기간은 2018년 4월 1일부터 5월 6일까지로 D광역시에 소재한 간호학과의 288명의 간호대학생을 대상으로 조사를 실시하였다. 연구결과 웰다잉에 대한 인식은 연명치료중단의 태도와 적극적 안락사, 소극적 안락사와 정(+)의 상관관계로, 연명치료중단의 태도는 적극적 안락사, 소극적 안락사와 정(+)의 상관관계로 나타났다. 간호사는 연명치료과정에서 많은 윤리적 갈등을 경험할 것으로 예상되는 바 간호대학생 시기에 웰다잉에 대한 인식, 생명윤리교육을 받는 것이 필요하고 교육프로그램의 개발과 효과평가가 요구된다.

일개 지역 사업장 근로자의 직무스트레스수준 평가 (The Evaluation of Worker's Job Stress Status in Workplace of a Local Area)

  • Purpose: This study was performed to evaluate the worker's job stress status in the workplace of a local area. Method: Data were collected from October to December, 2007. The subjects were 208 workers at 2 work sites in Busan Metropolitan area, who were examined using Job-Strain-Model Questionnaire. Data were analyzed by SPSS 12.0 Win Program to get the percentage, mean, standard deviation, t-test, ${\chi}^2$-test, ANOVA. Results: The results of this study were as follows: In the mean sub-factors job stress level, the mean of job demand was $28.7{\pm}4.4$(median 29.0), the mean of job discretion was $54.7{\pm}8.2$(median 54.0), the mean of social support was $21.8{\pm}2.9$(range:8-32). This study's subjects were appeared as active group with relatively higher score of job demand and job discretion than the average value of those. There was no statistically significant difference of general characteristics among the different job strain groups. There was statistically significant difference with of social supports among the different job strain groups. Conclusion: In conclusion, the subjects of this study's were active group. Thus, it is suggested that it is be necessary to repeated the education of the job task work for active group with high score of job demand and job discretion.

한국 정신간호영역에서의 간호업무환경 측정도구의 타당화 연구 (Validation Study of the Practice Environment Scale of Nursing Work Index for Psychiatric Hospital Nurses in Korea)

  • Purpose: The purpose of this study was to test the reliability and the validity of the Practice Environment Scale of Nursing Work Index (PES-NWI) scale when applied to Korean psychiatric nurses working in the psychiatric hospital in Korea. Methods: Data from 224 psychiatric nurses working in the 6 psychiatric hospitals in Korea were analyzed using Cronbach's ${\alpha}$, item total correlation, and exploratory factor analysis. Results: The Cronbach's ${\alpha}$ for the 27 items from the PES-NWI were .93, and the item total correlation coefficients ranged from .38 to .64. Six factors which explained 61.2 of variance in the perception of practice environment, emerged; nurse professional role and development, nurse manager ability, leadership, and support of nurses, staffing and resource adequacy, nursing foundation for quality of care, collegial nurse-physician relations, co-worker's ability and qualification. Five derived factors reconstructed, and two factors were renamed. Conclusion: Findings demonstrate that not all but 27 items from PES-NWI is a reliable and valid scale when applied to Korean psychiatric nurses working in Korea. Also findings indicate that Korean psychiatric nurses perceive practice environment differently than American nurses but have some point of similarities with Asian nurses working in the United States.
