With the dynamically changing environment of society, managing change is the vital element of organizations's survival and growth. Health care organizations have expended enormous resources to restructure patient care delivery. Despite the growing literature describing these organizational innovations, there is a paucity of credible data that reflects systematic measurement and evaluation of such changes. This study examined the nurses' psychological response toward the work process redesign, newly introduced by the nursing department in a acute care hospital. The aim of the study was to figure out how nurses's general perception of change and perceived attributes of change affected their acceptance of change during the organizational transition. This was descriptive- correlational. The sample for the study included 50 head nurses and 135 staff nurses. Data was analyzed using SPSS PC+, version 10.0. The major findings of the study were as follows: First, the mean score of the perception of change was 71.2(SD=13.8) with the range of 0-100, which means nurses generally perceived change positively. There were significant differences in perception of change by gender and education level. Head nurses perceived change more positively than staff nurses. The higher education level showed the more positive view of change. Second, among the perceived attributes of change, trialability had the highest mean score, which means nurses perceived the change more positively if it is testable on a limited basis. Relative advantage was perceived the most negatively. Finally, factors influencing the acceptability of the work-process redesign were perceived comparability, complexity, relative advantage, and observability, which accounted for 43.7% of the variance in the acceptability of change. This study evaluated the preliminary effects of the nursing process for reengineering, focusing on nurses' acceptability towards change. The usefulness of this research study was to determine the factors influencing acceptance of organizational members during transitional periods of change and to suggest effective strategies for increasing adoption as well as for decreasing resistance to change.
The nurse staffing level in the acute care hospitals affects patient safety and performance, and the nurse staffing legislation can be an important tool to guarantee the minimum nurse staffing. In Korea, although the medical law suggests the nurse staffing standards, it is necessary to revise the medical law for quality of nursing care and patient safety. Firstly, the nurse staffing standards in the current medical law enacted in 1962 needs to be revised to reflect changes in health care environment. Secondly, legal nurse staffing standards in the medical law are the minimum nurse staffing that medical institutions should comply with and thus must be managed so that all medical institutions should abide by them. Thirdly, the nurse staffing standards should apply on the basis of RN-to-patient ratios per shift in order to help patients understanding and ensure the easy management. Fourthly, the information of nursing staff level by the nursing unit and nursing shift in hospitals shall be released.
The role of Community Health Practitioner(CHP) should be continuously adapted to the social changes and the needs for health care services. That is the reason CHP needs to be retrained through the continuing education program. This paper showed CHP's roles to be reinforced by analyzing his present . task performance and ability in seven task areas as well as the changes of the social environment. In addition, this paper presented retraining areas needed for the reinforcement of the CHP's role in the future, and the development strategy of related continuing education package. The major results are as follows: 1. CHP's main practice area is health care services and management & guidance, whereas the development of health information system is neglected. 2. As a result, CHP plays a role mainly as a health care supplier, a consultant and a health instructor. Therefore CHP's roles to be reinforced are management of the community health system, act as a spokesman and a team member, promotion, assessment, collection & maintenance of information, coordination and research. 3. The areas to be reinforced in CHP's continuing education are (]) aged people's health, (2)?drinking & smoking, (3)?young people's health(including drug and sexualissues), (4) rehabilitation, (5)?administration and management for community health, (6)?partnership & membership, (7) local residents' participation and community development, (8) collection & treatment of health information and (9) environmental issues for community health. 4. The priority in developing continuing education package should be given to the area, which is encountered often in rural area but important, and has a good opportunity to be resolved. The health management of aged people was selected as a top priority by members of the Community Nursing Academic Society. 5. It is recommended that the instruction materials be accommodated to the small scale workshop or seminar in order that CHPs can participate actively in the continuing education program.
Forming health care management model in small-scale enterprises was the purpose of this study. For the purpose, we tried to investigate the characteristics of small-scale enterprises and analyzed the pattern of their health care management. The results are as follow: 1. The strength of health managing agency and technical supporting program lies in team approach by specialized manpower. However, if the liaison between each part of the organization is not smooth, the overall management will be very difficult. 2. Small scale enterprises are characterized by their short life after the establishment, use of rental building, lack of welfare facilities, weakness in sanitary management and aggregation of factories of similar type of industry. Because of these characteristics, it is very difficult to solve problem basically, such as improvement of working environment. Therefore, it is important to focus on health education and community based approach. 3. Many workers in small-scale factories are in middle and old age. They have health problems mainly related to personal habits. Implementation of an appropriate health promotion program is needed. 4. The number of workplaces, which should be managed by health managing agent. is increasing rapidly. But the number of health managing agent is limited. In the aspect of the requirement of manpower and equipment, training personal agent is more urgent than founding institutional agent. 5. The uniform method of health management hampers the choice of employer and workers. The types of provision of health management should be diversified. 6. For an efficient management, a frequent visit of personal agent and the following referral to a specialist should be done. The specialists in charge of secondary management are from the field of occupational medicine, occupational hygiene, ergonomics, etc. 7. The health management of small-scale facilities should have six components. They are community-based approach, multi-disciplinary cooperative system, program based on the need of recipient, forming partnership of employer and worker, change of lifestyle, and evidence-based program.

Purpose: This study aimed to characterize the relationship between parental stress and nurses' communication as perceived by parents of high-risk newborns in a neonatal intensive care unit (NICU). Methods: The participants were 54 parents of high-risk newborns in a NICU. Data were collected from January to March 2018. Parental stress and parents' perceptions of nurses' communication ability and styles were measured using a questionnaire. Results: The average scores for parental stress and nurses' communication ability were 3.39 and 4.38 respectively, on a 5-point scale. Parents most commonly reported that nurses showed a friendly communication style, followed by informative and authoritative styles. Mothers and fathers reported significantly different levels of parental stress. Parental stress showed a negative correlation with nurses' perceived verbal communication ability. Higher scores for nurses' verbal communication ability and for friendly and informative communication styles were associated with lower parental stress induced by the environment, the baby's appearance and behaviors, and treatments in the NICU. Conclusion: The findings of this study suggest that nurses need to offer proper information for parents and to support parents by encouraging them to express their emotions of stress and by providing parents with therapeutic communication and opportunities to participate in care.

Recently cancer, AIDS, chronic sickness have increased according to the elevation of socioeconomic level and fast change of lifestyle. The number of patients receiving terminal care increased fairly because the span of life is extended by development of medicinal technology. Also necessity of hospice and palliative care was risen according to the request of terminal patients that remove pain and keep calm life by interest about quality of life. However architectural plan and type specialization of facility which can correspond team's composition and supplied nursing program are not consisting. This study researches about care environment of hospice facility plan through investigation into terminal patient's special quality. The purpose of this study is to propose fundamental datas of hospice facility for architectural plan through comparative analysis of cases of domestic and outside facilities.
The tendency of having fewer or no children and increasing number of single household will make the protection of the elderly with geriatric disease even harder in the future. Nursing home can provide long-term care for those who can-not be cared by the family. Korean government has been changing welfare policy for the elderly to cope with the changing needs of them regarding nursing home and geriatric disease. Researchers from various field also showed great interest in nursing home. The purpose of this study is to analyze the number and contents of theses and papers regard-ing nursing home in the 12 journals, to find out overall the research streaming of the theme, method, subject, and district of the papers in the journals. Data were collected by using keyword through library database. 246 thesis and 116 papers published from 1990 to 2006 were analyzed in 7 categories, and 66 papers in the journals were used on the content analysis for lading out research streaming. As results, research streaming according to 5 periods were identified. The most frequent subjects of theses regarding nursing home are nursing home systems and policies. Otherwise, the various subjects of papers in the journals have been deal with, especially plans of nursing home and spacial behavior of the elderly and staffs' satisfaction. In the future, the research subjects related to the nursing home should deal with total space plan as living environment for the elderly and work environment for the staffs according to privacy, spatial use and behavior, culture and lifestyle in Korea, and so on.
Purpose: This study was done to conduct a meta-analysis of articles describing randomized controlled trials which showed substantiated effects for home-based child abuse prevention programs for all parents. Methods: The articles regarding child abuse prevention programs for parents were investigated using the systematic review method. A literature search was conducted in PubMed, CINAHL, PsycINFO, Cochrane central library, Eric, and RISS on material published. Article searches and quality appraisal through RoB (Risk of Bias) yielded 14 records. Results: The total random effect size was 0.344 (Parents outcome, 0.333; Child outcome, 0.310; Environment, 0.275). The home based intervention for parents was effective for prevention of potential child abuse. Conclusion: This study supports the effectiveness of health care professional directing child abuse prevention programs for all parents whose children are in the early years of life including during the period of pregnancy.
Purpose: The purposes of this phenomenological study were to explore the experience of preterm labor. Methods: The participants were 7 women admitted to two obstetric hospitals in Kyunggi-do Province with preterm labor. Data was collected with MP3 records through individual in-depth interviews and participated observation. The data was analyzed by Giorgi(1985) method. Results: The results were divided into six categories as follows: 1) Inappropriate coping: unexpected event, overwork, lack of insight of preterm labor, 2) Burn out: multiple role, burden, role conflict. 3) Restrictions of lifestyle: uncomfortable hospital environment, wearisomeness, limitations of personal hygiene, 4) Physical discomfort: headache, flush, tremor, palpitations, 5) Psychological distress : concerns about fetus health status, fear of possible preterm delivery, lack of information, financial worries, 6) A transition to new lifestyle: share of household chores, communication with self-help group, careful lifestyle. Conclusion: The findings of this study will offer a better understanding of women's preterm labor experiences and suggest clues to nurses on how to improve the care they provide.
Purpose: Falls are a common cause of unintentional injuries in infants. This study was conducted to examine the patterns of healthcare utilization following infant falls in South Korea. Methods: This cross-sectional descriptive study utilized an online survey designed to gather information regarding the general characteristics of parents and infants, fall-related variables, and healthcare use. Results: The most serious falls identified by parents occurred at an average infant age of 6.97 months. Most fall incidents took place indoors (95.7%), and many occurred under the supervision of caregivers (68.0%). Following the fall, 36.4% of the participants used healthcare services. Logistic regression analysis revealed that healthcare use following an infant fall was significantly associated with being a firstborn child (odds ratio [OR]=5.32, 95% confidence interval [CI], 2.19-15.28) and falling from a caregiver's arms (OR=4.22; 95% CI, 1.45-13.68). Conclusion: To prevent and decrease the frequency of infant falls, improvements are needed in both the domestic environment and parenting approaches.
