지역사회 케어코디네이터 교육과정을 위한 기초연구: 교육요구도 분석 (A Preliminary Study for the Curriculum Development of Community Care Coordinators: Educational Needs Analysis)

  • Purpose: A care coordinator is an emerging nursing professional role in South Korea. The purpose of this study was to identify educational needs and priorities for care coordinators among nurses. Methods: An online survey was conducted on 661 current or retired nurses from January 30 to February 28, 2021. A total of 17 essential competencies for care coordinators, recognized based on literature review, were used to analyze the educational needs. The data were analyzed using descriptive statistics, a paired t-test, and one-way analysis of variance with SPSS 25.0. The educational needs analysis was conducted by using a paired t-test, the Borich Needs Assessment Model, and the Locus for Focus Model. Results: Five contents were identified as the first priorities for educational needs: 'Health program planning and evaluation', 'Care planning', 'Coordinating community-based services', 'Case management', and 'Transitional care'. The second priorities for educational needs included 'Population health management' and 'Welfare resource linkages via communicating with social workers'. Conclusion: The priority items derived from this study offer underpinning insights for the development of care coordination training program.

유산 가능성 임부의 정서, 불편감 및 간호요구에 관한 연구 (A Study on Emotions, Feelings of Discomfort, and Nursing Needs of the Women in the Possibility of Abortion : Centered on the Threatened Abortion and Habitual Abortion during Hospitalization)

  • Maternity nurses' roles are to find out women's various responses related to the health problems during pregnancy, to intervene adequate nursing and supports, and to help them adapt satisfactorily to new situation, such as hospitalization. The purposes of this study are : firstly, to inquire about women's emotional status, feeling of discomfort, and the nursing need during hospitalization because of discomfort, and the nursing needs during hospitalization because of the possibility of abortion by threatened and habitual abortion : secondly, to provide fundamental data for developing adequate nursing intervention and improving hospital management. The subjects of this study were 62 women who were hospitalized, visited out-patient clinic for follow-up, and stayed at home after discharge since August, 1993 from one hospital located in Seoul, from March 23 to April 13, 1994. The questionnaire was consisted of 21 items of emotion(Reliability Cronbach's alpa, .77), 19 items of feeling of discomfort(.79), and 21 items of nursing needs (.89), and nurses' performance according to nursing needs,(.90). These were measured by using Likert Scale and analyzed by using SPSS / $PC^+$ with the descriptive statistics, $X^2$-test, and ANOVA. Research findings are as follows : 1) The subjects' average age, hospitalized days, and gestational age are 32.2, 15.7, 12.9 by respectively. 2) 88.7% of the subjects are getting antepatal care, 66.1% are experiencing 1.75 times of spontaneous abortion, and 82.3% are nuclear family. 3) The Emotions were mainly comprized negative and subjective state of minds such as uneasiness, anxiety, and discomfort. The domains of emotions were related to 'fetus', 'herself', 'hospitalization', and 'husband's 'families', in rank. The highest scored item was "I, above anyone else, am mostly stressed from abortion." 4) The feelings of discomforts were firstly related to 'personal discomfort arising from absolute bed rest', 'meals provided', Usage of hospital, perse', 'health team', in rank. 5) The nursing needs frequently pointed out were 'Nurses kindly explain of me the purpose, method, and results of the lab. test', 'Nurses sincerely pay attention to my word in communication'. The least nursing needs were 'Nurses observe my feces and whether I have constipation or not'. 'When I can't move, nurses help me by holding my arms or pulling my bed'. 6) The highest performed items were 'Nurses observe whether I bleed or not', and 'Nurses give me drug with exact dose in time'. The least performed items was about hospital environment, such as 'Temperature, ventilation & humidity in the room should be controlled'. 7) Besides religion that is related to nursing need with statistically significant difference, none of general characteristics were related to emotion, feeling of discomfort, and nursing needs.

상급종합병원 간호사의 사전연명의료의향서에 대한 지식, 자신감 및 교육요구도 (Knowledge, Confidence, and Learning Needs Regarding Advance Directives among Hospital Nurses)

  • Purpose : Nurses' knowledge regarding advance directives may affect their administration of and confidence towards end of life care. This study aimed to describe the relationships of knowledge, confidence, and learning needs with advance directives among hospital nurses. Method : This cross-sectional study was performed at a tertiary university hospital in Seoul between September 25 and October 14, 2017. Convenience sampling was used to recruit nurses who provided bedside care and had at least 1 year of clinical experience. We used a validated self-report questionnaire. Results : The mean score of knowledge, confidence and learning needs were $5.00{\pm}1.73$, $29.81{\pm}7.52$, and $64.54{\pm}8.48$ respectively. Hospital nurses' knowledge, confidence and learning needs were significantly different according to age, job position, educational level and perceived advance directives. Knowledge regarding advance directives was significantly associated with confidence (r = .27, p < .001) and learning needs (r = .16, p = .005). Conclusion : Knowledge regarding advance directives was relatively low compared to the findings of previous studies. Therefore, nurses should be knowledgeable and encouraged to initiate advance directives. It is necessary to develop a standardized educational program regarding advance directives based on Korean cultures.

임상간호사의 연구관련 활동 실태와 관련요인 (Research Related Activities and Its Related Factors in Clinical Nurses)

  • Purpose: This study was don(B to describe research-related activities and attitudes toward research, barriers to and support needs for undertaking research in clinical nurses. Method: Data were collected by a questionnaire from 238 clinical nurses with over one year clinical experience working at 2 university hospitals. Results: Research related activities included courses or lecture about Nursing Research 85.7%, journal reading at least once every 2 to 3 months 30.0%, memberships in academic societies 29.4%, participation in academic conferences 45.0%, conducting research 45.4%, research utilization 24.6%. The score for attitudes toward research was 3.08(range 1-5). The score of barriers to undertaking research was 3.37(1-5) and the score for support needs for undertaking research 4.14(1-5). Attitudes toward research significantly correlated with barriers to undertaking research(r=.- 36, p=.000). Barriers to undertaking research significantly correlated with support needs for conducting research(r=.23, p=.000). Nurses with experience in conducting research had more negative attitudes toward research(t=-2.130, p=0.034) and more barriers to undertaking research than those without experience in conducting research (t=2.194, p=0.029). Conclusion: These results suggest that it is necessary to increase positive attitudes toward research in clinical nurses and nursing organizations need to provide strong supports for nurses conducting research.

Nurses' Perceived Needs and Barriers Regarding Pediatric Palliative Care: A Mixed-Methods Study

  • Purpose: This study aimed to describe nurses' perceived needs and barriers to pediatric palliative care (PPC). Methods: Mixed methods with an embedded design were applied. An online survey was conducted for nurses who participated in the End-of-Life Nursing Education Consortium- Pediatric Palliative Care (ELNEC-PPC) train-the-trainer program, of whom 63 responded. Quantitative data were collected with a survey questionnaire developed through the Delphi method. The 47 items for needs and 15 items for barriers to PPC were analyzed with descriptive statistics. Qualitative data were collected through open-ended questions and analyzed with topic modeling techniques. Results: The mean scores of most subdomains of the PPC needs were 3.5 or higher out of 4, and those of PPC barriers ranged from 3.22 to 3.56, indicating the items in the questionnaire developed in this study properly reflect each factor. The needs for PPC were divided into 4 categories: "children and adolescents," "families," "PPC management system," and "community-based PPC." Meanwhile, PPC barriers were divided into 3 categories: "healthcare delivery system," "healthcare provider," and "client." The keywords derived from the topic modeling were perception, palliative, children, and education for necessities and lack, perception, medical care, professional care providers, service, and system for barriers to PPC. Conclusion: In this study, by using mixed-methods, items of nurses' perceived needs and barriers to PPC were identified, categorized, and weighted, and their meanings were explored. For the stable establishment of PPC, the priority should be given to improving perceptions of PPC, establishing an appropriate system, and training professional care providers.

Educational needs for practicing neonatal intensive care among Korean nursing students

  • Purpose: This study was conducted to investigate the educational needs for practicing neonatal intensive care among Korean nursing students. Methods: An explorative, sequential, mixed-methods design was used. Qualitative content analysis was conducted of in-depth interviews of six nursing students, five clinical practice faculty members, and five nurses in the neonatal intensive care unit. The results of a survey of 174 nursing students were analyzed quantitatively. Results: Nursing students, clinical practice faculty members, and nurses wanted opportunities for direct nursing practice and education in school during neonatal intensive care practice. In terms of specific educational content, nursing students expressed the highest observation-related educational needs for communication with medical team members, and they expressed the highest practice-related educational needs for operating medical equipment used for neonatal intensive care. The nursing students' needs with regard to the method of practice education were highest for orientation from the head nurses. Conclusion: Communication and operating medical equipment were found to be areas with high educational needs for practicing neonatal intensive care among Korean nursing students. Further research is needed to develop an educational framework and setting for practicing neonatal intensive care that would meet their needs.

간호사의 뇌졸중 관련 영양지식 및 영양교육 요구도 조사 (Assessment of Nurses' Nutritional Knowledge and Educational Needs Regarding Stroke Specific Diet Regimens)

  • Purpose: Although the prevalence rate of chronic diseases is rapidly increasing due to an unhealthy diet in Korea, nurses may not have enough nutritional knowledge to educate their patients. This study investigated the level of nurses' nutritional knowledge for chronic diseases as well as for strokes, and the needs for nurses' nutritional education. Methods: This research is a descriptive research conducted from April to August of 2014, on 242 nurses who work with stroke patients, in two tertiary general hospitals in the city of Seoul, South Korea. Results: The average nutritional knowledge score of our subjects was $19.9{\pm}2.51$ (range 12-24). The correct response rate was 83% which was higher than expected. However, many nurses (31-66%) answered incorrectly on items such as 'Drinking low-fat milk is better than whole milk', and 'Seasoning with a large amount of soy sauce instead of salt can reduce sodium consumption'. We also found that nurses who received continuing education regarding nutrition scored higher than those who did not (p=.020). There was no correlation between nutritional knowledge and the need for nutritional education (r=.034, p=.601). Conclusion: The level of nutritional knowledge of the nurses was relatively low and irrelevant to the completion of nutrition courses during university or duration of experience. Rather, the relevance was higher when the nurse received nutrition-related training after graduating from university. Our results imply that continuous nutritional education is necessary for hospital nurses.

간호 관리 능력 개발을 위한 교육 연구 (Needs on Management Development Program for Head Nurse)

  • The objectives of this study are to identify the actual educational contents of management for head nurse and to propose the educational subjects according to identity the needs of head, charge, and staff nurses. The subjects were investigated the actual Management Development Programs and educational needs of head nurses and prospective nurse manager(charge nurse, staff nurse with a lot of clinical experiences) in general hospitals. The tools were composed of two questionnaires: One was developed from the literature review for making items to measure actual situation. The other was revised Katz's model for measurement of educational needs. The first respondents of actual situation were 27 general hospitals with over 400 beds in Seoul and the second respondents were 89 head nurses, 67 charge nurses and 136 nurses at 3 hospitals by convenient sampling out of 27 general hospitals. Data were collected by telephone interview, mail questionnaire and visiting from 7th of October through 30th of November in 1997. In data analysis, general characteristics of the respondents and actual status of Management Development Programs were analyzed by frequency and percentage. Educational needs according to general characteristics were analyzed by ANOVA The results were as follows: 1. Actual situation of Management Development Program 1) Seven hospitals(26%) had Management Development Program for prospective managers and 14 hospitals (52%) for head nurses. 2) Education Department existed in 14 hospitals (52%). 3) One hospital(4%) had top level managers took part in the Management Development. 4) Two hospitals selected head nurse, who had finished courses of Management Development. Eight hospitals(30%) assessed educational needs. The assessment tools consisted of making a question via questionnaire(75%), determining at department meeting(12%) and interview(13%). 5) Educational programs had 3 types: 10 lecture type, 7 discussion type and 4 role play type programs. 6) One hospital evaluated the change of learner's attitude. 7) Four hospitals scored educational point, but that was measured only by attending. 8) Actual Management Development Programs were as follows. parenthesis indicates the number of hospitals. (1) Management Development Programs for Prospective manager. Role perception of Middle level Manager (1) . Role reconstruction of Nurse Manager (1). Workshop for Charge Nurse (1). The results of needs on Management Development subject 1) The educational needs of all respondents on 3 skill domains showed positive agreement to strongly positive agreement. 2) High priority(more than 4.5) items were 12 of 24 Human skill items(50%), 1 of 6 Technical skill items(16%), and 2 of 13 Conceptual skill items (15%). 3) Out of high priority items, 8 items were instituted. 4) All respondents showed high needs on 3 skill domains regardless of 3 positions (head nurse, charge nurse, and nurse). Educational needs of Human skill domain, according to position were 108. S, 108.7, 106.8 (mean score = 72) , needs of Technical skill domain were 26.5, 26.6, 26.I(mean score=18), and needs of Conceptual skill domains were 56.9,56.7, 55.1(mean score=39). 5) Needs of 3 skill domains according to clinical career showed significant difference. Out of respondents, nurses with career of over 16years showed lowest degree of needs in Human skill domains(F=4.47, P=.004) and Conceptual skill domain(F=2.93, P=.034). 6) Educational needs according to educational background were not significant difference. But out of respondents, nurses educated at 3-year junior college relatively showed lowest needs in all of the 3 skill domains. With the above-mentioned findings, further study is necessary for generalization of this study at hospitals with different bed size and location. Also it is needed to study about management skill of nurse and charge nurse, and effective educational method.

요양병원과 종합병원 간호사들의 섬망에 대한 인식과 간호지식정도 (Comprehension and Knowledge about Delirium in Nurses Working at Long-Term Care Hospitals or General Hospitals)

  • Purpose: This study was done to compare the comprehension and knowledge about delirium in nurses working at long-term care hospitals (NLH) with nurses working at a general hospital (NGH). Method: The participants were 191 nurses from a general hospital and 7 long-term care hospitals in Chungnam. The tool for comprehension consisted of the needs and importance of delirium education and the confidence of management for early detection, caring and prevention of delirium. The tool for knowledge of delirium developed by the author consisted of causing factors, symptoms, caring and prevention of delirium. Result: The comprehension of need and importance of delirium education was higher than the confidence for delirium management in participants. NLH nurses had a higher level in comprehension of the need and importance of delirium education and a lower level in confidence of delirium management than NGH nurses. The mean for delirium knowledge was 75.32% for all participants. The knowledge of NLH nurses was significantly lower than those of NGH nurses. All nurses showed low scores in confidence and knowledge of delirium prevention. Conclusion: A systematic educational program for nurses to enhance the ability for assessment, early detection and prevention of delirium for institutionalized elderly patients needs to be developed.

호스피스 병동 간호사의 죽음 불안 및 돌봄 요구: 성격유형을 중심으로 (Death anxiety and Needs of Interpersonal Caring Behaviors of Nurses in Hospice Wards: Focused on Types of Personality)

  • 목적: 본 연구는 호스피스 병동 간호사의 죽음 불안과 돌봄 요구를 성격유형을 중심으로 살펴본 서술적 조사연구이다. 방법: 호스피스 병동에서 근무하는 118명의 간호사를 대상으로 시행되었고, SPSS Statistics 26으로 분석하였다. 결과: 대상자의 죽음 불안은 2.41±0.27이었고, 하위 항목 중 '자신의 죽어감에 대한 두려움(2.60±0.38)'이 가장 높았다. 돌봄 요구는 3.58±0.60이었고, 하위 항목 중 '용서하기(3.74±0.64)'가 가장 높았다. 죽음 불안과 돌봄 요구 간의 유의한 정적 상관관계(r=.265, p=.004)를 확인하였다. 대상자의 성격유형별 죽음 불안이 돌봄 요구에 미치는 영향은 감정 중심 유형에서 타인의 죽어감에 대한 두려움(𝛽=-1.021, t=-2.388, p=.027)이 돌봄 요구에 영향을 미치는 요인(F=3.724, p=.020)으로 확인되었다. 결론: 죽음 불안과 돌봄 요구는 유의한 정적 상관관계를 보이며, 감정 중심 유형에서 죽음 불안이 돌봄 요구에 영향을 미치는 것으로 나타났다.
