A Comparison of Nursing Work Environment, Role Conflict, and Job Embeddedness of Nurses Working in Comprehensive Nursing Care Service Wards and General Wards in a Tertiary Hospital (상급종합병원에서의 간호·간병통합서비스 병동과 일반병동 간호사의 근무환경, 역할갈등, 직무 배태성 비교)

  • Purpose: The objective of this study was to compare and analyze the work environment, role conflict, and job embeddedness between comprehensive nursing care service (CNCS) ward nurses and general ward nurses. Methods: This descriptive research study involved 70 CNCS ward nurses and 69 general ward nurses working at an advanced general hospital in Seoul. Data were collected using the structured questionnaire from March 27 to April 14, 2019 and analyzed with the SPSS 24.0 program. Results: The work environment of the CNCS ward nurse was higher than that of the general ward nurse (t=4.38, p<.001), and the role conflict of the CNCS ward nurse was lower than that of the general ward nurse (t=-2.09, p=.038). However, job embeddedness did not show any statistically significant difference (t=0.22, p=.824). Conclusion: The results of this study show that the introduction of CNCS ward has shown improvement in the work environment and strengthened the establishment of the roles in their team, while maintaining the job embeddedness of nurses. These results indicate that improvements in the work environment, such as nurse staffing and material support, would contribute to the qualitative enhancement of nursing and that it would need to extend the introduction of CNCS wards.

The Difference in Circadian Rhythm and Level of Fatigue Between ER and General Ward Nurse (응급실과 일반병동 간호사의 밤번 근무시 Circadian rhythm과 피로도 비교)

  • The purpose of this study is to find out the difference in circadian rhythm and level of fatigue between ER and General Ward Nurse. This study was done through analyzing body temperature, heart rate and major electrolytes such as $Na^+$ and $K^+$ concentration on urinalysis, which are related to factors of circadian rhythm and level of fatigue. The subjects, 17 nurses from Emergency Room and 25 nurses from general ward, were selected from a General Hospital in Taegu City on August 8 to 23, 1998. And the data were analyzed by proportion, t-test, chi-square and Pearson's Correlation, ANCOVA, using SAS program. The results of this study were shown as follows: First, The difference of Maximal and minimal heart rate was significant between ER group and general ward group. However, body temperature difference wasn't notable. Second, Urine $Na^+$ concentration on urinalysis was lower and $K^+$ concentration on urinalysis was higher in ER group than general ward group significantly. Third, Fatigue level was higher in ER group than general ward group significantly. Fourth, through urine concentration, significant relations in circadian rhythm of ER nurse group and general ward group and their fatigue level were found. Urine $Na^+$ concentration on urinalysis of ER nurse was significant related with their fatigue level. In conclusion, the effect of fatigue on circadian rhythm is greater in ER group than general ward group during night shift. This study could be a help in improvement of nurse's health and understanding the effect of fatigue on health.

A Comparative Study regarding Health Condition and Work Stress of Nurses Working in Cancer Ward and General Ward (암병동간호사와 일반병동간호사의 건강상태와 직무스트레스 비교 연구)

  • The health of a nurse is very important because her mental and physical health can influence toward nursing patients directly, Especially, Cancer patients are growing annually. Also, terminal cancer patients' nursing and dead place are increasingly using hospital and the period of nursing for cancer patients are increasing. Nursing for cancer patients are different with acute disease. Therefore, I analysed comparing nurses' health working in cancer and general ward so that nurses working at cancer ward could be developed as a professional nurse, and I wanted to establish the foundation of nursing administration and reasonable manpower management to supply good quality of nursing to patients. In my research, I selected 117 nurses working in cancer ward and 134 nurses working in general word to analyse the stress rate and nurse's health comparing nurses working in general ward and cancer ward. The survey was conducted of nurses working in cancer ward nurses in 2 university hospitals, nurses working in a cancer hospital, and general ward nurses working in 3 public hospital. Also, the data was collected from Sep. 13, 2001 to Sep. 28, 2001. As health measuring tool, I used Cornell Medical Index(CMI) which are developed to fit Koreans by Ko Ungrin and Park Hang-bas (1980) using Cornell Medical Services which were designed by Weiser, Brosman, Mittelman, Wechler, Wolff in Cornell University(1945). As working stress measuring tool, I used Questionaries which were designed by Kim Mae-ja and Ku Mi-ok(1984) and then developed by Bae In-sook(1996). For managing the data, I used frequency, percentage, ${\chi}^2$ verification, t-test, and F-test (ANOVA). And in the case of significant data(p<.05). I did Duncan's test for post verification. The mutual relation between health condition and working stress rate have been conducted using Pearson's Correlation Coefficient. Followings are the results of my research. 1. Two groups showed significant differency at age after testing homogeneous character between two groups (${\chi}^2$ =9.919, p=.007). 2. Comparing two group's health condition, cancer ward(average 19.35${\pm}$18.34) were higher than general ward(14.42${\pm}$10.59) and showed statistical significant differency(p=.009). And, comparing two group's mental condition, cancer ward(9.00${\pm}$9.79) were higher than general ward(7.13${\pm}$6.35) and statistically no differency. 3. After comparing two group's working stress rate, the rate of cancer ward nurse's working stress(3.36${\pm}$.50) is higher than general ward nurse (3.32${\pm}$.48). There are no significant differency. However, in the detailed verification test, there were significant differency at inappropriate compensation (t=3.254, p=.001) and medication issue (t=2.170, p=.031). 4. After comparing health condition at general points, physical health condition showed significant differency at age(p=.020), the number of children (p=.015), religion (p=.015), position(p=.005), career(p=.008), working satisfaction(p=.003), activity after office hour(p=.045); and mental health condition showed significant differency at position(p=.010), career (p=.017), working satisfaction (p=.003). 5. After comparing the working stress rate according to general points, there were significant differency at working satisfaction (F=5.285, p=.006), predicted nursing(F=3.822, p=.023). 6. At the relation of health condition and working stress rate between two groups. physical and mental condition showed significant relation with working stress rate. i.e, if a nurse's health condition is not good, she are feeling much more stress than others. After considering all the factors in my research, I found that the health condition and stress rate of cancer ward nurses is much higher than general ward nurses. Considering that cancer ward nurses is necessary to care for increasing cancer patients with mental and physical nursing, the less stress for cancer ward nurses is very important to develop nursing quality and working efficiency by keeping good health condition, specializing cancer ward nurses. Therefore, we need following studies to find the factors which are effecting to cancer ward nurses' health and specialization. Also, we need to improve managing working condition to decrease working stress by improving working condition.

A Comparative Study of Floor Area and Average Distance of Nurse Working of Planned Courtyard in Ward (중정이 계획된 병동부 바닥면적과 간호동선 평균거리 비교 연구)

  • Purpose: The courtyard in the ward can light and ventilate in the central space and corridor of the ward. It is possible to improve the disadvantages of the existing double-corridor ward. Also, there is the advantage in that patients are able to contact with nature indoors and cause positive activities. So, the purpose of this study is quantitatively analyze the floor area and the average distance of nurse working of the courtyard in ward. Methods: The subjects are converted through the same criteria setting. And it investigate and analyze the influence of the courtyard in the ward through the analysis of the spatial composition. Result: The factors that affected the floor area increase are the planning courtyard, the corridor type and the dispersion of the medical rooms. The average distance of nurse working is more influenced by the nursing management system of the hospital than by the courtyard. So, It can improve according to the position and number of nurse station. Implications: It can be used as a primary data for courtyard plan in the ward for positive patient environment plan considering the floor area, average distance of nurse working.

A Study on the Visibility Analysis Model for the Ward of General Hospital Considering Nurse's Circulation (종합병원 병동부 간호 이동 동선을 고려한 가시 특성 분석 모델에 관한 연구)

  • The path and visibility of nurses in general hospital wards have been treated as architectural planning factors. However, the analysis approach of existing studies shows limitations that only fixed physical elements are considered without considering the behavior of users using space. Consider factors for analysis of ward and models based on this study model. Select a case hospital to apply the analysis technique and conduct the nurse questionnaire and route survey of the hospital. Establish a framework for analysis model applied with the path of nurse movements. The analysis model applies to the case hospital. The analysis results are aggregated to derive design suggestion for reference to the spatial improvement of the ward. Visible visibility to observe the bedside in the nursing station, visibility to observe the beds in the nurse's path, and visibility to observe patients moving in the nurse's path were derived from visual access frequency and exposure frequency. The survey of nurses' movements at the site allowed the nurses to calculate the distance required to move. Reflecting the path of nurse movement, a model was presented for a comprehensive analysis of nursing distance and nursing visibility, which could lead to improvement in the observation and visibility of nurses and the layout of patient rooms or day rooms.

Professional Self Concept of Psychiatric Mental Health Nurse Practitioner and General Nurse in Psychiatric Ward (정신병동의 정신보건간호사와 일반간호사의 전문직 자아개념)

  • Purpose : This study was conducted to investigate the extent to which the professional self concept between the psychiatric mental health nurse practitioner (PMHNP) and general nurse in psychiatric ward was comparable. The results were expected to provide basic data for developing the construct of professional self concept and making awareness of the importance of the program increasing professional self concept. Method : The subjects of this study were 227 PMHNP and 436 general nurse in psychiatric ward. The instruments used for this study were PSCNI by Arthur (1990), PSI by Heppner and Peterson(1982) and the index of work satisfaction by Slavitt et al.(1978). With the aid of the SAS, t-test. two-way ANOVA and stepwise multiple regression were conducted. Result : 1. The average item score of PSCNI of PMHNP was $2.82\pm0.27$. and that of general nurse was $2.66\pm0.27$. Statistically significant difference between two groups was found(p=0.0000) 2. There was statistically significant difference between two groups in the score of professional practice(p=0.0000), satisfaction(p=0.0024), leadersbip(p=0.0000) , flexibility(p=0.0000) and skill (p=0.0000). 3. Statistically significant differences between the two groups were observed in terms of age(p=0.0003), marital status(p=0.0001). education(p=0.0005), religion(p=-.0144), motive (P=-.0001), length of service as a nurse(p=0.0121), the length of service in psychiatric unit(p=0.0143). However there were no significant interaction effect with group and age, marital state. education, religion, motive, length of service as a nurse, length of service in psychiatric unit. 4. Job satisfaction (JS) and problem solving inventory score(PS) were found to be the highest factor predicting the professional self concept between the PMHNP and general nurse. JS and PS accounted for $43.4\%$ in the professional self concept of PMHNP, whereas PS, JS, age and religion accounted for $53\%$ in the professional self concept of general nurse in psychiatric ward. In conclusion, this study suggested that we need to develop programs and polices to increase the professional self concept of nurse, particularly of psychiatric mental health nurse practitioners.

  • This study was to investigate the relationship between hospital nurses 'knowledge of breast-feeding and their ability to provide care to breast-feeding mothers and their children. From Nov. 1 to 30, 1999, 105 nurses of the obstetric ward, delivery room, OPDs of obstetrics and gynecology, and nurseries of three general hospitals from Seoul and Pusan were evaluated. The results are as follows: 1.Mean score of nurse's knowledge to breast-feeding was 12.76. Mean score of performance of nursing activities was 109.80. 2.General characteristics and feeding characteristics were significantly related to nurse's knowledge to breast-feeding and performance of nursing activities related to breast-feeding. Nurse's knowledge about breast-feeding was significantly related to age, working ward, clinical experience, rank of position, experience of breast-feeding and experience education on breast-feeding. Performance of nursing activities for breast- feeding was significantly related to age, working ward, experience in other ward, marital status, and education on breast-feeding. 3.A positive correlation between nurse's know-ledge to breast-feeding and performance of nursing activities related to breast-feeding was statistically significant(r=.381, P<0.05).

A Comparative Study on Role Conflict and Job Satisfaction between Nurses in ICU and Nurses in General Ward (중환자실 간호사와 일반병동 간호사의 역할갈등과 직무만족도 비교연구)

  • Purpose: The purpose of this study was to identify and compare role conflict and job satisfaction between nurses in ICU and nurses in general ward. Method: The subjects of this study consisted of 141 nurses employed at ICU and 201 nurses employed at general ward, who were recruited from 5 university hospitals with 500 beds located in Seoul and Gyeonggido. The instruments included role conflict scale developed by Kim and Park(1995) and job satisfaction scale translated and modified by Lee(1996). The collected data were analyzed using the SPSS WIN 10.0 program for descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient. Result: In the two groups, there was a inverse correlation between role conflict and job satisfaction. In ICU, nurses' role conflict levels differed significantly depending on religion(p<.05), total experience(p<.05), scheduled to work as a nurse(p<.05). And nurses' job satisfaction levels differed significantly depending on total experience(p<.05), working period in current post(p<.001), scheduled to work as a nurse(p<.001), motive of choosing the nursing science(p<.01), and job satisfaction(p<.001). In general ward, nurses' job satisfaction levels differed significantly depending on marital status(p<.05), educational level(p<.01), total experience(p<.001), motive of choosing the nursing science(p<.01), and job satisfaction(p<.001). Conclusion: Summing up, the nurses at ICU showed higher level of role conflict than the nurses ay general ward. Therefore, it is needed to decrease the level of nurse's role conflict and improve their job satisfaction in ICU.

A Study on Behavior Health Care Competency between Psychiatric Ward Nurse and General ward Nurse (정신과병동과 일반병동 간호사의 행동건강간호역량에 관한 비교 연구)

  • This study is a cross sectional, descriptive research that utilized the Korean version of the BHCC (Behavior Health Care Competency) measurement tools to compare and contrast the BHCC level between nurses stationed in psychiatric and general ward departments. The research subjects were selected from 6 hospitals that have at least 300 beds located in Seoul city and Gyeonggi Province. There were a total of 190 nurses, consisting of 90 nurses from the psychiatric ward department and 100 nurses from the general ward department. The comparison demonstrated that nurses from psychiatric ward department showed a higher BHCC compared to general department nurses in most items. In the case of psychiatric ward nurses, compared to general ward nurses, the assessment was 5.29 times higher, the intervention was 6.06 times higher and the proper use of resources was 2.63 times higher. On the other hand, the treatment recommendation had no influence. Accordingly, education and training for general ward nurses should be improved and hospital administrators should pay more attention in conducting the BHCC education for the general ward nurses and on fostering education experts to develop related programs.

Change of Ward Design in Korean Hospital Architecture since 1945 (1945년 이후 한국병원건축 병동부의 변화)

  • Purpose: This paper aimis to analyze the characteristics of ward plan of Korean hospital architecture constructed in each periods and identify the change since 1945. Methods: Literature review was conducted while ward plans for analysis were sellected. And then the important points of ward design were analyzed in each periods. Results: The important points of ward design - type of corridor in ward plan, number of beds per nursing unit, location of nurse station, organization of multi-bed rooms, location of patient toilet, number of day room, concentration and separation of elevator halls for patient and general- were analyzed and the characteristics of change were identified in each periods. Implications: This paper could be a basic data for arranging the history of hospital architecture in Korea.
