The Effectiveness of Ultrasound-Guided Thoracic Paravertebral Block for Percutaneous Radiofrequency Ablation of Hepatic Tumors: A Pilot Study (간종양의 경피적 고주파 열치료에서 초음파 유도하 흉부 방척추블록의 효용성: 예비 연구)

  • Purpose: The purpose of this study was to evaluate the effectiveness of thoracic paravertebral block (TPVB) for management of pain during and after percutaneous radiofrequency ablation (RFA) of hepatic tumor. Materials and Methods: All patients were divided into non-TPVB (4 patients, 4 sessions of RFA for 4 tumors) and TPVB group (5 patients, 7 sessions of RFAs for 7 tumors). Ultrasound (US)-guided TPVB was performed at T7 level. The 15 mL of 0.375% ropivacaine was injected into right paravertebral space before RFA. If patients complained pain and asked analgesics or experienced pain with verbal numerical rating scale (VNRS) of more than 4, fentanyl $25{\mu}g$ (up to $100{\mu}g$), pethidine 25 mg, and midazolam 0.05 mg/kg (up to 5 mg) were sequentially given intravenously during RFA. Results: Total intravenous morphine equivalence of analgesics before, during, and after RFA was 129.1 mg and 0.0 mg in non-TPVB and TPVB group, respectively. Conclusion: US-guided TPVB may be an effective and safe anesthetic method for decreasing or eliminating pain during and after RFA for hepatic tumor and helpful in decreasing the usage of opioids.

A Case Study of Soyangin Patient with Restless Legs Syndrome Treated with Hyeongbangsabaeksan (형방사백산(荊防瀉白散)으로 호전된 소양인(少陽人) 하지불안증후군 환자 치험 1례)

  • Objectives The aim of this study was to report an improvement on the symptoms of Restless Legs Syndrome (RLS) after using Hyeongbangsabaeksan.Methods The patient was diagnosed with Soyangin Soyang-sangpung Symptomatology and treated with Hyeongbangsabaeksan. The patient's subjective symptoms of pain on the lower limb and discomfort in sleeping were observed using the Numerical Rating Scale(NRS) and Visual Analogue Scale(VAS) respectively during the treatment period. And other specific symptoms of RLS were assessed once per week for a total of three times with Korean Versions of the International Restless Legs Scale.Results and Conclusions The pain on the patient's lower limb decreased from NRS 4 to NRS 0 after one week of treatment. And discomfort in sleeping decreased from VAS 5.5 to VAS 0 after one week of treatment. Furthermore, the score of Korean Versions of the International Restless Legs Scale decreased from 40 to 5 after two weeks of treatment. In conclusion, this study shows that Sasang Constitutional Medicine can be an effective treatment for Soyangin patient with Restless Legs Syndrome as Soyangin Soyang-sangpung Symptomatology.

Correlation with Psychosocial, Workplace Coping Skills, Pain, and Degree of Disability of Office Workers with Non-Specific Chronic Neck Pain

  • Objective: The purpose of this study was to investigate the correlation between activity of daily living and social psychology and work mind of office workers with non-specific chronic neck pain (NSCNP). Design: Crossed-sectional study Methods: 86 patients with NSCNP were recruited for this study. Neck disability index (NDI) and Numerical pain rating scale (NPRS) were used to check the pain intensity and disability of patients with neck pain. To find out the occupational factors of the subjects, the Korean version of Latack Coping Scale was used. And,To find out the socio-psychological factors of the subjects, the Korean version of depression anxiety stress scale (DASS-21) was used. We performed correlation for each variable. Results: The correlation between NPRS and NDI and DASS-21 Scale were clear positive correlation (p<0.05). There was no statistical significance between the LATACK control group and the pain and disability index (p>0.05). A clear positive correlation was established between the avoidance group of LATACK and pain (p<0.01). Conclusions: Through this study, it is necessary to have time to manage depression, anxiety, and stress in the treatment of neck pain in office workers who spend a lot of time sitting. In addition, it is thought that there should be the ability to control oneself about one's duties in the workplace.

A Pilot Clinical Study on the Accuracy and Safety of Ultrasound-guided Gyeontonghyeol (BP-LE6) Acupuncture: A Prospective Randomized, Single Blinded Crossover Study

  • Background: The purpose of this study was to assess the accuracy and safety of procedures using ultrasound equipment for acupuncture treatment. Methods: A pilot, prospective randomized, single blinded, crossover clinical study on the accuracy and safety of ultrasound-guided Gyeontonghyeol (BP-LE6) acupuncture treatment was conducted. Patients (n = 13) with shoulder pain were randomly divided into 2 groups. During Visit 1, ultrasound-guided BP-LE6 acupuncture was administered to the experimental group. In the control group, patients received BP-LE6 acupuncture (without checking ultrasound images) by manipulating the ultrasound probe as if administering ultrasound-guided acupuncture. Visit 2 was arranged within 7-14 days and the remaining procedures, other than those administered in Visit 1, were performed. In both the experimental group and control group, the number of needle insertions, and time required for the treatment to result in the patients feeling de-qi was recorded. The numeric rating scale (NRS) score for shoulder pain was recorded before and after the acupuncture treatment. Results: The number of needle insertions was $5.31{\pm}3.50$ times in the experimental group, and $6.62{\pm}3.38$ times in the control group, however, there was no statistically significant difference between the groups (p > 0.05). The mean time required to perform the procedure was $151.54{\pm}48.59$ seconds in the experimental group and $86.69{\pm}37.17$ seconds in the control group, which was statistically significantly different (p < 0.05). The changes observed in numerical rating scale scores between groups were not statistically significantly different. Conclusion: Although there was no statistically significant difference, administering acupuncture using ultrasound guidance may lead to accurate needling with a reduced number of needle insertion attempts. A large-scale clinical study of better design should be conducted in the future.

  • The purpose of this study is to report the clinical effects of modified Seogakjihwang-tang on a patient who had headache and memory disorder after traumatic brain injury, which were considered as the result of blood stasis. The patient had headache and memory disorder after traumatic brain injury and treated with modified Seogakjihwang-tang three times a day for 60days. As a result, the numerical rating scale and pain rating score of headache decreased and the scores of mini-mental state examination-Korea, the Korean version of Montreal cognitive assessment and the mini-memory test increased during treatment with modified Seogakjihwang-tang. Therefore, this case supports a possibility that modified Seogakjihwang-tang might be an effective treatment to patients with headache and memory disorder after traumatic brain injury.

A Patient with Symptoms Caused by Electric Shock Treated with Traditional Korean Medicine

  • This case report describes a 60-year-old female patient diagnosed with intercostal neuropathy and vertebral compression fractures which occurred following an electric shock injury. The patient received acupuncture, pharmacopuncture, and herbal medicine administration between February 10th, 2020 and April 25th, 2020. The pain level in the thoracic and left intercostal areas was assessed using the Numerical Rating Scale. The Self-report of the Leeds Assessment of Neuropathic Symptoms and Signs Pain Scale were used to diagnose neuropathic pain. The Neuropathic-Pain -Scale was used to evaluate the degree of neuropathic symptoms. The Oswestry Disability Index and the European Quality of Life-5 Dimensions were used to assess quality of life scales and functional disorder. Following combined Korean medicine treatment, the patient exhibited reduced levels of pain and significant improvement in functional disorder symptoms and quality of life.

The Effects of Pre-operative Visual Information and Parental Presence Intervention on Anxiety, Delirium, and Pain of Post-Operative Pediatric Patients in PACU (회복실에 대한 사전 정보제공과 보호자 상주 중재가 수술 직후 각성 시 소아 청소년 환아의 불안, 섬망 및 통증에 미치는 효과)

  • Purpose: The purpose of this study was to test whether pre-operative visual information and parental presence had positive effects on anxiety, delirium, and pain in pediatric patients who awoke from general anesthesia in a post-surgical stage. Methods: This study used a non equivalent control-group post test design (n=76). Independent variables were provision of pre-operative visual information and parental presence for post-surgical pediatric patients in PACU (post anesthesia care unit). Dependent variables were anxiety, delirium, and pain in the pediatric patients measured three times at 10 minute intervals after extubation in the PACU. Measurements included Numerical Rating Scale for assessing state anxiety, Pediatric Anesthesia Emergence Delirium Scale by Sikich & Lerman (2004) for delirium, and Objective Pain Scale by Broadman, Rice & Hannallah (1988) for pain. Results: Experimental group showed significantly decreased state anxiety at time points-10, 20, and 30 minutes after extubation. Delirium was significantly lower at 10 minutes and 30 minutes after extubation in the experimental group. Pain was significantly lower at 10 minutes after extubation in the experimental group. Conclusion: The results of this study suggest that this intervention can be a safe pre-operative nursing intervention for post-surgical pediatric patients at PACU.

A Case Report of Acquired Rotating Torticollis, Treated by Muscle Energy Techniques and Oriental Medicine (Muscle Energy Techniques 및 한의학적 치료를 이용한 후천성 회전형 사경의 치험 1례)

  • Objectives : The purpose of this study is to investigate the clinical application of Muscle Energy Techniques(MET) and Oriental Medicine to a patient who had Acquired Rotating Torticollis. Methods : A patient diagnosed as acquired torticollis was hospitalized at Buchun Jaseng hospital of Oriental Medicine for 28 days and she was treated by MET, herbal medicine and acupuncture. To measure the outcome of the patient's improvement, we observed Numerical Rating Scale(NRS), Neck Disablility Index(NDI), Tsui's Scale and the X-rays of Whole spine. Results : After being treated by MET and Oriental Medicine, the patient's pain was reduced and the head rotating was significantly improved. The improvement of patient in the NRS, NDI, Tsui's Scale was detected and the X-ray of Whole spine became normal. Conclusions : As seen in this case, complexed therapy with MET and Oriental Medicine is effective treating method to control pain and head rotating for patients with acquired rotating torticollis.

Development and Validation of Questionnaire for Chronic Fatigue Syndrome (CFS) Diagnosis Based on Systemic Exertion Intolerance Disease (SEID) Criteria (전신성 활동불능증(Systemic Exertion Intolerance Disease) 진단 기준을 바탕으로 한 만성 피로 증후군(Chronic Fatigue Syndrome) 진단 설문지 개발 및 신뢰도 평가)

  • Purpose: This study aimed to develop a questionnaire for the diagnosis of chronic fatigue syndrome (CFS) designed based on the systematic exertion intolerance disorder (SEID) criteria, and to validate the reliability of the questionnaire. Methods: A literature search on questionnaires for CFS diagnosis was conducted to develop a SEID questionnaire (SEID-Q27), followed by a pilot survey to identify the reliability of the questionnaire. Adults (Daejeon university personnel) with a Chalder fatigue scale (CFQ) score ≥15 were invited for the survey. We commenced the survey in November 2019 with a two weeks of interval for the test and retest method. The reliability of the questionnaire was investigated in three angles: 1. Cronbach's α, 2. correlations (r) of the questions, numerical rating scale (NRS), and visual analog scale (VAS), and 3. kappa (k) analysis. Results: Among the total 275 adults registered, 55 (20%) participants with a CFQ score ≥15 were invited, and 31 (11%) [15 male, 16 female] completed the questionnaire. The total Cronbach's α was 0.944 for the test and 0.949 for the retest. The reliability (r) of questions by CFQ score (≥15, ≥18, ≥20) ranged from 0.533-0.928 (p <0.05), and the r score of the NRS and VAS were the highest in CFQ scores ≥20, at 0.933 (p<0.001). The agreement rate of the SEID-Q27 between the test and retest was 87% (kappa k=0.743). Conclusions: The SEID-Q27 seems to be reliable. Further studies are needed to measure the validity of the tool and the cutoff point.

Consumptive Disease and Chronic Fatigue Improved by Nokyonggunbi-tang and Other Korean Medical Treatments: A Case Report (녹용건비탕을 비롯한 한방치료로 호전된 허로 환자 1례)

  • Objective: This study investigated the efficacy of using Nokyonggunbi-tang and other Korean medical treatments for a patient with consumptive disease and chronic fatigue. Methods: A 59-year-old female patient with fatigue, headache, dizziness, and neck pain due to consumptive disease and chronic fatigue was treated with herbal medicine (Nokyonggunbi-tang), together with acupuncture, pharmacopuncture, cupping, and chuna manual therapy, for 22 days. The treatment effects on the pain were evaluated using the scores for the Fatigue Severity Scale (FSS), European Quality of Life Five Dimensions (EQ-5d) Scale, Neck Disability Index (NDI), and Numerical Rating Scale (NRS). Results: Following treatment, the patient showed a decrease in the FSS, NDI, and NRS scores and an improvement in the EQ-5d score. Conclusions: Nokyonggunbi-tang and Korean medical treatment significantly improved fatigue symptoms in a patient with no substrate disease.
