A Patient with Guillain-Barre Syndrome Treated with Combined Korean Medicine Treatments

  • A patient diagnosed with Guillain-Barre syndrome (GBS) suffering from numbness in the toes and fingers, paralysis in the face, and back pain received intravenous immunoglobulin on December 28, 2019. Without an improvement in symptoms the patient was admitted to a Korean medicine hospital on January 1, 2020 to receive combined Korean medicine treatment including herbal medicine (Sipjeondaebo-tanggami and Hyangbujasunkipalmul-tang), acupuncture treatment, moxibustion, cupping treatment, and physical therapy. Treatment was administered over 51 days, of which 17 days were during hospitalization. Paralysis of facial muscle, and numbness in fingers and toes were evaluated using the Yanagihara score and numeric rating scale (NRS) score, respectively. Facial paralysis improved (21 to 40 based on Yanagihara score) and paresthesia of the hands and feet disappeared (NRS score 6-7 to NRS score 0). There were no side effects or sequela. This study suggested combined Korean medicine treatment may be effective for the treatment of GBS.

A Case Report of Tension Type Headache Patient with Dangkisoo-san and Acupuncture (당귀수산과 침으로 호전된 긴장형 두통 환자 치험 1례)

  • Objectives : The purpose of this study is to report the clinical effects of Dangkisoo-san and acupuncture on a patient with Tension Type headache. Methods : We have given Dangkisoo-san and acupuncture treatment to a patient with Tension Type headache for 8days, and checked changes of symptoms. Main symptoms were numbness and pains in a head and a neck. Results : After treatment, the numbness and pains in a head and a neck were improved considerably. Conclusions : This study shows that Dangkisoo-san and acupuncture are significantly effective in treatment of Tension Type headache.

  • Diabetic neuropathy may affect every part of the nervous system with the possible exception of the brain. While it is rarely a direct cause of death, it is a major cause of morbidity. Distinct syndromes can be recognized, and several different types of neuropathy may be present in the same patient. The most common picture is that of peripheral polyneuropathy. Usually bilateral, the symptoms include numbness, paresthesias, severe hyperesthesias and pain. The pain, which may be deep-seated and severe, is often worse at night. In this case, chief complaints were numbness and paresthesia of lower extremities and the symptoms were improved through Oriental medical treatment.

A Double-blind Comparison of 0.125% Ropivacaine with Fentanyl and 0.125% Bupivacine with Fentanyl for Labor Analgesia Using Patient-Controlled Epidural Infusion (지속적 통증조절기를 이용한 경막외강으로 Fentanyl을 첨가한 0.125% Ropivacaine과 0.125% Bupivacaine 주입시 분만통 조절에 대한 효과)

  • Background: This study intends to evaluate the benefits of the administation of continuous infusion and demand doses of 0.125% ropivacaine compared with 0.125% bupivacaine after addition of fentanyl using patient controlled epidural analgesia (PCEA) for pain control during labor. Methods: Thirty-nine American Society of Anesthesiologists physical status 1 or 2 parturients were randomized by double blind design to receive either 0.125% bupivacaine with fentanyl 1 ug/ml or equivalent concentration of ropivacaine/fentanyl using PCEA; with 6~8 ml/hr basal rate, 3 ml bolus, 5 min lockout, 30 ml/hr dose limit. We assessed analgesia, the amount of study solution used in PCEA, sensory levels, motor block (0~3 scales), side effects and patient satisfaction. A postpartum questionnaire was carried out afterward. Results: There were no differences in visual analogue scores (VAS) for pain, hourly study solution use, sensory levels, side effects and patient satisfaction between groups. However, patients administered ropivacaine/fentanyl had significantly less demand, less administered in PCEA, less numbness and restriction of movement compared with patients in the bupivacaine/fentanyl group. Conclusions: Ropivacaine 0.125% with fentanyl, when administerd epidurally by PCEA for labor analgesia, was equally efficious as bupivaciane 0.125% with fentanyl, having with minimal numbness and restriction of motion.

A Case Report of a Guillain-Barre Syndrome Patient Suspected of Having Acute Transverse Myelitis Treated with Korean Medicine (급성 횡단성 척수염으로 의심된 길랑바레증후군 환자에 대한 한방 치료 증례보고 1례)

  • A 67-year-old female patient diagnosed with acute transverse myelitis visited our hospital in February 2023. Consultation papers indicated that she was diagnosed with acute transverse myelitis and that she complained of quadriparesis and numbness in both hands and feet. However, her spine magnetic resonance imaging (MRI) did not clearly indicate that she suffered from acute transverse myelitis. She was then sent to a high-level hospital and diagnosed with Guillain-Barre Syndrome through electromyography, cerebrospinal fluid test, and spine MRI. On March 2023, she visited our hospital again, still complaining about weakness in both hands and inability to walk. She was treated with Korean medicine, including acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine such as Yanggyeoksanhwatang and Sipimijihwang-tanggami. After three months, she was discharged on cane walking. The manual muscle test (MMT) grade of her upper limbs increased from 3-/3-/3-/3- (shoulder/elbow/wrist/fingers) to 4/4/4/3+, and the MMT grade of her lower limbs increased from 2/2/2/2 (pelvis/knee/ankle/feet) to 4/4/4-/4-. Her numbness in both feet disappeared, and her numbness in both hands decreased from a numeric rating scale of 7 to 2. No adverse events were reported during treatment. This case implies the therapeutic potential of Korean medicine for Guillain-Barre Syndrome, especially for those who had delayed diagnosis and missed the opportunity to get plasma exchange.

Regrowth of Cervical Intradural Lipoma without Spinal Dysraphism

  • A 49 years old male patient who suffered from deterioration of posterior neck pain, left hand numbness, left lower limb pain and gait disturbance for 3 years visited our outpatient department. He had been diagnosed as non-dysraphic cervical intradural lipoma and operated in August 1990. On the radiologic images, we found the regrowth of non-dysraphic cervical intradural lipoma from C2 to C7 level, which surrounds and compresses the cervical spinal cord. Previous subtotal laminectomy from C2 to C7 and severe cervical lordosis were also found. Appropriate debulking of lipoma mass without duroplasty was successfully done with intraoperative neurophysiological monitoring (IONM). We are following up the patient for 24 months via outpatient department, his neurologic symptoms such as hand numbness, gait disturbance, left lower limb pain and posterior neck pain have improved. We describe a rare case of regrowth of non-dysraphic cervical intradural lipoma.

  • Although Churg-Strauss syndrome (CSS) is a rare disease that is generally associated with vasculitis, nerve involvement is also common in cases of CSS. A 48-year old man was diagnosed with a herniated disc at L4-5 and an annular tear at L5-S1 after complaining of pain and numbness in the left lower leg. Peripheral edema was observed during physical examination and the patient was diagnosed with CSS after a biopsy was conducted. In addition, electromyography and nerve conduction velocity revealed the presence of multiplex mononeuropathy, which indicated the pain and numbness was due to peripheral neuropathy caused by CSS. The symptoms were relieved after oral administration of prednisolone. This case indicates that when symptoms of peripheral neuropathy do not match the radiographic evidence other causes, such as CSS, must be considered.

  • The purpose of this study was to describe the treatment of the clinical symptoms of syringomyelia using Korean medicine. A patient with syringomyelia complained of back and shoulder pain, numbness of the upper right limb, headaches and dyspnea. He was treated with herbal medicine, acupuncture, moxibustion, and physical therapy. The Visual analog scale (VAS) was used to score sensory symptoms (pain, numbness) and Modified Borg scale (mBorg) was used to score dyspnea. After the treatment, his symptoms were relieved. The patient's chief complaint was back and shoulder pain which was reduced greatly from a VAS score of 6 between 1-2. The results in this study may contribute to the development of the Korean medicine field of syringomyelia in the future.

  • Numb chin syndrome, is a rare neuropathy, characterized by facial and oral numbness restricted to the distribution of the mental nerve. Even though this neuropathy is uncommon, but this still has an important clinical meaning because it can be related with a malignancy. Because orofacial symptoms can even present the first clinical feature of a malignancy, dentists should pay careful attention to their meaning and importance to detect the malignant tumor early. Moreover, patients who present with a sudden numbness on chin should be investigated for the undiagnosed malignancy. In this report, we described a patient with stabbing orofacial pain and numbness of chin who was diagnosed with diffuse large B-cell lymphoma and placed the importance on the diagnosis of NCS.
