Clinical Effects of Sayuktanggami-bang on Cerebral Vascular Accident Patients with Normocytic Normochromic Anemia (뇌졸중환자의 정구성정색소성 빈혈에 대한 사육탕가미방의 임상적 효과)

  • Objective : Normocytic Normochromic Anemia is one of the most common cases in clinics of all types of morphological Anemia. This study is aimed at examining significant curative effects of Sayuktanggami-bang on Cerebral vascular accident(CVA) patients with Normocytic Normochromic Anemia. Methods : For this study to examine the effects of Sayuktanggami(四六湯加味方), Patients were divided into 2 groups. Group A(experimental group) consisted of 19 cases prescribed Sayuktanggami-bang, Group B (control group) consisted of 15 cases prescribed other herbs. Before presciption, They were tested for CBC, AST, ALT, Total protein, Cholesterol, TG, HDL, BUN, Creatinine, Uric acid, Na, K, Cl while fasting, then were given the herbs over two weeks. They were checked again for changes over those two weeks. Results : In cases of group A, the statistical significance of CBC changes before and after treatment were recognizable : RBC(p<0.01), Hb(p<0.01), Hct(p<0.01). In comparing CBC changes between group A and group B which developed cerebral infarction group, a type of CVA, the significance of curative effects in group A is recognizable over group B before and after treatment : RBC(p<0.01), Hb(p<0.01), Hct(p<0.05). Cytotoxic reaction was nonexistant. Conclusion : Results of this study suggest that Sayuktanggami-bang has curative effects for CVA patients with normocytic normochromic anemia.

  • Feline hemotropic mycoplasmosis (hemoplasmosis) is an infection of the red blood cells caused by the Mycoplasma haemofelis (Mhf), Candidatus Mycoplasma haemominutum (CMhm), and Candidatus Mycoplasma turicensis (CMt). The existence of Mhf, CMhm, and CMt has been demonstrated in feral cats in Korea using molecular methods, but no clinical cases have yet been reported. This study reports 2 clinical cases of hemotropic mycoplasmosis caused by CMhm and CMt in 2 anemic cats. The first case was a client-owned intact female domestic shorthair cat that presented with fever, pale mucous membranes, and normocytic normochromic non-regenerative anemia. Prior to referral, an immunosuppressive prednisolone dose was administered at the local veterinary clinic for 1 month. The cat was diagnosed with high-grade alimentary lymphoma. Organisms were found on the surface of the red blood cells on blood smear examination. The second case was of a rescued cat that presented with dehydration and fever. The cat had normocytic normochromic non-regenerative anemia. Necropsy revealed concurrent feline infectious peritonitis. Polymerase chain reaction assay targeting 16S rRNA revealed CMhm infection in case 1 and dual infection of CMhm and CMt in case 2. Normocytic normochromic non-regenerative anemia was observed in both cats before and during the management of the systemic inflammation. This is the first clinical case report in Korea to demonstrate CMhm and CMt infections in symptomatic cats.

Hematological manifestations in dogs progressing to the iron deficiency anemia by repeated phlebotomy

  • Progressing to the iron deficiency anemia was experimentally induced in 4 clinically healthy dogs by repeated phlebotomy to characterize hematologic features, serum iron values, and RBC indices. Abnormal RBC morphologies were also evaluated semiquantitatively on Wright's-stained blood films. Hematologic abnormalities in early stage of anemia included decreased both hematocrit and hemoglobin, and reticulocytosis, with no changes in mean corpuscular volume (MCV) and mean corpuscular hemoglobin concentration (MCHC) were represented. In intermediate stage, decreased serum iron concentration with microcytosis and hypochromia were prominent. In late stage, red cell distribution width and Mentzer's index were out of reference ranges in the majority of dogs. In this study microcytic anemia was appeared at the hemoglobin range of 5.1-7.2 g/dl. On most sampling days, platelet counts and white blood cells were within the reference ranges, with some minor variations. Iron deficiency was not necessarily associated with microcytic anemia. Judging from the sequential changes of both MCV and MCHC, 3 patterns of anemia were sequentially observed: initially normocytic normochromic, intermediate normocytic hypochromic or normocytic normochormic, and finally microcytic hypochromic. The most frequent morphologic abnormalities were target cells. Occasional elliptocyte, acanthocyte, stomatocyte, kinzocyte, dacrocyte and schistocyte were also noted on the blood films.

  • The anemia which is accompanied with chronic primary diseases has been showed most frequently in the clinic. The purpose of this study is to give an account of the effects of the Korean herbal medicine therapy which is based on the constitutional medicine for the patient who has suffered from mild anemia, pneumonia and general depressed condition in the chronic stage of post-cerebellar hemorrhage. The subject is a 75-year-old woman who has had the symptoms caused by hemorrhage, which are headache, dizziness, walking disturbance, dysphasia, and general weakness and so forth. Therefore, she has normocytic normochromic anemia in the hematomancy. We have diagnosed her as Soeumin(少陰人) Ulkwangjeung(鬱狂症) and have prescribed Palmulgunjatang(八物君子湯) in accordance with the principle of Seungyangikgi(升陽益氣). The consequence is that the accompanied anemia and genseal depressed condition have improved.

Isolated Lateral Sinus Thrombosis Presenting as Cerebellar Infarction in a Patient with Iron Deficiency Anemia

  • As a rare cerebrovascular disease, cerebral venous thrombosis (CVT) is caused by various conditions including trauma, infection, oral contraceptive, cancer and hematologic disorders. However, iron deficiency anemia is not a common cause for CVT in adult. Posterior fossa infarction following CVT is not well demonstrated because posterior fossa has abundant collateral vessels. Here, we report a case of a 55-year-old man who was admitted with complaints of headache, nausea, and mild dizziness. The patient was diagnosed with isolated lateral sinus thrombosis presenting as cerebellar infarction. Laboratory findings revealed normocytic normochromic anemia due to iron deficiency, and the patient's symptoms were improved after iron supplementation.

  • The author obtained index of red cell volume distribution width(RDW) and other red cell indices in 210 patients of various hematoncologic conditions and 200 healthy control group using, an automated blood analyzer, Coulter Counter Model S-plus II. This study performed to classify various etiologic anemia based on the MCV and RDW, to evaluate availability to the differential diagnosis in korean anemic distoders somewhat different from etiologies of anemias in foreginers. In the most of cases, the increase or decrease of MCV were always combined the pararell changes of MCH and MCHC. But the values of MCV and RDW were not correlated in control group and patient group. So the terms of heterogenous of homogenous anemia were meaningful morphologic classification than hypochromic or normochromic anemia. The heterogenous microcytic anemia contained iron deficiency anemia. In heterogenous normocytic anemia, myelophthisic anemia, acute leukemia were contained. In heterogenous macrocytic anemia, megaloblastic anemia, hemolytic anemia were contained. The homogenous microcytic anemia was observed in anemia of chronic disorders. In homogenous normocytic anemia, acute blood loss, chronic leukemia, multiple myeloma were contained. The aplastic anemia was belonged to homogenous macrocytic anemia. The diagnostic significance of RDW in hemoglobinopathies is most important. But this study was not contained hemoglobinopathies. Instead RDW was very helpful to differential diagnosis of most common anemias, iron deficiency anemia and anemia due to chronic disorders in Korea.

Canine Uterine Rupture with Septic Peritonitis Results from Adenocarcinoma in a 2-year-old Maltese Dog

  • A 2-year-old female Maltese dog was presented with a history of anemia and vaginal hemorrhagic discharge. Physical examination revealed severe vaginal hemorrhagic discharge, abdominal pain, pale mucous membranes, low blood pressure and dehydration. Results of serum biochemistry, hematology, venous blood gas, and electrolyte canine C-reactive protein (CRP) test revealed severe normocytic normochromic anemia, severe neutropenia, a high level of CRP, hypoglycemia, and imbalanced electrolytes. Abdominal ultrasound examination showed focal hypoechoic defect with loss of layering in uterine horn wall. A laparotomy revealed a clear reddish fluid in the abdomen, the fistula of left and right uterine horn, the purulent discharge from fistula, and symptoms of septic peritonitis near by the fistula site. The bitch underwent ovariohysterectomy and recovered without complication. Histopathological diagnosis of the uterine fistula site was adenocarcinoma.

Multiple Brain Calcification in Chronic Lead Poisoning (만성 연중독자에서의 다발성 뇌석회화 병변)

  • We experienced a case of occupational lead poisoning employed in a secondary lead smelting plant for 12 years. The patient was 39-year-old male and had been felt dizziness, recent memory impairment and intermittent severe abdominal pain for 2 years. On admission, blood lead level was $92.9{\mu}g/dl$, urinary lead level was $19.9{\mu}g/l$ and zinc protoporphyrin level was $226.0{\mu}g/dl$. On the blood test, hemoglobin was 10.6g/dl and showed normocytic normochromic anemia. There were no abnormal findings in the biochemical and hormonal tests. Decrease of I.Q. and use of words in speaking were found in the psychiatric and psychologic examinations. We observed the finding of motor polyneuropathy in the nerve conduction velocity test. Computed tomographic finding showed calcification lesions in the basal ganglia, dentate nuclei, caudate nuclei, and especially characteristic multiple calcifications were located in the subcortical white matter.

  • Pure Red-Cell Aplasia [P.R.C.A.] is rare disease characterized by absence of erythroid precursors in the bone marrow, normocytic normochromic anemia with profound reticulocytopenia in the peripheral blood, and relatively or completely spared granulopoiesis and thrombopoiesis. The association rates of P.R.C.A. with Thymoma is approximately 50%, but only 5-10% of all patients with a Thymoma have a P.R.C.A.. P.R.C.A. is thought to be a variety of autoimmune disease, and humoral inhibitor, i.e. IgG, has been demonstrated experimentally. Its treatments such as thymectomy, immunosuppressants, steroid, androgenic hormone, and splenectomy have been tried but the result is not satisfactory and the prognosis is poor. We experienced a case of P.R.C.A. with Thymoma treated with thymectomy and postoperative steroid therapy, and which showed good postoperative recovery clinically and hematologically.

  • Hematological value were determined from 140 Fasciola gigantica infested and 138 non-infested apparently healthy (control) zebu cattle from eight different sites of Bangladesh. The F. gigantica infested cattle have experienced of normocytic normochromic anemia indicated by the significant (p<0.01) decline of total erythrocyte count, hemoglobin level, packed cell volume and mean corpuscular hemoglobin concentration and significant (p<0.01) increase of erythrocyte sedimentation rate, as compared with non-infested control animals. Values of mean corpuscular volume and mean corpuscular hemoglobin content were not differed significantly between the infested and control animals. There observed significantly (p<0.01) higher eosinophil and neutrophil counts and significantly (p<0.01) lower lymphocyte counts in fasciola infested cattle in comparison with the control animals.
