  • 1. Out of 21 examples on a clinical base Alzheimer type dementia were 8 examples(38%), 11 vascular types(52%), 1 Alzheimer and vascular mixed type and rest 1 was secondary dementia type(NPH).2. Between the genders, there were 5 males and 3 females for Alzheimer types, 6 males and 5 females for the vascular types, 1 male for Alzheimer and vascular mixed type, and 1 female for secondary dementia type(NPH).3. For the degrees of Alzheimer type dementia there were 2 mild dementia, 4 moderate dementia, and 2 severe dementia. 4. Among the Alzheimer types 2 mild dementia were almost recovered back to normal in approximately 2 months, 2of 4 moderate dementia became significantly better in approximately 2 months and recovered to almost normal state in 3 to 5 months. The rest 2 have been under treatment for 3 months, but showed a little improvement. Out of 2 severe dementia examples, one showed a little improvement even if it has been under treatment over 2 years. The other example did not show any improvement, but dementia did not proceed any more.5. Among the 11 vascular examples, 7 recovered in 2 to 3 months, 1 in 5 months, 2 recovered in a year, and the treatment was stopped arbitrary for the last one.6 In the case of Alzheimer and vascular mixed type dementia, even the moderate dementia did not show any apparent result in 6 months.7. For NPHI (Normal Pressure Hydrocephalus), there was an improvement on dementia in 2 months after the treatment, but gait disturbance and urinary incontinence did not show any noticeable difference. As a conclusion, almost all the Alzheimer and vascular type patients recovered, but the treatment periods varied depending on the types of the dementia and the degree of seriousness, especially in the case of the severe Alzheimer patients, the treatment oniy suppression the progression of the dementia. The most important aspect from clinical point of view was even if almost all the patients were almost completely cired, they need to keep being cured for a long period of time. The method with respect to Four Constitutions is thought of the most desirable.

Transcallosal Ipsilateral Motor Pathway from the Unaffected Hemisphere in a Patient with Traumatic Brain Injury

  • Purpose: We presented with a patient with traumatic brain injury who showed a transcallosal ipsilateral motor pathway from unaffected motor cortex to affected extremities, as evaluated by diffusion tensor tractography (DTT). Methods: One patient and six age-matched normal subjects were enrolled. A 42-year-old left hemiparetic male who suffered from brain injury by motor vehicle accident 9 years ago showed a leukomalactic lesion in the right corona radiata and parieto-temporal lobe. His left extremities were completely paralyzed initially, but recovered slowly over 2 years. At the time of the evaluation, he was able to grasp and release an object, and to walk with spastic gait pattern. DTT was performed using 1.5 T with a Synergy-L Sensitivity Encoding head coil. DTT was obtained with termination criteria of FA <0.2 and an angle change > $45^{\circ}c$. Results: The motor tracts of the unaffected (left) hemisphere of the patient and control subjects originated from the motor cortex and descended along the known corticospinal tract without any transcallosal tract. By contrast, the tract of the affected (right) hemisphere originated from the left premotor cortex, descended through the left corona radiata, and then crossed the mid-portion of the corpus callosum. The tract then descended through the known corticospinal tract pathway to the right medulla. Conclusion: We conclude that the transcallosal ipsilateral motor pathway from the unaffected hemisphere appeared to contribute to the motor recovery in this patient.

Study on Change of the Flatfoot's Ankle Angle in Sagittal plane before and after Wearing FFO (기능성 발보조기 착용 전후의 시상면의 편평족 발목각도 변화에 관한 연구)

  • The foot performs an important function in supporting the body and keeping body balance. An abnormal walking habit breaks the balance of the human body as well as the normal function of the foot. The influence of a flatfoot(pes planus) occupies a considerable portion of the various causes resulting in the wrong walking habit. But, little studies has been done by the functional foot orthotics for the flat foot. The object of this study, therefore, is to propose a new approach method to reveal the effects of the improvement of the foot function by using orthotics. The essential point of this study is to measure and analyze the change of ankle angle in the sagittal plane for flat foot subjects wearing the orthotics. Before and after wearing the functional orthotics, the gait analysis of flat foot subjects was conducted in three experimental aspects : the change of ankle angle, the change of the total Ankle ROM and the difference of left & right ankle angle in the sagittal plane. 1. The average ankle angle differences of before-and-after wearing the orthotics have declined like this; left : $2.71^{\circ}$, right: $1.91^{\circ}$ (p<0.05). 2. Total ankle ROM also showed decrease in both sides while the left side's is rather slight; left : $0.57^{\circ}$, right : $2.07^{\circ}$ (p<0.05). 3. The difference of left and right ankle angle in the sagittal plane decreased by $0.71^{\circ}$ (p<0.05). In result, it is confirmed that the functional foot orthotics have a significant effect on mechanical movement of ankle joint for flat foot. it is expected that this paper will be further studied and improved as a practical estimation method in the research on the effect of foot-orthotics.

A Navigation Algorithm using a Locomotion Interface with Programmable Foot Platforms for Realistic Virtual Walking (실감의 가상 걸음을 위한 발판타입 이동인터페이스의 네비게이션 알고리즘)

  • This paper describes a novel navigation algorithm using a locomotion interface with two 6-DOF programmable foot platforms. When a human walks on the locomotion interface (LI), the walking motions of the human are recognized by several sensors. Then, the sensed information is used by the LI for generation of infinite surfaces for continuous walking and the virtual environments for scene update according to motions of the human walking. The suggested novel navigation system can induce user's real walking and generate realistic visual feedback during navigation. A novel navigation algorithm is suggested to allow natural navigation in virtual environments by utilizing conditions of normal gait analysis. For realistic visual feedback, the virtual environment is designed with three components; 3D object modeler for buildings and terrains, scene manager and communication manager component. From experiments, the subjects were satisfied with the reality of the suggested navigation algorithm using the locomotion interface. Therefore, the suggested navigation system can allow a user to explore into various virtual terrains with real walking and realistic visual feedback.

  • Patients with cerebellar infarction often complain of many symptom-vertigo, gait disturbance, nausea, vomiting. Especially, Vertigo is the main symptom, and patients cannot achieve normal activity in daily life. When we prescribed Banhabakchulchunma-tang and Gastrodiae Rhizoma extract to a 61year-old female stroke patient with vertigo, we could observe that the symptom was improved rapidly after treatment. Therefore we suggest that Banhabakchulchunma-tang which increase Chunma in quantity is very useful for vertigo after stroke. Further case studies of herbal treatment of this ailment are needed.

Functional Evaluation of Tennis Shoes Using Foot-Pressure Distribution (족저압력분석을 활용한 테니스화 기능성평가)

  • The purpose of this study was to analyze the foot-pressure distribution of Tennis Shoes for assessing their functionality. 10 university male students (shoe size: 265mm) who had no history of injury in the lower extremity and a normal gait pattern participated in this study. Four types of tennis shoes, most popular in Korea (A, B, C & D company), were selected and tested. Using the PEDAR-X system and PEDAR-X insoles, 4 different motion stages were analyzed for the foot-pressure distribution: (a) straight running; (b) c-cut($45^{\circ}$ left turn running; (c) forehand stroke; and (d) backhand stroke. Results revealed that in all stages, there were no statistically significant differences among the types of shoes; however, descriptive statistics indicated that functionality of shoe types was somewhat different depending on the type of stages. The order in functionality found was C>A>B>D.

  • The purpose of this study was to compare core muscle activity and thickness in the abdomen (internal Oblique, IO; External Oblique, EO; Transverse Abdominis, TrA) according to walking training methods. Tests were performed on 20 healthy men who randomly assigned to two groups, divided by Nordic walking (n=10) or Power walking group (n=10). They were performed Nordic walking or Power walking training for 2 weeks that is consistent with each of the assigned groups. Results demonstrated that Nordic walking was more effective than Power walking in improving IO and EO activities. Nordic walking is believed to be useful method for a variety of therapeutic exercise as a stable balance with the stick in addition to normal gait and trunk stability.

The Effects of Asymmetric Bag Carrying during Walking on Plantar Pressure (보행 시 비대칭성 가방 휴대가 족저압에 미치는 영향)

  • PURPOSE: The purpose of the present study was to examine changes caused by asymmetric bag carrying methods to carry the bag with one shoulder only to plantar pressure during walking. METHODS: Twenty three normal adults without any gait problem participated in the present study. Experimental conditions used consisted of walking without carrying any bag(condition 1), walking wearing a bag on both shouders (condition 2), and walking wearing a bag on the right shoulder(condition 3) and the weight of the bag was set to 15% of each subject's body weight. All the subjects were instructed to participate in all experiments under these three conditions and plantar pressures were measured from the subjects' right and left feet using an F-scan system while the subjects were walking under the three conditions. To analyze the measured plantar pressure, the sole was divided into seven areas (Hallux, Toe, Met1, Met23, Met45, Mid foot and Heel) and maximum plantar pressures in individual areas were measured. RESULTS: The results of measurement of plantar pressures under three walking conditions did not show significant changes in any areas of the left and right feet except for the mid foot area of the right food. The asymmetry between the left and right feet was examined and the results showed significant differences only in area Met23 under condition 2 and did not show significant differences in any other areas. CONCLUSION: On comprehensively considering the results of the present study, it could be seen that asymmetric bag carrying did not have large effects on changes in plantar pressure during walking compared to symmetric carrying. The reason for this is considered to be posture adjusting mechanisms against load positions.

Development of Joint Angle Measurement System for the Feedback Control in FES Locomotion (FES보행중의 피드백제어를 위한 관절 각도계측 시스템 개발)

  • The purpose of this study is to develop a minimally constraint joint angle measurement system for the feedback control of FES (functional electrical stimulation) locomotion. Feedback control is desirable for the efficient FES locomotion, however, the simple on-off control schemes are mainly used in clinic because the currently available angle measurement systems are heavily constraint or cosmetically poor. We designed a new angle measurement system consisting of a magnet and magnetic sensors located below and above the ankle joint, respectively, in the rear side of ipsilateral leg. Two magnetic sensors are arranged so that the sensing axes are perpendicular each other. Multiple positions of sensors attachment on the shank part of the ankle joint model and also human ankle joint were selected and the accuracy of the measured angle at each position was investigated. The reference ankle joint angle was measured by potentiometer and motion capture system. The ankle joint angle was determined from the fitting curve of the reference angle and magnetic flux density relationship. The errors of the measured angle were calculated at each sensor position for the ankle range of motion (ROM) $-20{\sim}15$ degrees (dorsiflexion as positive) which covers the ankle ROM of both stroke patients and normal subjects during locomotion. The error was the smallest with the sensor at the position 1 which was the nearest position to the ankle joint. In case of human experiment, the RMS (root mean square) errors were $0.51{\pm}1.78(0.31{\sim}0.64)$ degrees and the maximum errors were $1.19{\pm}0.46(0.68{\sim}1.58)$ degrees. The proposed system is less constraint and cosmetically better than the existing angle measurement system because the wires are not needed.

Effects of the moment arm length of backpacks on balance and walking while carrying a backpack (백팩 착용시 모멘트 팔의 길이가 균형과 보행에 미치는 영향)

  • The present study investigated the effects of the moment arm length on balance and walking when carrying a backpack. In total, 30 normal adults without orthopedic and neurological injuries were assessed. For each subject, balance and gait were measured under three conditions: 1) the no backpack condition, 2) the general backpack condition, and 3) the decreased length of moment arm backpack condition. There were significant differences in the center of the pressure area and velocity between the three conditions, whereas there was no significant difference in the center of the pressure area and velocity according to the moment arm length. There were significant differences in double limb support time and walking velocity under the three conditions, and there was a significant difference only in double limb support time according to the moment arm length. The results of the present study showed that a change in the length of the moment arm can be helpful for walking when carrying a backpack.
