Isolated aortic valvular heart disease : analysis of etiology and surgical experience (대동맥판막질환의 임상적 고찰 -판막병리 및 임상성적에 대하여-)

  • Clinical and pathologic data were reviewed in 20 patients who had have surgery for isolated aortic valvular heart disease between April 1978 and April, 1987. Hospital mortality was 10%, with no additional late mortality during a mean follow-up period of 24.1 months. Prosthetic valve failure developed in 3 patients and two had reoperation. Niety four percent of the survivors who were in NYHA Funtional class III or IV before operation are now in class I or II. Ninety percent of all patients are still alive at a maximum follow up of 9 years. The clinical histories, gross and histologic examination of valves estabilished the causes for isolated aortic valve disease: 3 rheumatic, 2 congenital bicuspid, 2 hypertention, 2 aortitis and each one case of floppy valve, medial cystic necrosis of aorta, bacterial endocarditis. But etiology was unknown in 8 cases. Sixteen patietns had myxoid degeneration, defined as significant disruption of the valve fibrosa and its replacement by acid mucosaccharides and cystic changes. Myxoid degeneration was also the primary pathologic abnormality in the patients with 2 hypertention, 2 rheumatic, 1 aortitis, 1 bacterial endocarditis, 1 floppy valve, 1 congenital bicuspid. The patients with myxoid degeneration of uncertain origin were 8. Histologic finding of all of them revealed nonspecific patients with myxoid degeneration of uncertain orgin were 8. Histologic finding of all of them revealed nonspecific chronic valvulitis with myxoid degeneration. This finding may indicate that the etiology w uld be infectious.

  • The cellular and nonspecific immune response of EPA were investigated in mice. ICR male mice were divided into 8 groups and received intraperiteneal injection of EPA (5 mg, 10 mg, 20 mg/kg) for 4 weeks. Cyclophosphamide (5 mg/kg) was administered i.p. 2days prior to secondary immunization. Immune responses were evaluated by hypersensitivity to SRBC(DTH), rosette forming cell(RFC), natural killer(NK) cell activity and macrophage activity. The obtanined results were as follows: As compared to normal group, 1) DTH was increased by EPA 5 mg, 10 mg administration groups. 2) RFC was significantly increased by EPA 20 mg administration group. 3) NK-Cell activity was significantly increased by EPA 10 mg administration group. 4) Macrophase activity was enhanced by EPA 5 mg administration group.

Eye Irritation, Skin Irritation and Skin Sensitization Tests for Nonspecific Immunostimulator BARODON® (비특이 면역증강제 BARODON® 의 안점막 및 피부에 대한 국소자극시험)

  • Two local irritation and skin sensitization studies of nonspecific immunostimulator, $BARODON^{\circledR}$ were carried out with New Zealand White rabbits and Hartley guinea pigs. In skin irritation test of male New Zealand White rabbits, body weights were not significantly changed and there were no responses after treatment for 24 or 72 hours and the Primary irritation index (P.1.1.) was '0'. And, in the eye irritation test, there were chemosis in some of rabbits. One of 3 rabbits in washing group was detected chemosis after 24 and 72 h following treatment and 2 of 6 rabbits in non-washing group were detected chemosis after 24h and 7 days following treatment. Therefore, total score is '4' after 24 h and '2' after 72 h following treatment by conforming article "some blood vessel are clearly hyperemic" . However evaluation value is non-irritant because M.O.I. (Mean ocular irritation index) score is below during the all experimental period and no significance through individuals and exposure time. In skin sensitization, the score of skin reaction was graded 1 with 0% sensitization rate. Taken together, these results indicate that $BARODON^{\circledR}$ may be non-irritant material. material.

Primary Laryngeal Lymphoma (후두에 발생한 원발성 림프종)

  • Primary lymphoma of the larynx is rare, accounting for less than 1% of all laryngeal neoplasm. Early symptoms are ambiguous and nonspecific, and confirmation of the diagnosis is often difficult. The aim of this study was to report the experience of our department in the management of these aggressive lesions, as they require special diagnostic and therapeutic attention. We enrolled 3 patients who diagnosis of lymphoma involving the larynx were retrospectively reviewed. The histopathological diagnosis revealed 1 case of diffuse large B cell lymphoma, and 2 case of NK-T cell lymphoma. Details of the presentation, diagnostic procedures, treatment, and outcome of these patients were presented. Primary laryngeal lymphoma is a rare entity. Early symptoms are subtle and nonspecific, and confirmation of the diagnosis is often difficult. Because of the rarity of this tumor type, the optimal management remains controversial and it seems that should be managed not as a distinct disease entity but as an unusual presentation of non-Hodgkin lymphoma, according to the recent treatment trends.

Immunotoxicological Effects of Mouse CpG Oligodeoxynucleotides in Lupus-prone NZB/NZW F1 Mice

  • Despite wide therapeutic use of CpG ODN against infection, allergy and cancer, the safety and toxicity of CpG ODNs were poorly delineated. Thus, we investigated whether optimal dosing of CpG ODN would affect immunotoxicological parameters in NZB/NZW F1 mice. Comparisons were made among control, non-CpG ODN and mouse CpG ODN ($10{\mu}g$)-treated groups for 4 weeks. To gauge the immunotoxicity of CpG ODNs, we measured nonspecific parameters, degree of lupus nephritis, proteinuria, or autoantibody, and cytokine expression in mRNA level of lymphocytes. We found that there were no significant differences among groups in nonspecific immunotoxicological profiles and in evaluation profiles of glomerulonephritis. However, titer of anti-dsDNA and anti-cardiolipin antibodies in mouse CpG ODN group rose three or eight-fold higher than in control group. Collectively, CpG ODN might be clinically less immunotoxic in terms of clinical profiles in lupus-prone NZB/NZW F1 mice, in spite of high autoantibody titer in CpG ODN treated groups.

Effects of Interferential Current Treatment on Pain, Disability, and Balance in Patients with Chronic Low Back Pain: A Randomized Controlled Study

  • PURPOSE: This study investigated the efficacy of interferential current (IFC) treatment on the improvement of pain, disability, and balance in patients with chronic nonspecific low back pain. METHODS: A double-blind randomized clinical trial was conducted with 40 patients with chronic nonspecific low back pain. The patients were randomly allocated into two groups: the IFC treatment group (n = 20) and the placebo treatment group (n = 20). The IFC group received 30 minutes of IFC treatment on the lumbar region, while the placebo group received IFC treatment without real electrical stimulation. The intervention was administered five days a week for two weeks. RESULTS: The primary outcomes of resting pain and pain during functional movement were measured by a visual analogue scale. The secondary measurements included the Oswestry disability index (ODI) for low back pain and postural sway. The measurements were performed before and after the two-week intervention period. Compared to the placebo treatment group, the IFC treatment group showed significantly greater improvement in pain during anterior trunk flexion in the standing position (p = .029), ODI (p = .039), and postural sway when subjects stood with their eyes closed (p = .010) at the end of the intervention. CONCLUSION: Our findings show that IFC treatment can improve pain, disability, and postural sway, thus, highlighting the benefits of somatosensory stimulation from IFC.

  • Colloid uptake in various hepatic conditions such as focal nodular hyperplasia, regenerating nodules in the cirrhotic liver, hamartoma, hemangioma and rarely hepatoma has been documented. Extrahepatic tumors may show colloid uptake and they include splenic hemangioma, malignant fibrous histiocytoma, breast carcinoma and Kaposi's sarcoma. The mechanism of colloid uptake in those lesions is associated with phagocytic activity in or around the tumors. We report a pancreas islet cell tumor that showed colloid uptake on $^{99m}Tc$-phytate liver scan without histologic evidence of phagocytosis by tumor cells or infiltration of phagocytes in the tumor Microscopically the tumor was highly vascular and showed diffuse hemorrhage throughtout the tumor. We postulated that extravasation of the colloid into the tumor insterstitium caused nonspecific colloid uptake in this tumor. It is expected that hemorrhagic tumor may show nonspecific colloid uptake without phagocytosis in or about the lesion.

Toxic Effects on the Nonspecific Immune System of the Rock Bream Oplegnathus fasciatus upon Exposure to Di-2-ethylhexyl Phthalate

  • The aim of this study was to investigate the in vivo toxicity of di-2-ethylhexyl phthalate (DEHP), on the immune system of the rock bream, Oplegnathus fasciatus. Fish were injected twice intraperitoneally with DEHP (200, 400, and 800 mg/kg BW), and the cellularity and functional activity of phagocytes in the spleen and head kidney were measured. The number of head kidney leukocyte cells was significantly greater in fish treated with 800-mg DEHP/kg BW. Nonspecific immunity, as determined by the phagocytic index, was significantly decreased at 800-mg DEHP/kg BW in the head kidney. A significant reduction in phagocytic capacity was observed in the head kidney at ${\geq}$400-mg DEHP/kg BW. Furthermore, significantly increased levels of serum glutamic oxaloacetate transaminase and glutamic pyruvate transaminase indicated a marked hepatic dysfunction in immunosuppressed fish. Total serum protein was significantly reduced at ${\geq}$400-mg DEHP/kg BW; however, there were no significant changes in lysozyme activity. These results demonstrate that immune responses in the rock bream, Oplegnathus fasciatus can predict immunotoxicity at doses ${\geq}$400-mg DEHP/kg BW.

  • This study was divided into two parts. In the first experiment, the in vitro antimicrobial effect was tested in order to evaluate the effect of vapors, and the effectiveness of the nonspecific endodontic medicaments (formocresol, camphorated parachlorophenol and eugenol). In the second experiment, the intracanal effect was tested in vitro under simulated clinical condition. The actual bactericidal effect of the nonspecific endodontic medicaments (formocresol, camphorated parachlorophenol and eugenol) was quantitated. The results were obtained as follows: 1. The zone of inhibition was appeared on the vapors of formocresol only, however there were no zone of inhibition appeared on the vapors of camphorated parachlorophenol and eugenol. 2. Formocresol produced the widest zone of inhibition and eugenol, the next and camphorated parachlorophenol, the narrowest. 3. All of the tested medicaments were vaporized in the root canal. They proved to be the effective antimicrobial activity in the root canal. 4. All of the tested medicaments were showed more bactericidal effect at 72 hours than 48 hours. 5. In comparing with the bactericidal effect of the tested medicaments in the root canal, formocresol was showed the most bactericidal medicament, camphorated parachlorophenol was showed the least. 6. Complete sterilization of the root canal was not achieved in any medicaments applied in this study.

Inhibitory Effects of Chicken Egg Yolk Antibody on Infection of Escherichia coli in Macrophage

  • The present study evaluated the potential use of immunoglobulin prepared from egg yolk of chickens immunized with Escherichia coli K88 (IgY-Ec) in the control of E. coli K88 infection in RAW 264.7 murine macrophage. The binding activity of IgY-Ec against E. coli K88 surface protein was more specific and increased than control IgY. In infection assay of E. coli in macrophage, the specific IgY-Ec to E. coli K88 remarkably inhibited the phagocytic activity comparing to nonspecific IgY (p<0.001). In adherence assay, bacterial adhesion on macrophage cells was definitely reduced by preincubation of IgY-Ec compared with nonspecific IgY (p<0.05). These findings suggested that IgY-Ec have the protective effects against pathogens and IgY-based diets may have potential benefits for preventing or treating various infections in domestic animals.
