비소세포성 폐암에서 수술 후의 국소 제어효과 (The Local Effect after Surgery in Non-small Cell Lung Cancer)

  • 배경: 암에 대한 수술적 치료의 치근의 발전은 임상적으로 가장 혁신적인 발전 중의 하나이다. 저자들은 비소세포성 폐암환자에서 시행한 수술적 치료의 효과를 검토해 보았다. 대상 및 방법: 1995년부터 2005년까지 저자들이 수술적 치험한 증례 중 완전 절제가 시행되었던 폐암환자 432예를 대상으로 하여 임상적 소견들과 수술방법 그리고 재발 양상 등에 대하여 분석하였다. 결과: 폐엽절제술이 가장 많이 시행되었으며(66.7%),소매 폐엽절제술(5.6%)이 가장 적게 시행되었다. 179예(42.6%)에서 관찰기간 중 재발이 확인되었으며, 국소 재발 예는 67예(16.0%)로서 전신적 재발 112예(26.7%)보다 현저히 작았다. 주된 국소 재발 부위는 폐문부(37.3%)와 동측 종격동(25.4%)이었으며, 병원 사망률은 2.8% (12/432예)였고 수술 유병률은 12.5% (52/432예)였다. 결론: 낮은 사망률과 국소 재발률을 얻었으며, 따라서 저자들의 수술적 치료방법은 안전하며 유효한 방법이라고 생각되었다.

근관 충전제의 과충전에 의한 하치조 신경손상에 관한 비수술적 치료 증례 (Case Report : Non-surgical Treatment of Inferior Alveolar Nerve Injury as a Result of Overinstrumented Root Canal Treatment)

  • 근관치료 중 수동 또는 엔진구동파일을 사용하다 과도한 기구 조작을 하게 되면, 근관충전제, 드레싱제 그리고 근관세척제가 근관에서 나와 하악관으로 확산될 수 있다. 이 때 환자는 통증, 지각과민, 지각감퇴, 무감각, 이상감각 등을 호소하게 될 것이다. 이런 문제들은 근관충전제에 포함되어 있으면서 생체적합성이 떨어지는 물질들에 의한 비가역적인 손상을 피하기 위해 가능한 빨리 해결되어야 한다. 비록 근관치료와 관련하여 발생한 하치조신경의 손상을 치료하는 진료지침이 비교연구 되어 있는 것이 없으나, 이 합병증에 대한 통상의 치료는 통증과 염증을 조절하는 것이고, 가능하다면 수술적인 치료로 근원을 처치하는 것이다. 그러나, 비수술적인 치료로 통증을 완전히 개선하거나 감소시키고 또는 감각이상을 치료하는 것이 보고되어왔다. 가바펜틴(gabapentin) 또는 프리가발린(pregabalin) 같은 항간질제는 신경병증 통증의 치료에 이용되어 왔다. 이번 논문에서는 하악 우측 제 2대구치의 근관치료 후의 하치조신경의 손상과 이에 대해 프레드니솔론과 가바펜틴으로 비외과적 치료를 시행한 것과 임상적으로 신경감각검사와 신경전류인지역치검사(Neurometer)를 통해 경과관찰을 시행하였다.

Long-term Surgical Outcomes in Oligometastatic Non-small Cell Lung Cancer: A Single-Center Study

  • Background: We reviewed the clinical outcomes of patients with oligometastatic (OM) non-small cell lung cancer (NSCLC) who received multimodal therapy including lung surgery. Methods: We retrospectively analyzed 117 patients with OM NSCLC who underwent complete resection of the primary tumor from 2014 to 2017. Results: The median follow-up duration was 2.91 years (95% confidence interval, 1.48-5.84 years). The patients included 73 men (62.4%), and 76 patients (64.9%) were under the age of 65 years. Based on histology, 97 adenocarcinomas and 14 squamous cell carcinomas were included. Biomarker analysis revealed that 53 patients tested positive for epidermal growth factor receptor, anaplastic lymphoma kinase, or ROS1 mutations, while 36 patients tested negative. Metastases were detected in the brain in 74 patients, the adrenal glands in 12 patients, bone in 5 patients, vertebrae in 4 patients, and other locations in 12 patients. Radiation therapy for organ metastasis was performed in 81 patients and surgical resection in 27 patients. The 1-year overall survival (OS) rate in these patients was 82.8%, and the 3- and 5-year OS rates were 52.6% and 37.2%, respectively. Patients with positive biomarker test results had 1-, 3-, and 5-year OS rates of 98%, 64%, and 42.7%, respectively. These patients had better OS than those with negative biomarker test results (p=0.031). Patients aged ≤65 years and those with pT1-2 cancers also showed better survival (both p=0.008). Conclusion: Surgical resection of primary lung cancer is a viable treatment option for selected patients with OM NSCLC in the context of multimodal therapy.

Risk Factor Analysis of Morbidity and 90-Day Mortality of Curative Resection in Patients with Stage IIIA-N2 Non-Small Cell Lung Cancer after Induction Concurrent Chemoradiation Therapy

  • Background: Major pulmonary resection after neoadjuvant concurrent chemoradiation therapy (nCCRT) is associated with a substantial risk of postoperative complications. This study investigated postoperative complications and associated risk factors to facilitate the selection of suitable surgical candidates following nCCRT in stage IIIA-N2 non-small cell lung cancer (NSCLC). Methods: We conducted a retrospective analysis of patients diagnosed with clinical stage IIIA-N2 NSCLC who underwent surgical resection following nCCRT between 1997 and 2013. Perioperative characteristics and clinical factors associated with morbidity and mortality were analyzed using univariable and multivariable logistic regression. Results: A total of 574 patients underwent major lung resection after induction CCRT. Thirty-day and 90-day postoperative mortality occurred in 8 patients (1.4%) and 41 patients (7.1%), respectively. Acute respiratory distress syndrome (n=6, 4.5%) was the primary cause of in-hospital mortality. Morbidity occurred in 199 patients (34.7%). Multivariable analysis identified significant predictors of morbidity, including patient age exceeding 70 years (odds ratio [OR], 1.8; p=0.04), low body mass index (OR, 2.6; p=0.02), and pneumonectomy (OR, 1.8; p=0.03). Patient age over 70 years (OR, 1.8; p=0.02) and pneumonectomy (OR, 3.26; p<0.01) were independent predictors of mortality in the multivariable analysis. Conclusion: In conclusion, the surgical outcomes following nCCRT are less favorable for individuals aged over 70 years or those undergoing pneumonectomy. Special attention is warranted for these patients due to their heightened risks of respiratory complications. In high-risk patients, such as elderly patients with decreased lung function, alternative treatment options like definitive CCRT should be considered instead of surgical resection.

A comparative study of immediate wound healing complications following cleft lip repair using either absorbable or non-absorbable skin sutures

  • Objectives: The aim of the study was to compare wound healing complications following the use of either absorbable or non-absorbable sutures for skin closure in cleft lip repair. Materials and Methods: This was a randomized controlled trial conducted at the Department of Oral and Maxillofacial Surgery, Lagos University Teaching Hospital, Idi Araba, Lagos State, Nigeria. Sixty subjects who required either primary or secondary cleft lip repair and satisfied all the inclusion criteria were recruited and randomized into two groups (Vicryl group or Nylon group). The surgical wounds in all subjects were examined on 3rd, 7th, and 14th postoperative days (POD) for presence or absence of tissue reactivity, wound dehiscence, and local wound infection. Results: Hemorrhage, tissue reactivity, wound dehiscence, and local wound infection were identified as wound healing complications following cleft lip repair. The incidence of postoperative wound healing complications on POD3 was 33.3%. Tissue reactivity was more common throughout the evaluation period with the use of an absorbable (Vicryl) suture compared to a non-absorbable (Nylon) suture, although the difference was statistically significant only on POD7 (P=0.002). There were no significant differences in the incidences of wound dehiscence and infection between the two groups throughout the observation period. Conclusion: There were no statistically significant differences in the incidences of wound dehiscence and surgical site wound infection following the use of either Vicryl or Nylon for skin closure during cleft lip repair. However, more cases of tissue reactivity were recorded in the Vicryl group than in the Nylon group on POD7. Particular attention must be paid to detect the occurrence of wound healing complications, most especially tissue reactivity, whenever a Vicryl suture is used for skin closure in cleft lip repair.

치주큐렛 파절에 대한 연구 (Study on broken periodontal curets)

  • Purpose: The purpose of this study was to investigate the incidence of curet fracture and its contributing factors. Material and Methods: Fifty-eight periodontal curets which were broken during periodontal treatment in Kangnung National University Dental Hospital for 1 year were used as study materials. The blade thickness of new curets and broken ones was measured using a digital micrometer. Types of treatment procedures, clinical experience of operators, point of breakage, and method of removal of broken fragments were recorded for each broken curet. Results: The incidence of curet fracture in root planing (16.4 curets per 1,000 procedures) was higher than those in flap surgery (7.5) or supragingival scaling (2.7). No curet was broken during supportive periodontal treatment. The incidence of fracture did not seem to be related with clinical experience of operators. The most frequent breakage point of the curets were upper 1/3 of blades. Fifty-six of 58 broken fragments were removed by non-surgical methods. Two broken tips which could not removed non-surgically were left in the pockets, and proved to be removed spontaneously 1 week later. Conclusion: Root planing showed higher incidence of curet fracture than any other type of periodontal treatment. Most of the fractured fragments were removed by non-surgical method. Further study is needed to develop methods of removal of the fragments which can not be removed non-surgically.

Does platelet-rich fibrin increase bone regeneration in mandibular third molar extraction sockets?

  • Objectives: This study determined the effect of platelet-rich fibrin (PRF) on extraction socket bone regeneration and assessed the patterns and determinants of bone regeneration after the surgical extraction of impacted mandibular third molars. Materials and Methods: This prospective study randomly allocated 90 patients into two treatment groups: A PRF group (intervention group) and a non-PRF group (control group). After surgical extractions, the PRF group had PRF placed in the extraction socket and the socket was sutured, while the socket was only sutured in the non-PRF group. At postoperative weeks 1, 4, 8, and 12, periapical radiographs were obtained and HLImage software was used to determine the region of newly formed bone (RNFB) and the pattern of bone formation. The determinants of bone regeneration were assessed. Statistical significance was set at P<0.05. Results: The percentage RNFB (RNFB%) was not significantly higher in the PRF group when compared with the non-PRF group at postoperative weeks 1, 4, 8, and 12 (P=0.188, 0.155, 0.132, and 0.219, respectively). Within the non-PRF group, the middle third consistently exhibited the highest bone formation while the least amount of bone formation was consistently observed in the cervical third. In the PRF group, the middle third had the highest bone formation, while bone formation at the apical third was smaller compared to the cervical third at the 8th week with this difference widening at the 12th week. The sex of the patient, type of impaction, and duration of surgery was significantly associated with percentage bone formation (P=0.041, 0.043, and 0.018, respectively). Conclusion: Placement of PRF in extraction sockets increased socket bone regeneration. However, this finding was not statistically significant. The patient's sex, type of impaction, and duration of surgery significantly influenced the percentage of bone formation.

  • To understand the current status of pediatric surgical practice of the members of the Korean Association of Pediatric Surgeons, a survey of the practice of the 31 members in both 1994 and 1995. Twenty five members(80%) representing 20 hospitals responded. An average of four hundred and seventy four cases pediatric surgical operations were performed at individual institution in 1995 with 40 newborn cases. Eighteen members(72%) are currently working at university hospital. Fourteen institutions(70%) are currently classified as tertiary by the health insurance agency. The majority(15 to 60%) of members are working in the metropolitan Seoul area, while five in Taegu area. Sixteen members reported having the title of department head/director. Four members reported occasional non-pediatric surgical practice. Nine members out of 20 reported having independent pediatric surgical out-patient clinic before the establishment of the association(1985). Eight out of 15 members reported being appointed chief of pediatric surgery before 1985. In 20 institutions, 34 full time physicians(27 members, reportedly) are working in pediatric surgery. In regarding to pediatric surgical training, 16 members(64%) received an average of 16 months of training abroad, 5 members trained at home and abroad, and 4 from only at home. There are no differences in length of training periods in these groups. Twenty one members received their basic pediatric surgical training before 1985, the year of inauguration of the association. Twelve members received post-pediatric surgery refresher courses averaging 11 months' duration, after 2-11 years. Thirteen participants of this study belongs to the founding members of the association.

구강외과영역의 염증성질환에 대한 PROFENID (Ketoprofen)의 치험성적에 관하여 (CLINICAL STUDY ON THE EFFECT OF PROFENID (KETOPROFEN) TO THE INFLAMMATORY DISEASES)

  • We selected 100 patients necessitating the surgical or non-surgical treatment among the patients who visited our hospital. We administered the newly developed non-steroid, PROFENID capsule (25mg per capsule), to the selected patients Via per OS and obtained the following results. 1) Cases of 56% at about 15 minutes after the administration of Profenid capsules, 35% at about 30 minutes after the administration of Profenid capsules, 35% at about 30 minutes and 6% at 60 minutes or more presented the initial effects of that drug respectively. 2) In the patients with the comparatively heavy swelling, the appearances of the effect of that drug were relatively delayed. 3) Sexual difference was not seen in that drug effect. 4) Untoward effects of that drug seemed to be not found in this case study.
