Non-invasive different modalities of treatment for temporomandibular disorders: review of literature

  • Temporomandibular disorders (TMDs) are diseases that affect the temporomandibular joint and supporting structures. The goal of treatment for TMDs is elimination or reduction of pain and return to normal temporomandibular joint function. Initial treatment for TMDs is non-invasive and conservative, not surgical. Oral and maxillofacial surgeons should fully understand and actively care about non-invasive treatments for TMDs. The purpose of this study is to review the validity and outcomes of non-invasive and surgical treatment modalities for TMDs.

  • Cystitis and proctitis are defined as inflammation of bladder and rectum respectively. Haemorrhagic cystitis is the most severe clinical manifestation of radiation and chemical cystitis. Radiation proctitis and cystitis are major complications following radiotherapy. Prevention of radiation-induced haemorrhagic cystitis has been investigated using various oral agents with minimal benefit. Bladder irrigation remains the most frequently adopted modality followed by intra-vesical instillation of alum or formalin. In intractable cases, surgical intervention is required in the form of diversion ureterostomy or cystectomy. Proctitis is more common in even low dose ranges but is self-limiting and improves on treatment interruption. However, treatment of radiation proctitis is broadly non-invasive or invasive. Non-invasive treatment consists of non-steroid anti-inflammatory drugs (NSAIDs), anti-oxidants, sucralfate, short chain fatty acids and hyperbaric oxygen. Invasive treatment consists of ablative procedures like formalin application, endoscopic YAG laser coagulation or argon plasma coagulation and surgery as a last resort.

수면장애에서 비침습적 뇌자극술의 치료 효과 고찰: 경두개자기자극술과 경두개직류전기자극술을 중심으로 (A Review on Effects of Non-Invasive Brain Stimulation in the Treatment of Sleep Disorders)

  • Sleep disorders, increasingly prevalent in the general population, induce impairment in daytime functioning and other clinical problems. As changes in cortical excitability have been reported as potential pathophysiological mechanisms underlying sleep disorders, multiple studies have explored clinical effects of modulating cortical excitability through non-invasive brain stimulation in treating sleep disorders. In this study, we critically reviewed clinical studies using non-invasive brain stimulation, particularly transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), for treatment of sleep disorders. Previous studies have reported inconsistent therapeutic effects of TMS and tDCS for various kinds of sleep disorders. Specifically, low-frequency repetitive TMS (rTMS) and cathodal tDCS, both of which exert an inhibitory effect on cortical excitability, have shown inconsistent therapeutic effects for insomnia. On the other hand, high-frequency rTMS and anodal tDCS, both of which facilitate cortical excitability, have improved the symptoms of hypersomnia. In studies of restless legs syndrome, high-frequency rTMS and anodal tDCS induced inconsistent therapeutic effects. Single TMS and rTMS have shown differential therapeutic effects for obstructive sleep apnea. These inconsistent findings indicate that the distinctive characteristics of each non-invasive brain stimulation method and specific pathophysiological mechanisms underlying particular sleep disorders should be considered in an integrated manner for treatment of various sleep disorders. Future studies are needed to provide optimized TMS and tDCS protocols for each sleep disorder, considering distinctive effects of non-invasive brain stimulation and pathophysiology of each sleep disorder.

A Study on the Effectiveness of Skin Care Solution System using Non-Invasive Air Technology

  • The effectiveness of an innovative skin treatment system that delivers an anti-aging solution deep into the skin without invasiveness and pain using a non-invasive air technology was investigated. In addition, an effective change using a non-invasive technique for delivering a solution for skin improvement was confirmed. The equipment named Cellre Jet is an effective skin care and drug delivery equipment that instantly opens the skin epidermis by using a maximum output pressure of 6 bars and high-pressure purified oxygen of 75-90% purity to deliver various nano-sized vital substances deep into the skin, and it uses the method of precisely controlling the equipment through an 8-inch digital touch display to accurately dispense the prescribed dosage. In this study, changes in skin condition were analyzed using this equipment and nano ampoules on subjects with actual skin problems through a related comparison and effectiveness judgment program. Through this study, skin care and drug delivery are possible, which will contribute to verifying the effectiveness of this non-invasive drug delivery equipment in the future, and is expected to establish the systematic effect in observing and studying changes in the skin.

체외강력집속초음파치료 (Extracorporeal High Intensity Focused Ultrasound Therapy)

  • 의학의 발달과 더불어 종양에 대한 국소치료는 광범위근치술(extended radical surgery) 로부터 기능보존술(function preserving surgery)로 발달하고 이것은 다시 최소침습술(minimal-invasive surgery)로 발전하여 왔다. 그러나 최근 들어서는 기술의 혁신적인 진보로 비침습적수술법(non-invasive surgery)이 개발되어 감마나이프(gamma knife), 사이버나이프(cyber knife), 및 하이푸나이프(HIFU knife) 등이 출현하게 되었다. 본 논문에서는 이들 중 하이푸나이프를 이용한 체외강력집속초음파치료(extracorporeal high intensity focused ultrasound therapy)에 대하여 발달사, 치료기 구조 및 치료과정, 조직학적 변화와 기전, 임상적용, 장단점 및 전망 등을 살펴보고자 한다.

흉선종의 외과적 치료 (Surgical Treatment of Thymoma)

  • We experenced 18 patients with surgically treated thymoma from January 1986 to December 1993. There were 13 male and 5 female ranged from 23 to 69 years of age. Among them Myasthenia gravis was present in 8 patients (44%) The predominant cell type was lymphocytic(11 patients), followed by epithelial (3) and mixed (4), and had no value in predicting prognosis. Treatment consisted of complete resection in 15 patients, partial resection in 2 patients and 1 patient was performed biopsy. Only, and then adjuvant radiation therapy was done in 7 patients and 3 patients needed adjuvant chemotherapy. Invasion of the adjacent tissue in thymoma was the most improtant prognostic value. There were 6 non-invasive tumors and 12 invasive tumors. Two patients with invasive thymomas resulted in death and one of 6 patients with non-invasive thymomas died during follow up ranged from 25 day to 60 months. The causes of death were myasthenic crisis in 1 patient, C. N. S. problem in 1 patient and pulmonary & mediastinal metastasis in 1 patient.

  • 1. Purpose : The present study was carried out to evaluate the effects of simultaneous treatment with acupuncture, herb medication and non-invasive laser irradiation of blood vessel on the pain control of primary headache. 2. Methods : 30 outpatients who visited in the oriental medical hospital of Kyungsan University from April 10, 2001 to Oct. 10, 2001, based on symptoms and the results of pulse diagnosis, stress test(ABR-2000) and iridology test, were observed on the pain control of primary headache like tension or stress-induced headache. 1)Acupuncture and herbal medicine : Acupuncture treatment and herb medication widely used for headache were carried out simultaneously. 2)Non-invasive laser irradiation : Laser irradiation was undertaken on brachial vein with Lapex-2000 for 30minutes a day for 5 to 15days. 3. Result and Conclusion: In patients with headache, triglyceride mean values decreased from $168.57{\pm}26.90mg/dL$ to $154.23{\pm}28.66mg/dL$ and total cholesterol mean values decreased from $202.23{\pm}22.17mg/dL$ to $194.57{\pm}19.32mg/dL$, after simultaneous treatment with acupuncture, herb medication and non-invasive laser irradiation of blood vessel. And these results suggest that these simultaneous treatment has significant effects in tension and stress-induced headache.: Experiment subjects considered as shown the validity in the headache estimated 80%.

IGRT를 위한 비침습적인 호흡에 의한 장기 움직임 실시간 추적시스템 (A Non-invasive Real-time Respiratory Organ Motion Tracking System for Image Guided Radio-Therapy)

  • A non-invasive respiratory gated radiotherapy system like those based on external anatomic motion gives better comfortableness to patients than invasive system on treatment. However, higher correlation between the external and internal anatomic motion is required to increase the effectiveness of non-invasive respiratory gated radiotherapy. Both of invasive and non-invasive methods need to track the internal anatomy with the higher precision and rapid response. Especially, the non-invasive method has more difficulty to track the target position successively because of using only image processing. So we developed the system to track the motion for a non-invasive respiratory gated system to accurately find the dynamic position of internal structures such as the diaphragm and tumor. The respiratory organ motion tracking apparatus consists of an image capture board, a fluoroscopy system and a processing computer. After the image board grabs the motion of internal anatomy through the fluoroscopy system, the computer acquires the organ motion tracking data by image processing without any additional physical markers. The patients breathe freely without any forced breath control and coaching, when this experiment was performed. The developed pattern-recognition software could extract the target motion signal in real-time from the acquired fluoroscopic images. The range of mean deviations between the real and acquired target positions was measured for some sample structures in an anatomical model phantom. The mean and max deviation between the real and acquired positions were less than 1mm and 2mm respectively with the standardized movement using a moving stage and an anatomical model phantom. Under the real human body, the mean and maximum distance of the peak to trough was measured 23.5mm and 55.1mm respectively for 13 patients' diaphragm motion. The acquired respiration profile showed that human expiration period was longer than the inspiration period. The above results could be applied to respiratory-gated radiotherapy.

Temporomandibular joint dislocation: experiences from Zaria, Nigeria

  • Objectives: Dislocation of the temporomandibular joint may occur for various reasons. Although different invasive methods have been advocated for its treatment, this study highlights the value of non-invasive treatment options even in chronic cases in a resource-poor environment. Materials and Methods: A seven-year retrospective analysis of all patients managed for temporomandibular joint dislocation in our department was undertaken. Patient demographics, risk factors associated with temporomandibular joint dislocation and treatment modalities were retrieved from patient records. Results: In all, 26 patients were managed over a seven-year period. Males accounted for 62% of the patients, and yawning was the most frequent etiological factor. Conservative treatment methods were used successfully in 86.4% of the patients managed. Two (66.7%) of the three patients who needed surgical treatment developed complications, while only one (5.3%) patient who was managed conservatively developed complications. Conclusion: Temporomandibular joint dislocation appears to be associated with male sex, middle age, yawning, and low socio-economic status, although these observed relationships were not statistically significant. Non-invasive methods remain an effective treatment option in this environment in view of the low socio-economic status of the patients affected.

목 및 가슴신경뿌리병증의 임상적 고찰: 비침습적 중재시술치료 (Clinical Characteristics of Cervical and Thoracic Radiculopathies: Non-Invasive Interventional Therapy)

  • Cervical and thoracic radiculopathies are among the most common causes of neck pain. The most common causes are cervical disc herniation and cervical spondylosis in patients with cervical radiculopathy, and diabetes mellitus and thoracic disc herniation in thoracic radiculopathy. A thorough history, physical examination, and testing that includes electrodiagnostic examination and imaging studies may distinguish radiculopathy from other pain sources. Although various electrodiagnostic examinations may help evaluate radiculopathy, needle electromyography is the most important, sensitive, and specific method. Outcome studies of conservative treatments have shown varying results and have not been well controlled or systematic. When legitimate incapacitating symptoms continue despite conservative treatment attempts, more invasive spinal procedures and intradiscal treatment may be appropriate. Surgery has been shown to have excellent clinical outcomes in patients with disc extrusion and neurological deficits. However, patients with minimal disc herniation have fair or poor surgical outcomes. In addition, conventional open disc surgery entails various inadvertent surgical related risks. Although there has not yet been a non-surgical interventional procedure developed with the therapeutic efficacy of open surgery, conservative procedures can offer substantial benefits, are less invasive, and avoid surgical complications. While more invasive procedures may be appropriate when conservative treatment fails, prospective studies evaluating cervical and thoracic radiculopathies treatment options would help guide practitioners toward optimally cost-effective patient evaluation and care.
