당뇨환자의 치은조직내 Superoxide Dismutase와 Catalase의 활성도에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON SUPEROXIDE DISMUTASE- AND CATALASE- ACTIVITY IN GINGIVAL TISSUES IN DIABETIC PATIENTS)

  • Oxygen derived radicals($O_2\;^-$, $H_2O_2$, and $OH^-$) are thought to play a role in a lot of human diseases. And it has been believed that antioxidant enzymes such as superoxide dismutase(SOD) and catalase could protect the tissues from damage resulting from the oxygen derived free radicals. The purpose of this study was performed to investigate the activity of the SOD(CuZn- and Mn-SOD) and catalase in inflammatory gingival tissues and the correlation between boold glucose level and antioxidants and age in non-insulin dependent diabetes mellitus(NI- DDM) patients. For this study, the patients were classified into normal, inflammatory, and diabetic, and ten their papillary bleeding index(PBI) and gingival index were checked. Subjects consisted of 11 healthy patients with no inflammatroy gingiva, 20 adult periodontitis patients, and 8 diabetic patients, aged 33 to 66(average: 44.62). The blood glucose level of diabetic group was ranged from 120ml/dl to 160ml/dl(physical status 0 : averge : 135.67ml/dl). Gingival tissues were surgically obtained from the patients during periodontal surgery, extraction, and clinical corwn lenghening procedure. The activity of CuZn and Mn- SOD and catalase in the gingival tissues was measured by using UV-spectrophotometer by the same methods that Crapo et al. And Aebi did, respectively. The results were as follows : 1. The Mn-SOD activity was significantly lower in inflammatory group in comparison to normal group(P<0.05), and the activities of antioxidants in diabetic group were not significant in comparison to normal inflammatory group(P>0.05). 2. The activities of antioxidants showed little variation among individuals of different ages (P>0.05). 3. The higher blood glucose level was, the higher gingival index was(P<0.05). 4. There was no correlation between blood glucoe level and activity of antioxidant in inflammatory gingival tissues of NIDDM patients(P>0.05). In conclusion, these results, within the limits of the present experiment, suggest that the activity of Mn-SOD might reflect the inflammatory status of gingival tissue, and the activity of antioxidants was independent of blood glucose level of diabetic patients in physical status 0.

제2형 당뇨병 환자 식생활습관 및 보건소 영양교육 후 혈당개선효과 (Older Adults with Type 2 Diabetes Improve Glycemic Control after Nutritional Education Program at the Public Health Center)

  • This study was carried out to provide information on the effect of nutrition education program for diabetic patients at the Guri City's Public Health Center. Subjects of this study were 31 persons(male 7, female 24) who attended all courses of "2002 Diabetes Education Class". They were indicated as the 'education group'(EG). Eating and living habits of EG were investigated before the education. EG's weight and blood glucose (post prandial 2 hours, PP2) were examined as well. EG's PP2 reduction was compared with a 'control group'(CG) who didn't join any course in that class. All of the subjects were non-insulin-dependent diabetes mellitus(NIDDM) patients. EG's average age was 62.4$\pm$8.8. Before taking the course, EG's PP2 was 251.5$\pm$29.6mg/dl, and body mass index(BMI) was 26.3$\pm$2.3 on average. Most of them were stressed out from their daily lives and usually had no exercise. Most people of EG ate meals rapidly and liked sweet and fatty foods. After the course of training, EG's weight and BMI before the training were not decreased significantly. However, all of the EG's PP2s, which were measured 4 times(before the meal at the special lunch session, after 2 hours at this meal, after 2 weeks and 4 weeks dietary assembly), were decreased in comparison with the PP2 which was checked prior to joining the training. EG's average PP2 was more reduced than CG's one. In addition, all groups' PP2s were decreased for 8 weeks. After all, this nutritional education at the public health center was effective in glycemic control for diabetes mellitus patients. Especially, when the dietary assembly as practical training was included in the educational process, the patient's dietary intake and PP2 was improved more effectively. Therefore, this study suggests that nutrition work at public health centers is necessary for the Health Promotion Policy.

Cellular and Molecular Roles of $\beta$ Cell Autoantigens, Macrophages and T Cells in the Pathogenesis of Automimmune Diabetes

  • Type I diabetes, also known as insulin-dependent diabetes mellitus (IDDM) results from the destruction of insulin-producing pancreatic $\beta$ cells by a progressive $\beta$ cell-specific autoimmune process. The pathogenesis of autoimmune IDDM has been extensively studied for the past two decades using animal models such as the non-obese diabetic (NOD) mouse and the Bio-Breeding (BB) rat. However, the initial events that trigger the immune responses leading to the selective destruction of the $\beta$ cells are poorly understood. It is thought that $\beta$ cell auto-antigens are involved in the triggering of $\beta$ cell-specific autoimmunity. Among a dozen putative $\beta$ cell autoantigens, glutamic acid decarboxylase (GAD) has bee proposed as perhaps the strongest candidate in both humans and the NOD mouse. In the NOD mouse, GAD, as compared with other $\beta$ cell autoantigens, provokes the earliest T cell proliferative response. The suppression of GAD expression in the $\beta$ cells results in the prevention of autoimmune diabetes in NOD mice. In addition, the major populations of cells infiltrating the iselts during the early stage of insulitis in BB rats and NOD mice are macrophages and dendritic cells. The inactivation of macrophages in NOD mice results in the prevention of T cell mediated autoimmune diabetes. Macrophages are primary contributors to the creation of the immune environment conducive to the development and activation of $\beta$cell-specific Th1-type CD4+ T cells and CD8+ cytotoxic T cells that cause autoimmune diabetes in NOD mice. CD4+ and CD8+ T cells are both believed to be important for the destruction of $\beta$ cells. These cells, as final effectors, can kill the insulin-producing $\beta$ cells by the induction of apoptosis. In addition, CD8+ cytotoxic T cells release granzyme and cytolysin (perforin), which are also toxic to $\beta$ cells. In this way, macrophages, CD4+ T cells and CD8+ T cells act synergistically to kill the $\beta$ cells in conjunction with $\beta$ cell autoantigens and MHC class I and II antigens, resulting in the onset of autoimmune type I diabetes.

당뇨교육이 당뇨환자의 식사요법에 대한 지식과 실천 및 혈당에 미치는 영향 (Effects of Diabetes Education on Diabetic Management in Non-Insulin-Dependent Diabetics Mellitus Patients)

  • The purpose of this study was to examine the effect of diabetes education on diabetic management by determining the changes of the knowledge and practice for diet therapy and blood glucose level pre-training and post-training. Statistical data analyses were completed using the SPSS 11.0 program. The results can be summarized as follows : The average age of the subjects was 52.8 years old, the period of suffering from diabetes was 7.9 years and 31% of the patients had a history of diabetes in their family members. In life style for self-management, they showed lower levels in drinking post-training, and significantly higher exercise levels post-training (P<0.05). Regarding the level of knowledge for diet, they showed significantly higher levels post-training in eight items such as importance of diet therapy for diabetes (p<0.005), principle of diet therapy (p<0.005), nutrient composition of foods (p<0.005), carbohydrate composition of foods (p<0.005), calorie prescribed to themselves (p<0.001), exchange units prescribed to themselves (p<0.005), exchange food items and exchanges units of cereal & grains (p<0.005) and exchange food items and exchanges units of fruit & juices (p<0.005). Regarding the practices of diet, they showed significantly higher levels of practice in keeping permitted meal size (p<0.005), using food exchange list (p<0.005), keeping exact meal times (p<0.001) and restricting most foods to eat (P<0.01) post-training. When measuring their bodies, average weight was lower post-training. Obesity was significantly lower post-training (p<0.01), and blood pressure both in systolic and diastolic was lower. Postcardinal-2hour blood glucose level decreased significantly from 268.4$\pm$98.9 pre-training to 180.9$\pm$48.4 post-training (p<0.001).

  • MODY는 일반적으로 제 2 형 당뇨의 특수형으로 25세 이전에 발병하며 상염색체 우성으로 유전되 며 인슐린 분비장애를 특징으로 한다. GCK와 HNF-1α의 유전자 돌연변이는 제 2 형 당뇨에서 가장 큰 원인 중에 하나이다. 따라서 이들 유전자다형성의 연관성을 다양한 분석 방법으로 연구할 필요성이 대두되고 있다. GCK와 HNF-1α 유전자의 exon과 exon에 근접한 intron을 실험하였고, 또한 promotor까지 PCR-DHPLC (Polymerase Chain Reaction - Denaturing High Performance Liquid Chromatography) 방법과 direct sequencing 방 법을 사용하였다. 시료는 11명의 환자와 20명의 정상인에서 DNA를 추출하였고 GCK와 HNF-1α의 단일 염기 다형성을 PCR-DHPLC 방법으로 확인하였다. 결과적으로 GCK 유전자에서는 1개(R135G)를 검출하였고 HNF-1α 유전자에서는 2개(I27L, S487N)를 검출하였으며 intron 8에서도 돌연변이를 검출할 수 있었다.

C57BL/KsJ-db/db 제2형 당뇨병 마우스에서 발아일품벼 추출물의 항당뇨효과 (Anti-diabetic Activity of Germinated Ilpum Rough Rice Extract Supplement in Mice)

  • 본 연구에서는 제2형 당뇨병 모델인 C57BLKS/J-db/db 마우스에서 발아일품벼 추출물의 농도별로 급여한 후 혈당 조절에 미치는 영향을 알아보고자 조사하였다. C57BLKS/J-db/db 마우스에게 급여한 발아일품벼 추출물의 양에 따라 5군 즉, 정상군(Normal Control: NC), 당뇨대조군(Diabetic Control: DC), 1% 발아벼 추출물 급여군(DM-1% GIRRE), 0.5% 발아벼 추출물 급여군(DM-0.5%GIRRE), 0.25% 발아벼 추출물 급여군(DM-0.25%GIRRE)으로 나누었다. 8주 동안 매일 식이 섭취량과 음료섭취량 및 매주 체중과 공복혈당을 측정하고 마지막 주에 경구포도당 부하검사(OGTT)를 실시하였으며, 부검 시 혈액을 채취하여 혈청인슐린 농도와 당화혈색소를 측정하였다. 그 결과, DM-1% GIRRE군에서 당뇨유발에 의한 체중감소 및 당뇨 증상 중에 다음(poly-dipsia), 다식(polyphasia) 등이 어느 정도 완화되고 있음을 알 수 있다. DM-1%GIRRE군에서 당뇨동물의 공복 혈당, 경구 내당능 검사, 혈중 HbA1c 수준을 유의적으로 낮추었지만, 혈장 인슐린 농도는 당뇨실험군들 간에는 차이가 나타나지 않았다. 이상의 결과를 종합해보면 발아일품벼추출물의 급여가 혈당상승을 억제하는 효과가 있는 것으로 생각되며 그 기전에 대해서는 향후 추가 연구가 필요하다고 본다.

식용피 밀양3호[Echinochloa esculenta (A. Braun)] 에탄올 추출물의 당뇨모델 마우스에 대 한 항당뇨 활성 (Anti-diabetic Effects of Barnyard Millet Miryang 3 [Echinochloa esculenta (A. Braun)] Grains on Blood Glucose in C57BL/KsJ-db/db Mice)

  • 잡곡의 하나인 식용피는 폴리페놀과 수용성 식이섬유성분이 풍부하고 전분함량이 타 곡류보다 낮으면서 단백질 함양이 높아 영양학적 건강지표가 높은 잡곡이다. 본 연구에서는 항염증 활성이 강하게 확인된 밀양3호를 선발하여 이의 항당뇨 활성에 관해 조사하였다. 식용피 밀양3호의 80% 에탄올(EtOH) 추출물을 당뇨모델인 C57BL/KsJ-db/db mice에 600 mg/kg로 8주간 투여하여 혈당과 혈중 지질에 대한 영향을 조사한 결과, 공복혈당이 당뇨 대조군에 비해 유의적으로 감소하여 개선 되었으며, 경구 내당성도 1주 투여 시부터 개선되는 효과를 보이다가 8주간의 투여 시는 양성대조군인(메트포민 100 mg/kg) 수준으로 경구 내당성이 개선됨을 확인하였다. 이때 체중증가는 메트포민 투여군(10.36±3.15)에는 못 미치나 8.54±2.24로 당뇨 대조군 체중 증가(8.71±1.64)의 수준이었으며, food efficiency rates를 고려하면 식용피가 식이 섭취에는 큰 영향을 주지 않는 것으로 사료되었다. 총 콜레스테롤 농도는 당뇨 대조군의 총콜레스테롤(229.3±47.8)에 비해 식용피 EtOH 추출물 투여군(160.7±7.6)과 메트포민 투여군(176.0±25.6)에서 유의적으로 감소하였다. LDL-콜레스테롤 농도는 식용피 투여군에서 메트포민 투여군 수준으로 당뇨 대조군에 비해 약 41%정도 감소한 것에 반해, 중성지방은 식용피 추출물의 영향을 받지 않았다. 간독성은 식용피 추출물군의 GOT 값이 메트포민 투여군 보다 더 낮았으며, GPT 활성도는 모든 실험군들 간에 유의한 차이가 없었다. 이상의 연구 결과는 식용피 밀양3호가 항 당뇨 관련 우수한 소재 함유 곡류로 관련 질환 예방과 치료에 활용할 수 있음을 시사한다.

Fermented Chaga Mushroom (Inonotus obliquus) Effects on Hypolipidemia and Hepatoprotection in Otsuka Long-Evans Tokushima Fatty (OLETF) Rats

  • The effects of fermented chaga mushroom (Inonotus obliquus) powder on the lipid concentrations and the activities of liver marker enzymes of serum in genetically diabetic Otsuka Long-Evans Tokushima fatty (OLETF) rats were investigated. Rats were fed a semisynthetic diet supplemented with 50 g/kg chaga mushroom powder (CM) or 50 g/kg fermented chaga mushroom powder (FCM) for 8 weeks (26 to 34 weeks of age). Nondiabetic Long-Evans Toknshima Otsuka (LETO) rats were used as age-matched nondiabetic control animals. Water consumption was significantly higher in the OLETF control than the LETO rats. Water consumption in the FCM-fed OLETF rats tended to be less than in both the OLETF control and CM-fed OLETF rats. Serum concentrations of triglycerides and total cholesterol were significantly higher in the OLETF control rats than in the LETO rats while within the OLETF rat groups, the consumption of FCM resulted in a significantly lower serum triglyceride concentration and slightly lowered serum total cholesterol concentration when compared to the OLETF control and CM-fed rats. The alanine aminotransferase (ALT) activity was significantly higher in the OLETF control than in the LETO rats, but this difference was significantly reduced compared to the CM-fed rats and essentially no difference in the ALT levels was observed between the LETO and OLETF-FCM rats. This observation suggests an adaptive effect of the fermented chaga mushroom in liver function. Livers of the LETO rats showed no histopathological changes, whereas those of the OLETF control rats were characterized by many fat depositions in the central zone of the hepatocytes. The livers of the OLETF CM-fed rats showed less fatty changes compared to the OLETF control rats and fat deposition in the hepatocytes was nearly absent. These results suggest that orally ingested fermented chaga mushroom has a potential beneficial effect on the complications known to occur in the obesity-related non-insulin dependent diabetes mellitus (NlDDM) OLETF rat.
