강직성 척추염 환자의 자기 효능과 운동지속과의 관계 (The Relationship between Self-efficacy and Exercise Adherence in Patients with Ankylosing spondylitis)

  • The purpose of this study was to determine the relationship between self-efficacy and adherence of exercise in patients with ankylosing spondylitis. The subjects for this study were the 50 patients with ankylosing spondylitis who had been diagnosed as out patients in the Rheumatism Center of one university hospital in Seoul and participated in the 8 weeks exercise program. The data were collected by a questionnaire the period from April 1 to May 2, 2000. Data were analyzed by t-test, ANOVA, Pearson's correlation coefficient, Frequencies, using the SAS program. The results were as follows: The mean duration for exercise adherence of ankylosing spondylitis was 12.2 months. The mean self-efficacy score was 74.3 in a possible range of 10 to 100. A significant difference of self-efficacy was found between adherers and non-adherers and self-efficacy of adherer was higher than that of non-adheres(t=4.25, p=.001). Self-efficacy was significantly associated with the total duration of an exercise adherence(r=.42, p=.001). These findings may indicate that the relationship between self-efficacy and exercise adherence in patients with ankylosing spondylitis was supported. From these results, it can be suggested that the relationship between self-efficacy and exercise adherence in patients with ankylosing spondylitis. Therefore, intervention is needed to promote self-efficacy of ankylosing spondylitis and futher studies should develop self-efficacy promoting programs for patients with ankylosing spondylitis.

약물 이행 모델 기반 다제약제 복용 노인의 약물 불이행과 관련 요인 (Medication Non-adherence and Related Factors of Older Adults Who Use Polypharmacy Based on Medication Adherence Model)

  • 본 연구는 약물 이행 모델을 기반으로 다제약제 복용 노인의 약물 불이행과 관련 요인을 파악하기 위해 수행되었다. 연구 대상은 서울과 경기도에 거주하는 다제약제 복용 노인 190명이었다. 연구 결과, 대상자의 43.2%가 임의로 처방된 약의 복용을 중단하는 의도적인 약물 불이행을 보였다. 의도적인 약물 불이행의 이유는 '증상이 조절된다고 느낄 때', '약을 복용하는 것이 귀찮고 불편해서', 그리고 '약을 복용하였을 때 더 나빠지는 것 같아' 순으로 나타났다. 또한, 다제약제 복용 노인은 능동적으로 약 정보의 내용 및 경로를 탐색하는 정도가 매우 낮았다. 의도적인 약물 불이행군과 의도적인 약물 불이행이 아닌 군 사이에 성별, 복용 약의 종류, 그리고 약물 부작용 경험에 따라 유의한 차이가 있었지만, 약 정보의 탐색 내용 및 경로에는 통계적으로 유의한 차이가 없었다. 본 연구를 통해, 다제약제 복용 노인의 의도적인 약물 불이행을 줄이기 위해서는 성별을 고려하고 복용 약의 종류를 확인하며, 약물 부작용 경험 여부를 사정하는 것이 중요하다. 그리고 능동적으로 약 정보의 내용 및 경로를 탐색하여 찾는 정도가 낮으므로 노인이 선호하는 경로를 통해 약 정보의 내용을 제공하여 다제약제 복용 노인의 약물 이행 의사결정 과정을 돕는 것이 필요하다.

한국인 HIV/AIDS 환자의 복약순응도에 미치는 위험 인자 연구 (Risk Factors Associated with Medication Adherence in HIV/AIDS Patients)

  • Background: Adherence to antiretroviral treatment (ART) is crucial for maintaining the HIV-RNA suppression in patients living with HIV/AIDS. This study aims to analyze the risk factors contributing to low medication adherence among individuals with HIV/AIDS by analyzing data from the Korean HIV/AIDS cohort study. Methods: The dependent variable is ART medication adherence. The depressive symptom and anxiety scores were collected as main independence variables. Covariates included gender, age, transmission route, alcohol and smoking information, and antiviral treatment regimen details. To predict the relationship between ordinal dependent variables and independent variables, an ordered logistic regression analysis was conducted, and odds ratios (OR) were calculated. Results: The results of the ordered logistic regression analysis showed that female was associated with a higher risk of low medication adherence (OR=2.91, 95% CI=1.08, 7.83). Among the subjects who were non-smokers and non-drinkers, the risk of low medication adherence was lower (OR=0.36, 95% CI=0.18, 0.70). Depending on the ART treatment group, individuals taking integrase inhibitor had a lower risk of medication adherence (OR=0.31, 95% CI=0.13, 0.76), and those experiencing depressive symptoms were related with a higher risk of low medication adherence (OR=1.97, 95% CI=1.12, 3.46). Conclusions: The encouragement and emotional support of healthcare professionals are essential for patients living with HIV/AIDS who experience depressive symptoms to maintain ART adherence. Additionally, further research is needed to ensure that HIV/AIDS infected female with concurrent depressive symptoms can achieve appropriate ART therapeutic effect.

  • Tuberculosis (TB) is a significant public health concern. Globally, TB is among the top 10 and the leading cause of death due to a single infectious agent. Providing standard anti-TB therapy for at least 6 months is recommended as one of the crucial strategies to control the TB epidemic. However, the long duration of TB treatment raised the issue of non-adherence. Non-adherence to TB therapy could negatively affect clinical and public health outcomes. Thus, directly observed therapy (DOT) has been introduced as a standard strategy to improve anti-TB medication adherence. Nonetheless, the DOT approach has been criticized due to inconvenience, stigma, reduced economic productivity, and reduced quality of life, which ultimately could complicate adherence issues. Apart from that, its effectiveness in improving anti-TB adherence is debatable. Therefore, digital technology could be an essential tool to enhance the implementation of DOT. Incorporating the health belief model (HBM) into digital technology can further increase its effectiveness in changing behavior and improving medication adherence. This article aimed to review the latest evidence regarding TB medication non-adherence, its associated factors, DOT's efficacy and its alternatives, and the use of digital technology and HBM in improving medication adherence. This paper used the narrative review methodology to analyze related articles to address the study objectives. Conventional DOT has several disadvantages in TB management. Integrating HBM in digital technology development is potentially effective in improving medication adherence. Digital technology provides an opportunity to improve medication adherence to overcome various issues related to DOT implementation.

만성질환자의 상용치료원 이용과 복약순응도 간의 관계 (The Association between having a Usual Source of Care and Adherence to Medicines in Patients with Chronic Diseases)

  • Objective: This study was to explore the association between having a usual source of care and adherence to medicines in patient with chronic diseases. Methods: The 2012 Korea Health Panel was used as a data source. We analyzed 4,418 respondents that were diagnosed with chronic diseases and utilized health care services. Non-adherence to medication, a dependent variable, was defined as "not taking the medicines that were prescribed for treating chronic disease" or "not following the direction for medication". Whether having a usual source of care or not was used as a key independent variable, which was defined as having a regular site or a regular doctor for medical test, treatment, and consultation. Sex, age, education level, marital status, income, the type of health insurance, the number of chronic disease and CCI (Charlson Comorbidity Index) were included as covariates in the analysis. We conducted a multivariate logistic regression. Results: Totally, 30 percent of respondents reported to experience non-adherence to medication. Having a usual source of care was significantly associated with lower non-adherence to medication regardless its type, which is a regular doctor (OR=0.61, 95% CI=0.53-0.70) or a regular site (OR=0.67, 95% CI=0.58-0.78). Furthermore, having a usual source of care was associated with both of medication persistence (OR=0.66, 95% CI=0.54-0.81) and compliance (OR=0.65, 95% CI=0.56-0.76). Conclusion: Our results showed the possibility that usual source of care is able to conduct a positive role in improving adherence to medication with better management of chronic disease.

만성 관절염 환자의 자기효능과 수중 운동지속과의 관계 (The Relationship between Self-efficacy and Aquatic Exercise Adherence in Patients with Chronic Arthritis)

  • The purpose of this study was to identify the relationship between self-efficacy and adherence of aquatic exercise in patients with chronic arthritis. Subjects were 54 patients with rheumatoid arthritis and osteoarthritis who had participated in the 6-week aquatic exercise program. The results indicated that the relationship between self-efficacy and aquatic exercise adherence in patients with chronic arthritis was statistically significant. A significant difference of self-efficacy was found between adherers and non-adherers and self-efficacy of adherer was higher than that of non-adherer(t=5.21, p=.000). Self-efficacy was significantly associated with the total duration of an aquatic exercise adherence(r=.44, p=.001). Based on these results, self-efficacy was the important factor which affect adherence of aquatic exercise in patients with chronic arthritis. Further study was suggested to identify the relationships among self-efficacy, exercise barrier and other possible factors.

  • Background: This study was conducted to evaluate the factors affecting adherence in patients with hypertension and type 2 diabetes mellitus before and after a clinic based patient incentive program in Incheon. Methods: An observational follow-up study was done for 28,355 patients in one registered group and 245,598 patients in a non-registered group from March 16th 2009 to December 31th 2010 in Incheon. The registration, mandatory laboratory tests and number of clinic visits were collected by merging the Incheon Chronic Disease Management System data and the National Health Insurance Corporation (NHIC) data. As a measure of patient adherence, we used a variable of prescription days from the NHIC and defined above 80% of average prescription days as an appropriate patient adherence. Repeated measures analysis of variance and logistic regression were used to analyze the differences in patient adherence and factors affecting adherence. Results: The changes in prescription days for the registered group are larger than for the non-registered group. In the logistic regression model, including the variables with sex, age, income status and number of clinic visits, the registered group exhibited a higher Odds ratio in the patient adherence. Conclusion: This study revealed the association between registration and appropriate patient adherence in patients with hypertension or type 2 diabetes mellitus.

Cancer Screening Adherence of Asian Women According to Biochemically-verified Smoking Status: Korea National Health and Nutrition Examination Survey

  • Background: Men and women who smoke tend to show less compliance to screening guidelines than non-smokers. However, a recent study in Korea showed that self-reported female smokers constituted less than half of cotinine-verified smokers. Therefore, the aim of this study was to identify hidden smokers using cotinine-verified method and examine cancer screening behavior according to biochemically verified smoking status. Materials and Methods: Among 5,584 women aged 30 years and older who participated in the Fourth and Fifth Korea National Health and Nutrition Examination Survey (KNHANES), 372 (6.66%) hidden smokers were identified based on interview responses and verified by urinary cotinine levels. We compared cancer-screening behavior (cervical, breast, stomach, and colon cancer) of female hidden smokers to that of non-smokers and selfreported smokers by cross-sectional analysis. Results: Hidden female smokers had significantly lower adherence to breast cancer screening compared to non-smokers (aOR (adjusted odds ratio) [95% CI] = 0.71 [0.51-0.98]). Adherence to stomach cancer (aOR [95% CI] = 0.75 [0.54-1.03]) and cervical cancer (aOR [95% CI] = 0.85 [0.66-1.10]) screening was also lower among hidden female smokers compared to non-smokers. Self-reported (current) smokers showed lowest adherence to cervical cancer (aOR: 0.64, 95% CI0.47-0.87), breast cancer (0.47 [0.32-0.68]), stomach cancer (0.66[0.46-0.95]), and colon cancer (0.62 [0.38-1.01]) screening compared to non-smokers, followed by female hidden smokers, then non-smokers. These lower adherence rates of current smokers were attenuated after we incorporated hidden smokers into the current smoker group. Conclusions: Cancer screening adherence of female hidden smokers was lower than cotinine-verified non-smokers but higher than current smokers. Considering the risk of smoking-related cancer among women, identifying hidden smokers is important to encourage appropriate cancer screening.

  • Objectives: To identify the factors affecting long-term adherence to methylphenidate treatment in children with attention-deficit hyperactivity disorder (ADHD). Methods: A retrospective medical record review of 239 ADHD patients (mean age $9.3{\pm}2.6$ years, range 6.0-17.4 years) who had visited the child and adolescent psychiatry clinic at a university hospital, in Seoul, Korea from March 2005 to February 2008. Subjects were diagnosed as ADHD based on the criteria set forth in the Diagnostic and Statistical Manual of Mental Disorders 4th edition, text revision version (DSM-IV-TR) and underwent neuropsychological tests including the continuous performance test (CPT). Treatment discontinuation was defined as the last prescription date when the medication possession rate (MPR) became less than 0.80. Subjects were divided into three groups and labeled as Group I, non-adherence without pharmacotherapy, Group II, non-adherence with short-term pharmacotherapy, and Group III, adherence with long-term pharmacotherapy. Results: Ninety (37.7%) patients were grouped as non-adherent (Groups I+II) and 149 (62.3%) as adherent (Group III). The adherence group exhibited lower intelligence, higher symptom severity, and a higher number of comorbid psychiatric disorders than controls. The use of stimulants was significantly associated with long-term adherence to treatment. Additionally, the duration of interval between the date of the first visit and the date of the first prescription was positively associated with long-term adherence. Conclusion: About two-thirds of patients diagnosed as ADHD adhered to the treatment six months after the first visit. With respect to patient evaluation and the development of treatment strategies, factors affecting early drop-out and longer follow-up must be considered.

Quality indicators for cervical cancer care in Japan

  • Objective: We aimed to propose a set of quality indicators (QIs) based on the clinical guidelines for cervical cancer treatment published by The Japan Society of Gynecologic Oncology, and to assess adherence to standard-of-care as an index of the quality of care for cervical cancer in Japan. Methods: A panel of clinical experts devised the QIs using a modified Delphi method. Adherence to each QI was evaluated using data from a hospital-based cancer registry of patients diagnosed in 2013, and linked with insurance claims data, between October 1, 2012, and December 31, 2014. All patients who received first-line treatment at the participating facility were included. The QI scores were communicated to participating hospitals, and additional data about the reasons for non-adherence were collected. Results: In total, 297 hospitals participated, and the care provided to 15,163 cervical cancer patients was examined using 10 measurable QIs. The adherence rate ranged from 50.0% for 'cystoscope or proctoscope for stage IVA' to 98.8% for 'chemotherapy using platinum for stage IVB'. Despite the variation in care, hospitals reported clinically valid reasons for more than half of the non-adherent cases. Clinically valid reasons accounted for 75%, 90.9%, 73.4%, 44.5%, and 88.1% of presented non-adherent cases respectively. Conclusion: Our study revealed variations in pattern of care as well as an adherence to standards-of-care across Japan. Further assessment of the causes of variation and non-adherence can help identify areas where improvements are needed in patient care.
